Genetic Tests in AF Patients May Flag Heart Failure Risk
Genetic testing in patients with atrial fibrillation (AF) may identify those at higher risk of developing incident cardiomyopathy or heart failure, new research from the Netherlands suggested. In a study that included two longitudinal cohorts (the All of Us Research program and the UK Biobank), the prevalence of inherited rare gene variants associated with cardiomyopathy was twice as high in patients with AF and up to five times as high in patients with early-onset AF, defined as that occurring in people younger than 45 years. The analysis, of 44,182 patients with AF, also found those with gene variants associated with cardiomyopathy were at a higher risk for incident cardiomyopathy or heart failure after a diagnosis of AF — about 50%-70% higher compared with noncarriers — independent of clinical and polygenic risk. The findings appeared in JAMA Cardiology . 'If you see a young person in the clinic with AF, first of all, it's not a good sign,' Sean J. Jurgens, MD, MSc, PhD, assistant professor in the Department of Experimental Cardiology at Amsterdam UMC, and a co-author of the study, told JAMA in an interview about the research. 'The risk of heart failure and cardiomyopathies are relatively high and substantially elevated in any person with AF at a younger age. It's not a benign disease where you only have to treat the ischemic stroke risk.' People carrying rare pathogenic variants 'are the patients you really need to look after because these will be the ones who will go on to develop heart failure more likely,' he said. Jurgens acknowledged barriers remain with widespread genetic testing, citing cost, insurance coverage, a lack of cardiovascular genetic expertise, and the lack of genetic counselors. Genetic testing is also more widely available in a smaller country like the Netherlands. In the United States, especially outside of urban centers, accessibility is particularly limited. 'This is a very difficult problem, is why it is so important to do the right stratification,' Jurgens said. 'We can't offer it to everyone. One thing that's good to see is that the prices, at least on the technological side are dropping.' Jurgens said the new research may help reduce the cost of genetic testing, as it narrowed the panel of pathogenic variants from more than 100 to 26, with a high degree of confidence. Notable Strengths In an editorial accompanying the journal article, Olivia G. Anderson, MS, CGC, with the Division of Cardiovascular Medicine at the University of Pennsylvania Perelman School of Medicine in Philadelphia, and her colleagues called the new study 'methodologically rigorous.' The work 'affirms the observation that AF can be an early phenotypic manifestation' of genetic cardiomyopathy, they wrote. Anderson and her co-authors praised the study for generating concordant results from a sensitivity analysis using a 12-month blanking period, which reduced the likelihood patients were diagnosed with AF and cardiomyopathy at the same time. They also said the Dutch team's study makes a strong case for genetic testing in patients with AF, particularly for those younger than 45 years. 'Their findings also highlight the increasing need for genetic counselors embedded in cardiology practices and an expanded availability of specialized cardiovascular genetics clinics,' they wrote. Advancing access to clinical trials for cardiomyopathy
( NewMediaWire ) - May 27, 2025 - DALLAS — Despite scientific advances in cardiovascular care, people in living in rural areas and other communities with long-term economic or social challenges still face barriers to cutting-edge therapies such as gene editing, according to a 2020 American Heart Association presidential advisory. To address gaps in care, the Association, devoted to changing the future of health for all, is using its Get With The Guidelines(R) data to support patient populations that may be underrepresented or overlooked in clinical trials. Get With the Guidelines programs connect hospitals with current evidence-based guidelines and accurate measurement tools to improve care quality and industry practices. As part of this work, the Association today announced an initiative to improve education, outreach and access to clinical trials for gene editing therapies for