Long hours in bed tied to impaired thinking in post-stroke patients
ST. PAUL, Minn., May 28 (UPI) -- The suspected connection between lack of proper sleep and stroke got stronger Wednesday with the publication of a study associating brain tissue damage and impaired thinking with long in-bed time among post-stroke patients.
The study found that people who spent the most time in bed either sleeping or trying to sleep after suffering mild strokes or transient ischemic attacks were more likely to have areas of damage to the white matter and in their brains known as "hyperintensities," which are visible in magnetic resonance imaging or computed tomography scans.
Areas of hyperintensity in white matter are possible indicators of stress or damage to the brain's many small blood vessels, a condition known as cerebral small vessel disease, or SVD. They are associated with cognitive impairment, triple the risk of stroke and double the risk of dementia.
Stroke patients who spent the most time in bed also scored slightly lower in cognition tests of thinking and memory skills, according to researchers in Scotland and Hong Kong in a study published in the online version of the U.S. medical journal Neurology.
In that study, some 420 people from Edinburgh and Hong Kong with an average age of 66 who had a mild stroke or a transient ischemic attack, also known as a "mini stroke," underwent brain scans, filled out sleep questionnaires and took cognitive tests within one to three months after the stroke.
Those who spent a longer time in bed were more likely to have signs of damage to small blood vessels in the brain, such as white matter hyperintensities, and also were more likely to have "slightly lower" scores on the test of thinking and memory skills.
The results provide the latest piece in the mosaic of evidence pointing to poor or disturbed sleep as a risk factor for both initial and recurrent stroke or TIAs.
Stroke is the fifth-leading cause of death and a top culprit in long-term disability in the United States, with nearly 800,000 Americans each year experiencing one. Given the tremendous health burden stroke imposes, identifying new ways to reduce secondary episodes and risk of death in stroke survivors is a major priority for the global health community.
In probing sleep as a possible means of intervention, researchers have generally found that its relationship with stroke is complex -- sleep disturbances are likely both a contributor to, and consequence of, stroke, they have found.
A 2023 review of the relatively limited number of studies available concluded that insomnia symptoms and/or poor sleep quality, as well as long sleep duration, were probably associated with increased risk of stroke. Sleep, "a modifiable behavior," was deemed a promising "secondary prevention target" to reduce the risk of recurrent event and death after a stroke.
The new study strengthens the case that sleep patterns are an important marker for preventing further strokes and TIAs, according to one of the authors.
Dillys Xiaodi Liu, a postdoctoral scholar in the Department of Psychiatry and Behavioral Sciences at the University of California-San Francisco, told UPI in emailed comments the findings yielded some important new insights about stroke patients who spend long periods of time in bed.
"Most of the previous studies have focused on sleep duration, [while] in-bed time is less investigated," she said. "One longitudinal study has reported long in-bed time is associated with higher risk of dementia and cognitive decline in older adults.
"We found it particularly interesting that longer in-bed time was associated with greater white matter hyperintensity burden, a marker of cerebral small vessel disease, or SVD, which causes 'slow' thinking and movement.
"This indicates the associations may be bidirectional -- patients with higher white matter hyperintensity burden may have longer in-bed time), which requires further validation in longitudinal study," she said.
Liu added she'd like to see future research focus on whether the association found between sleep patterns and small vessel disease burden remains in place over time.
"For example, do stroke patients with longer time in bed or longer sleep duration at the time of stroke onset exhibit greater SVD burden after one year or more? It will offer valuable insights into the role of sleep health in the management of SVD," she said, noting that another of the study's co-authors, the prominent neurologist Dr. Joanna Wardlaw of the University of Edinburgh, is currently leading a longitudinal stroke cohort study.
"I hope more interesting findings will emerge from it in the future," Liu said.
More evidence is emerging that links disturbed sleep patterns and stroke, according to Dr. Joyce K. Lee-Iannotti, a professor of neurology at Barrow Neurological Institute in Phoenix and the inaugural director of its Sleep Division.
