Monday briefing: How the Air India ​crash ​happened​ and why ​it ​raises ​questions ​about ​aviation ​safety

Good morning. An Air India plane bound for London Gatwick took off from Ahmedabad, India, at 1.38pm local time last Thursday. On board were 242 people. Within moments of takeoff, the airliner crashed, sending up a fireball of exploding jet fuel.
Initial reports suggested all passengers and crew had perished. Then, miraculously, there was one survivor – a British man, Vishwash Kumar Ramesh, who while badly injured was able to walk out of the wreckage by himself. Ramesh, who was returning to his family in London, told the Hindustan Times: 'I don't know how I survived. I saw people dying in front of my eyes – the air hostesses, and two people I saw near me … I walked out of the rubble.'
Air India said the passengers on board the Boeing 787 Dreamliner were made up of 169 Indian nationals, 53 British nationals, one Canadian and seven Portuguese nationals. Footage shows the plane flying low over a residential area before disappearing behind buildings followed by an explosion. At least 24 people on the ground died as the flight crashed into a hostel housing medical students in a crowded residential area of Ahmedabad.
A black box was recovered from the crash site last Friday. The UK Foreign Office said officials were working with Indian authorities to establish the facts around the crash and provide support to those involved.
The Indian prime minister, Narendra Modi, said he was stunned and saddened while the British prime minister, Keir Starmer, called the crash 'devastating'.
There is still much we don't know in the days since the crash, most importantly about why it happened. To help put things into context, I spoke to Gwyn Topham, the Guardian's transport correspondent, about the Boeing airliner involved and why the crash is likely to renew scrutiny of the manufacturer. That's after the headlines.

The plane crash is the deadliest involving Britons in recent years, casting a shadow of mourning over large parts of the UK.
Gareth Thomas, MP for Harrow West and the chair of the APPG on British Gujarati, told First Edition: 'Harrow is home to a large British-Gujarati community, many of whom have close family ties to Gujarat, and this devastating news will be felt particularly strongly here.'
Among those confirmed dead are businessman Akeel Nanabawa, his wife, Hannaa Vorajee, and their four-year-old daughter Sara (pictured above), according to the Gloucester Muslim community.
Also killed in the crash were Fiongal and Jamie Greenlaw-Meek, who ran the Wellness Foundry in south London and Ramsgate. The business offered psychic readings, tarot, reiki, and yoga.
Adam Taju, 72, and his wife, Hasina, 70, were also on board, along with their son-in-law, Altafhusen Patel, 51, who lived in London with his wife.
Javed Ali Syed, a hotel manager at the Best Western Kensington Olympia Hotel, and his wife, Mariam, were also reportedly among the victims.
What do we know about the Boeing 787-8 Dreamliner?
The Air India plane that was headed to London was a Boeing 787 Dreamliner. This model of plane has been in service since 2011. There are more than 1,100 787s in service worldwide, used by most major international airlines. This is the first time one has crashed.
'It was seen as revolutionary when it came in because it was so much lighter,' Gwyn Topham explains. 'People called it the 'plastic plane' at the time because of the composite materials. That made it more fuel-efficient, quieter, and generally a nicer plane to be on. It doesn't feel that novel now, but at the time it was a leap forward.'
Gwyn adds: 'There have been issues over the years. About a decade ago a lithium-ion battery caught fire on a plane parked at Heathrow, which was obviously alarming. And in 2013 there were a couple of electrical fires in Japan that led to the aircraft being grounded briefly. But nothing since that time that would make most people worry about flying this plane.'
He notes that while a whistleblower last year urged Boeing to ground all 787 Dreamliners worldwide during Washington hearings, Boeing rejected the claims.
Air India, which operates a fleet of Dreamliners, has been using the US-built long-haul plane since 2012. This is the airline's first serious incident since 2020, when a smaller Boeing 737-800 operated by Air India Express skidded off the runway on landing during bad weather.
Tata Group, the parent company of Air India, said it would provide 10m rupees (£86,000) to the families of those killed in the crash. The company also said it would cover the medical costs of those injured and provide support in the 'building up' of the medical college hit by the plane.
Further scrutiny on Boeing
This latest crash piles further pressure on Boeing, which has faced significant scrutiny over the past few years due to a series of production issues and safety concerns.
Most notable are the two major crashes that were due to faults on Boeing's 737 Max planes, in Indonesia and Ethiopia in 2018 and 2019. That model was taken out of service for almost a year, before being relaunched and returned to widespread use.
Last month, Boeing agreed to pay $1.1bn (£812m) in a deal with the US justice department to avoid prosecution over these two crashes, which together killed 346 people. The families of the crash victims described the deal as 'morally repugnant'.
'That really shocked the industry and the public. At first, many didn't want to believe it could be the plane and people thought it might be pilot error. But it turned out to be Boeing's fault, and that severely damaged trust,' Gwyn says.
'Since then, every Boeing incident, from relatively minor ones to the door panel that blew off on an Alaska Airlines flight last year, ends up getting massive attention. Ryanair has even complained publicly about receiving planes with faults.
'Even if this crash turns out to have nothing to do with the aircraft itself, it still puts a fresh spotlight on Boeing.'
What impact will this have on the aviation industry?
Hours after the crash, flights were allowed to resume at the airport in Ahmedabad. Despite these high profile crashes, Gwyn doesn't expect passengers to be deterred from flying.
'It's important to remember that aviation is very safe. I think 2023 had no fatal passenger jet crashes at all. When crashes do happen, they're usually the result of a combination of rare and unexpected factors,' he says.

