
NHS should ‘tear up the outpatient model'
Around 130,000,000 outpatient appointments – procedures or appointments which take place in hospitals or clinics but do not require an overnight stay – take place in the NHS in England every year.
The majority of people on the NHS waiting list are waiting for outpatient appointments.
An estimated 7.42 million treatments were waiting to be carried out at the end of March, relating to 6.25 million patients, according to official figures.
The latest performance figures for the NHS in England are due to be published on Thursday.
Sir Jim Mackey, chief executive of NHS England, told the NHS ConfedExpo conference that the service should take 'big leaps' such as 'tearing up the outpatient model' and 'tearing up the frustrations about technology' and 'tearing up the multiple touch points for patients when it should be one'.
And Dr Penny Dash, chairwoman of NHS England, told delegates: 'There are seven and a half million people on the waiting list, half of them waiting more than 18 weeks, and the majority of those people waiting for an outpatient appointment.
'Surely we can fix outpatients to help bring down the waiting list.'
Matthew Taylor, chief executive of the NHS Confederation, said: 'Outpatient services should and could be transformed with most checks and follow-ups done in neighbourhoods or remotely.'
Meanwhile, Sir Jim said the NHS has 'lost its way' on quality of care, as he told delegates that the forthcoming 10-year plan for health, which is to be published in July, will 'focus on quality'.
'Neighbourhood care – I think that part of the plan is still a really big standout for me,' he said.
'The focus on quality, I think it's really welcome – we have lost our way a little bit on quality coming out of Covid, and again, looking at quality in the round and not just safety.'
But he said that the NHS would move on from the 'over-prescription centrally' and the 'labyrinthine approval processes' to give local health leaders more freedom.
Dr Dash added: 'We do need to have a comprehensive strategy for quality.
'We've had a big focus on safety, rightly, over the last 10 years, but that has been a bit at the expense of patient experience and also effectiveness. We need to regroup, set out that strategy.'

A Surrey-based start up has secured investment from the Alzheimer's Society Accelerator programme for their innovations in dementia Health and Hydro Care will together develop their technologies to assist in helping people better understand and manage changes in their symptoms. Caroline Cake, co-founder of Neu Health, said: "We've seen how transformative digital tools have been with other conditions, diabetes for example, and we wanted to see if we could do the same for dementia."The pairing will focus on technology that monitors hydration levels in dementia patients. Dr Tess Morris-Paterson, founder of Hydro Care, said: "We know that carers often find it difficult to monitor whether their loved one is drinking enough, a challenge that commonly leads to issues like urinary tract infections that lead to hospital admissions."Neu Health has developed a device that monitors changes in symptoms, while Hydrocare has designed an app to keep an eye on hydration. Covid Nimbus variant with painful symptom UK hospital cases rise
New figures show the viral infection is on the rise after hitting UK shores - with NHS advising 'stay home' with symptoms Covid cases being admitted to hospital have shown a big increase as a new variant with a 'painful' symptom hit the UK. Doctors have said the new strain of Covid - codenamed Nimbus has one specific symptom. Cases of the variant have been found in the UK as it starts to dominate across the world, having been detected in January. The increase in cases has seen the World Health Organisation say it is monitoring the new variety, also known as NB.1.8.1, closely. New figures updated by the UK Health Security Agency show that the number of hospitalistations in the UK has risen almost 10 per cent in the last week for which figures are available. The statistics show a 947 patients admitted with Covid in the week to May 31 - an increase of 9.4 per cent on the preceding week. Number of detected cases, and bearing in mind there is no widespread testing going on, is 1,211 in the week to June 4, a rise of 6.9 per cent. Number of deaths was 63, in the week to May 30, down 12.5 per cent on the previous week. The NHS advice is currently that if a person believes they have Covid they should stay at home. Nimbus is now the dominant strain in China and makes up a third of cases in the USA, while it is being blamed for a spike in cases in Australia. The mutated Nimbus strain is able to spread more easily and more infectious than previous variants. Vaccines and previous infection have been found to be about half as effective at blocking Nimbus as some other strains. A World Health Organisation spokesman said: 'Currently approved COVID-19 vaccines are expected to remain effective against this variant against symptomatic and severe disease. Despite a concurrent increase in cases and hospitalizations in some countries where NB.1.8.1 is widespread, current data do not indicate that this variant leads to more severe illness than other variants in circulation.' Patients have reported this variant giving them 'razor blade throat', a very sore throat which has left some people with a hoarse voice. Director & Superintendent Pharmacist, Prescription Doctor, Mohamed Imran Lakhi, said: 'People commonly report a severe sore throat, persistent fatigue, mild cough, fever, muscle aches, and nasal congestion. Some also experience digestive issues like nausea or diarrhoea, which isn't always typical with Covid variants.' According to Dr Naveed Asif, GP at The London General Practice, the NB.1.8.1 variant has a distinctive symptom, a severe sore throat known as a 'razor blade sensation'. This is a sharp, stabbing pain when you swallow, often at the back of the throat. It can be treated in the same way with rest, fluids and painkillers. If your symptoms persist or get worse, or if you are worried about you or someone else you can speak to a GP or call 111. If someone is in serious distress, for example struggling to breathe, you should call 999. The UKHSA said: 'You might have seen news or social media conversations about the COVID-19 variant NB.1.8.1, which you might know as the 'Nimbus variant'. NB.1.8.1 has been detected in small numbers in the UK to date, but international data suggests that it is growing as a proportion of al Covid-19 cases. Based on the available information so far however, there is no evidence to suggest that this variant causes more severe disease than previous variants, of that the vaccines in current use will be less effective against it.' The UKHSA has confirmed the emergence of the 'Nimbus' variant in the UK, highlighting its relatively rare status but noting its faster growth rate compared to other Covid-19 cases globally. Regions hit by NB.1.8. 