Pay for nurses in Wales is at a "standstill" and "lagging behind" other public sector workers, according to a union.
Nurses have been offered a 3.6% pay rise for this financial year, below the offer for other NHS staff including doctors and dentists.
The Royal College of Nursing (RCN) Wales has asked whether "downtrodden" nurses "have got the fight in them" to go back on strike.
The Welsh government said it had accepted the recommendations made by the independent NHS pay review body in full and was working to ensure staff receive the pay increase and back pay "as soon as possible".

Announcing the pay offer, Wales' Health Secretary Jeremy Miles said nurses provided "exceptional care", adding their services were "truly appreciated".
The minister accepted the advice of the NHS pay review body, including a 4% pay increase for doctors and dentists, and what has been described as an average rise of 5.4% for resident doctors, until recently known as junior doctors.
Speaking to BBC Walescast, Nicky Hughes, RCN Wales' associate director of nursing, said the Welsh government should "respect all professions and provide equal pay or at least parity across all the public sectors".
"Yet for the second year in a row we find that nurses...are lagging behind and that really shows a complete disrespect for the profession."
With the rise in everyday prices currently running at 3.5%, Ms Hughes said the 3.6% award for nurses was "about on inflation".
She added the cost of living crisis, particularly for food, was also an issue.
"We know that nurses are using food banks so, at the moment, what we're saying is, nurses deserve more."
Ms Hughes also pointed to the 2,000 nursing vacancies in Wales, out of a workforce of about 40,000.
She said the offer did not reflect nurses' work in recent months in reducing NHS waiting lists.
In March, the number of patients waiting two years or more for planned treatment stood at 8,389 - its lowest point since April 2021.
The RCN Wales will consult its members on the pay offer.
Welsh nurses voted to go on strike several times at the end of 2022 and the start of 2023.
Asked whether RCN members were willing to return to the picket lines, Ms Hughes said it would be "very difficult" currently.
"They're tired, they're burnt out, they're trying to do the best for their patients that they can," she said.
"So, although there will be an appetite with some of our members to go on strike, we will have to see where they go because they do feel very downtrodden at the moment.
Ms Hughes added nurses were fighting "every day" to get through their shift, make sure patients had safe care, and to ensure colleagues were looked after
A Welsh government spokesperson said: "We have accepted the recommendations made by the independent NHS Pay Review Body in full and are working to ensure that staff will receive both the pay increase and back pay as soon as possible.
"We greatly value the nursing and midwifery workforce in Wales and the vital work they do."

The Government has cut funding for nursing courses in a move that risks undermining its pledge to end the NHS's reliance on foreign workers. Ministers have quietly frozen grants that are paid to universities to help them cover the higher costs of training medical workers. Critics reacted by warning that the move, which represents a real-terms cut, would 'worsen an already difficult situation' in the sector and 'leave us more reliant' on imported labour. It will also prompt fears of further cuts down the line, with education set to be one of the losers in Rachel Reeves's spending review this week. Universities are already struggling to keep nursing courses afloat, with research showing that many are cutting lecturer jobs to save cash. In last year's manifesto, Labour pledged to 'end the long-term reliance on overseas workers' with a 'workforce and training plan' for the NHS. However, Bridget Phillipson, the Education Secretary, has now frozen grants which are paid to universities to cover the extra costs of running medical courses. Those include courses for nursing, midwifery and 'allied health professionals' such as paramedics, radiographers and occupational therapists. In a letter to the Office for Students, she said the per-student funding levels should be kept the same as last year, representing a real-terms cut. That is part of an overall reduction of £108 million in the cash that has been made available for the Strategic Priorities Grant this year. Patricia Marquis, the executive director for the Royal College of Nursing in England, said nursing courses were already under threat from the 'financial crisis' at universities. 'A freeze in per student funding, effectively a real-terms cut, could make a bad situation worse, resulting in not only more job losses, but impacting the very financial viability of nursing courses,' she warned. 'University courses are the single biggest route into the profession, and this would be a disaster. Additional funding should be released to protect nursing courses in response to rising costs.' Vivienne Stern, the chief executive of Universities UK, also warned that the cuts to the grant risked 'worsening an already difficult situation'. She said that the move, combined with a recent decision to slash cash for apprenticeships, would have a 'detrimental impact on training our healthcare workforce'. 'Universities are committed to delivering what this country needs, including a strong and sustainable NHS workforce, and are crucial to training talent,' she said. 'We can't create a future-proof NHS workforce without long-term, sustained investment from the government.' Ms Phillipson said in her letter to the OfS that universities should prioritise funding for nursing courses as 'a strategically important subject'. She targeted grants for other courses and in particular media studies and journalism, which have been derided by some as 'Mickey Mouse' degrees. But despite that, it appears there was still only enough money in the pot for her to restrict the hit to medical courses to a real-terms cut. Laura Trott, the shadow education secretary, said the move was 'yet more evidence that Labour aren't serious about cutting immigration'. 