Machine operator killed after concrete pile falls at Rawang construction site
KUALA LUMPUR: A machine operator was killed after he was struck by a reinforced concrete (RC) pile that sprang loose from its hook near a construction site in Rawang, Selangor, on Thursday (April 24).
The Selangor Department of Occupational Safety and Health (DOSH) said the incident occurred at 8.40am, when the victim, a foreigner working as a piling machine operator, was carrying out foundation work to erect piles with the assistance of a colleague and a project supervisor.
"During the process of pulling, erecting and positioning the pile, one of the piles being lifted slipped from the hook and fell onto the machinery cabin, causing the operator to panic and jump out.
"Unfortunately, the victim was hit by the falling pile and suffered severe injuries," DOSH said in a statement today.
The victim was later pronounced dead after being taken to Sungai Buloh Hospital.
Following the incident, the Selangor DOSH issued a Prohibition Order to keep the accident site undisturbed until investigations are completed, as well as a Prohibition Notice to halt piling activities.
"Further investigation is being carried out to identify the cause of the accident and the parties responsible for it," it added.
The investigation is conducted under Section 15(1) of the Occupational Safety and Health Act 1994, which carries a maximum fine of up to RM500,000 for an employer if found guilty.
Legal action can also be taken against employers if violations of worker safety and health regulations are found.
DOSH also stressed that such incidents can be prevented if employers carry out comprehensive risk assessment and control in accordance with the Hazard Identification, Risk Assessment, and Risk Control guidelines.

Police defied court order, failed to probe Dutch model's death, lawyer claims
Published on: Tuesday, June 10, 2025 Published on: Tue, Jun 10, 2025 By: Ho Kit Yen, FMT Text Size: Dutch model Ivana Smit was last seen with Alex Johnson and Luna Almaz on Dec 7, 2017 at their apartment, before she died. (Instagram pic) Kuala Lumpur: The High Court here was told today that the police had failed to conduct a thorough probe into the death of Dutch model Ivana Smit eight years ago, despite a court order directing an investigation. Lawyer SN Nair, appearing for the family, said another High Court judge had in 2019 ruled that Smit's death had been caused by 'persons known or unknown' and directed the police to take further steps in the matter. Advertisement Despite that, Nair said the subsequent investigation was done 'half-heartedly', pointing out that the police had not sought the extradition of the two main suspects, Alex Johnson and Luna Almaz. Smit was last seen with the American-Kazakh couple at their apartment on Dec 7, 2017, before she died. 'The investigating officer even conceded that (an Interpol) blue notice issued was not effective in tracking down Alex and Luna,' said Nair. Smit's family is suing the police and government for breach of statutory duties and negligence in her death probe. Nair said the issuance of the blue notice cannot be considered sufficient action in the probe into Smit's death. He said the police ought to have secured the extradition of Johnson and Almaz, taken further witness statements, and re-examined the available forensic evidence. 'This case is a sobering reminder of what happens when those charged with protecting justice are themselves negligent,' Nair added. In reply, senior federal counsel Nur Ezdiani Roleb, appearing for the government and police force, told the court that investigations into Smit's death are still ongoing. She insisted that the police had not breached their statutory function but had performed their investigative duties properly. 'The police were entitled to probe this case as a sudden death in 2017, based on the findings of the pathologist (Dr Nurliza Abdullah),' said Ezdiani. She also told the court that Smit's family was not entitled to seek damages over a purported breach of statutory duties and negligence. Trial judge Justice Roz Mawar then asked Nair to quantify the damages the family was seeking—in the event the court ruled in their favour. Nair said previous decisions indicated that RM500,000 would be an adequate sum, but left the matter for the court to determine. Ezdiana said the government will file additional submissions to address the quantum of damages issue. The court then fixed July 15 for decision. Dutch ambassador Jacques Werner was present at the hearing today. * Follow us on our official WhatsApp channel and Telegram for breaking news alerts and key updates! * Do you have access to the Daily Express e-paper and online exclusive news? Two policemen, ex-cop claim trial to graft charges
ALOR STAR: Two policemen and a former cop claimed trial to corruption charges at the Sessions Court here today. Lance Corporal Ahmad Rabani Rusnizal, 29, and Lance Corporal Mohamad Shukre Mohamed Rowi, 37, together with former police corporal Wan Mohd Nor Imran Wan Seman, 35, entered the plea before judge N. Priscilla Hemamalini. According to the charge, the three of them, acting with common intent, allegedly agreed to accept a bribe of RM20,000 in cash from Azizi Norhafizi Abu Bakar as an inducement not to take action against him for being in possession of methamphetamine (syabu). At the time of the alleged offence, all three were serving in the Crime Prevention Branch at Merbok police station. The offence is alleged to have taken place at the Semeling police post, Kuala Muda, at 11.30pm on July 23, 2023. Meanwhile, in the same court, Ahmad Rabani, Mohamad Shukre and another policeman, Corporal Azmil Rizal Azhar, 34, also pleaded not guilty to two other charges. For the first of these charges, the three accused allegedly agreed, with common intent, to accept a bribe of RM10,000 from Azizi Norhafizi as an inducement not to take action against him for possessing methamphetamine. They also claimed trial to a separate charge for allegedly receiving RM5,000 bribe from the same individual under similar circumstances. The two offences were allegedly committed at the Semeling police post and at a 7-Eleven outlet in Taman Lembah Bujang Indah, Merbok, on Oct 25 and 26, 2023, respectively. All charges were framed under Section 17(a) of the MACC Act 2009. If found guilty, they face a maximum prison sentence of 20 years and a fine of not less than five times the amount of the bribe or RM10,000 — whichever is higher. The Malaysian Anti-Corruption Commission prosecutor, Mohd Zulfadli Azharudin prosecuted all cases, while all the accused were represented by counsel Siti Salwa Ja'afar. Zulfadli proposed that the court set RM10,000 bail for each of the accused, but Siti Salwa pleaded for half of the amount. The court subsequently set bail at RM8,000, with Ahmad Rabani and Mohamad Shukre each required to pay RM16,000. July 20 has been fixed as mention date.