Lee-Iannotti, who was not connected to the new study, told UPI there is "growing and compelling evidence linking insufficient sleep to increased stroke risk.
"From my perspective as both a sleep medicine and vascular neurology specialist, this relationship is multifactorial. Chronic sleep deprivation promotes systemic inflammation, endothelial dysfunction and autonomic dysregulation -- all of which are known contributors to cerebrovascular disease," she said.
Short sleep duration also is associated with traditional stroke risk factors such as hypertension, diabetes and atrial fibrillation, Lee-Iannotti said, adding, "Clinically, I have also observed that patients with sleep disorders like obstructive sleep apnea -- which fragment sleep and reduce sleep efficiency -- often present with more severe strokes and slower recovery trajectories.
"This underscores that sleep is not a passive state, but a critical pillar of brain and vascular health."
Lee-Iannotti, who serves as the chair of American Academy of Neurology's Sleep Section, said future research should aim to clarify the mechanisms that link sleep quality and architecture with both ischemic and hemorrhagic stroke subtypes.
"There's also a need to integrate wearable technology and home-based diagnostics into large, longitudinal studies that can capture real-world sleep patterns over time," she said.
"Biomarker discovery, including inflammatory and neurovascular markers influenced by sleep, could offer predictive insights."

The federal law enforcement agency released its "2024 Active Shooter Incidents in the United States" report, which shows 24 such reported incidents last year versus 48 in 2023. The active shooter incidents killed a total of 23 and wounded 83 others. The 106 casualties were down 57% from 244 in 2023. The federal law enforcement agency defines an active shooter incident as one or more people actively engaged in killing or trying to kill others in a populated area. Mostly lone male shooters Such incidents require a coordinated response by law enforcement and other first responders to minimize casualties, the FBI says. The FBI says lone shooters accounted for all but one of the active shooter incidents, and two were involved in the lone incident with more than one shooter. Males, 88%, greatly accounted for active shootings, with 22 reported vs. three, 12%, involving females. The shooters were between 14 and 73 years of age, and five incidents involved shooters who had an identified connection with the location or at least one victim. Last year's active shooter events occurred in 19 states and five types of locations. Texas, California and North Carolina had the most Texas led all states with four active shooter incidents, followed by two each in California and North Carolina, according to the report. Common shooting locations were open spaces, businesses, schools, government locations and houses of worship, according to the FBI. Places of business and educational locations each accounted for four active shooting incidents, followed by three in a governmental location and one in a house of worship. June recorded the most active shooter incidents with five, followed by September with four. Navy ordered to rename USNS Harvey Milk in deliberate Pride Month move
Defense Secretary Pete Hegseth has ordered the U.S. Navy to rename a ship honoring late LGBTQ rights hero Harvey Milk — in a move intentionally set to coincide with Pride Month. According to a memo from the Office of the Secretary of the Navy reviewed by officials have already drawn up a plan for the USNS Harvey Milk to be renamed. The ship is a John Lewis-class oiler, part of a series of vessels named after civil rights leaders and activists. While its new name has yet to be announced, Navy Secretary John Phelan and former Fox News host Pete Hegseth are expected to unveil the change on June 13, according to the memo. An unnamed defense official confirmed to the decision to make the announcement in June was deliberate. The move was slammed by former House Speaker Nancy Pelosi as a 'shameful, vindictive erasure of those who fought to break down barriers for all to chase the American Dream.' Milk, who was elected to the San Francisco Board of Supervisors in 1977, made history as California's first openly gay elected official. He was assassinated inside San Francisco City Hall on Nov. 27, 1978 — just months after helping pass a landmark city ordinance banning discrimination in housing and employment based on sexual orientation. Before his short-lived career in public office, the trailblazing LGBTQ+ rights leader also served as a diving officer in the Navy, but resigned with the rank of lieutenant junior grade in 1955 after being questioned about his sexual orientation. In late 2016, the Navy announced it would name a ship after the slain LGBTQ icon. Construction began in late 2019 and, two years later, Navy Veteran Paula Neira christened the Navy's USNS Harvey Milk (T-AO 206) in a ceremony in San Diego Bay attended by state and local leaders. 