NHS bosses took months to meet over GP firm complaints

NHS officials took months to meet doctors following complaints about surgeries run by GP management company, eHarley Street.
The company has been criticised by patients, doctors and the first minister, and is now handing back most of its Welsh practices, which apart from one in Cardiff, are in the Aneurin Bevan health board area.
It follows safety, staffing and supply concerns revealed by BBC Wales at the end of last year.
Aneurin Bevan University Health Board has been asked to comment.

Concerns about surgeries in Blaenau Gwent, Torfaen, Newport and Caerphilly county were raised last August.
But health board officials only met the GPs responsible in late October, according to an internal health board document seen by BBC Wales.
It also shows the health board gave the GPs who also run eHarley Street more public money after concerns were raised.
The document states: "In August 2024, concerns were highlighted to the health board by local staff."
It goes on to add: "The health board has instigated a period of enhanced monitoring for the GP partnership with bi-weekly meetings.
"The focus of these meetings is governance, workforce and finance as well as any specific concerns.
"The first of these meetings was held on 28 October 2024".
Internal Aneurin Bevan University Health Board documents have been seen by BBC Wales after a freedom of information request to the Welsh government.
The documents show the health board blamed social media for the rise in patient concerns.
"There have been several social media posts which have encouraged the public to contact the health board to raise concerns, as a result there has been an increased number of concerns for Brynmawr in particular," it said.
The internal "Briefing for Chair" document outlines some of the concerns raised, including a lack of clinical cover, a failure to pay locum staff, a failure to pay suppliers and the cleanliness of practice premises.
eHarley Street has been approached for comment.
In a previous statement, the partners who run the surgeries said they faced "significant financial constraints" but were "committed to addressing these challenges".
Senedd members have already called for answers to how the Leicestershire-based private company came to operate within the Welsh NHS.

eHarley Street manages nine surgeries in Wales but is handing back five to the Aneurin Bevan health board.
The documents show that when concerns were raised about the practices, "assurances" were given to the health board "by the central operations manager on behalf of the partnership".
Each NHS surgery is contracted to individual GPs who have responsibility for that practice.
The documents show the health board gave extra public money to the doctors responsible for the surgeries - who are also the bosses of eHarley Street - to pay for locum GPs.
"In order to maintain safe levels of clinical cover the health board has agreed financial support for locum reimbursement", the document said.
However, some locum GPs are still waiting to be paid months after working in eHarley Street surgeries - some are taking legal action to recover their wages.
In December, there were demands for an inquiry into eHarley Street after patients, some with terminal illnesses, said they were having difficulty accessing appointments and treatments.
"It frightens me - I don't feel safe," said Katrina Hughes, 69, a terminal cancer patient who struggled to get an appointment or see the same doctor at Brynmawr Medical Practice in Blaenau Gwent.
"There's no continuity of care at all."
She was among around 100 people who attended a public meeting to discuss services at the practice - many said they struggled to get appointments or were waiting for medication and test results.

Consultants and some doctors at a hospital have seen staff parking charges increase by more than 100%, a union says. Price rises accompanied the introduction of automatic number plate recognition (ANPR) at James Cook University Hospital in Middlesbrough. A University Hospitals Tees (UHT) spokesperson confirmed £1.8m would be invested in car park improvements over the coming year. The British Medical Association (BMA) has described the new charges as "shocking and unacceptable". Some members of staff on lower salaries would see a decrease in charges but consultants would pay a tariff for a dedicated car park for them, said the UHT. The BMA has said charges for consultants at the hospital have gone up by 108%, with some doctors on specific shift patterns seeing increases in excess of 125%. The price changes will bring the South Tees hospitals in line with the policy of the North Tees and Hartlepool NHS Trust, which has had varied prices for staff over the last 10 years. Dr Andy Thornley, representing the BMA, said: "Charging staff who are just trying to come to work to care for patients at the hospital to make financial gains for a struggling trust is wrong. "Worse still is the unfair charges that patients and their visitors are being forced to pay, with no choice in order to access the hospital. "This is just the latest in what can only be described as chaotic implementation of new parking measures at the hospital, impacting not only consultants but also resident doctors who are still unsure what the new charges will mean for them. "We want assurances that staff who are members of the parking scheme will not be fined additional charges for simply coming to work to care for patients." A UNISON spokesperson reinforced this message, stating that imposing higher parking charges on NHS staff and patients "shows a shocking lack of understanding" of the financial struggle many are facing. The trust has said it is the first increase in two years and that any surplus funds will be diverted into patient care. A spokesperson said: "We understand the price rise is not welcome news, but we have kept the increase as low as possible. "We would like to reassure all our patients and visitors that the income from car parking is used to maintain our car parks including the costs of repairs and maintenance, lighting, gritting and security, all of which helps us to retain our 'Park Mark' Safer Parking Scheme accreditation. Missouri continues to lag behind most states in children's health, report finds