transthyretin amyloid cardiomyopathy (ATTR-CM). ATTR-CM is a progressive and often underdiagnosed condition that disproportionately affects older adults and certain racial and ethnic groups. In ATTR-CM, a misshapen form of the protein transthyretin builds up in the heart, preventing the left ventricle from relaxing and filling properly. Over time, this can impair the heart's ability to pump blood, leading to heart failure. This new effort builds on the American Heart Association's ongoing commitment to improving diagnosis, care and outcomes for people with cardiomyopathy. The objectives are to close knowledge gaps, raise awareness and improve access to early diagnosis and emerging treatments for cardiac amyloidosis — no matter where people live — through research, clinical education and public outreach. The nationwide initiative, conducted with financial support from Intellia Therapeutics, aims to elevate understanding of gene editing, advance research and support clinical trial opportunities among populations often excluded from research. 'Too many people remain unaware of or disconnected from lifesaving cardiovascular clinical trials,' said Michelle Kittleson, M.D., PhD, American Heart Association volunteer, professor of medicine at Cedars-Sinai and director of education, heart failure and transplantation at Smidt Heart Institute. 'This effort is designed to help close that gap, ensuring that medical innovation is matched with education, trust and opportunity for all.' The initiative includes a multi-pronged education and research initiative to assess current ATTR-CM awareness and develop new insights related to gene editing and cardiovascular disease. These observations will inform new educational materials, a series of national webinars and outreach strategies designed for patients, families and clinicians. The first webinar, Understanding Amyloidosis & Emerging Therapeutic Frontiers, will be held June 18, and will feature experts in cardiology and gene therapy. Additional components of the initiative focus on enhancing patient identification for access to emerging therapies and clinical research opportunities. This includes activating a referral network of non-trial sites, supporting multidisciplinary provider education and developing tools that leverage clinical data to help identify potentially eligible participants. Cytokinetics Presents Additional Data Related to Aficamten at the European Society of Cardiology Heart Failure 2025 Congress
Two New Analyses from SEQUOIA-HCM on Effect of Aficamten Between Patientswith Mild and Moderate-to-Severe Symptoms, and Across Geographic Regions HEOR Analyses of Real-World Data Reveal Disparities in Outcomes In Females and Older Patients with Non-Obstructive HCM SOUTH SAN FRANCISCO, Calif., May 18, 2025 (GLOBE NEWSWIRE) -- Cytokinetics, Incorporated (Nasdaq: CYTK) today announced that additional data arising from two analyses from SEQUOIA-HCM, (Safety, Efficacy, and Quantitative Understanding of Obstruction Impact of ficamten in HCM), the pivotal Phase 3 clinical trial of aficamten in patients with symptomatic obstructive hypertrophic cardiomyopathy (HCM), and results from a real-world analysis related to non-obstructive HCM were presented at the European Society of Cardiology Heart Failure 2025 Congress. The results from an analysis of the efficacy of aficamten in patients with obstructive HCM and mild symptoms in SEQUOIA-HCM were also simultaneously published in The European Heart Journal.1 'These analyses from SEQUOIA-HCM demonstrate that the effect of aficamten on exercise capacity, symptoms, hemodynamics and cardiac biomarkers is consistent in patients with obstructive HCM regardless of baseline symptom severity and geographic region,' said Stephen Heitner, M.D., Vice President, Head of Clinical Research. 