The morning after Phyllis Rabinowitz brought her newborn daughter home from the hospital, she knew something was terribly wrong. The baby, Rebecca, had thick mucus, trouble breathing and lethargy unlike anything Ms. Rabinowitz had seen in her first child.
But during multiple visits to the emergency room, doctors told Ms. Rabinowitz that Rebecca had a common cold and sent them home. At nine days old, Rebecca died; her parents learned from an autopsy that the cause was a viral infection that could have been managed had she been admitted.
More than 80 percent of emergency departments in United States hospitals are not fully prepared for pediatric cases, a new study finds, despite the fact that children make up about 20 percent of visits each year.
The new analysis, published Friday in the journal JAMA Network Open, estimated that if every emergency department in the United States had the core features of 'pediatric readiness,' more than a quarter of the child deaths that follow E.R. visits could be prevented, a figure that equates to thousands of young lives each year.
Even in the most ill-prepared states, the cost to ready every emergency room would be less than $12 per child living there, the researchers found.
'You can now find your state and see: How many children who would otherwise die could we expect to save if we implemented universal pediatric readiness at a high level?' said Dr. Craig Newgard, who was the lead author on the paper, and is the director of the Center for Policy and Research in Emergency Medicine at Oregon Health & Science University.
'The numbers are nontrivial, and I think they're pretty compelling at a state-by-state level — enough to motivate people to want to raise readiness,' he said.
The Cost of Status Quo
The checklist for so-called readiness, established by the National Pediatric Readiness Project, includes stocking child-size equipment and supplies and establishing protocols and trainings for youth-specific care, such as optimal vital signs for children and resuscitation techniques. From a public health perspective, those preparations are considered extremely cost-effective — even more so than several routine childhood vaccines.
Clinicians who study the effects of hospital readiness say the checklist is achievable regardless of the hospital's size or financial resources: Most general emergency departments stock more than 90 percent of the supplies that are that needed to properly care for children, and an on-staff nurse or doctor can undergo training and double as a specialized pediatric care coordinator, for example.
'Even a tiny, little five-bed emergency department in a rural or frontier setting can reach the same level of readiness as a large tertiary care hospital in an urban setting,' Dr. Newgard said. 'If they saw this paper and decided, 'Hey, this is something we should really do,' there are resources to walk them through and figure out where best to start.'
Still, those preparations can be low on the priority list of struggling health centers, and large, mostly rural swaths of the country have become deserts for specialized pediatric emergency care. In a national assessment, overall 'pediatric readiness' scores for emergency rooms decreased slightly between 2013 and 2021, and an ongoing consolidation of pediatric care to large urban tertiary care centers has forced families to travel even farther for treatment.
Only a third of children live near an emergency department that is considered highly 'pediatric-ready,' and among them, nine out of 10 live closer to a less-prepared one. Data shows that those families pay a harsh price: Critically ill children are four times as likely to die in hospitals and twice as likely to die in trauma centers that score low in 'pediatric readiness.'
Hope in the Data
The goal of the researchers in the new study was to estimate the annual cost of making emergency rooms ready for children — and the number of children's lives that could be saved each year if the effort were successful.
Using data from 4,840 emergency departments in all 50 states and in the District of Columbia from 2012 to 2022, they estimated that about 669,000 children were acutely ill or injured when they sought care, meaning that they were admitted to a hospital, transferred to another or died in the unit.
Researchers found that 842 of the 4,840 emergency rooms — less than one-fifth — were considered well-prepared for pediatric emergencies. The level of preparedness at a statewide level ranged from 100 percent of hospitals in Delaware to just 2.9 percent in Arkansas.
They used predictive models to examine how mortality rates would be affected if every emergency department earned a pediatric readiness score of at least 88 out of 100. Of about 7,619 deaths that occur among children who show up at emergency rooms, the researchers concluded that 2,143 could be saved — more than a 25 percent reduction in mortality. In most states, that would be dozens of children saved per year.
The model predicted that the annual cost for all emergency departments in the country to achieve that status would be more than $207 million. The cost per state ranges from $0 in Delaware to more than $18 million in Texas, equating to up to $11.84 per child resident depending on the state.
A Road Map
Authors of the paper highlighted several ways to incentivize hospitals to become better prepared for pediatric emergencies, such as by adding a readiness score threshold to hospital accreditation requirements or by using performance-based reimbursement models.
But hospitals can make the investment themselves, and the four-bed emergency room at South Lincoln Hospital District in Wyoming is a prime example: After a 2021 assessment ranked its preparedness at a 44 out of 100, 'we decided, no, we're going to take this very seriously, and we're going to do what it takes,' said Andrew Appleby, a nurse who serves as pediatric coordinator there.
