
'Companions' ease pain of China's bustling, bamboozling hospitals
Yawning funding gaps and patchy medical coverage have long funnelled many Chinese people towards better resourced city hospitals for much-needed care.
Sprawling, overcrowded and noisy, the facilities can be exhausting for patients and their families, especially the elderly.
The problem has fuelled the rise of patient companions, or "peizhenshi", a lucrative and unofficial service in the country's growing gig economy.
Tian, 83, said most Beijing hospitals were "overwhelmingly confusing".
"We have to go up and down all the floors, wait for elevators, wait in lines... it's really troublesome," he told AFP.
Elsewhere at the People's Liberation Army General Hospital in the Chinese capital, patients faced long queues, myriad check-ins and a whirl of digital payment codes.
Hospital aides wearing bright red sashes rattled off directions into headsets as hundreds of patients filed through the colossal lobby.
Armed with a sheaf of papers at a traditional Chinese medicine ward, Meng breezed through check-in before joining Tian and wife Gao Yingmin in a consultation room.
Leaving Gao to rest in a waiting area, Meng then brought Tian to a payment counter before explaining to the couple how to pick up prescribed medications.
For a four-hour service, patient companions like Meng charge around 300 yuan ($40).
It is worth every penny for Gao, 78, who is undergoing treatment for complications from throat surgery.
The helpers are "convenient, practical and (give us) peace of mind", she said, straining against a breathing tube.
"We no longer have to worry... they do all the work for us."
'Real need'
Hundreds of advertisements for patient companions have sprung up on Chinese social media in recent years.
Authorities appear to allow the companions in hospitals because they are broadly in line with the government's promotion of health services for seniors.
Meng, 39, had no medical background before enrolling in a weeklong training programme run by Chengyi Health, an online platform that connects patients and companions.
Founder Li Gang, a former anaesthesiologist, said "there's a big knowledge gap when it comes to medical care".
Large Chinese hospitals can have over 50 clinical departments, each with numerous sub-specialities.
That means many people "don't know how to go to the doctor", Li said.
While some young people -- such as expectant mothers -- hire companions, some two-thirds of Chengyi's clients are aged 60 or older.
Trainee Tao Yuan, 24, said he left his job at an internet company to pursue a vocation "more valuable than money".
A generation born under China's now-abolished one-child policy are approaching middle age and caring for their elderly parents alone.
Increasing work and family pressure had left them with a "real need" for help, Tao said.
Ageing nation
China's healthcare system has long struggled to tackle deep-seated regional funding gaps and inconsistent access to equipment and medical staff.
Limited treatment options, especially in rural areas, push many patients into municipal hospitals for comparatively minor ailments.
"It's a perennial structure problem," said Wang Feng, an expert on Chinese demographics at the University of California, Irvine.
Working adults have no time to take elderly parents to hospital, while technology cannot yet replace human caregivers, he said.
China "will have a larger... demand for personal assistance" as the elderly account for an ever bigger proportion of the population, Wang said.
Authorities are betting big on the "silver economy" -- products and services for older people, which totalled seven trillion yuan ($970 billion) last year, according to the nonprofit China Association of Social Welfare and Senior Service.
The figures are a bright spot in an economy struggling to maintain strong growth and robust youth employment.
Xiao Shu, who asked to be identified by a nickname for privacy, told AFP he made around 10,000 yuan ($1,400) per month –- a tidy wage in China's competitive capital.
But the former dentistry worker said there were limits to the service.
The 36-year-old once refused to take a client's nearly 90-year-old father to a post-surgery check-up.
"If something happened to him, who would be responsible for it?" he said.