Missouri ranked in the bottom third of all states for children's health, according to a report released Monday — due in part to a high rate of child and teen deaths. The annual Kids Count Data Book from the Annie E. Casey Foundation, which used data from 2023, evaluated all states on four metrics of child well-being: health, economic well-being, education, and family and community. Missouri ranked near the middle of states for overall child well-being, at 27th out of 50, weighed down by poor performance in health and education. Missouri's rankings in the four categories were: 13th in economic well-being, 33rd in education, 35th in health And 25th in family & community. 'Children's health remains an area of concern,' noted a press release Monday from Family and Community Trust, the Missouri-based nonprofit partner to Kids Count. Only nine states had higher rates of child and teen deaths in 2023 than Missouri, one of the factors considered in the health ranking. (Those were: Mississippi, Louisiana, New Mexico, Alabama, Tennessee, Arkansas, Alaska, Oklahoma and Montana.) Missouri generally ranks among the states with the highest rate of firearm deaths for kids. Firearms became the leading cause of kids' deaths in the United States in 2020, surpassing car accidents. While the national average in 2023 was 29 child and teen deaths per 100,000, that number was 37 deaths per 100,000 in Missouri. The national average rose overall in 2023. The report notes that while covid deaths contributed to the increase, the rise was largely due to rising firearm deaths and drug overdoses, particularly among teens ages 15 to 19. Also bringing Missouri's health ranking down: Missouri's rate of low-birth weight babies increased in 2023 from 2019 and is above the national average. Other factors helped Missouri's score. For one, Missouri has seen major improvements in children's insurance coverage since the state implemented Medicaid expansion in 2021. The rate of uninsured kids fell from 7% in 2019 to 5% in 2023, which is now on par with the national average. Nationally, Missouri saw among the sharpest declines in uninsured people overall from 2019 to 2023 with the expansion of Medicaid. Those gains could be threatened by Congress' budget proposal to reduce Medicaid spending in part by imposing more barriers to care. Teen births in the state have gone down, in line with national trends — though the state's average is still above the national one. The rate of overweight or obese kids has also improved in Missouri and is down to 31% of kids in 2023, on par with the national average. The rate of kids in poverty declined to 14% in the state in 2023, below the 16% national average. Missouri's education ranking slipped in recent years. In 2023, 77% of Missouri eighth graders were not proficient at math, according to the report, which is nearly 10 percentage points worse than 2019 and is worse than the national average. Canadian doctor after devastating Sudan assignment: 'The worst humanitarian crisis in the world'

