
5 Takeaways From New Research About A.D.H.D.
As diagnoses of A.D.H.D. and prescriptions for medications hit new record highs, scientists who study the condition are wrestling with some fundamental questions about the way we define and treat it. More than 15 percent of American adolescents have been diagnosed with A.D.H.D., according to the Centers for Disease Control and Prevention, including 23 percent of 17-year-old boys. A total of seven million American children have received a diagnosis.
Normally, when a diagnosis booms like this, it's because of some novel scientific breakthrough — a newly discovered treatment or a fresh understanding of what causes the underlying symptoms. I spent the last year interviewing A.D.H.D. scientists around the world for my magazine article, and what I heard from them was, in fact, the opposite: In many ways, we now understand A.D.H.D. less well than we thought we did a couple of decades ago. Recent studies have shaken some of the field's previous assumptions about A.D.H.D. At the same time, scientists have made important discoveries, including some that are leading to a new understanding of the role of a child's environment in the progression of his symptoms.
At a moment of national concern about our shrinking attention spans, this science suggests that there may be some new and more effective ways to help the millions of young people who are struggling to focus.
Below are the key findings from the new research.
A.D.H.D. is hard to define — and recent science has made it harder, not easier.
A.D.H.D. has always been a tricky condition to diagnose. One patient's behavior may look quite different from another's, and certain A.D.H.D. symptoms can also be signs of other problems, from anxiety and depression to childhood trauma and autism spectrum disorder. Twenty years ago, researchers thought they were on the verge of ending that controversy by finding a distinct 'biomarker' for A.D.H.D. — a single gene that would reliably predict the disorder, or a physical difference in the brain that you could spot on an M.R.I. But today scientists acknowledge that the search for a biomarker has mostly come up empty, which means the diagnosis remains fluid and somewhat subjective.
Adding to the confusion, a study published last October found that only about one in nine children diagnosed with A.D.H.D. experiences consistent symptoms all the way through childhood. More often, the researchers found, symptoms come and go, sometimes disappearing for a few years, sometimes returning. Together with other research, this study has led some in the field to conclude that our traditional conception of A.D.H.D. as an inherent biological fact — something you simply have or don't have, something wired deep in your brain — is both inaccurate and unhelpful. A new model considers A.D.H.D. differently: not as a disorder you always have in some essential way, but as a condition you experience, sometimes temporarily.
Medications like Adderall and Ritalin can have a positive effect on children's behavior – but the results often don't last.
The biggest long-term study of A.D.H.D. treatments found that after 14 months of treatment, a daily dose of Ritalin did a better job of reducing children's symptoms than nondrug interventions like therapy or parent coaching. But then the effect started to fade, and by 36 months, the relative benefit of the drug treatment had disappeared altogether. The symptoms of the children in the medication treatment group were no better than those of the ones assigned to behavioral interventions — and no better than a comparison group that was given no intervention at all.
The medications can improve students' behavior in the classroom — but they don't seem to help them learn.
Medications like Ritalin and Adderall reliably improve students' behavior — at least in the short term — but they don't seem to do much to improve academic achievement. Research suggests that on medication, children are working harder and faster but not more effectively. So when it comes time to take a test, they haven't actually learned anything more.
Still, it's a puzzle: If A.D.H.D. medications don't improve academic performance, why do so many students and their parents think they do? Researchers investigating this question have found that drugs like Ritalin and Adderall mostly work on your emotions, not your cognition. They don't make you smarter, in other words — but they make you believe you're smarter by increasing your emotional connection to the work you're supposed to be doing.
There's some history to this. Today's leading A.D.H.D. medications are all versions or derivatives of amphetamine, and ever since World War II, a main attraction of amphetamine pills is that they make boring activities (watching for German planes, doing endless loads of laundry, driving a truck across the country) seem more interesting. Today Ritalin and Adderall may be having the same effect for high school students — making boring school work seem temporarily fascinating.
There is no clear dividing line between those who have A.D.H.D. and those who don't.
Rather than conceiving of A.D.H.D. as a black-or-white, yes-or-no diagnosis, many researchers now believe that it's more accurate to consider A.D.H.D. symptoms as existing on a continuum. As the British researcher Edmund Sonuga-Barke said to me: 'There literally is no natural cutting point where you could say: 'This person has got it, and this person hasn't got it.' Those decisions are to some extent arbitrary.'
On one end of the continuum are children who could probably get an A.D.H.D. diagnosis — but also might be fine without one. On the other extreme are cases that are much more serious. Joel Nigg, a researcher in Oregon, has identified one group of children — about a third of the diagnosed total — whose A.D.H.D. symptoms are accompanied by intense anger. They are at much higher risk of future problems, including school dropout, criminal behavior and early death. We should put our treatment focus on those children, he says, and consider other approaches for those with less serious cases.
Changing a child's environment can change his or her symptoms.