Gaza rescuers say 62 killed by Israeli forces
The reported killing of people seeking aid marks the latest in a string of deadly incidents near aid sites in Gaza, where a US- and Israeli-backed foundation has largely replaced established humanitarian organisations. Civil defence spokesman Mahmud Bassal told AFP that 62 Palestinians had been killed Friday by Israeli strikes or fire across the Palestinian territory. When asked by AFP for comment, the Israeli military said it was looking into the incidents, and denied its troops fired in one of the locations in central Gaza where rescuers said one aid seeker was killed. Bassal told AFP that six people were killed in southern Gaza near one of the distribution sites operated by the Gaza Humanitarian Foundation (GHF), and one more in a separate incident in the centre of the territory, where the army denied shooting "at all". Another three people were killed by a strike while waiting for aid southwest of Gaza City, Bassal said. The health ministry in the Hamas-run territory says that since late May, more than 500 people have been killed near aid centres while seeking scarce supplies. GHF has denied that fatal shootings have occurred in the immediate vicinity of its aid points. 'Slaughter' Medical charity Doctors Without Borders (MSF) on Friday slammed the GHF relief effort, calling it "slaughter masquerading as humanitarian aid". It noted that in the week of June 8, shortly after GHF opened a distribution site in central Gaza's Netzarim corridor, the MSF field hospital in nearby Deir el-Balah saw a 190 percent increase in bullet wound cases compared to the previous week. Aitor Zabalgogeaskoa, MSF emergency coordinator in Gaza, said in a statement that under the way in which the distribution centres currently operate: "If people arrive early and approach the checkpoints, they get shot." "If they arrive on time, but there is an overflow and they jump over the mounds and the wires, they get shot". "If they arrive late, they shouldn't be there because it is an 'evacuated zone', they get shot," he added. Meanwhile, Bassal said that ten people were killed in five separate Israeli strikes near the southern Gaza city of Khan Yunis, east of which he said "continuous Israeli artillery shelling" was reported Friday. Hamas's armed wing, the Ezzedine Al-Qassam Brigades, said they shelled an Israeli vehicle east of Khan Yunis Friday. The Al-Quds Brigades, the armed wing of Hamas-ally Palestinian Islamic Jihad, said they had attacked a group of Israeli soldiers north of Khan Yunis in coordination with the Al-Qassam Brigades. Bassal added that thirty people were killed in six separate strikes in northern Gaza on Friday, including a fisherman who was targeted "by Israeli warships". Verdict expected in Italy 'forever chemicals' trial
The now-shuttered Miteni factory near the northeastern city of Vicenza is alleged to have polluted one of Europe's largest groundwater basins with PFAS, dubbed "forever chemicals" because they never break down. The prosecution alleges that the plant in Trissino, which produced PFAS from 1968 and was run by three companies until its closure due to bankruptcy in 2018, leaked chemical-laced wastewater into a waterway, polluting a vast area between Vicenza, Verona and Padova. Fifteen managers from Mitsubishi, International Chemical Investors (ICIG) and Miteni are charged with contaminating nearly 200 square kilometres (77 square miles) of drinking water, as well as soil. Prosecutors in the trial, which began in 2021, have called for the managers to be sentenced to a total of 121 years in jail, lawyer Edoardo Bortolotto told AFP Thursday. Over 200 civil plaintiffs have joined the trial, including Greenpeace and local mothers who united after discovering their families had the chemicals in their blood. PFAS have been used since the late 1940s to mass produce the nonstick, waterproof and stain-resistant treatments that coat everything from frying pans to umbrellas, carpets and dental floss. But chronic exposure to even low levels of the chemicals has been linked to liver damage, high cholesterol, reduced immune responses, low birthweights and several kinds of cancer. The contamination was discovered in 2013 after Italy's environment ministry ordered tests of the Po River following a 2006 European project assessing exposure to such chemicals in rivers. Of all the rivers studied, the Po had the highest concentrations of one specific PFAS called PFOA, a known carcinogen. Further investigation identified Miteni as the source. Thai cannabis stores fret as government moves to tighten rules on sales
The kingdom was the first country in Southeast Asia to decriminalise the drug when it removed cannabis from the list of banned narcotics in June 2022. The intention was to allow sales for medical rather than recreational use, but the move led to hundreds of cannabis "dispensaries" springing up around the country, particularly in Bangkok. While the relaxation has proved popular with some tourists, there are concerns that the trade is under-regulated. Health Minister Somsak Thepsuthin signed an order late on Tuesday requiring an on-site doctor to approve sales for medical reasons. The rule would only come into force once it is published in the official Royal Gazette. It is not clear when this would happen. Thanatat Chotiwong, a long-time cannabis activist and store owner, said it was "not fair" to suddenly change the rules on a sector that was now well established. "This is a fully-fledged industry -- not just growers selling flowers. There are lighting suppliers, construction crews, farmers, soil and fertiliser developers, and serious R&D," he told AFP. "Some of us have invested tens of millions of baht in greenhouses and infrastructure. Then suddenly, the government steps in to shut it all down." Thanatat urged the government instead to "implement proper taxation and regulation -- so this revenue can be returned to society in a meaningful way." - 'Too few doctors' - The government has made several previous announcements of plans to restrict cannabis, including legislation moved in February last year, but none has come to fruition. The new rules would mean cannabis could only be sold to customers for medical reasons, under the supervision of licensed professionals such as medical doctors, traditional Thai medicine doctors, folk healers or dentists. "It's going to work like this: customers come in, say what symptoms they have, and the doctor decides how many grams of cannabis is appropriate and which strain to prescribe," Kajkanit Sakdisubha, owner of The Dispensary cannabis shop in Bangkok, told AFP. "The choice is no longer up to the customer -- it's not like going to a restaurant and pick your favorite dish from a menu anymore." And he warned that many of the shops that had sprouted since decriminalisation would not be able to adapt to the changes. "The reality is there are too few doctors available. I believe that many entrepreneurs knew regulations were coming, but no one knew when," he said. While waiting for the rules to come into force, The Dispensary is halting cannabis sales as a precaution, store manager Bukoree Make said. "Customers themselves are unsure whether what they're doing is legal. I've been receiving a lot of calls," Poramat Jaikla, the lead seller or "budtender", told AFP. The cannabis move comes as the government led by Prime Minister Paetongtarn Shinawatra's Pheu Thai party is hanging by a thread after losing its main coalition partner, Bhumjaithai. Though conservative, the Bhumjaithai party has long supported more liberal laws on cannabis. The party quit the coalition this month in a row over a leaked phone call between Paetongtarn and former Cambodian leader Hun Sen.