'These findings are informative as we continue to explore the potential application of aficamten across a range of patient phenotypes in obstructive HCM.' Effect of in Patients with Mild Symptoms and Moderate-to-Severe Symptoms Data from an additional analysis from SEQUOIA-HCM related to the effect of aficamten in patients with mild symptoms were presented in a Late Breaking Clinical Trial session and simultaneously published in The European Heart Journal. Patients in SEQUOIA-HCM (n=282) were divided into two groups according to baseline symptom severity: mild symptoms, defined as New York Heart Association (NYHA) Functional Class II and Kansas City Cardiomyopathy Questionnaire Clinical Summary Score (KCCQ-CSS) ≥80 (n=118; 62 randomized to aficamten), and moderate-to-severe symptoms defined as NYHA Functional Class II/III/IV and KCCQ-CSS <80 (n=150; 71 randomized to aficamten). The effect of aficamten on the primary endpoint of change from baseline to Week 24 in peak oxygen uptake (pVO2) was similar between symptom groups (1.6 and 1.8 mL/kg/min in mild and moderate-to-severe symptom groups respectively; interaction p=0.8). While both groups experienced improvements in KCCQ-CSS, the magnitude of improvement was greater in the moderate-to-severe symptom group compared to the mild symptom group (interaction p=0.02) as expected given the lower baseline KCCQ-CSS score. At the end of the treatment period, 54% of patients with mild symptoms and 36% of patients with moderate-to-severe symptoms were asymptomatic. Additionally, more than half of patients in both groups had an improvement of at least one NYHA Functional Class (interaction p=0.6). Improvements in resting and Valsalva left ventricular outflow tract (LVOT) gradients and NT-proBNP also did not differ significantly between the two groups (all interaction p≥0.3). The safety and tolerability profile of aficamten was similar to placebo in both subgroups. These data indicate that, in SEQUOIA-HCM, the effect of aficamten was observed independent of baseline symptom burden in patients with obstructive HCM. Effect of in Patients with Obstructive HCM Consistent Across Geographic Regions Data from an additional analysis from SEQUOIA-HCM evaluating geographical differences in the effect of aficamten were presented in a Late Breaking Clinical Trial session. Participants from SEQUOIA-HCM (n=282) were classified into three geographic regions: Europe, including Israel (n=142; 50%), North America (n=94; 33%) and China (n=46; 16%). At baseline, patients in Europe and North America were on average older, with a higher BMI, lower LVOT gradients, lower KCCQ-CSS and were more likely to have comorbidities than those in China. A greater proportion of patients in North America were NYHA Functional Class III/IV compared to Europe and China. Peak VO2, Valsalva LVOT gradient, NT-proBNP and hs-cardiac troponin I were similar across geographical regions. The distribution of doses of aficamten was similar across regions. The effect of aficamten on the primary endpoint of change in pVO2 and all secondary endpoints was consistent, with no significant differences across regions (all interaction p>0.15). The incidence of serious adverse events was similar in the aficamten and placebo groups across regions, and occurrences of left ventricular ejection fraction (LVEF) <50% were infrequent. These results demonstrate that in SEQUOIA-HCM, while there were regional differences in patient baseline characteristics, the dosing, safety profile and effect of aficamten was consistent across the geographic regions studied. Analyses of Real-World Data Reveal Association Between Age, Sex and Cardiovascular Outcomes in Patients with Non-Obstructive HCM Data presented from a health economics and outcomes research (HEOR) analysis evaluated the association between age, sex and cardiovascular outcomes in patients with non-obstructive HCM. This retrospective cohort study included adult patients diagnosed with non-obstructive HCM from January 1, 2013 to December 31, 2021 using real-world data from Optum Market Clarity database. Of the 9,842 patients included, 46.2% were female, 53.8% were male and the age distribution was as follows: 24.2% were ages 55 to 64 years, 22.1% were ages 75 or older, 22.1% were ages 65 to 74, 19.9% were ages 40 to 54 and 11.7% were ages 18 to 39. Female patients had increased rates of stroke (risk ratio [RR] 1.32), heart failure (RR 1.22), cardiovascular hospitalization (RR 1.23) and cardiovascular rehospitalization (RR 1.15) compared to male patients (all p<0.01). However, female patients were less likely to have atrial fibrillation (RR 0.83) and ventricular tachycardia (RR 0.69) (p<0.001). Compared to patients aged 75 years or older, younger patients were less likely to have atrial fibrillation, stroke, heart failure, cardiovascular