As for the impact on passenger confidence, he adds: 'Passenger numbers have kept rising year after year. Even when Ryanair offered refunds to passengers booked on the 737 Max, no one took them up on it. So while something like this may shake confidence in the short term, it doesn't usually translate into people flying less.'
While public confidence in air travel is likely to endure, the aviation industry will once again be forced to engage in serious soul-searching over safety, accountability and public trust.

Keir Starmer and Ukrainian president Zelensky will this week try to pressure Donald Trump to hit Russia with fresh sanctions if Vladimir Putin continues to resist calls for an unconditional ceasefire in Ukraine. The Prime Minister said targeting Moscow with further action would 'undoubtedly' form part of talks at the international summit this week unless the Kremlin engages in peace talks. Mr Zelensky is expected to attend the conference in Canada , where leaders from the world's major economies are descending on a luxury mountain lodge in the Rockies for talks on global security. And Sir Keir is expected to hold talks with Mr Trump, who at the weekend told Putin over the phone that the war in Ukraine 'should end'. Mr Zelensky has accepted a US-backed ceasefire proposal and offered to talk directly with his counterpart in Moscow, but the Russian leader has so far shown no willingness to meet him. Speaking to reporters traveling with him to Kananaskis, Sir Keir was asked if he expects additional sanctions on Russia to be agreed by international partners. 'We've got President Zelensky coming, so that provides a good opportunity for us to discuss again as a group,' he said. 'My long-standing view is, we need to get Russia to the table for an unconditional ceasefire. 'That's not been really straightforward. But we do need to be clear about we need to get to the table and that if that doesn't happen, sanctions will undoubtedly be part of the discussion at the G7.' Sir Keir faces a busy week of diplomacy with spiraling conflict in the Middle East and the war in Ukraine top of the agenda in the talks between leaders from Canada, the US, France, Italy, Japan, Germany and Britain. It comes as the Foreign Office advised against all travel to Israel following further retaliatory attacks by Iran. Downing Street has not ruled out moving to evacuate UK nationals from the country if the situation deteriorates. In his first bilateral meeting at the summit, the Prime Minister and his Italian counterpart Giorgia Meloni reiterated their 'enduring support' for Kyiv. '(They agreed) that it is a topic of our common security that they looked forward to discussing in the next two days,' a Downing Street readout said, describing the conference as coming at a 'vitally important moment for the world'. Sir Keir is also likely to have talks with Emmanuel Macron, with whom he has been making plans for a so-called 'coalition of the willing' to send peacekeeping forces to Kyiv. The Prime Minister told reporters he had a 'good relationship' with the US president, which is 'important' at a time of heightened global instability. 'I've been saying, for probably the best part of six months now, we're in a new era of defense and security, a new era for trade and the economy,' he said. 'And I think it's really important for Britain to play a leading part in that, and that's what I'll be doing at the G7, talking to all of our partners in a constructive way.

