Report into alleged ‘toxic' atmosphere in Children's Health Ireland has been sent to the gardaí

The persons involved have not had an opportunity to respond, the Taoiseach Micheal Martin noted in refusing to publish the report. But they may now have that chance in any interviews undertaken by detectives. Meanwhile the general collapse in morale is set out in the 2021 report and makes for disturbing reading. There is an alleged case where a consultant outside the country was allegedly refusing to allow another to help a child leave hospital in time for their First Holy Communion. This accusation, however, is currently unproven. The report looks into alleged bullying and disruptive behaviour at Children's Health Ireland and involved 49 interviews with 44 participants over a four-month period. The average duration of each interview was two hours. The report says: 'Over the course of this examination, 89pc of participants interviewed described a culture where change was slow, which lacked governance and robust processes, and was heavily influenced by strong and exceptionally challenging personalities working in the organisation. 'Within (detail deleted), 66pc of participants interviewed acknowledged that there was a lack of collaborative working. During the interview process, multiple participants described unprofessional and disruptive behaviour from consultants that significantly contributes to the culture which exists today.' Six consultants were identified only be letters of the alphabet, running from A to F, although there are other health professionals named elsewhere in the report. It says that '27pc of participants described unprofessional behaviour from consultant A and 52pc from consultant D. 'As the examination progressed, additional areas for exploration were highlighted by participants which directly related to behaviours of particular staff.' ADVERTISEMENT Learn more A senior admin staffer and a consultant were described as having considerably challenging behaviour by 100pc of their colleagues with whom these concerns were specifically explored. "These behaviours, which appear to have gone unmanaged by site leadership, have incrementally led to a number of challenges for the organisation, such as staff attrition, overall staff health and wellbeing, operational and financial impacts and — of huge significance — quality issues, which ultimately have significant potential for impacting patient safety," the report says. Nearly half of participants (48pc) voiced concerns about the effectiveness and decision-making of site leadership. 'Of participants interviewed, 43pc expressed that they wanted to see real change across CHI and were willing to be part of the solution. 'Many participants describe significant inter-personal difficulties among the surgical team, leading to what many narrate as a toxic environment.' The report also refers to a series of emails back and forth between general surgeons and other specialists, disputing what was in the remit of one another's roles. The tone of the emails between the parties is said to suggest 'a complete lack of professionalism by some, ultimately wasting professional consultants' time, negatively impacting relations and significantly potentially affecting patient care'. The report also makes clear that senior hospital figures disputed the smallest things, such as individual items to be used in treating patients. There was an apparent inclination amongst some to pass the buck or to avoid taking responsibility, which sometimes involve disputed about who did what in Monday to Friday working. In one case, consultant D called out 'unprofessional behaviour and abuse of the emergency list' on the alleged part of another. Consultant D is said to have written: 'The Department cannot have things both ways. This is why I have stopped going to these meetings. 'Decisions are made and apparently agreed, and then after the meeting, individuals just do their own thing.' Meanwhile, consultant E supposedly said of consultant D that their conduct and actions 'have the potential to undermine professional working relationships and patient care. This, unfortunately, has not been an isolated incident, but rather a pattern of behaviour.' In one case, a consultant was asked to attend a special meeting, but the email received no response for a period of six weeks. Finally, there was a response which involved no retraction of a previous statement, as requested. Instead it said: 'I acknowledge receipt. I have nothing further to add.' 'The report says the above scenario is a snapshot of the problematic communication and fraught relationships' that exist between general surgeons and specialists. 'Although patient care was not compromised in this instance, the communication style, accusatory language and indeed, unprofessional antics that continued over this period add to further compromise inter-interpersonal relations, heightened levels of mistrust among colleagues, and ultimately are a distraction from patient-centric care.' The authors noted that 'research has shown that poor behaviours such as verbal abuse, publicly humiliating others, and discontinuation of communication (such as not engaging in meetings), destabilises psychological safety and undermines speaking up about patient safety issues. 'This increases the risk of harm to patients.'