The emergency room at South Lincoln, the only health care facility in a 50-mile radius, raised its score by 25 points in two years, and thanks to new pediatric protocols and training simulations, Mr. Appleby said, he recently gave lifesaving epinephrine to a child in anaphylaxis before the sole physician on duty could arrive.
Ms. Rabinowitz, who now runs a foundation advocating pediatric readiness in emergency rooms, believes that kind of vigilance among the staff could have saved her baby Rebecca's life. When she learned of the new data set that quantifies the potential for preventing similar tragedies, she wept.
'I actually feel optimistic for the first time,' she said through tears. 'If these changes can be mandated in the whole emergency system — if they can be ready for children — my charity can go out of business. That would be the best-case scenario.'

The researchers defined "healthy aging" as "the absence of 11 major chronic diseases, lack of cognitive and physical function impairments, and having good mental health." Experts Reveal Hidden Ways Sugar Accelerates Aging Beyond Just Weight Gain The study was led by researchers from the Jean Mayer USDA Human Nutrition Research Center on Aging (HNRCA) at Tufts University and Harvard T.H. Chan School of Public Health. "The main takeaway of the study is that consuming dietary fiber and high-quality carbohydrates — those from fruits, vegetables, whole grains and legumes — is associated with positive health status in older adulthood," lead author Andres Ardisson Korat, a scientist at HNRCA, told Fox News Digital. The researchers collected data from the Nurses' Health Study, in which 47,000 women reported their dietary habits and health outcomes between 1984 and 2016, according to a Tufts press release. The women ranged in age from 70 to 93 by the end of the study period. Eating This Breakfast Food Could Help You Live Longer, Study Suggests The researchers looked at the women's consumption of dietary fiber, total carbohydrates, refined carbohydrates, high-quality (unrefined) carbohydrates, and carbohydrates from whole grains, fruits, vegetables and legumes. They also analyzed glycemic index (each food's score indicating how much it raises blood sugar) and glycemic load, which also takes into account portion size to provide a more accurate measure of each food's impact on blood sugar. Women who consumed higher amounts of total carbohydrates; high-quality carbohydrates from whole grains, fruits, vegetables and legumes; and total dietary fiber in midlife were 6% to 37% more likely to have healthy aging and to score higher in several areas of mental and physical wellness, the study found. Conversely, consuming refined carbohydrates — which come from added sugars, refined grains and potatoes — and starchy vegetables resulted in a 13% reduced chance of healthy aging. "This study reinforces something many of us intuitively feel: quality matters," noted Melanie Avalon, a health influencer, entrepreneur and biohacker based in Atlanta, Georgia. "It's not just about 'carbs versus fats versus protein' — it's about the kind of carbs you're eating." Avalon, who was not involved in the research, discussed some of the more notable findings with Fox News Digital. "Perhaps most surprising was the finding that replacing just 5% of calories from carbohydrates with protein (from either plant or animal sources) was associated with lower odds of healthy aging by 7% to 37%," she said. Healthy Eating In Middle Age Has This Key Longevity Benefit Avalon also noted that when carbohydrates were segmented by type — processed versus unprocessed — the associations with healthy aging were independent of BMI (body mass index), a metric typically closely linked to metabolic health. "This suggests the effects of carbohydrate quality on healthy aging were not solely explained by weight-loss effects," she said. The study also briefly touched on the controversial topic of seed oils. "It found that higher intake of polyunsaturated fatty acids (PUFAs) — which are commonly found in seed oils — was linked to decreased odds of healthy aging, adding nuance to the ongoing debate," Avalon added. Based on the study findings, the biohacker recommends favoring whole, unprocessed foods, including fruits, vegetables, legumes and whole grains. "Shop the perimeter of the grocery store for produce and whole grains, as well as the freezer aisles for frozen fruit and vegetables," she suggested. "Consider dipping into the aisles only for pantry staples like canned legumes." For those who tolerate grains, people can reap the benefits of foods like quinoa, brown rice, oats and whole wheat products, Avalon noted. "Prioritize fiber-rich foods and minimize refined carbohydrates like white bread, sugary beverages and ultraprocessed snacks." "These shifts can meaningfully support healthy aging and reduce the risk of chronic disease." One chief limitation of the study is that the participants were mainly white female healthcare workers. "Because of the observational nature of the study, we cannot rule out confounding by other variables," Korat said. "We would have liked to have data on men to evaluate the associations in this group." "We hope our findings help inform consumers about the importance of healthy diets in the promotion of healthy aging," Korat added. "The more we can understand about healthy aging, the more science can help people live healthier for longer." Avalon added that while diet is "foundational," it's just one piece of the "healthy aging puzzle." The Cast and Crew of ‘St. Denis Medical' Found Joy and Warmth in the Show's Hospital Setting

