Inside the C.D.C., a Final ‘Love Letter' Before Mass Layoffs

When every email inbox in the division pinged with a new message at 5:07 p.m. on Friday, the staff collectively held their breath.
But it wasn't the dismissal notification that these employees of the Centers for Disease Control and Prevention had been waiting for. Quite the opposite: It was, the subject line said, a 'love letter.'
The author was Sarah Roby, a health communications specialist in the C.D.C.'s injury center, and the note would be read and reread throughout the weekend by her colleagues as they awaited their fate. The recipients worked in the injury center's division of violence prevention, which focused on driving down rates of child abuse, sexual violence and gun deaths by collecting data, funding research and rolling out state and local programs that used evidence-based strategies.
Injuries are the leading cause of death for Americans under 45 years old, and interventions like bicycle helmets, car seats and crisis hotlines are considered wildly cost-effective, since the horrors they prevent down the road are so expensive to society.
But much as with vaccines, experts said, it can be a challenge to convince the public that injury prevention is worth investing in, because it is hard to help people recognize an event that never occurred.
Ms. Roby told her colleagues about what had initiated her career: a shift at a shelter in Henry County, Ga., when a woman arrived on a Greyhound bus from Oklahoma after being thrown off the balcony of her apartment.
As she drove home that night, years ago, Ms. Roby wrote, she wondered about a better way to respond to domestic violence — not just to serve those who experience it, but also to prevent it in the first place.
Recipients of Ms. Roby's email said they wept at its resonance, holding out their iPhones so that spouses could see the words or reading them aloud to friends over dinner. One showed it to a 16-year-old nephew to help him understand 'what this country is losing.'
They said Ms Roby's words had captured the dedication of public health workers at a pivotal moment in history, when the rise of anxiety, depression, social media and fentanyl have collided, driving teenage injury deaths so high that they reversed decades of a downward trend.
The injury center maintained a widely used database that tracked meticulous details of injury trends in various age groups and demographics so that effective interventions could be designed. More than 80 percent of the injury center's budget went directly to states and local governments and nonprofit organizations so they could collect the data and roll out well-designed programs.
'Talk about government efficiency: It's much better to figure out the best way to do something and replicate it 50 times than to have all 50 states trying separately,' said Dr. Mark Rosenberg, who helped found the injury center and served as its first permanent director. 'That careful coordination was the injury center's magic.'
In the end, Ms. Roby's letter made its way to the injury center's statisticians and data scientists, economists and epidemiologists. It reached grant coordinators, program officers and some of the world's leading experts on car crashes, falls, drownings, poisonings and traumatic brain injuries in sports.
By sunrise on Tuesday morning, most recipients of the letter had been terminated.

(NEXSTAR) – You've likely heard public health officials' repeated pleas to check yourself, your kids and your pets for tick bites, which can spread serious diseases. While Lyme disease gets the most attention – and affects the most people – there are other tick-borne bacteria and diseases that have also been found spreading in the U.S. Just last month, Connecticut researchers said they identified the first case of a longhorned tick infected with a pathogen called ehrlichia chaffeensis. Infections can give people fever chills, headaches, muscle aches and stomach issues. 'We also worry about a parasite called Babesia, which causes a disease called Babesiosis,' said Nicole Baumgarth, a professor of molecular microbiology and immunology at the Johns Hopkins Bloomberg School of Public Health, in a recent briefing on tick-borne illnesses. The same ticks that carry the bacteria that cause Lyme disease can carry this parasite, which then infects people's red blood cells, causing anemia and other 'very serious consequences,' Baumgarth said. Some people infected by the parasite may feel flu-like symptoms, explains the Centers for Disease Control and Prevention, while others may have no symptoms at all. That's a problem if people live with the parasite and then donate their infected blood, which can cause harm to the people who receive it as a transfusion. Baumgarth also warned of viruses that can be spread by ticks, such as tick-borne encephalitis. It's an infection that can cause inflammation of the brain and spinal cord. It's a bigger problem in Europe than in the U.S., she said, but here we have Heartland virus and Powassan virus to contend with. 'Both viruses are still very rare, but we have seen our first case of Heartland virus in Maryland last year, and the fact that we are now seeing these virus infections in areas that we haven't seen them before, it probably indicates the tip of the iceberg,' said Baumgarth. Because these viruses are rarer, and their symptoms can be non-specific, doctors don't usually think to test for them, Baumgarth explained. Many cases of Heartland and Powassan virus could be left uncounted as a result. 'So it would be rare that we diagnose it because we are not looking hard enough for it right now,' she said. Both viruses can present with generic first symptoms like fever, headache and vomiting, but can cause serious illness if left unchecked. Powassan virus could lead to seizures, loss of coordination and confusion in severe cases where the brain and spinal cord are impacted. Yet another concern is the tick that can cause people to become allergic to red meat if they're bitten. It's called alpha-gal syndrome and is caused by bites from the Lone Star tick, first identified in Texas. However, this type of tick is 'very aggressively' moving north and becoming a problem in other regions of the country, according to Baumgarth. Some of these rare viruses don't have a treatment or cure, so prevention is what's most important, the CDC says. You should avoid ticks by wearing long sleeves and pants in bushy and wooded areas, using bug repellent, and doing a thorough tick check after spending time outside.

Jonathan Martin, my son, overdosed after he was turned away from HealthRight360's detox facility on Aug. 27, 2024. Jonathan was desperately ill and seeking help. He died alone early the next morning. Jonathan's death was the subject of a Chronicle story ('He overdosed waiting for help. Why did S.F.'s largest drug treatment center provider turn him away?' San Francisco, Feb. 27). When the story was published, I sent a letter to Mayor Daniel Lurie, District Attorney Brooke Jenkins, the chiefs of the Department of Public Health and relevant agencies, HealthRight360 CEO Vitka Eisen and every member of the Board of Supervisors, hoping to find out why my son was turned away. I wanted to understand what prevented him from receiving the services he so urgently needed. I have still not received a single response, explanation or even a note of condolence from any of these officials. If the people who oversee drug treatment in the city will not answer a grieving mother, who will give my family answers? Citizens of San Francisco: You voted these people into office.
