Proper DRG roll out can take more than six months, says private hospitals association

KUALA LUMPUR: A proper rollout of the diagnosis-related group (DRG) payment model could take more than six months given the complexities in data gathering and analysis, says the Association of Private Hospitals Malaysia (APHM), adding that it is ready to provide any help.
Its president Datuk Dr Kuljit Singh said accurate clinical data and a national electronic health record system is needed for any DRG-type mechanism to work
He said this foundational data is not available yet.
"As the process of gathering and analysing such data is complex and time-consuming, APHM anticipates that a proper rollout will require considerably more than six months," he said.
Dr Kuljit also said the APHM welcomed Health Minister Datuk Seri Dr Dzulkefly Ahmad's announcement that a basic medical and health insurance/takaful product would be introduced later this year and the DRG rolled out in phases.
To support this national initiative, Dr Kuljit said APHM member hospitals have offered to share relevant clinical data with the Health Ministry and the Finance Ministry.
"APHM strongly advocates that adequate time and resources are allocated to ensure that the DRG initiative is thoroughly conceptualised, piloted and implemented, to ultimately deliver sustainable improvements for all Malaysians," he said.
The DRG payment system involves paying a fixed sum based on the diagnosis and treatment provided, rather than itemising each charge.
It is said the DTG model can help regulate medical costs by limiting excessive charges.
Examples of countries that have implemented this are Sweden, Canada and Australia.
Earlier, in his opening speech, Dr Dzulkefly said introducing DRG in phases to pay for healthcare services, beginning with the base medical and health insurance/takaful products, would be a key driver for value-based healthcare.
He also said the Health Ministry is hoping to introduce the DRG payment model by the end of 2025.

KUALA LUMPUR: The diagnosis-related group (DRG) system will be introduced alongside the basic Medical and Health Takaful Insurance (MHIT) starting next year. Deputy Health Minister Datuk Lukanisman Awang Sauni stated that the basic MHIT will act as a standard platform to ensure DRG-based payment efficiency. He explained that the private sector DRG is an interim measure to curb rising medical costs burdening the public. The private sector DRG differs slightly from the national DRG but will eventually integrate through system interoperability. Lukanisman shared these details during a Dewan Rakyat session in response to Suhaizan Kaiat's query on healthcare cost reforms. The national DRG was formulated in 2023 under a five-year plan for full implementation by 2027. A Joint Ministerial Committee on Private Healthcare Costs (JBMKKS) was formed to oversee cost-control initiatives, including DRG. The private sector DRG will support a more inclusive national system by improving data integration and analysis. Government and private agencies are collaborating on technical studies and ICT system development for the national DRG. This year, clinical coding workshops and system development will take place in both government and private hospitals. By 2026, the national DRG algorithm and coding model will be finalised before system testing begins. The official launch of the national DRG system is scheduled for 2027. Lukanisman confirmed no plans for a patient tribunal but noted existing complaint mechanisms under Act 586. Complaints against private hospitals are handled by the Private Medical Practice Control Section (CKAPS). KUALA LUMPUR: Full enforcement of the medicine price display policy in private healthcare centres and community pharmacy facilities will begin on Jan 1 next year, the Dewan Rakyat heard today. Deputy Health Minister Datuk Lukanisman Awang Sauni said the move follows an enforcement education period of three months ending on July 31 for the premise management to adapt to the policy where no penalty and compounds will be issued. "This is then continued with the phased administrative enforcement period Aug 1, during which inspections and continuous monitoring will be conducted until Dec 31. "During this period, reminder letters will be issued to non-compliant facilities, while compounds for repeated offences will be imposed from October 1," he said. A full enforcement will be made next year, Lukanisman said in response to the question posed by Datuk Dr Ahmad Yunus Hairi (PN-Kuala Langat) on the status of the prudent implementation phase of the medicine price display at private healthcare facilities and community pharmacies, as well as its effectiveness in controlling healthcare service charges from unreasonable hidden costs. Lukanisman added that initial assessment of the policy's implementation showed that, as of July 31, 55 per cent, or 900 out of 1,639 private healthcare facilities inspected, had satisfactorily complied with the requirements. "However, 45 per cent, or 739 facilities, had yet to meet the expected compliance level, including having incomplete price displays or displays that were not easily accessible to patients. "These findings indicate the need for further action, such as strengthening monitoring, conducting additional guidance sessions, and phased enforcement against non-compliant premises," he said. He said the Health Ministry will also conduct a study under the Medicine Prices Monitoring Study beginning September 2025 to evaluate the impact of the policy, with its findings serving as a key reference for policy improvements, particularly in pricing and transparency. The aim, he said, is to create a more transparent, fair, and consumer-friendly private healthcare system. The mandatory price labelling for medicines at private healthcare centres and community pharmacy facilities was rolled out on May 1. It was aimed to ensure the public can make informed choices by knowing, comparing, and selecting the best prices when managing their medication expenses.
