Recognized for Outstanding Outcomes Over the Past Year
GRANVILLE, N.Y., June 4, 2025 /PRNewswire/ -- Granville Center, a Centers Health Care facility providing rehab, short-term care, and long-term care alongside comfortable accommodations and refined amenities, recently received a five-star rating in nursing home quality from the Centers for Medicare & Medicaid Services (CMS). CMS created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare nursing homes more easily, and facilities with five stars are considered to have significantly above average quality.

Lowering the hospitalization rate of residents from 24% to 15% and implementing new intervention and documentation procedures in place to maintain newer, stronger standards;
De-escalation of behavioral issues lowering the use of antipsychotic medications to 2.2% (compared to the national average of 14.6%);
No reported urinary tract infections or catheter use (compared to the national averages of 1.9% and 1.3%, respectively).
In addition to optimizing clinical procedures, Granville Center has dedicated significant resources to improving the quality of life of its residents, including introducing upbeat music during the day and hosting novel events to engage families and the local community.