A.D.H.D. is usually portrayed primarily as a medical condition — a neurodevelopmental disorder with a genetic cause — which is why we often look first to medication to treat it. But researchers are now discovering that A.D.H.D. symptoms can be highly responsive to the environment as well. When the surroundings of a person with an A.D.H.D. diagnosis improve — a more engaging classroom, a more stimulating job, a more congenial home life — his symptoms often improve as well.
These findings are leading some researchers away from the traditional 'medical model' of A.D.H.D., which sees the brains of people with A.D.H.D. symptoms as biologically deficient, and toward a new model that considers A.D.H.D. primarily as a mismatch between a child's unique brain and his environment. Medication can still be useful in helping to resolve those mismatches, but changing the environment can sometimes work just as well.
When A.D.H.D. is bad, it's bad. And when it's bad, medication can help. But many children and parents have been led to think of A.D.H.D. as a permanent malfunction, when in fact it might be better thought of as a temporary misalignment, brought on by external forces as well as internal ones. Finding a solution for that misalignment is important, especially for a family in crisis. But identifying the right solution starts with having a more accurate understanding of A.D.H.D.

"We are compelled to speak up when our leadership prioritizes political moment over human safety and faithful stewardship of public resources," more than 300 scientists wrote Monday to NIH Director Jay Bhattacharya in a so-called "Bethesda Declaration" published by Stand Up For Science in rebuke to Trump administration research funding cuts and staff layoffs. They added in the letter to U.S. Health and Human Services Secretary Robert F. Kennedy Jr. and members of Congress overseeing NIH that they "dissent" to Trump's policies that "undermine" the NIH mission, "waste" public resources and harm "the health of Americans and people across the globe." In the open letter, they said the current endeavor to "Make America Healthy Again" referred to "some undefined time in the past." "Keeping NIH at the forefront of biomedical research requires our stalwart commitment to continuous improvement," the letter's writers said, adding that the life-and-death nature of NIH work "demands that changes be thoughtful and vetted." According to the letter, the Trump administration terminated at least 2,100 NIH research grants since January, totaling around $9.5 billion and contracts representing some $2.6 billion in new research. "We urge you as NIH Director to restore grants delayed or terminated for political reasons so that life-saving science can continue," the letter added in part. "This undercuts long-standing NIH policies designed to maximize return on investment by working with grantees to address concerns and complete studies," it said. It further accused the White House of creating a "culture of fear and suppression" among NIH researchers. Bhattacharya, a Stanford University professor and health researcher, called the agency the "crown jewel of American biomedical sciences" and said he had the "utmost respect" for its scientists and mission during his confirmation hearing in March. On Tuesday, Bhattacharya is scheduled to testify before the Senate's Appropriations Committee on Trump's 2026 NIH budget proposal which seeks to cut roughly 40% of NIH's $48 billion budget. "This spending slowdown reflects a failure of your legal duty to use congressionally-appropriated funds for critical NIH research," the scientists penned to Bhattacharya. The letter goes on to characterize it as "dissent" from Trump administration policy, quoting Bhattacharya during his confirmation as saying "dissent is the very essence of science." "Standing up in this way is a risk, but I am much more worried about the risks of not speaking up," says Jenna Norton, a program officer at the NIH's National Institute of Diabetes and Digestive and Kidney Diseases. Measles outbreaks in Michigan and Pennsylvania end, while Texas logs just 4 new cases
The U.S. logged 122 more cases of measles last week — but only four of them in Texas — while the outbreaks in Pennsylvania and Michigan officially ended. There were 1,168 confirmed measles cases in the U.S., the Centers for Disease Control and Prevention said Friday. Health officials in Texas, where the nation's biggest outbreak raged during the late winter and spring, said they'll now post case counts only once a week — yet another sign the outbreak is slowing. There are three other major outbreaks in North America. The longest, in Ontario, Canada, has resulted in 2,009 cases from mid-October through June 3. The province logged its first death Thursday in a baby that got congenital measles but also had other preexisting conditions. Another outbreak in Alberta, Canada, has sickened 761 as of Thursday. And the Mexican state of Chihuahua had 1,940 measles cases and four deaths as of Friday, according to data from the state health ministry. Other U.S. states with active outbreaks — which the CDC defines as three or more related cases — include Colorado, Illinois, Kansas, Montana, New Mexico, North Dakota, Ohio and Oklahoma. In the U.S., two elementary school-aged children in the epicenter in West Texas and an adult in New Mexico have died of measles this year. All were unvaccinated. Measles is caused by a highly contagious virus that's airborne and spreads easily when an infected person breathes, sneezes or coughs. It is preventable through vaccines, and has been considered eliminated from the U.S. since 2000. There were a total of 742 cases across 35 counties, most of them in West Texas, state health officials said Friday. Throughout the outbreak, 94 people have been hospitalized. State health officials estimated less than 1% of cases — fewer than 10 — are actively infectious. Fifty-five percent of Texas' cases are in Gaines County, where the virus started spreading in a close-knit, undervaccinated Mennonite community. The county has had 411 cases since late January — just under 2% of the