Rockford hospitals receive mixed safety grades; Illinois shows improvement
ROCKFORD, Ill. (WTVO) — While Illinois ranked 20th in the nation on the annual safety report, each received a B, a C, and a D, respectively.
The 2025 Leapfrog Group report grades hospitals on 22 safety measures, including hand hygiene, falls and trauma, and death rates among surgical patients.
In total, 35 Illinois hospitals earned 'A' grades, moving the state up the ranking from 30th last year.
In Rockford, OSF Saint Anthony Hospital earned the highest grade in the city, a scoring worse than average on surgical site infections after colon surgery, blood clots, and responsive staff, but average or higher than average on nearly all other safety criteria.
UW Health SwedishAmerican Hospital rating, scoring average to better than average on infections, but worse than average on practices to prevent errors, handwashing, and doctor-patient communication.
And Javon Bea Mercyhealth – Rockton scoring worse than average on MRSA, C. diff, and blood infections, death from treatable complications, collapsed lung and blood clot issues, prescription communication issues, and issues surrounding specially trained doctors in the ICU.
Javon Bea Mercyhealth – Riverside was not listed.
Beloit Memorial Hospital grade; FHN Memorial in Freeport ; OSF St. Katharine Medical Center in Dixon .

Former teen mom credits Rockford nurse for inspiration to pursue a nursing career
ROCKFORD, Ill. (WTVO) — A mother who gave birth at 15 years old gave credit to a nurse who was instrumental in guiding her after pregnancy for inspiring her to become a nurse. Cherish Adams, now a mother to three children, has completed her nursing degree. Adams said Jeannine Joseph, a Neonatal Intensive Care Unit Nurse, helped her realize she wanted to pursue the career. The realization started to take shape when Adams gave birth to her oldest child and was thinking about giving him to the Swedish American Hospital in a shoebox. Adams explained that this was the most terrified she had ever felt in her life. 'When I finally did go up to the hospital, I just remember being really scared and very overwhelmed because, again, I hadn't told anyone anything about this,' said Adams. Joseph was one of the first people to know how the situation was taking a toll on Adams personally. 'She's in school, but this does not mean that you have to give up on your dreams,' Joseph explained to Adams. 'You still can go to school, you still can graduate from high school, you still can go to college, and you can make the best life that you can for you and Allen.' After 21 years, Adams is graduating from Rasmussen University-Rockford with an Associate's Degree in Nursing. She gave Joseph credit for helping put her on this path through all the chaos and confusion. 'Honestly, it just makes me feel very excited and just very accomplished,' said Adams. 'On top of graduating from nursing school, I'm actually getting to take a position in the same hospital where Jeannie inspired me. So, it's really this a huge full circle moment.' Illinois Department of Healthcare data breach reveals Social Security and child support details
SPRINGFIELD, Ill. (WTVO) — The Illinois Department of Healthcare and Family Services (HFS) is alerting the public to a data breach that may have exposed customers' private information to hackers. According to HFS, the agency became aware of a phishing campaign targeting its employees around February 11th, 2025. The agency said employees' emails and documents were compromised, and may have included customer names, social security numbers, driver's license or state ID number, financial information related to child support, child support or Medicaid identification and case numbers, and birth dates. HFS said 933 individuals were impacted in the breach, 564 of which were Illinois residents. The Department said all affected individuals were notified by May 23rd. 'Potentially affected individuals can also contact consumer reporting agencies to place a free fraud alert or security freeze on their accounts, or the Federal Trade Commission to learn more about fraud alerts, credit freezes, or other identity theft resources,' HFS said. 815 BlackFest celebrates Black culture and tackles issues
ROCKFORD, Ill. (WTVO) — Rockford's Levings Park will host the return of the 815 BlackFest, a summer festival celebrating Black culture. The event will be headlined by Just Fine, a Mary J. Blige tribute act from Pittsburgh. Along with the food, vendors, and music, the stated goal of the event is to address more serious issues in the Black community, including hunger, domestic violence, mental health, education, financial literacy, and homelessness. 'When we look at those six pillars, almost every avenue of those six pillars I've touched at some point in my life. And a lot of people have,' said community advocate Tina Renee. 'But the one thing that has brought me through those pillars, and the struggles, is having those connections and those resources, or having people who surround me that can help me through those.' 815 BlackFest will take place on June 29th, 2025, from 3 p.m. to 7 p.m. at Levings Park, 1420 S. Pierpont Ave. Admission is free. Other performers include the Phenomenal Dolls, FAB Dance Team, Yavo, DJ H-Fresh, Trinity Rucker, and a fashion show with Shereese Croft.
