
Invasive Lobular Carcinoma: Why It Deserves More Focus
Invasive Lobular Carcinoma (ILC) is the second most common type of invasive breast cancer, accounting for about 10-15% of all invasive cases. Of all breast cancers, ILC is notable for its frequency and biological differences which make it distinct from others.
Invasive Lobular Carcinoma, while related to the more prevalent Carcinomas, exhibits distinct characteristics in its growth, diagnosis, and response to treatment.
Ongoing breast cancer research continues to enhance the understanding of ILC's biological mechanisms, its variations across subtypes, and the optimization of treatment strategies. However, current clinical knowledge and the development of targeted therapies for ILC still present areas for improvement.
At the cellular level, ILC is defined by the loss of E-cadherin, a protein that helps cells stick together. ILC originates in the milk glands and spreads from these lobules. Without this molecule, ILC cells grow in single file lines rather than forming cohesive tumors like IDC [2], as IDC originates in the milk ducts.
This subtle and diffuse pattern makes it harder to detect on standard imaging tools like mammograms and ultrasounds. ILC begins in the terminal duct lobular units and the loss of E-cadherin is significant in its progression.
ILC is not a one size fits all diagnosis. It has several subtypes—classic, pleomorphic and solid—each with unique biological and clinical behaviors [4]. These variants can differ in how they look under the microscope, how aggressive they are and how they respond to treatment. More than 80% of ILC cases are hormone receptor positive, making estrogen and progesterone receptors significant markers in its characterization and treatment options.
One of the biggest challenges with ILC is that it often goes undetected until later stages. Because the tumor cells infiltrate the breast tissue without forming a lump, patients and doctors may miss early signs, delaying diagnosis and treatment.
Treatment Options
Although ILC behaves differently from IDC, they are often treated the same way—surgery, radiation, endocrine therapy and chemotherapy as needed [2]. But as we learn more about ILC, researchers are finding that a more tailored approach, considering hormone receptor status may lead to better results.
Hormone therapy is a treatment option for hormone receptor positive ILC, often used in combination with other therapies such as radiotherapy and chemotherapy.
One of the breakthroughs in this area is the use of genomic tools such as the 21-gene recurrence score (RS). This test determines whether a patient is likely to benefit from chemotherapy by evaluating the expression of 21 cancer related genes. While it was initially validated in IDC, it's being used more and more to guide treatment decisions in ILC as well [1].
Breast conserving surgery is also an option for ILC patients, especially when neo-adjuvant therapies are used to shrink the tumor before surgery.
Interestingly, ILC patients with high RS scores are less likely to receive chemotherapy compared to IDC patients with similar risk profiles. This may be due to the uncertainties around how ILC responds to chemotherapy or differing opinions among oncologists. Plus new treatments emerging from clinical trials are showing promise in addressing the unique challenges of lobular carcinoma.
These genomic tests are influencing treatment options, helping to tailor therapies more precisely to the individual characteristics of the cancer.
When it comes to results, ILC holds its own against IDC but there are nuances. Prognostic factors play a big role in treatment results for ILC patients. For patients with N1 ILC and high RS, chemotherapy does seem to offer a survival benefit. This is a key finding as it reinforces the value of using genomic tests to individualize care.
Early detection of ILC makes a big difference in survival rate, with localized cases having a 99% 5 year relative survival rate. But the survival rate declines as the stage of cancer progresses.
Also, researchers haven't found significant differences in local recurrence, distant metastasis or overall survival between ILC subtypes [5]. So while subtypes may behave differently biologically, they don't necessarily translate into different prognosis—at least with current treatment. If the cancer has spread into nearby tissue or lymph nodes, the prognosis is worse, highlighting the importance of staging in determining patient outcomes.
ILC presents several clinical and research challenges.
Advanced Diagnosis: The subtle spread of ILC means it's often found at later stages when treatment is more complex. Conventional imaging can miss ILC lesions, so there's a need for advanced imaging and potential use of neoadjuvant therapies—treatments given before surgery to shrink tumors [3].
The challenges in detecting ILC mean we need to use multiple diagnostic tests beyond standard mammograms. MRI is particularly useful in producing internal images, helping to assess the size and extent of the tumor.
Conflicting Results: While some studies show ILC patients do as well as IDC patients, others show worse long term results. This may be due to underrepresentation of ILC in clinical trials or lack of treatment guidelines [5]. ILC is often diagnosed at an advanced stage, which complicates treatment, so early detection and specialized care is key.
Personalized Treatment: Given the heterogeneity of ILC, researchers are advocating for more personalized approaches. That means considering not only the subtype but also the molecular profile—factors that may one day lead to more effective, less toxic therapies.