'At a time when we should be training more British people to become nurses, they've cut teaching grants for universities and scrapped almost all higher-level apprenticeships, leaving an 11,000 shortfall in the NHS workforce plan,' she said. 'These decisions will leave us more reliant on foreign labour, when we should be training British people for British jobs.' The Department for Education is facing steep real-terms cuts to its budget when the Chancellor unveils her spending review on Wednesday. She will set out the annual funding totals that all Whitehall departments receive from next year until almost the end of the decade. Most are expecting to see their budgets squeezed to free up more cash for health and defence, which will be the two big winners. Within education, the schools budget is ring-fenced, meaning it cannot be cut. Early years provision, meanwhile, is a flagship Labour policy with the party pledging to fund the creation of 100,000 new nursery places. That means that higher education, including universities and apprenticeships, is expected to bear the brunt of the cuts imposed on the department. A Government spokesman said: 'We hugely value the work of talented nurses and midwives which is why we have prioritised Strategic Priorities Grant funding for important, high-cost courses, including nursing and midwifery, that are vital for economic growth and our Plan for Change. 'The dire situation we inherited has meant this Government must take tough decisions to put universities on a firmer financial footing and we're continuing to support them, including by increasing the amount they can charge in tuition fees. 'We are rebuilding the NHS for the benefit of patient and staff, and ensuring nursing remains an attractive career choice, including by unveiling a refreshed workforce plan to ensure we have the right staff in the right place so the NHS can be there for everyone.' 'My left breast was moved to my right after cancer recurred'

Five months into a pregnancy for a much wanted second child in 2024, Nicola Purdie found a lump in her right breast. It is a nightmare scenario for anyone. But for Nicola this was not a new experience. She had already had a double mastectomy, chemotherapy and breast reconstruction after being diagnosed with cancer in 2020. But a radical idea which she suggested to her surgeon has led to what is believed to be a pioneering world first procedure - transplanting a healthy reconstructed breast to replace a cancerous one. In September 2020, Nicola had just finished with breastfeeding her first child and was returning to work as a geography teacher in her home town of Swansea when she found a lump. By October it was diagnosed as cancer and she started five months of chemotherapy at Singleton Hospital, followed by the double mastectomy. Nicola, now 38, said: "Even though I only had cancer in the one breast, my maternal aunty and maternal grandmother also had had breast cancer previously. I thought 'I'm young, let's just get rid of everything and do as much as we can'." She had reconstructive surgery using skin and tissue from her abdomen in a procedure known as a DIEP (deep inferior epigastric perforator) flap. "It meant I had natural breasts which would grow and shrink with my bodyweight over time and I wouldn't need implants that would need changing in 10 or 15 years," she said. Because the cancer was oestrogen-fuelled, she was placed on hormone suppression therapy for at least two years. "We knew at the time I was diagnosed that we wanted to have another baby eventually, so we waited for two-and-a-half years actually. "All the oncologists we saw were of the same opinion that I was at no increased risk, because I'd had a complete pathological response [no sign of cancer post-treatment]." Then lump number two appeared. Nicola explained: "This time it was in the skin of the breast because that was the only tissue that was remaining really from the first surgery. There must have been some cancer cells there. "I knew straight away as soon as I found the lump, this is not a coincidence. This is cancer again." Apart from family and a close friend, Nicola did not tell anyone about the cancer this time. Partly it was to shield her daughter from knowing but she had another reason. "I wanted to protect the baby. I wanted him to come into the world and it to be all about him. I didn't want it to be focusing on 'oh gosh Nicky, you OK?'." She attributes this mindset to helping her "shut the door" on thinking about it until her son was born. Because of pregnancy, she could not begin chemotherapy, or have the usual scans to establish if the cancer had spread. She had some chest X-rays and an ultrasound of her stomach and liver, which gave a degree of certainty, although not "100%", that it remained contained in the breast. She also had a lumpectomy to remove the lump from the breast skin. The plan was to deliver the baby at 32 weeks, at the end of August, but even in the midst of the urgency for treatment, she had an eye to her son's future. "I think it was 26 August and I said 'can I put my teacher's hat on now and push it to the week after'. "My husband was like 'what?' But I said 'I've got to give him the best chance'. "So we waited until the first Monday in September and he was born at 32 weeks and six days." Fraser was in the neonatal unit for three weeks. "He went from strength to strength and that made things easier as well because of what I was dealing with. "I don't know how I would have been if he was poorly as well." Nicola started chemotherapy two weeks after giving birth and would go from the baby unit upstairs to have her treatment and then straight back down to her son. She had 16 rounds of chemo which took her all the way through to the beginning of February. Woman has pioneering breast reconstruction surgery 'Reconstructive breast surgery wait just adds to the ordeal' The cancer survivors waiting years for breast reconstruction Initially the oncology team had not been sure what operation to suggest as Nicola had multiple cancer sites in the right breast area and a tumour in one lymph node. The most likely option was removal and an LD (latissimus dorsi) flap, which involves swinging a muscle and skin from the back to cover the area left behind by the removed breast, but leaving the area flat in her case. Reconstruction using an implant on the right side instead would not have been possible as so much skin had to be removed, there would not be enough to cover it, or to withstand damage from subsequent radiation treatment. However, Nicola's aunt had had the LD flap operation and she knew it came with an arduous recovery process and possibly reduced range of movement - not an option for her as the mother of two small children. She also knew she wanted "symmetry" - either two breasts or none. It was while talking to her sisters she initially had the idea that, if she did end up having both breasts removed, why not use the skin from the healthy left breast to cover the larger affected area on the right. But it was only once she was in the consulting room with consultant plastic, reconstructive and breast surgeon Reza Arya, that the idea of moving the whole breast intact dawned. She realised it would allow her to create a right breast and have reconstructive surgery on the left side using an implant because there was enough skin left in situ from the first operation in 2020. When she put this to Reza - "could we rob Peter to pay Paul?" - she "could literally see the cogs whirring" in his head. He recalled being speechless at the idea. "I had never seen or heard of such a procedure being talked about or published. "I wasn't even sure at that stage whether I would be adding to Nicola's care or taking away from it." Reza said it was very unusual to have a patient come up with such a radical idea. "Kudos to Nicola because she's amazing. "I had extensive discussions with colleagues all over the UK who are all experts in flap reconstruction from the abdomen. The conclusion was maybe it is possible and just go ahead with it. "This is not run of the mill mastectomy we're talking about," he said. "We're removing the whole of the footprint of the breast [on the right]. It is a very, very large area of skin and to be able to close it you have to borrow from somewhere, some skin and tissue, that can withstand future radiotherapy. "Left is a side that's easy to reconstruct in future, because it's not subject to radiotherapy and all other treatments, and the cancer itself." When Nicola went into the surgery, she knew she could wake up to very different outcomes - a successful left to right transplant, or an LD flap reconstruction on her right side after all if he decided the transfer would not work, with the left breast removed anyway at her request. She gave Reza her blessing for any outcome. And her outrageous gamble paid off. "What makes it a world first is that the tissue has been ported to two different sites," she said. "We didn't know if that would be possible or probable, the microsurgery of actually sewing the vessels together - if it would work or if it would be so scarred that it would be completely impossible." She is starting a few weeks of radiotherapy, and around Christmas time will have reconstructive surgery on the left side using a saline implant. She will be on hormone therapy for the next 10 years to manage the risks of recurrence but has been given the all-clear in terms of this treatment cycle. For someone who has been through such gruelling experiences, she is remarkably upbeat. How does she stay positive? "If I am having those moments when I feel 'oh my god this is awful, why has this happened to us?' "I let myself think about it for a minute and then move on to the next thing. If you don't allow yourself to think about it, it won't get you down. "I have to sit back and think 'this is life. Worse things can happen'. I'm lucky that I've had incredible treatment from the word go, and an amazing team who I trust." She added: "Both the kids were just the best distraction ever. It's hope isn't it? When you see little children. You think there's hope for the future." Urgent call for south London blood donors amid 'slump' after bank holidays

NHS Blood and Transplant is asking South Londoners to help fill a donation shortfall caused by recent bank holidays. There are hundreds of appointments available at Brixton Donor Centre, with 1,000 slots needing to be filled each week to meet demand. The high number of bank holidays, combined with the start of the half-term break, has increased pressure on blood stocks. The NHS is particularly keen to see new and lapsed donors return. "We are extremely grateful to everyone who has donated so far this year, but we must act now to avoid falling behind. "To avoid facing a 'spring slump' in donations, we urgently need donors to book an appointment today – and help us fill appointments over the weeks ahead. "If you can't find an appointment straightaway, please book further ahead or keep checking back to help fill last-minute availability or cancellations. "Each donation has the power to save up to three lives." There is a particular need for donors of black heritage to meet the growing need for blood to treat sickle cell, the country's fastest-growing inherited blood disorder, which predominantly affects black communities. Black heritage donors are 10 times more likely than the white population to have the specific Ro blood type used to treat sickle cell patients. Around 1,300 black donors a month are needed to provide life-saving transfusions to sickle cell patients, as well as for use in emergencies, childbirth, during surgery, cancer treatments, and other medical conditions. Brixton Donor Centre, which opened in December 2024, has had 7 per cent of all donations with the in-demand Ro subtype, compared with 2 per cent nationally. South Londoners with O negative, O positive and A positive blood types are also being encouraged to donate, as these blood groups are in high demand. Brixton Blood Donor Centre manager Rexford Osei-Bonsu said: "I'm immensely proud of the thousands of people who have given blood in Brixton, but the holidays have meant that we need more people to come forward. "Giving blood is easy and painless and takes about an hour of your time to make a life-saving difference."
