Mum of Chesterfield boy, 3, who died of sepsis criticises 'chaotic' hospital care

The mother of a three-year-old boy who died from Sepsis has told the inquest into his death that he was not given antibiotics early enough and doctors failed to recognise the condition. Oscar Neillings died at Chesterfield Royal Hospital on 8 November 2023 after contracting a Strep A infection. At the inquest into his death at Chesterfield Coroner's Court on Wednesday, jurors were told Oscar developed a cold about a week before his death from which he never recovered. As his symptoms worsened, his parents called 111 twice in the days leading up to his death and he was given appointments with two separate out-of-hours GPs. Both doctors said he had a viral infection and did not think he needed to be referred to hospital, the court was told. Oscar continued to deteriorate, suffering from abdominal pain, vomiting, and breathing difficulties. On the morning of 8 November he had a GP appointment in the morning, where an ambulance was called and he was taken to Chesterfield Royal Hospital. He died there at 5.24pm after suffering a cardiac arrest. Giving evidence to the inquest, Oscar's mother Stephanie Neilling paid tribute to her son, saying he was "so full of life, I think that's why its so hard to believe he's gone." She said: "His personality was infectious. He had confidence I could only dream of." Ms Neillings also criticised the care he had been given, both by the second out-of-hours GP and at the hospital. She said she thought something should had been done after the second appointment, telling the court: 'It's madness how he was sent home diagnosed with a viral infection but obviously in pain.' She said at the hospital no-one recognised that Oscar had sepsis and described the care he received as "chaotic" with a lack of communication and leadership. Ms Neillings told the court: "It was so unorganised - I didn't know what was going on." She added: "There was a lack of leadership – no-one took control throughout the day. He didn't get the antibiotics when he needed them, they missed their timescales for that." Doctors who cared for Oscar, both before and during his time in hospital, are due to give evidence to the inquest. It is expected to last until Friday 23 May.

He said of his symptoms at the time: "I was waking up four to five times needing the toilet. "Then I started to lose weight quite quickly. I lost about a stone and a half in three weeks, but I was still eating the same and going to the gym every day." PETALING JAYA : Although Malaysia has modernised its tax system and launched outreach efforts aimed at young adults, many new entrants to the workforce still misunderstand their income declaration duties, especially those earning below the taxable threshold. Universiti Utara Malaysia School of Economics associate professor Dr Logasvathi Murugiah said this misconception is especially common among fresh graduates, part-time workers, freelancers and individuals in the informal or gig economy. 'Many young people assume that if they don't owe any tax, they don't need to declare their income. 'Some believe that tax filing only applies to permanent employees or that low-income earners go unnoticed by the system, which simply isn't true.' Logasvathi, who is a financial literacy expert, said the misunderstanding stems from several factors, including low tax literacy, confusion between declaring income and paying tax and the common belief that taxes only become relevant once a person starts earning a higher income. 'There's a cultural mindset that tax is a 'later in life' issue. But the reality is, income declaration is a legal and financial responsibility that starts the moment you earn, even if it's just from side jobs or freelance gigs.' According to the Inland Revenue Board (IRB), individuals with an annual employment income exceeding RM37,333 (or about RM3,111 per month) and who have a monthly tax deduction are required to pay tax. This threshold is calculated after deducting allowable tax reliefs and rebates from one's gross income. However, even those earning below the taxable threshold are strongly encouraged to declare their income. Logasvathi said there are many benefits in doing so, beyond just compliance. 'Young earners may not realise the advantages they're missing out on. Filing a tax return helps build a financial record, something banks and institutions look at when you apply for loans, credit cards or government aid. 'One might even be eligible for tax refunds or reliefs they're unaware of.' She added that the consequences of not declaring income, even when no tax is owed, can be significant over time. 'Individuals may lose out on financial opportunities, face difficulties verifying their earnings for official purposes and risk legal penalties if they later fall under audit. 'On a national level, undeclared income contributes to revenue loss, weakens economic planning and shifts the tax burden onto those who are already compliant. 'It also erodes trust in the fairness of the system. When only some people follow the rules, it discourages broader participation.' Logasvathi said to bridge these gaps, IRB has rolled out several digital and youth-focused initiatives in recent years. 'Since January 2024, the MyTax portal has allowed Malaysians to register, file and manage their taxes entirely online. At the same time, outreach initiatives such as HASiL Your Tax Buddy and Program Sahabat HASiL have turned to social media to engage younger audiences. 'The government is doing a lot to meet young people where they are, online. But the system still needs to be demystified. Tax education should be introduced in schools and universities so young Malaysians understand the process before they even start working.' For those new to the system, she said the steps to get started are more straightforward than many expect. 'The first step is to register for a Tax Identification Number using the e-Daftar system on the MyTax portal. From there, you can access the e-Filing platform, declare all sources of income, including freelance or part-time work, and submit your returns before the deadline. 'There are tools to help, too. The MyTax portal is user-friendly, and IRB also offers a mobile app, a chatbot and a tax calculator to make the process easier.'

For the first time in the nearly 70-year history of Billboard's albums chart, a pair of Spanish-language albums — Bad Bunny's ode to Puerto Rico, 'Debí Tirar Más Fotos,' and Mexican-American band Fuerza Regida's '111xpantia' — lead the charge. 'Debí Tirar Más Fotos' is the No. 1 album in the United States, climbing up from its previous No. 7 position following its first release on vinyl. It's the fourth total week at No. 1 for the album, which spent three consecutive weeks at the peak starting Jan. 25. The 17-song set returns with a total of 84,500 equivalent album units, with more than half of that total deriving from vinyl purchases, according to Luminate. It earned a total of 76,000 units with 39,000 album sales and 50 million streams. With 39,000 copies sold, '111' surpasses the previous largest sales week for a regional Mexican album, when Selena's 'Amor Prohibido' sold 36,000 copies in 1995. It's also the highest-charting Spanish-language album by a duo or group, and the highest-charting regional Mexican music album ( Peso Pluma's 'Génesis' held the title since 2023), as reported by Billboard. Prior to '111,' the band — led by Jesús Ortiz Paz (JOP) with José 'Pelón' García, Moisés López, Khrystian Ramos and Samuel Jáimez — scored its biggest debut on the Billboard 200 in 2023 with the release of 'Pa Las Baby's y Belikeada.' The second debut in the Top 10 of the list this week comes from rapper Key Glock and 'Glockaveli,' which debuts at No. 8 with 34,000 units. The 18-song set was also expanded with a deluxe edition featuring an additional three tracks. As for the remaining top titles: SZA's 'SOS' moves to No. 3 from No. 2; Morgan Wallen's 'One Thing at a Time' is at No. 4; Kendrick Lamar's 'GNX' is at No. 5; Sabrina Carpenter's 'Short n' Sweet' is at No. 6; PartyNextDoor and Drake's '$ome $exy $ongs 4 U' is at No. 7; and Wallen logs his second simultaneous Top 10 album with 'Dangerous: The Double Album' at No. 9. Martin Fowler actor James and his wife of thirteen years Victoria share four children – Edward, ten, Louis, eight, Hugo, five, and Rufus, who turns two next month. Victoria runs an Instagram account documenting life events for the family, and told her 130,000 followers that Rufus had been hospitalised after their recent holiday. 'So, it all started earlier this week, when just before bed the boys were outside on the trampoline.