'Hospitals are very sexy places.' The seasoned TV viewer probably knows that; from 'E.R.' to 'Grey's Anatomy' to 'The Pitt,' fictional hospitals are always teeming with attractive people and tense relationships — which is what makes them such fruitful narrative territory. NBC's 'St. Denis Medical' is no exception, as the cast and creative team told IndieWire as part of a virtual panel for Universal Studio Group's USG University. 'My friend who's an oncological surgeon assured me it's a very real thing.' That was the spark for Season 1, Episode 8 of the show, which opens with the St. Denis staff being summoned to a meeting after two of them are caught in the aforementioned on-call room. Hospital administrator Joyce (Wendi McLendon-Covey) hopes it'll be a quick and efficient conversation, but it ends up opening the floodgates regarding sex, gossip, and more. 'It gets Alex (Allison Tolman) in her head about is she having enough sex in her marriage — which was something that was very relatable to me and many married people,' Ledgin said. 'Then the room sort of piles on these ideas of, what if Joyce gets really paranoid that she's not in the loop, and it just was such a funny point of view for that character. One by one, it all comes together in the room of how to turn this into a story about our characters.' The break room is just one of many settings that showcase the work of production designer Elliot LaPlante, and how she and Ledgin worked to create 'the level of hospital' that St. Denis would be from a visual standpoint. 'This was not a cold, inner city hospital, and that allowed us to have a little bit more charm and a little bit more of a community feel, while keeping the balance of feeling real,' said Ledgin. For LaPlante, the show's Oregon setting took her back to her Pacific Northwest roots and to the atmosphere of that region. 'How do we bring the heart that we experience in these scripts to the visuals that we're seeing, and how do we make it as authentic as possible?' she said. 'What are those things that someone who is a medical professional will see and be like, 'Oh my gosh, that is right on. We have that in our hospital.' That's what we were always looking to find in all of our sets.' That extends to Joyce's office, cluttered with memorabilia from her time at the hospital and clues about how much time she devotes to work (as much as she wants to have a richer life outside). 'She sometimes does feel removed from things, but we wanted to get out that reminder [that] she still is integral into everything that's happening and has had such an impact on the community,' Laplante said. 'As we go on through the season, you just understand that Joyce is excluded from a lot of things, and it's because she is a disaster of a person,' McLendon-Covey offered. 'She sure wants to be a cuddly person, but she just isn't. All her plants have to be fake, because she doesn't have time to take care of them, and she has stuffed animals because those are her pets.' She and Tolman expressed gratitude for the stability of something like 'St. Denis.' The sets stay up, the departments have found their rhythm, and production rarely goes into overtime, making everyone 'able to be fully present and able to be pleasant at work for every single hour of every single day, because we weren't working crazy schedules, and we're working these coveted sitcom hours,' Tolman said. Even in a hospital, 'St. Denis' is a workplace comedy, a genre now comfortably depicted in the single-camera mockumentary style. The handheld cameras require minimal set up (cutting down prep time for someone like Tolman, who usually counts on her quick memory to learn lines on set), and performers develop a relationship with their camera operators, who function as moving pieces of the scene like anyone else. 'Whatever brand of gentle psychosis it is where you go through your life as if there was an audience — I've had that since I was a kid,' Tolman said. 'You can share things with the audience, and you can look at the camera and draw them in. There's all these opportunities to make other jokes and have other reactions, and then we get to surprise each other, and we get to surprise our writers and our directors. It just keeps it really alive.' Everyone from the 'St. Denis' team was eager to praise other departments, from the camera crew and writing staff to costume designer Alex Hester, producer Meg A. Schave, and more. As they prepare for Season 2, there was palpable excitement about working on the show with so much established. 'Normally we build something that is for a season and then it comes down,' LaPlante said about her job specifically. 'This set had to be waterproof. It had to be engineered. We built an ambulance bay with a 40-foot cantilevered awning, and we really had to be strategic throughout the Season of how we were able to shoot that area… I'm so glad it's done. It's ready to go for Season 2.' For Ledgin, his years of experience as a writer and executive producer led him to being a full-fledged showrunner — on unexpected levels. 'I am a little surprised by how much I couldn't get away from it, even when I went to sleep,' Ledgin said of the show's first season, which is now streaming in full on Peacock. 'It was in my dreams. I was waking up with an idea, and the problem was that I liked the idea so I had to actually get my phone and write it down. I was like, 'This is going to save me an hour tomorrow then I'm banging my head against the wall trying to think about what it was…' I think that was probably the biggest challenge that I am working on for Season 2.' Tying Alcohol to Poor Sleep to Discourage Drinking