Taxpayers will foot a £239m bill for a new civil servants' pension scheme administrator despite the company's 'failing' track record, its critics have said. Outsourcing giant Capita will run the Civil Service Pension Scheme from December, which involves handling £7bn in pension contributions and making payments of almost £8bn a year to retirees. However, the Cabinet Office has already withheld almost £10m in contractual payments after the company missed vital deadlines while taking over from the scheme's current administrators, a National Audit Office (NAO) report said. Capita previously administered the Teachers' Pension Scheme for 27 years, but Government officials opted to switch to a new provider in 2023 amid a string of delays. Earlier this year it emerged teachers had been left waiting months for pension payouts, while some were unable to file for divorce due to administrative failures. Capita said at the time that it was experiencing delays in calculating cash equivalent transfer values, which provide a cash value for the pension someone has built up and are often requested by people transferring it elsewhere or getting divorced. The provider is now facing legal action over the backlog. There are 1.7 million members of the Civil Service Pension Scheme, which has almost £189bn in pension liabilities. Six missed milestones already The seven-year contract was awarded in November 2023 and Capita will take over running the scheme from December. The terms include a two-year transition from the current administrator MyCSP. However, MPs said Capita had already missed three of its six milestones during the transition period, and the Cabinet Office subsequently withheld £9.6m in payments. In its role, Capita will be required to maintain members' records, calculate and pay pensions, manage contributions and deal with queries from members and employers. The outsourcing giant will now operate a simplified IT solution to avoid further delays, reducing the service offered to both scheme members and employers until at least March 2026, the NAO said. Capita previously spent 27 years running the Teachers' Pension Scheme, beginning in 1996. It received multiple renewals that cost taxpayers more than £170m, but the Department for Education handed the new 10-year £223m contract to Indian IT company Tata in June 2023. Last year Capita also lost its £107m contract for managing SATs tests. 'Track record of losing contracts' Shimeon Lee, of the TaxPayers' Alliance said: 'It's hard to believe that such a massive pension scheme has been handed to a company with a track record of losing contracts. 'While Civil Service pensions do desperately need reform, it's vital that the system itself is robust and efficient. The Cabinet Office must get tougher on poor performance and handing taxpayers' cash to failing contractors who can't deliver.' A Capita spokesperson said: 'Capita is proud to be working in partnership with the Cabinet Office to modernise the administration of the Civil Service Pension Scheme from December 2025. 'While the NAO report reflects the status of our transition to scheme administrator in May, we have since met the referenced milestones and are on track to deliver enhanced, innovative services for members for when the contract commences. 'We remain committed to offering seamless, tailored experiences to all Civil Service Pension Scheme members.'

Sir Keir Starmer has stood by his claim that Tories calling for a national inquiry into grooming gangs were 'jumping on the bandwagon of the far-Right'. Downing Street's official spokesman said the Prime Minister had been 'specifically' referring to Tory ministers who had 'done nothing' to tackle the grooming gang scandal while in Government. The comments came after Sir Keir announced an about-turn to approve a national inquiry into the scandal after repeatedly rejecting demands from senior Tories, campaign groups and victims. He said he had changed his mind after a national inquiry was recommended by Baroness Casey following her review into the scale and nature of grooming gangs. He added that it was the 'right thing to do' after having read 'every single word of her report.' Asked to justify his criticism after the about-turn, Sir Keir's spokesman said: 'The Prime Minister's comments about bandwagons were specifically about ministers from the previous government who sat in office for years and did nothing to tackle this scandal. 'And as the Prime Minister said: 'We will not make the same mistake.' 'And those cheerleading for Tommy Robinson, who was jailed for almost collapsing a grooming case, are not interested in justice, and members of the previous government did not implement any of Alexis Jay's recommendations, but then called for an inquiry when it became politically convenient. 'The Prime Minister also told Parliament in January that, again, 'reasonable people can agree or disagree on whether a fear of the inquiry is necessary' but he has always been focused on getting on with getting justice for the victims of child sexual abuse.'
