Child (12) missing for 18 months from international protection centre, Hiqa report

A 12-year-old child missing for 18 months was among 30 unaccounted for during a recent inspection of the
Tusla
welfare service provided to unaccompanied and separated children seeking international protection.
There were no records of strategy meetings with gardaí or contacts with UK or Northern Ireland counterparts to attempt to locate the 12-year-old, the inspection report outlined.
A further 27 children were reported missing at various stages of the inspection and returned to their placements.
The inspection was published on Thursday by the
Health Information and Quality Authority
(Hiqa), and found that in ten cases where children were reported as missing, the service was not managing these cases in line with the national protocol for children missing in care.

The inspection, which was carried out between January 28th and 30th and on February 5th, also found that in multiple cases, children in the service were not allocated a social worker.
A total of 195 children were instead placed with a 'dedicated team' while they were awaiting the allocation of a social worker.
The report notes that two children complained about being unable to enrol in school due to their lack of a social worker.
Another child raised concerns about their social worker not answering their calls and not responding to their needs.
Hiqa stated that not all child-protection concerns were managed in line with Children First, the national guidance for the protection and welfare of children.
It cited gaps in the identification of child-protection concerns by staff newly recruited to the team, gaps in referrals to An Garda Síochána and delays in carrying out safeguarding visits to children once they were accommodated.
In three cases, there were no records of safeguarding visits to children.
The watchdog states that the Separated Children Seeking International Protection service has been impacted by high referral rates and continued staff vacancies 'which impacted on the teams' ability to sustain service improvement'.
Hiqa escalated a number of cases to seek assurances about the management of issues identified during the inspection, saying that it received 'satisfactory assurances' that an improvement plan would be put in place.
A compliance plan was also submitted outlining actions that would be taken to address the areas of noncompliance.
In a statement today, Tusla noted 'while the report highlights risks and ongoing challenges, it also recognises meaningful progress made since the previous inspection in November 2023, including improvements in governance, staff training, case management, and oversight structures'.
'The report notes the commitment of the staff to deliver the highest possible care in what is a high-pressure, demand-led and at times a crisis-driven environment.'
Lorna Kavanagh, area manager of National Services and Integration at Tusla, said the agency is 'clear about the range of challenges and risks faced by the service', and is 'deeply committed to addressing the issues raised and ensuring that all children in our care receive a safe, timely and appropriate service'.
She added: 'significant steps have already been taken – including an increase in staff, stronger interagency collaboration and more robust governance – and we are determined to build on this momentum to deliver continuing improvement.'