'Harvey fought for the dignity and worth of every person,' Pelosi, who represents California's 11th congressional district, which includes most of San Francisco, said in a statement Tuesday. 'In San Francisco, we take great pride that our Harvey's name adorns a mighty ship among a new class of Navy vessels — named for the conscience of the Congress, John Lewis — which honors titans in the fight for freedom,' Pelosi said. Canada, US warn of air quality hazards as Canadian fire smoke reaches Europe
Canada's wildfires, which have already forced evacuations of more than 26,000 people, continued their stubborn spread Tuesday, with heavy smoke choking millions of Canadians and Americans and reaching as far away as Europe. Alerts were issued for parts of Canada and the neighboring United States warning of hazardous air quality. A water tanker air base was consumed by flames in Saskatchewan province, oil production has been disrupted in Alberta, and officials warned of worse to come with more communities threatened each day. "We have some challenging days ahead of us," Saskatchewan Premier Scott Moe told a news conference, adding that the number of evacuees could rise quickly. Every summer, Canada grapples with forest fires, but an early start to the wildfire season this year and the scale of the blazes -- over two million hectares (494,000 acres) burned -- is worrying. The provinces of Saskatchewan and Manitoba have been hardest hit. Both declared wildfire emergencies in recent days. "This has been a very difficult time for many Canadians," federal Emergency Management Minister Eleanor Olszewski told reporters in Ottawa. "This wildfire season has started off more quickly, and it's stronger, more intense," she said, adding that the Canadian military has deployed aircraft to evacuate remote towns in Manitoba and was ready to also assist Saskatchewan and Alberta with firefighting. Climate change has increased the impact of extreme weather events in Canada, which is still recovering from the apocalyptic summer of 2023 when 15 million hectares of forests were scorched. As of Tuesday, there were 208 active fires across Canada. Half of them were listed as out of control, according to the Canadian Interagency Forest Fire Centre. Many of the affected populations are Indigenous, and some small communities have burned to the ground. - 'Very intense few weeks' - Heavy smoke from the fires, meanwhile, has engulfed part of the continent, forcing residents of four Canadian provinces and the US states of Michigan, Minnesota, Nebraska and Wisconsin to limit outdoor activities. "Smoke is causing very poor air quality and reduced visibility," Environment Canada said in a statement. Wildfire smoke is comprised of gaseous pollutants such as carbon monoxide, along with water vapor and particle pollution, which can be particularly hazardous to health. Some of the worst smoke was in Alberta where three major oil sands producers -- Canadian Natural Resources, MEG Energy Corp, and Cenovus Energy -- this week evacuated workers and temporarily shut down hundreds of thousands of barrels of production per day. Huge plumes of smoke even reached Europe, the European Union's climate monitoring service said Tuesday. Due to their very high altitude, they do not pose an immediate health risk, according to the Copernicus Atmosphere Monitoring Service (CAMS), but are likely to result in hazy skies and reddish-orange sunsets. Additional plumes are expected to shade both continents in the coming days. "Central regions of Canada have experienced a very intense few weeks in terms of wildfire emissions," noted Mark Parrington, scientific director at CAMS. Canadian authorities have forecast a more intense fire season than usual in central and western Canada, due in particular to severe or extreme drought. "The significant reduction in snowpack in the spring led to early exposure of soil and vegetation, accelerating surface drying," explained University of Ottawa professor Hossein Bonakdari. "This early exposure acted as a silent amplifier, subtly setting the stage for extreme fires long before the first flame ignited," he said. Elsewhere, extensive forest fires have been raging in Russia's Far Eastern Federal District since early April, particularly east of Lake Baikal, generating carbon emissions of around 35 million tons, Copernicus reported. bur-amc/mlm