hospitalization and cardiovascular rehospitalization (all p<0.001). All-cause mortality was significantly greater in female patients compared to male patients (p=0.002). Patients aged 75 years or older had the highest all-cause mortality (16.6%; p<0.001), followed by patients aged 65 to 74 (8.3%), 55 to 64 (3.5%), 40 to 54 (3.1%) and 18 to 39 years (1.4%). These findings highlight disparities in morbidity and survival among females and older patients with non-obstructive HCM, emphasizing the potential role for novel treatments to help reduce the clinical burden. About Aficamten is an investigational selective, small molecule cardiac myosin inhibitor discovered following an extensive chemical optimization program that was conducted with careful attention to therapeutic index and pharmacokinetic properties.2 Aficamten was designed to reduce the number of active actin-myosin cross bridges during each cardiac cycle and consequently suppress the myocardial hypercontractility that is associated with HCM. In preclinical models, aficamten reduced myocardial contractility by binding directly to cardiac myosin at a distinct and selective allosteric binding site, thereby preventing myosin from entering a force producing state. About Hypertrophic Cardiomyopathy Hypertrophic cardiomyopathy (HCM) is a disease in which the heart muscle (myocardium) becomes abnormally thick (hypertrophied). The thickening of cardiac muscle leads to the inside of the left ventricle becoming smaller and stiffer, and thus the ventricle becomes less able to relax and fill with blood. This ultimately limits the heart's pumping function, resulting in reduced exercise capacity and symptoms including chest pain, dizziness, shortness of breath, or fainting during physical activity. AMVUTTRA® (vutrisiran) Significantly Reduces Mortality and a Range of Important Cardiovascular Events in Patients with ATTR Amyloidosis with Cardiomyopathy: Additional Data from HELIOS-B
CAMBRIDGE, Mass.--(BUSINESS WIRE)--May 17, 2025-- Alnylam Pharmaceuticals, Inc. (Nasdaq: ALNY), the leading RNAi therapeutics company, today presented the most contemporary analysis of the HELIOS-B Phase 3 study of vutrisiran for the treatment of ATTR amyloidosis with cardiomyopathy (ATTR-CM) as a late-breaking abstract at the Heart Failure 2025 Congress, a scientific congress of the European Society of Cardiology, taking place May 17-20 in Belgrade, Serbia. The results demonstrate that vutrisiran, which rapidly knocks down transthyretin, reduces key cardiovascular (CV) events such as CV hospitalizations, and heart failure (HF) hospitalizations. Additionally, in the analysis, urgent HF visits were reduced by 46% (95% CI: 0.30, 0.98; p = 0.041) in the overall population during the double-blind period, compared to placebo. These CV events often precede all-cause mortality (ACM) and are key indicators of disease progression. Importantly, results from the November 2024 data cut, including further follow up through up to 42 months, reinforce the primary HELIOS-B analysis showing vutrisiran's effect on ACM, and further demonstrate that vutrisiran reduces CV mortality. Through 42 months, the risk of ACM was reduced by 36% (95% CI: 0.46, 0.88; p = 0.007) and the risk of CV mortality was reduced by 33% (95% CI: 0.47, 0.96; p = 0.038) in the overall population, compared to placebo. For both the primary analysis and the current analysis, vital status through 42 months was ascertained for over 99% of all randomized patients from the HELIOS-B study, underscoring the robustness of the results. The study was conducted in a contemporary patient population with patients receiving robust background therapy, inclusive of treatment with a TTR stabilizer and SGLT2 inhibitors. The analysis of the HELIOS-B Phase 3 study, including mortality data through up to 42 months, was simultaneously published in JACC. 