Perching on white thrones beside three-tiered cakes adorned with white icing, their make-up perfect and tiaras perched on their heads, the girls looked every inch the perfect brides in their voluminous white gowns. But these scenes were not from a lavish wedding day, however. Girls from the Traveller community have been enjoying the UK and Ireland's most lavish First Holy Communions throughout April and May when the religious ceremony is traditionally held. With white, flowing gowns, balloon arches, and a carriage suitable for Cinderella herself, one might easily mistake the events, where Catholic children receive the Eucharist for the first time, for weddings. In addition to marking the important rite of passage of receiving the body and blood of Christ for the first time, a child's First Holy Communion appears to be an excuse for parents and loved ones to throw an unforgettable bash to remember the occasion. One young woman employed the help of hair stylist Tori Boylan, who describes herself on her TikTok as a 'pro bridal and party hair specialist', to achieve her jaw-dropping look. The girl from Rathkeale, County Limerick, put on a glittering display in a dramatic white ball gown, white sash, floor-length veil and net gloves. She finished off her look with glamorous makeup, complete with a pink blusher and matching lipstick. Her look isn't one-of-a-kind, and another bridal specialist, @kimsqueenstyles, worked with a duo of girls called Sofia and Vallerina to mark their First Holy Communion in a similar fashion. Kim shared an image of one of the young girls stood on the stage, which featured a balloon arch and a LED light up cross, mirroring an engagement pose, as a man, presumably her father, knelt down on one knee with her hand in his for a photograph. Prior to the event, Sofia and Vallerina posed on the stage in silk white robes before changing into their elaborate gowns. Both of them opted for similar hair and makeup styles, which included glowing, bronzed faces, and their hair up to ensure the focus was on their dazzling gold and pearl tiaras. Elsewhere, a different young lady appeared to have trumped the others when it comes to having the most dramatic First Holy Communion entrance. TikTok account @royalbridalweddings shared a clip of a girl, dressed in an elegant princess dress and holding a silver wand, arriving at her First Holy Communion in a magical white carriage with her family. She was helped in and out of the carriage by presumably her mother, who also dressed in white for the happy occasion. And it was no easy feat, with her mother and other guests throwing the flowing material from the girl's dress into the carriage before they could set off to their destination. Meanwhile, another girl called Savanna looked equally as elegant as she donned a long-sleeved white glittering gown for her special day. Annie had a stylist spritzing perfume in her hair before she walked outside of her home, which was decorated with a white carpet rolled out for her to step out onto. 'Here is Annie', @kimsqueenstyles wrote alongside the footage of the young girl on her special day. One dress was clearly not enough for her, and Annie later changed into a stunning after-party dress made from gathered white material. In a different post, Kim shared footage of another event she worked on for a young girl called Juliet, who sat posing on a floral embellished swing in the sunshine for photographs. Later on, the swing appeared to move inside as Juliet posed by what appeared to be the same swing under the neon lights of the after party. Meanwhile, Nicole Mcadorey from Belfast shared footage of her daughter posing in her white dress before her First Holy Communion. 'We have waited forever for this day - my little darling's First Holy Communion,' Nicole wrote alongside the clip of her daughter. It's not just elaborate gowns and fancy decorations that families have splashed out on for their children's communions, but also lavish cakes. One young girl called Nellie enjoyed a stunning three-tiered cake, complete with white frosting and pink icing flowers designed by White Blossom Bakes. Meanwhile, another event planner called Andrea Shillingford, who goes by Extravagant Travellers Events, shared images of another three-tiered cake draped in flowers for a young girl called Rosie. Rosie appeared delighted as she posed on the marble dancefloor, which was inscribed with the words, 'Rosie's Holy Communion'. It comes after a bride who tied the knot in lavish style at The Dorchester has pulled off what appears to be the UK's most expensive Traveller wedding to date. Louise Durant, 21, and Billy Maguire, 22, celebrated their union in a winter wonderland wedding at the luxurious Mayfair hotel - where room hire can start at £10,000 - surrounded by 350 guests. The bride, whose family is from the Traveller community, said the festive nuptials - which took place in December - were 'more than a dream come true'. 'It was beyond my wildest imaginations,' she told MailOnline. 