1 have seen rises in cases and hospital admissions, however, health professionals have yet to ascertain whether it leads to more severe health complications than previous strains. UKHSA suggests that if you must leave your home whilst having symptoms of a respiratory infection and either a fever or feeling poorly, you're advised to "avoid close contact with anyone who you know is at higher risk of becoming seriously unwell, especially those whose immune system means that they are at higher risk of serious illness, despite vaccination." UKHSA guidance on minimising the risk of transmitting infection to others: Wear a well-fitting face covering made with multiple layers or a surgical face mask Avoid crowded places such as public transport, large social gatherings, or anywhere that is enclosed or poorly ventilated Taking any exercise outdoors in places where you will not have close contact with other people Covering your mouth and nose when you cough or sneeze; wash your hands frequently with soap and water for 20 seconds or use hand sanitiser after coughing, sneezing and blowing your nose and before you eat or handle food; avoid touching your face. Race row as top UK health boss says mum got 'Black service, not an NHS service'

Lord Victor Adebowale believes his mum faced NHS discrimination and worse care because of the colour of her skin. Grace Adebowale, who worked as an NHS nurse for 45 years, died in January after a traumatic visit to A&E. Lord Adebowale, who is the chairman of the NHS Confederation, the membership body for NHS organisations, said his mum lived in pain for many years and was only diagnosed with suspected cancer after a biopsy was taken post mortem. During an emotional interview, he told the Mirror: 'She lived to 92 and you think 'she had a good innings' but for a lot of those years she lived in some discomfort. The reason I'm mentioning it is that it's still the case that if you look like me, or shades of me, you're more likely to discover you've got cancer in A&E. That for me is an example of two different services. 'You only have to look at the stats. Across all the major disease categories Black people have a worse experience and worse outcomes. My mother is a fairly solid example of a systemic problem. And it's important that people like me are not seen as above it all, because I'm not, I'm in it… I'm sick of it and I'm sick of it not changing.' Grace Adebowale came to the UK from Nigeria as a young nurse before dedicating her career to the NHS working in maternity, mental health and acute hospital services. She and her husband settled in Wakefield, Yorkshire, where Victor grew up. The 62-year-old Life Peer is one of the most influential people in the NHS and its most senior Black leader as well as a government adviser. Lord Adebowale revealed his experience when delivering the opening address at the NHS Confed Expo conference in Manchester this week, telling thousands of medics and health leaders he had 'experienced the culture of the NHS for myself'. He told delegates: 'It was not the dignified death that we would have wanted for her. It wasn't the death she deserved. So it makes me clear about the need to address the inequity. I think she got a black service, not an NHS service.' NHS data shows Black women are more than twice as likely to die in childbirth. People from African and Caribbean backgrounds are more likely to have mental health issues and less likely to receive treatment. Becoming tearful when speaking afterwards, Lord Adebowale said: 'She presented in A&E in a very poor condition. It was a very, very busy day at the hospital and there wasn't any space for anyone. And there had to be a negotiation to get her a space, which you shouldn't have to do.' He added: 'It made me angry because how can you live that long with something and not be [diagnosed]. It looks like she had lung cancer. She never smoked and lived a very healthy life. I just think there are too many situations where people who look like me, and different shades of me, don't get the service.' Lord Adebowale said that he did not want to blame anyone, but he said that he wanted to highlight a "systematic problem", adding: 'If you're in A&E and it is under stress then it's really difficult. It's not because people are cruel or nasty or don't want to do a good job. The first thing that goes is care. People deal with what's in front of them in the best way they can.' He added: "It is not acceptable that someone who looks like me, on average, waits 20 minutes longer in A&E than white patients. To achieve an inclusive, equitable NHS we need an inclusive equitable culture from top to bottom." Lord Adebowale said he was reluctantly speaking out because since becoming a public figure he has had 'racists sending me emails, trying to find out where I live'. He said: "And my mum, I think, God bless her, I think she would have wanted me to say it." He added: 'I know people have chronic diseases, and they don't know they have them, then they die from them. I know it's more likely to happen if you're Black and more likely to happen if you're poor. If you grow up in Dagenham your healthy life expectancy is 55 and if you live in Richmond Upon Thames it's over 78. It's all part of a systemic problem.' Lord Adebowale also highlighted the example pulse oximeters which estimate someone's oxygen levels and were vital in knowing when Covid-19 patients needed emergency care. It was later discovered that they over-estimated the oxygen levels of Black people. Lord Adebowale was one of the first individuals to become a People's Peer in 2001 for his work helping the homeless and people with addiction and mental health problems as chief executive of charity Centrepoint and then care organisation Turning Point. He has advised the Government on issues including mental health, learning disability and the role of the voluntary sector. The Peer established the NHS Race and Health Observatory in 2021 to try and tackle inequalities experienced by black and minority ethnic patients in healthcare.
