According to federal prosecutors, Plasencia repeatedly provided Perry with ketamine — a powerful anesthetic with psychedelic properties — without any legitimate medical reason in the weeks leading up to his death. The drugs were delivered in person, including at Perry's Los Angeles home and even a public parking lot, where Plasencia reportedly injected the actor while he sat in the back seat of his car.
Court documents reveal that Plasencia supplied Perry with 20 vials of ketamine, a package of lozenges, and syringes between late September and mid-October 2023. Prosecutors also allege he left drugs with Perry's assistant, despite knowing the assistant had no medical training.

As part of a plea agreement filed in Los Angeles federal court, Plasencia faces a potential sentence of 15 to 21 months in prison. However, under federal law, the judge could impose a sentence of up to 40 years and a $2 million fine.
The Los Angeles County Medical Examiner determined that Perry, who was 54, died from the 'acute effects of ketamine,' with additional contributing factors including drowning, coronary artery disease, and the effects of buprenorphine, an opioid.

Federal investigators have charged a total of five individuals in connection with Perry's death. Three have already pleaded guilty, including Perry's longtime assistant, Kenneth Iwamasa, who admitted to helping procure ketamine through doctors and a drug dealer. Another key figure in the case, Jasveen Sangha — known as the "Ketamine Queen" — is accused of supplying the fatal dose and is scheduled to stand trial in August. She has pleaded not guilty.
Prosecutors say Plasencia tried to conceal his misconduct by fabricating a false "treatment plan" for Perry to justify his unauthorized ketamine prescriptions.
Perry, who had been open about his decades-long struggle with addiction, was found unresponsive in his hot tub on October 28, 2023. His sudden death shocked fans around the world and reignited scrutiny around the misuse of therapeutic psychedelics like ketamine.