'The day started amazingly and just got better and better. It was perfect.' The family had been saving for two years, and indeed no expense was spared to create the perfect magical occasion. Louise's parents couldn't confirm how much exactly the wedding set them back, but said it was well over £100,000 - and didn't baulk when figures of around £250,000 were mentioned. Those in attendance at the five-star venue on Park Lane - which has been visited by the likes of David Bowie and Elizabeth Taylor - were treated to glittering décor with flowers as white as snow, festive candles, icy blue lighting and an elegantly trimmed Christmas tree. The azure dancefloor, which featured a glitzy disco ball gave the illusion of an ice rink for the happy couple to enjoy dancing on, while each table was adorned with snow globes and Christmas cracker party favours containing Bible verses. Friends and family compared the beaming bride to a 'princess', who was the image of glamour in a structured ivory gown, while followers on social media likened the celebrations to a ' royal wedding '. Louise, 21, and Billy, 22, live in Yorkshire and had been together for four years before deciding to tie the knot. Festivities on December 14 kicked off with a bang as four ex-London double decker buses picked up guests from their hotel, the Hilton on Park Lane, at 11.30am and took them all to St John's Church in Hyde Park. Louise's dress - by Eleni Bridal Couture of North London - featured a corset-style bodice embroidered with lace detailing, which fanned out into a sweeping skirt, and long sleeves which puffed at the shoulders. The bride and her 12 bridesmaids were taken there in a horse-drawn carriage from The Dorchester. She entered the church with music from the London Fanfare Trumpets who had dressed for the ceremony to look like Coldstream Guards. The service was an hour, and then guests were taken to The Dorchester for the wedding breakfast and the evening service. Starters was a mushroom soup with pastry on top, while the main featured a festive Christmas turkey dinner. To finish up, desert was sticky toffee pudding with the couple's initials and the date of the wedding written in chocolate sauce. Each table was adorned with snow globes containing photos of Billy and Louise, white shortbread biscuits in the shape of a dove, and Christmas crackers which contained a verse from bible and a chocolate in. The evening ceremony was in The Dorchester's ballroom and ran until 1am - and Louise and Billy's first dance was Ain't Nobody by US funk band Rufus and Chaka Khan. Music was provided by The Function Band and DJ Andrea Shillingford. One of the guests - Belsie Connors, who regularly sings at Traveller fairs and events - also sang for the occasion. Louise and Billy - who work for the family's holiday and lettings business - stayed at The Dorchester until December 16 and then embarked on a three and a half week honeymoon to Barbados, Orlando and New York. 'We had family from all corners of the UK, it was brilliant to see everyone. I had the best bridesmaids,' the bride gushed. 'The staff at the Dorchester were incredible, they helped make our big day all the more memorable. 'These are memories that will last a lifetime.' Her mother Vicky Morrison, who lives with relatives on a gated compound made up of five static homes near the village of Bagworth in Leicestershire, said the whole family 'chipped in' to make the couple's dream wedding come to life.

GAA club news Strong finish secures two more valuable points - Last Saturday night in balmy Killavullen, and a well prepared but rock hard pitch, Killavullen saw off Meelin, in the latest game in the Red FM County Division 7 league. As always it was not so straight forward, as Meelin are a seasoned tough, uncompromising team and you certainly knew you had played a game, afterwards. With several changes from the previous game, due to injuries and unforeseen and unavoidable absences, we started well and had the opening score of the game from Owen Magner, a lovely point from in and around midfield. Meelin hit back though and scored from two close in free's and then from open play before Jamie Magner scored two points in quick succession, to bring us level by the sixth minute. Mikey O Connor edged up ahead from his centre forward berth, but Meelin tracked us all the way and scored the next two points to regain the lead. Sean Woulfe equalised for us, in the 10th minutes, ten minutes that had simply flown by. Darragh O Grady edged us ahead again only for Meelin, to equalise after 15 minutes had elapsed. We were getting very little change out of a teak tough full back line of Meelin, but Colm O Mahony with a deft touch to a high centre from Owen Magner, changed the trajectory of the ball, and was very unlucky to see the sliotar strike the upright and rebound back into the goal keepers grasp and away. We were not quite firing on all cylinders up front though, and the management effected a change by bringing on Seamus O Mahony for Seamus Fox. Jamie edged us ahead again from a dead ball situation but again Meelin, tenaciously fought back to level. Jamie scored a lovely point on the run, but Meelin edged level and then ahead with two close in frees. Colm O Mahony got us level again, with a point from open play, created by Kian Lane and Jamie had us to a score of 10 points with another point from open play, but just before the interval, Meelin got back on level terms, with another point from a free to leave the score at the break 0-10 apiece. Eoghan Buckley was sprung from the bench for Sean Woulfe at the