This transcript has been edited for clarity. Welcome to Impact Factor , your weekly dose of commentary on a new medical study. I'm Dr F. Perry Wilson from the Yale School of Medicine. You may not have realized it yet, but we are full-on in the era of biofeedback. I remember a few years ago when my smartwatch started reporting on my heart rate variability during sleep. To be honest, I didn't even know what this metric really meant. But I did notice something fairly early on. On nights when I drank alcohol, my heart rate variability went down. Human brains are really good at linking pieces of data like that, and I kept an eye on it, with additional nights of drinking or not drinking and more reports of heart rate variability only strengthening the association. This is not unique to me, of course — multiple studies have demonstrated the link between alcohol and decreased parasympathetic tone, which is the thing that causes the decrease in heart rate variability. But for me, it was the first time I really saw a physiologic consequence of alcohol intake right before my eyes. Sure, everyone has had a hangover, but this phenomenon was there even if I had only had a glass of wine and felt fine waking up. And it had an effect on me. There was something about seeing that number — maybe it's my competitive nature — that made me want to improve it. I guess the behavioral scientists might call this gamification, but whatever you want to call it, the net-net was that I started drinking less. This is the power of biofeedback: You get information about your physiologic state to make the effects of your actions more real. And, with wearable devices now ubiquitous, it seems like the perfect time to test whether anecdotes like mine hold up in the real world. We got a randomized trial to answer that question just last week. I'm thinking about my wearable device this week because of this study, appearing in JAMA Network Open, examining the complex relationship between alcohol use and sleep in a population of 120 young adults aged 18-25. Full disclosure: This was a study out of Yale, but I was not involved in any way. The researchers, led by Lisa Fucito, wanted to test whether biofeedback — in this case, making explicit the link between alcohol intake and sleep quality — could modify alcohol use behavior. Let's digress for a minute to talk about alcohol and sleep. There is something of a two-way street here. In general, alcohol use decreases the quality of sleep; you wake up more, sleep less, and have less restful sleep overall. But bad sleep can also influence alcohol intake. After a poor night's sleep you have less self-control, less discernment — you might make worse decisions — and none of that is conducive to saying 'no thanks I've had enough' when someone is offering you a shot at the local watering hole. To track alcohol intake and sleep quality, the researchers strapped two wearable devices on to each participant. The first was a sleep actigraphy monitor — basically a device that can tell how much you sleep, how deeply, the quality, and so on. The second device was an alcohol monitor. You may have heard of this 'SCRAM' technology that police sometimes use to ensure someone isn't drinking. It samples the alcohol we excrete in our sweat. While every participant wore these devices for 2 weeks, only half of them were given the information the devices provided. The rest were divided into two control groups. One group was given some web-based information about the importance of sleep, and the other group received the same web-based information but were also asked to keep a diary of their drinking behavior and subjective sleep quality. The intervention group is the interesting one here, of course. At the end of week 1 and 2, a trained coach reviewed each participant's data with them. They showed them their blood alcohol content, their sleep data, and crunched this all into clear numbers like the ones you see here. This participant can see that on their non-drinking nights, their sleep quality is better than on their drinking nights. So, with my personal anecdotal data as a guide, this intervention must clearly have led to decreased drinking, right? Well, sort of. But not really. This is the number of drinks per week at each of four time points. Remember, the intervention only lasted for the first 2 weeks. The primary outcome was the number of drinks at week 12. You can see quite clearly an interesting effect; all three groups drank less over time. The intervention with the cool biofeedback, the people who just kept a drinking diary, even the people who got the web-based sleep information program. Everyone. This is actually a classic finding in studies like this known as the Hawthorne effect. It occurs when individuals change their behavior because they are aware they are being studied. You see it in all kinds of behavioral intervention studies. If they know you're watching, people are more likely to wash their hands, leave a tip, hold a door for a stranger. It's powerful. But it's also short lived. When the study is over, people tend to go back to their old behavior. Unfortunately, this makes it tough to assess the intervention itself. Perhaps it really would work in the real world, when big brother isn't watching, but we can't tell in the world of a clinical trial because all the guinea pigs know they are guinea pigs. That said, some of the secondary outcomes were positive. The intervention group had lower rates of sleep impairment and sleep disturbance. That's an odd finding since we know this improvement couldn't have been mediated by less alcohol drinking. Maybe feedback about your sleep quality makes you change other things independent of alcohol? Maybe you're just a bit more mindful about the whole thing? We should also remember that these were all 18 to 25-year-olds. I don't want to sound like an old fogey here, but I will say that I think I was well into my 30s before I took a hard look at some of my less than healthy behaviors and started to make real changes. There are a lot of other pressures on these young adults. Cutting back on drinking may just not be the most important thing to them at this point in their life. So, no, I'm not ready to give up on biofeedback as an agent of behavior change. I still think this area is promising, but we may need to come up with some more clever ways to study it. For now, I'm keeping an eye on my heart rate variability at night; I am a data guy after all. It may not work for everyone, but it works for me, and if it works for you, then that's good enough.