Sudan is facing an emergency of staggering proportions, one that has displaced over 12 million people and decimated the nation's healthcare system. But unlike other global crises, this one seems to be happening almost entirely in silence. 'This is not just a war between two parties,' says Sana Bég, executive director of Médecins Sans Frontières (MSF) Canada. 'It's a war on the people of Sudan.' What began as a power struggle between the Sudanese Armed Forces (SAF) and the Rapid Support Forces (RSF) in April 2023 has spiralled into what aid workers now describe as the world's largest displacement crisis. According to the World Health Organization, 70 to 80 per cent of the country's healthcare facilities are either nonfunctional or overwhelmed. In parts of Darfur, even the UN is absent, leaving MSF — also known as Doctors Without Borders — as the only international organization delivering aid on the ground. 'Having to choose between one life-saving activity for another are the kinds of decisions our teams are left to make because of this increasingly isolated environment where we're the only ones being relied on to carry out an entire scope of what should be an ecosystem of aid,' Bég says. This is not just a war between two parties. It's a war on the people of Sudan. For Dr. Reza Eshaghian, a Vancouver-based physician who recently returned from Darfur, the devastation is unlike anything he has seen before. 'What's happening in Sudan is the worst humanitarian crisis in the world, full stop,' he says. 'There are bombings, sexual violence, armed robbery. There's a lack of protection and international humanitarian law.' Eshaghian, who began working with MSF in 2014, was stationed in Nyala, a city in South Darfur. He noted that it's not just the violence itself that is killing people: 'So much of the suffering and death is from preventable illness. We saw women dying from eclampsia and postpartum hemorrhage. People are dying of malaria, malnutrition, diarrhea, dehydration, sepsis. These are things we know how to treat, but there's no infrastructure left to do it.' The hospital Eshaghian helped support saw overwhelming gaps in supplies and staffing. Although MSF brings in its own equipment, the logistics are gruelling, and his team saw supply shipments often delayed. There was also never enough. People are dying of malaria, malnutrition, diarrhea, dehydration, sepsis. These are things we know how to treat, but there's no infrastructure left to do it. Despite the danger, MSF has continued to provide help where it can, but the work has come at immense personal cost, especially for local staff. 'Eighty percent of our workforce globally is locally hired,' Bég notes. 'These are people whose homes are war zones. [They] tell us they have a fear of retaliation or repercussions for any actions that could be interpreted as siding with one warring party or the other. Having to live in fear on a regular basis is far different than what some of our teams that have the ability to get on a plane and leave face.' In other words, simply providing medical care can be seen as a political act. But that doesn't make it any more secure. The safety protocols MSF follows — clearly marked medical uniforms, identified facilities — are no longer a guarantee. 'We used to rely on international humanitarian law to protect aid workers and patients,' she says. 'That's not the reality anymore. Not in Sudan. The sad reality now is that we are no longer able to guarantee safety in the countries that we're operating in, so we're taking it day by day.' Among the most at-risk in Sudan, as is often the case in most war zones, are women and children. In one year alone, MSF supported approximately 8,500 births and performed over 1,600 emergency C-sections across Sudan. 'But the numbers don't tell the full story,' Bég says. 'We're talking about women walking 100 kilometres on foot, often pregnant or with sick children, just to reach care.' Eshaghian recalls one such woman who fled violence in Khartoum and arrived in Nyala with her three-year-old daughter — the last surviving member of her family. 'Her child had a fever. We were able to treat her. But the mother had lost her husband and other children along the way. Everything had been taken from them. And there are endless stories like this one." In many cases, the trauma goes even deeper. MSF staff are seeing widespread evidence of sexual violence being used as a weapon of war. 'Women have been raped while fleeing,' Bég says. 'Survivors end up giving birth to babies born out of rape, then they have to deal with the shame and fear of stigma and retaliation.' The scale of the trauma has pushed MSF to adopt community-based care models that, for example, can look like training trusted women in each village to serve as peer counsellors and connectors to formal care. 'We can't be everywhere,' Bég says. 'But we can empower people to help each other.' Both Bég and Eshaghian say that the world's silence on Sudan is not accidental — it's systemic. 'There are no foreign correspondents in Sudan. No international media,' Bég says. 'We are the eyes to the world, and we see that as a responsibility to speak out about what we're seeing and to call for an urgent scaling of aid.' That lack of visibility means less pressure on governments and organizations like the UN to act. 'UNICEF, UNFPA — their response is small and, in the case of Darfur, they're barely present,' Eshaghian says. 'There's clearly a lack of political and diplomatic pressure from governments who have the power to push for the end of this war.' Bég doesn't mince words. She asks, 'Who gets the privilege of our attention? There are certain conflicts that make it to our feeds, to our social media, to the news. There is an inherent inequity and racism in that itself. Where you live shouldn't determine whether you live, and that is what we're seeing in Sudan — willful ignorance.' Where you live shouldn't determine whether you live. Still, there are reasons to keep going — glimpses of joy, even in the bleakest corners. 'When you visit patients in the hospital, especially children who bounce back from illness and are quite resilient, you see their happiness. [We] took a lot of pride in ... being able to support the community, to see the impact,' Eshaghian says. What MSF is now demanding is clear: A significant increase in humanitarian access, stronger diplomatic pressure on Sudan's warring factions, and more support from countries like Canada. 'Our patients have no choice but to wake up with hope every day,' Bég says. 'The least we can do is show up for them and provide care. Hope is not optional, it's our moral imperative. We have to reject apathy.'