interval. Colm O Mahony scored a close in free for us, that he certainly earned for himself, but Meelin again equalised. Eoghan Buckley then scored a fine point for us, after good initial play from Eddie Cotter and we were unlucky to see a high ball bounce off the crossbar and away to safety. Colm scored another close in free after Darragh was fouled, and then from a breaking ball, Eoghan Buckley pulled first time, and the ball flew over the cross bar at pace, it could have been a brilliant goal, if only a few inches lower. Our half back line of the Cotter brothers on the flanks and Liam Cronin, anchoring in the centre were very assured all night long, when in behind, the returning Kevin Fox, Eoin Birchill and Ronan Barry were tough, and very tough, on a night, when anything else would have been found out. Mikey scored another point from an acute angle, over 20 metres out, but the crowd congregating in and around the shop area, certainly informed the referee that it was over the bar with their vociferous approval. There was 15 minutes gone in the half, at that stage. Meelin scored from a free, and we got a reprieve when their full forward got inside our full back line, and from a very tight angle, his ground shot hit the outside of the post, and away over the end line, Meelin got another converted free, before Colm O Mahony, the recipient of some rough house treatment all night long, got a free that drew ironic jeers from the crowd. We lost a player to a straight red card with seven minutes plus injury time to go, and in the context of the game, it was a very harsh decision. Meelin scored a free, and then playing the ball around well, to exploit their numerical advantage, they brought their score to 0-15 as against 0-16 to us, with four minutes remaining. Eoghan Buckley went back int the centre position of the half back line, and was instrumental in stuffing out any further threats from Meelin. Our forwards worked desperately hard for the remainer of the game, to cover for our numeric loss, and that hard work was rewarded by Owen Magner when he snapped up a ball in a crowded ruck, and his impetus was impended and his brother Jamie scored the resultant free, from in and around midfield. Jamie scored the next point as well, from another hard earned free, before Colm O Mahony fired one over his shoulder to bring our tally to 0-0-19 in the 3rd additional minute of the half. Killavullen finished impressively though, and Brian Cotter set up Owen Magner for another point, before Colm O Mahony scored the final score of the game, to bring our tally to 0-21 in the 35th minute. Killavullen had a shop running over two nights of this weekend, and it was very well supported, so a big thank you to those who managed it, on the nights. It is a great addition to the facilities at the venue. Lotto Jackpot Shared - There was two very local and deserving winners of the local GAA lotto on last Tuesday night, 13th May in the draw, which took place in the Community Centre. The numbers drawn were 7, 12 and 22. The jackpot on the night was, at €2,100. Pa Sheehan, former player and now, minor coach was one winner and got €1,050 for his efforts, and the other winner was Michael Linehan, Main Street, who is the Assistant Lotto Manager, and much, much more in our club. Pa bought his ticket in the Haven Bar, and Micheal sold the winning ticket to himself. Michael also won €1,050 and in an interesting little anecdote, Michael won the Freemount Lotto, on the same date, 6th May, all of 30 years ago. Some coincidences that. The winners of the lucky dips, last Tuesday night, were as follows: €50 - Sheila Barrett (seller John Angland) €30 – Eric Piggott (seller Haven Bar); €20 – Eric Piggott (seller Haven Bar). The next jackpot on next Tuesday night, 30th May will be at €1,000 when we start all over again, with a new jackpot. Killavullen player in Primary School Game - Congratulations to Aveen Curtin who has been selected to play in the primary school game, in Pairc Ui Chaoimh next Sunday, at the half time interval between Cork and Waterford. With Cork needing to win, there is sure to be a huge crowd at the game, and here's hoping that the result favour Cork, and that Aveen enjoys her day. Golf Classic Good interest in been expressed in our annual Golf Classic, which is creeping up steadily upon us. Killavullen GAA Golf Classic for 2025 will be on in Doneraile Golf Club, on the 13th and 14th of June. There is also competitions, for the longest drive for men and women, and nearest the pin on the 8th hole, plus a raffle on the days; it will make for a very enjoyable occasion. Equipment Funding is available for eligible persons over 65 years for the installation of an monitored alarm on a landline in the person's home. The alarms are now available for persons aged 65 or older and living alone, living with another person who meets the terms and conditions, living alone for significant periods of time during the day, or is a carer to someone else in their household. First year monitoring is free for all new applicants. Subsequent annual monitoring charges are then payable by the user. Other systems are available at various costs for those who do not possess a landline. Overall this monitoring product adds peace of mind for both the users, family and friends at minimal cost.