county's residents. The April 3 death in Texas was an 8-year-old child, according to Health Secretary Robert F. Kennedy Jr. Local health officials said the child did not have underlying health conditions and died of 'what the child's doctor described as measles pulmonary failure.' A unvaccinated child with no underlying conditions died of measles in Texas in late February; Kennedy said the child was 6. How many measles cases are there in New Mexico? New Mexico added two cases in the last week for a total of 81. Seven people have been hospitalized since the outbreak started. Most of the state's cases are in Lea County. Sandoval County near Albuquerque has six cases, Eddy County has three, Doña Ana County has two. Chaves, Curry and San Juan counties have one each. An unvaccinated adult died of measles-related illness March 6. The person did not seek medical care. How many cases are there in Oklahoma? Oklahoma added one case last week for a total of 15 confirmed and three probable cases. The state health department is not releasing which counties have cases. How many cases are there in Colorado? Colorado has seen a total of 12 measles cases in 2025, which includes one outbreak of seven related cases. The outbreak is linked to a Turkish Airlines flight that landed at Denver International Airport in mid-May, and includes three cases each in Arapahoe and El Paso counties and one in Denver, plus a person who doesn't live in Colorado. Other counties that have seen measles this year include Archuleta and Pueblo. How many cases are there in Illinois? Illinois health officials confirmed a four-case outbreak on May 5 in the far southern part of the state, and it's grown to eight cases as of June 6, according to the Illinois Department of Public Health. The state's other two cases so far this year were in Cook County, and are unrelated to the southern Illinois outbreak. How many cases are there in Kansas? Kansas has a total of 71 cases across 11 counties in the southwestern part of the state, with three hospitalizations. All but two of the cases are connected, and most are in Gray County. How many cases are there in Montana? Montana had 17 measles cases as of Thursday. Ten were in Gallatin County, which is where the first cases showed up — Montana's first in 35 years. Flathead and Yellowstone counties had two cases each, and Hill County had three case. There are outbreaks in neighboring North Dakota and the Canadian provinces of Alberta, British Columbia and Saskatchewan. How many cases are there in North Dakota? North Dakota, which hadn't seen measles since 2011, was up to 34 cases as of Friday. Two of the people have been hospitalized, and all of the people with confirmed cases were not vaccinated. There were 16 cases in Williams County in western North Dakota on the Montana border. On the eastern side of the state on the Minnesota border, there were 10 cases in Grand Forks County and seven cases in Cass County. Burke County, in northwest North Dakota on the border of Saskatchewan, Canada, had one case. How many cases are there in Ohio? Ohio remained steady for a third week at 34 measles cases and one hospitalization, according to the Ohio Department of Health. That count includes only Ohio residents. The state has two outbreaks: Ashtabula County near Cleveland has 16 cases, and Knox County in east-central Ohio has 20 — 14 among Ohio residents and the rest among visitors. Allen, Cuyahoga, Holmes and Defiance counties have one case each. How many cases are there in Tennessee? Tennessee has had six measles cases since early May, but no change since. Tennessee's outbreak appears to be over, as health officials say there have not been any new cases in six weeks. Where else is measles showing up in the U.S.? Measles cases also have been reported in Alaska, Arkansas, California, Florida, Georgia, Hawaii, Indiana, Iowa, Kentucky, Louisiana, Maryland, Michigan, Minnesota, Missouri, Nebraska, New Jersey, New York, Pennsylvania, Rhode Island, South Dakota, Vermont, Virginia and Washington. Earlier outbreaks in Indiana, Michigan and Pennsylvania were declared over by health officials after six weeks of no new cases. Cases and outbreaks in the U.S. are frequently traced to someone who caught the disease abroad. The CDC said in May that more than twice as many measles have come from outside of the U.S. compared to May of last year, and most of those are in unvaccinated Americans returning home. In 2019, the U.S. saw 1,274 cases and almost lost its status of having eliminated measles. What do you need to know about the MMR vaccine? The best way to avoid measles is to get the measles, mumps and rubella vaccine. The first shot is recommended for children between 12 and 15 months old and the second between 4 and 6 years old. Getting another MMR shot as an adult is harmless if there are concerns about waning immunity, the CDC says. People who have documentation of receiving a live measles vaccine in the 1960s don't need to be revaccinated, but people who were immunized before 1968 with an ineffective vaccine made from 'killed' virus should be revaccinated with at least one dose, the agency said. People who have documentation that they had measles are immune, and those born before 1957 generally don't need the shots because so many children got measles back then that they have 'presumptive immunity." Measles has a harder time spreading through communities with high vaccination rates — above 95% — due to 'herd immunity.' But childhood vaccination rates have declined nationwide since the pandemic and more parents are claiming religious or personal conscience waivers to exempt their kids from required shots. What are the symptoms of measles? Measles first infects the respiratory tract, then spreads throughout the body, causing a high fever, runny nose, cough, red, watery eyes and a rash. The rash generally appears three to five days after the first symptoms, beginning as flat red spots on the face and then spreading downward to the neck, trunk, arms, legs and feet. When the rash appears, the fever may spike over 104 degrees Fahrenheit, according to the CDC. How can you treat measles?