Tailored ILC treatment is crucial due to the unique challenges of this rare cancer, requiring a multidisciplinary approach and ongoing research to improve outcomes.
There's a growing consensus that ILC deserves the same research attention and funding as IDC. Dedicated clinical trials and precision medicine approaches could make a big difference in care for this overlooked breast cancer subtype.
Invasive Lobular Carcinoma is a distinct yet under-recognized breast cancer, with research focusing on the behavior of breast cancer cells in this subtype. Its diffuse pathology, subtle imaging and varied treatment responses make it different from the more common IDC. Cancer cells are key to ILC progression and diagnostic and treatment decisions.
While traditional protocols have lumped them together, tools like the 21-gene recurrence score are helping clinicians make more personalized decisions for ILC patients. But there's still work to be done, especially in early detection and outcome prediction.
The way forward requires ILC-specific trials, molecular research and diagnostic innovations so patients get care that matches their disease. Progress in general cancer research and treatment will help with the complexities of all cancers, including ILC.

Originally appeared on E! Online A 29-year-old woman is speaking out about her rare condition that causes excessive vomiting. Sharisse Zeroonian has been diagnosed with cyclic vomiting syndrome, a chronic disorder that can make her throw up as often as 15 times an hour during a severe episode. "The biggest thing was that people wouldn't understand that it's a disorder," she said in a Today interview published June 13. "They'd just think it was me being overdramatic." The ordeal started for Zeroonian in 2021, when she felt extreme nausea during a time of stress, anxiety or her period, leading her to relentlessly throw up for days on end. She spent the next two years going through a gauntlet of medical tests—including an endoscopy and a gastric emptying study to measure how quickly food passed through her digestive tract—before finally learning what was wrong. According to the Cyclic Vomiting Syndrome Association, patients are often misdiagnosed with the stomach flu or food poisoning since there are no procedures to test for the disorder. Doctors can only diagnose the condition, which can flare up at any age, by ruling out other diseases. More from E! Episodes may recur several times a month or several times a year," the CVSA noted on its website. "One mother aptly described her child's state during the episode as a 'conscious coma.' The symptoms are frightening to the person and family, and can be life-threatening if delayed treatment leads to severe dehydration." Fortunately for Zeroonian, she now takes an antipsychotic drug and an anti-nausea drug to help manage her symptoms. While she still experiences vomiting episodes once in a while, she told Today that they're shorter and less intense than before. In fact, she's even making a self-described "vom-com" centered around her condition. Titled Nothing Solid, the movie is loosely based on Zeroonian's own health journey. 'Being open about it and making this movie has helped so much because I think I suffered a lot more when I was not being open," she added. "I felt a lot more liberated once I started talking to people about it." And she won't be the first in Hollywood to a shine light on the disease. Grey's Anatomy tackled the rare condition in 2012. The season nine episode—titled 'Second Opinion"—was directed by actress Chandra Wilson (a.k.a Dr. Miranda Bailey) and was inspired by her daughter, who was diagnosed with CVS. 'She would get these terrible bouts of vomiting and stabbing abdominal pains,' Wilson told People in 2017. 'I thought, 'This was crazy.' Barry Sanders reveals he suffered heart attack last year, hopes to raise awareness
Barry Sanders revealed in an interview with CBS Sports he suffered a heart attack last June during Father's Day weekend while on a recruiting visit for his son. At the time, the former Detroit Lions superstar running back publicly announced on social media he had experienced a heart-related "heath scare," although Sanders has since opened up about the medical emergency in "The Making of a Heart Attack," an A&E special that will premiere Saturday at 1 p.m. ET and will highlight the hidden risks of heart attacks and strokes. Sanders explained in the interview that he woke up that day with a burning sensation in his chest. Before his son's recruiting visit wrapped, he drove himself to the emergency room, where tests indicated that Sanders' enzyme levels were alarmingly high and still increasing. A heart catheter procedure determined that Sanders had actually suffered a heart attack. "Of all things, I don't know why, that just never entered my mind," Sanders said, via CBS Sports. "I'm learning through this process that there aren't necessarily any warning signs, unless you do what we're encouraging people to do, which is to go the doctor, get tested for LDLC levels, or bad cholesterol. That's the only way to find out if you have high cholesterol. It's not something you're going to be able to feel. You don't have to fit a certain physical profile." Sanders' experience will be unpacked in the documentary, as will the stories of other heart attack and stroke survivors. While everyone's heart health is different, Sanders is encouraging people to talk to their doctors about it. Foodborne illnesses increase during the summer. A doctor explains how to keep safe