'From the primary analysis of HELIOS-B, we know that AMVUTTRA profoundly impacts all-cause mortality, while preserving patients' functional capacity and quality of life,' said Pushkal Garg, M.D., Chief Medical Officer of Alnylam. The results from the analysis underscore the rapid and sustained benefits of vutrisiran in treating ATTR-CM across key endpoints: In a separate presentation on Tuesday, May 20, Alnylam will share findings from a subgroup analysis of HELIOS-B evaluating the impact of vutrisiran on ACM and recurrent CV events among patients identified by investigators as having experienced disease progression while being treated with tafamidis. Also at the Heart Failure 2025 Congress, Alnylam will present the study design and rationale for TRITON-CM, a Phase 3, randomized, double-blind, study of nucresiran in patients with ATTR-CM. Nucresiran is an investigational next-generation RNAi therapeutic targeting TTR that has been shown to deliver rapid knockdown of TTR greater than 95% with twice-annual dosing in a Phase 1 study. TRITON-CM is an event-driven CV outcomes trial with a primary endpoint of composite ACM and CV events. The study is on track to initiate in the first half of 2025 and will enroll approximately 1,200 patients with wild-type or variant TTR and confirmed cardiomyopathy, including those receiving background stabilizer therapy. Additional details of the study's secondary endpoints and key inclusion and exclusion criteria will be shared on Monday, May 19. AMVUTTRA ® (vutrisiran) was approved by the U.S. Food and Drug Administration (FDA) and the Brazilian Health Regulatory Agency (ANVISA) for treatment of the cardiomyopathy of wild-type or hereditary transthyretin-mediated amyloidosis in adults. The Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) has adopted a positive opinion recommending the approval of vutrisiran for the same indication. A formal regulatory decision by the European Commission of the EMA is expected by the third quarter of 2025. Vutrisiran is currently under review for the treatment of ATTR-CM by the Japanese Pharmaceuticals and Medical Devices Agency (PMDA). Alnylam remains on track to proceed with additional global regulatory submissions for vutrisiran in 2025 and beyond. (Nasdaq: ALNY), the leading RNAi therapeutics company, today presented the most contemporary analysis of the HELIOS-B Phase 3 study of vutrisiran for the treatment of ATTR amyloidosis with cardiomyopathy (ATTR-CM) as a late-breaking abstract at the Heart Failure 2025 Congress, a scientific congress of the European Society of Cardiology, taking place May 17-20 in Belgrade, Serbia. The results demonstrate that vutrisiran, which rapidly knocks down transthyretin, reduces key cardiovascular (CV) events such as CV hospitalizations, and heart failure (HF) hospitalizations. Additionally, in the analysis, urgent HF visits were reduced by 46% (95% CI: 0.30, 0.98; p = 0.041) in the overall population during the double-blind period, compared to placebo. These CV events often precede all-cause mortality (ACM) and are key indicators of disease progression. Importantly, results from the November 2024 data cut, including further follow up through up to 42 months, reinforce the primary HELIOS-B analysis showing vutrisiran's effect on ACM, and further demonstrate that vutrisiran reduces CV mortality. Through 42 months, the risk of ACM was reduced by 36% (95% CI: 0.46, 0.88; p = 0.007) and the risk of CV mortality was reduced by 33% (95% CI: 0.47, 0.96; p = 0.038) in the overall population, compared to placebo. For both the primary analysis and the current analysis, vital status through 42 months was ascertained for over 99% of all randomized patients from the HELIOS-B study, underscoring the robustness of the results. The study was conducted in a contemporary patient population with patients receiving robust background therapy, inclusive of treatment with a TTR stabilizer and SGLT2 inhibitors. The analysis of the HELIOS-B Phase 3 study, including mortality data through up to 42 months, was simultaneously published in JACC. Also at the Heart Failure 2025 Congress, Alnylam will present the study design and rationale for TRITON-CM, a Phase 3, randomized, double-blind, study of nucresiran in patients with ATTR-CM. Nucresiran is an investigational next-generation RNAi therapeutic targeting TTR that has been shown to deliver rapid knockdown of TTR greater than 95% with twice-annual dosing in a Phase 1 study. TRITON-CM is an event-driven CV outcomes trial with a primary endpoint of composite ACM and CV events. The study is on track to initiate in the first half of 2025 and will enroll approximately 1,200 patients with wild-type or variant TTR and confirmed cardiomyopathy, including those receiving background stabilizer therapy.