People react in different ways to grief, but the day after the death of a close family member, Amy Dennis developed stomach cramps and diarrhoea. Just a teenager at the time, her GP put it down to being bereaved, but the symptoms persisted and the bouts of diarrhoea became a recurring issue that played havoc with her life. As time went on, and her situation didn't improve, it was thought that she could have had an intolerance to gluten – the protein found in grains – but it wasn't until she began to get 'properly sick' that a thorough investigation was undertaken. It was discovered that she had Crohn's disease – a chronic and debilitating digestive condition with lifelong consequences. The condition can require the need for surgery, hospitalisation, immuno-suppressing treatments and higher risks for some forms of digestive cancers. READ MORE 'At the start of 2021, during the second Covid lockdown, I began to lose extreme amounts of weight from both diarrhoea and vomiting,' says the now 23-year-old. 'I went from 84kg down to 48kg in under a year. I was hospitalised for an unrelated issue, but that is when I had more blood tests and a colonoscopy, which led to me being diagnosed with Crohn's disease in December 2022.' [ Inflammatory bowel disease: 'I remember being in primary school and thinking maybe if I skip breakfast, my stomach won't hurt' Opens in new window ] Once diagnosed, living well with this condition is possible, but it does come with a cost, as medication, new dietary requirements and other treatments add up. Crohn's disease and colitis, two types of inflammatory bowel disease (IBD), affect about 40,000 people in Ireland. Research published last month showed that 60 per cent of people living with IBD face 'financial difficulty' as a result of their condition. The report, launched by Crohn's and Colitis Ireland, showed that patients spend an average of €3,252 annually on managing their condition including treatment and dietary needs , while 47 per cent have avoided seeking necessary medical care due to cost. Figures showed 62 per cent reported that taking time off work due to their condition has negatively affected their financial situation 'to some or a great extent'. Costs associated with attending medical appointments also play a significant role. Some 85 per cent said that mileage and travel costs added to their financial burden, 83 per cent had issues with parking fees, 62 per cent with overnight stays for medical appointments, and 49 per cent cited childcare fees as taking a toll financially. Dr Orlaith Kelly, consultant gastroenterologist at Connolly Hospital in Dublin, says Crohn's disease can be diagnosed at any age, but commonly presents in people aged 15-35, with a second wave of diagnoses occurring at 50-60. Treatment is not curative, but can control the disease, which can have very debilitating symptoms. 'Given the age profile of patients, IBD can often impact people when they are ordinarily at the height of their working lives or entering work life or parenthood,' says Dr Kelly. She says that 60 per cent of people living with IBD face financial difficulty directly because of their condition, while almost half have avoided necessary medical care because of the cost. 'In a cost-of-living crisis, for some, often it becomes a choice between paying for school uniforms, a utility bill and maybe forgoing medication or missing an appointment to work to do so,' she says. 'And this has obvious impact on patients' wellbeing and health down the track, possibly resulting in an emergency visit or more hospitalisation when the disease flares or is out of control.' It would be good for us to be able to enrol in the long-term illness scheme, or at least lowering the cost of the drugs payment scheme for us — Amy Dennis Amy Kelly, chief operating officer at Crohn's & Colitis Ireland, urges the Government to 'listen to the needs of the community and to improve access to care in Ireland'. 'This includes including Crohn's disease and ulcerative colitis in the chronic disease management programme and expanding medical card eligibility, ensuring equitable access to essential medical care,' she says. 'This inclusion would provide free, structured care, potentially reducing patients' out-of-pocket healthcare expenses through fewer emergency room visits, hospital admissions and lower medication costs.' According to Dr Orlaith Kelly, care for patients should cover access to more IBD physicians and multidisciplinary team members such as specialist nurses, psychologists and surgeons in a more timely and uniform manner across the country. This also includes timely access to intestinal ultrasound, endoscopy and MRI scans when needed and to infusions and theatre slots. 'This requires joined-up thinking at national programme level, provision of adequate resources to assess current needs through accurate data capture and adequate resourcing of this area in hospital and national health budgets,' she says. 'The truth is this type of strategy will in fact save money along with countless improvements in quality of life for the patients by avoiding the long-term complications and [emergency department] visits, which inadequate care or lack of access can so often lead to.' Dennis says this would be a welcome relief for herself and others with IBD. 'It would be good for us to be able to enrol in the long-term illness scheme, or at least lowering the cost of the drugs payment scheme for us,' she says. 'I shouldn't have to choose between putting petrol in my car or getting the food and medication which is necessary to control my condition.'

The new national children's hospital has attracted 'a great deal of adverse and often ill-informed comment', the chief operating officer of building contractor BAM has told the Minister for Health. In a letter to Jennifer Carroll MacNeill last month, John Wilkinson said 'all necessary resources' were 'being and will be deployed' to the project. The letter comes after the Minister wrote to the BAM Group in April seeking assurances it remained committed and able to meet the agreed programme for the hospital. The correspondence was partially released to The Irish Times under the Freedom of Information (FOI) Act. READ MORE The Oireachtas Public Accounts Committee (PAC) was told last month that BAM had extended the substantial completion date of the hospital from June to September 30th of this year. Patients are now not expected to be treated at the facility until June 2026 at the earliest. David Gunning, chief executive of the National Paediatric Hospital Development Board (NPHDB), the body overseeing the project, told the PAC that BAM had achieved about 60 per cent of its planned progress set out within its programme in the past seven months. In her letter to the company, dated April 15th, Ms Carroll MacNeill also sought assurances for early access to 'necessary areas' of the hospital that are completed to the required standards and finishes to 'support our shared ambition to open this hospital as soon as possible'. She said the NPHDB had informed her officials that actual progress had fallen behind what was required 'in order to meet your own programme, including completion and commissioning of those areas required for early access'. [ Numbers working on children's hospital project dropped by third since January, politicians hear Opens in new window ] In response, Mr Wilkinson said: 'As you will be fully aware, this is a project which has attracted a great deal of adverse, and often ill-informed, comment in the past but I hope that it is now becoming clear that the hospital, once fully completed, will make an exceptionally important and valuable contribution to children's health in Ireland for a long time to come.' Mr Wilkinson said it was 'correct' that actual progress had fallen behind programme. He said since meeting the then minister for health Stephen Donnelly in October 2024, BAM had received more than 60 further change orders and employer's requirements directions 'including several that require substantial additional works to be undertaken'. A change order is a document used to alter the original agreement on a construction project. Mr Wilkinson said under those circumstances 'it is inevitable that the project timeline extends'. 'Notwithstanding the disruption this creates, our project team has worked tirelessly to re-plan the revised work scope and minimise the overall delay, all in the interests of completing the project as early as possible,' he added. Mr Wilkinson also said BAM was committed to allowing Children's Health Ireland early access to the hospital. 'We believe that early access can begin in the coming weeks, if the current positive progress continues,' he said. The latest cost for the project was put at €2.2 billion with building on the site at St James's Hospital in Dublin beginning in 2016.