As a salmonella outbreak linked to eggs has sickened 79 people across seven states as of June 5, a California-based company has recalled 1.7 million dozen eggs because of potential contamination, according to the US Food and Drug Administration. This rash of illnesses follows a separate outbreak of salmonella infections tied to recalled cucumbers and the US Department of Agriculture's Food Safety and Inspection Service issuing a public health alert for raw ground beef products that could be contaminated with E. coli. As summer approaches, I wanted to understand more about these foodborne infections. What are common causes and symptoms? What are possible consequences of salmonella and E. coli infections? Why are foodborne infections more common in the summer, and what are food safety practices that everyone should follow? To help me with these questions, I spoke with CNN wellness expert Dr. Leana Wen. Wen is an emergency physician and adjunct clinical associate professor at George Washington University. She previously served as health commissioner of Baltimore, where her responsibilities included overseeing food safety. CNN: What are the most common causes of foodborne infections? What symptoms do people experience? Dr. Leana Wen: Foodborne infections are caused by pathogens, including viruses, bacteria and parasites. Norovirus is the leading cause of foodborne illness in the United States. The virus is implicated in about half of all food-related illnesses, according to the US Centers for Disease Control and Prevention. The pathogen can spread through contaminated food and drink or from person to person when someone touches objects that have been handled by an infected individual and then touches their mouth, or through sharing cups, bowls and utensils. A number of bacteria can also cause foodborne illness. Infections can occur from eating raw or undercooked food. Other illnesses occur due to bacterial contamination in food. Common bacteria implicated in foodborne illness include campylobacter, listeria, salmonella and E. coli. Symptoms of foodborne infections include nausea, vomiting, diarrhea and abdominal cramping. Depending on the cause and on the health of the individual, other symptoms may develop such as fevers, bloody diarrhea and dehydration that worsens underlying health problems. CNN: What are possible consequences of salmonella and E. coli infections? How are they treated? Wen: Salmonella most often spreads to people through contaminated food. While most people who fall ill recover quickly with no long-term consequences, some can become very ill. In the current outbreak involving the California-based egg distributor, 21 people have been hospitalized out of the 79 people sickened, according to the CDC. The key to treatment is hydration. Individuals with diarrhea should keep up fluid intake to prevent dehydration. Those who are vomiting and unable to keep fluids down may need to take anti-nausea medications. Most people will get better without using antibiotics, but they may be needed for individuals at risk of severe illness. E. coli lives in the intestines of humans and many animals. A specific type of E. coli is known to cause a serious infection. This strain, called O157:H7, produces a toxin that can damage the intestinal wall and lead to bloody diarrhea. This is the strain implicated in the recent ground beef recall. The specific danger of O157:H7 is that some people who fall ill from it could develop a complication called hemolytic uremic syndrome, or HUS. This syndrome can lead to permanent health problems such as kidney failure, and in some cases, it is fatal. Individuals who develop an O157:H7 infection should also drink plenty of fluids to prevent dehydration from diarrhea. Antibiotics are specifically not given for these infections because they may increase the risk of developing HUS. Most foodborne infections can be treated at home. Reasons to contact your doctor include inability to keep down fluids, persistent diarrhea over three days, high fevers and bloody stools. Babies, older people and individuals with chronic medical conditions should have a lower threshold for seeking help. CNN: Why are foodborne infections more common in the summer? Wen: The risk of foodborne illness is higher during summer months because bacteria grow more quickly in warmer weather. In addition, people are more likely to have barbecues and outdoor events that involve food. Inadequate preparation or leaving out food for too long can lead to foodborne illnesses. CNN: What are food safety practices that everyone should follow? Wen: It's important to abide by recall notices. If you have purchased a product that is now recalled, do not consume it. Follow other instructions as directed by health authorities. Keep an eye on not only federal advisories but your local health department, which will inform you of possible foodborne illnesses associated with local stores and restaurants. Follow best practices in food preparation. That includes washing produce such as vegetables and fruits, even if they end up getting peeled. Make sure to cook meat and fish to the recommended temperature. Be aware of what utensils and plates you are using with raw meat and fish, and don't use them for other food items. Remember that bacteria multiply quickly at room temperature. Two hours is the limit for leaving food out before throwing it away or storing it in a refrigerator or freezer. If you are outdoors and the ambient temperature is very warm, reduce the limit to one hour. It's also a good idea to avoid unpasteurized or 'raw' milk and uncooked eggs. Of course, be sure to wash your hands often and well with soap and water. People who have diarrhea, vomiting and stomach cramps should not prepare food. Those most vulnerable to severe illness, including older adults, babies, and those who are pregnant or have serious underlying conditions, should be especially careful of foodborne illness as they are of other infections.
