Matt Pilmoor uploaded a snap showing him and his beloved Yorkshire-based mum sitting alongside each other.
His update on her condition came after she had been hospitalised for 120 days with a "rare" health issue doctors had struggled to diagnose.
Showing his hugely emotional state at the time of posting, Matt wrote: "As I sit here in floods of tears writing this post, mum went into her care home on Tuesday where she will be for the foreseeable future.
"Mum has only just got out of hospital after six months with early set dementia, hyper delirium, brain damage from mild nutrition.
"It's absolutely heart breaking and I can't quite get my head round it all.
"I have been there every single step of the way for my mum and dad but the time has come where they both need full time care."
In the lengthy post, 2023 MAFS star Matt added: "I'm navigating life now without my parents, it is so hard but I know it's something I will have to get used too. I am so incredible close to parents but more so my mum we have been joined at the hip my whole life, so this new stage I'm struggling to come to terms with.
"This picture was taken today, I know she looks very different but they are doing there best to bulk her up and try get more food down her.
"She's the most incredible woman and I will be there always for her.
"There is days where she will know me and other days she doesn't but today she did, She's said today, you will live one life for the two of us.

His co-star Peggy Rose was quick to offer support and put: "Oh Matt this breaks my heart it's difficult to know the right words to say but just know that I'm thinking of you and always know that I'm here for you and Shona."
Ella Morgan then added: "Sending my love. You're incredible Matt. Thank you for being so real and open and honest about the struggle and reality of caring. Always here for you."
Matt's mum is a familiar face to MAFS viewers after she appeared on the show two years ago.
Mafs couples that have stood the test of time
Loved-up Tayah Victoria and Adam Aveling of series six fame had the first Mafs baby.
The pair couldn't keep their hands off each other on the programme and quickly found their feet in the outside world, moving into Adam's Doncaster home.
Just 18 months after meeting, the couple welcomed their daughter Beau.
Season five couple Michelle Walder and Owen Jenkins also managed to make their marriage work away from the cameras and had their first child in December.
Teacher Michelle, 29, has no regrets about taking part in the experiment. She told us: 'I just feel very lucky and thankful that it has worked out - and excited for everything to come.'
Michelle and Owen were both sick of dating apps when they applied in 2019.
Owen recalled: 'I had been out for some drinks with a friend after work.
"While he was out for a cigarette I was scrolling on Instagram waiting for him to come back in.
'The MAFS advert was the last thing I saw, and I joked, 'Wouldn't it be funny if I signed up?'
'A few beers later when I was back at home I sent in the application, and the rest is history.'
Another couple to make Mafs UK history is Zoe Clifton and Jenna Robinson.
Despite a slight rocky start, where they clashed over Jenna's vegan lifestyle, the show's first same sex pairing are still going strong.
They even have a successful podcast together called Life With a Pod.
Jenna shed light on being involved in the show earlier this year when she told us: "We're not legally married, and I never felt like we were. I definitely feel the process makes you take the relationship a lot more seriously and having the help of the experts… if you can survive that process it sets a firm foundation for a long-lasting relationship."
At the time, she expressed her concern over the fact his then-partner Adrienne Naylor did not want kids.
Earlier this year, he told how it "breaks me" when his mum didn't recognise who he was.
On Mother's Day in March, he uploaded a special picture post tribute with the words: "Appreciate your mum and spend as much time with her as possible, because you don't know what's around the corner.'
MEETING ON MAFS
Shona and Matt have been going strong for over a year since their series of the E4 show in 2023 - even though they weren't paired together.
Shona left the show because of her unhealthy relationship with Brad Skelly – whose behaviour was deemed controlling and manipulative by charity Women's Aid.
But she found love afterwards with another star on the show - window cleaner Matt – who originally 'married' Adrienne Naylor.
In the reunion show aired earlier this year, the pair faced a fan backlash after scenes showed Matt "proposing" to Shona with a promise ring.

Cricket WAG Emma Lyon stuns in designer outfit as she watches hubby Nathan play in London - and you won't believe how much it costs
Emma Lyon has turned heads in a stunning designer outfit worth at least $17,800 as she watched her husband Nathan Lyon try to steer Australia to victory in the World Test Championship in London on Wednesday. The mum-of-one, who shares a one-year-old daughter with the spin bowler, was in the good seats at London's famous Lord's stadium after s paring no expense on her flight over to the UK. She treated her Instagram followers to photos of her standing on a balcony at the 'home of cricket', cradling her baby daughter and looking like she could've just done a photo shoot for Vogue. Daily Mail Australia has tracked down the various piece of her ensemble, which is far out of the average Aussie cricket fan's price range. Emma was dressed in an Integrated Fluid Silk Skirt from Dutch fashion house Róhe, which would have set her back about $1200. She paired that with another piece from the designer, a matching Sleeveless Gliet-Jacket worth $1100. Emma's jewellery puts those items into the shade in terms of cost, however: she's wearing what appears to be a Cartier Love Bracelet worth $9050 as well as another piece from the famed luxury company, a Juste Un clou bracelet priced at $6450. The 32-year-old also flaunted her high-end tastes when she documented her journey to England for the match recently. Lyon showed off $22,000 in accessories and $1500 worth of in-flight skin care as she and her daughter flew to London. That included a Chanel bag worth $19,230 and a $3725 tote bag from Goyard. Emma finally confirmed she and Nathan welcomed a baby girl to the world in September 2023. They made the reveal in October 2024, with Nathan now a father to three girls as he shares two children with his ex-partner Mel Waring. The off-spinner lost his wicket for a duck on Wednesday night, Australian time, as the Aussies stumbled to a first-innings total of just 212. He only got one over in before stumps as pacemen Mitchell Starc, Pat Cummins and Josh combined to leave South Africa reeling at 4/43. She flew first-class to England with her daughter late last month and brought her $19,230 Chanel bag (pictured) Lyon has been one of Australia's best players since making his Test debut in 2011, taking 553 wickets at an average of just 30.19. That's reflected in his contract with Cricket Australia, which pays him a rumoured $1.1million a year. He is also contracted to the Melbourne Renegades in the Big Bash League. Lyon's ex-wife Mel Waring recently gave Daily Mail Australia an exclusive update on her life after she moved to Canberra with the two daughters she shares with the star.

Orlando Bloom's just paid £10,000 to have his blood 'cleaned' - but here are 7 much cheaper ways to reduce the quantity of microplastics in your body
At 48, Orlando Bloom still looks as fresh-faced (and drop-dead gorgeous) as when he started out in Hollywood three decades ago. It's unsurprising, then, that the Lord of the Rings star is no stranger to rejuvenating – and often intrusive – procedures, from purging his body with frog poison to ingesting bentonite clay to cleanse his digestive system. But the actor's latest treatment might be his wildest yet. Bloom headed to London 's Clarify Clinic for a £10,000 procedure where a machine supposedly removed the microplastics from his blood.
