The U.S. is approaching a dangerous measles precipice, scientists say

In December, Stanford School of Medicine colleagues Dr. Nathan Lo and Mathew Kiang got to talking.
Childhood immunization rates were slowly but steadily falling nationwide, from 95% in the years before the pandemic to less than 93% in the 2023-24 school year.
If even that relatively small decline in vaccinations for measles, mumps and rubella (MMR); diphtheria, tetanus and pertussis (DTaP); polio; and varicella held, they wondered, what would U.S. infectious-disease prevalence look like in 10 years, or 20? What would happen if vaccination rates went up by a little bit, or fell by a whole lot?
Lo and Kiang put together a statistical model representative of the U.S. population and ran the results.
They found that if current vaccination rates hold steady over coming decades, measles — currently spreading in many parts of the country, but primarily in the Southwest — will once again be endemic in the U.S. within 25 years.
Their results were published Thursday in the Journal of the American Medical Assn.
Measles was technically 'eliminated' in the U.S. in 2000, meaning the disease became rare enough — and immunity widespread enough — that even if a case or two were to arise within a community, local transmission would quickly grind to a halt. In the 25 years since, there have been 10,570 measles cases, including the 800 people sickened in the outbreak that began in west Texas in January.
But at current vaccination rates, Lo and Kiang estimate there could be as many as 851,300 measles cases between now and 2050. By their calculations, in that time frame, more than 170,000 people will be hospitalized, and nearly 900 will experience debilitating and potentially deadly neurological complications. And some 2,550 people will die.
We wouldn't be back in the pre-vaccine days, when measles affected more than 4 million people annually and regularly claimed the lives of at least 400 U.S. children per year. But the disease would once again become endemic, meaning ever-present at some baseline level, like influenza, which sickens millions and kills thousands in the U.S. every year.
'Right now we should really be trying to up vaccination rates,' said Kiang, an assistant professor of epidemiology and population health. 'If we just kept them the way they are, bad things are going to happen within about two decades.'
Other vaccine-preventable diseases would also probably pop up in the coming quarter-century — 190 cases of rubella, 18 of poliomyelitis, eight of diphtheria, according to the Stanford team's models.
But none would be as likely to come roaring back as measles, far and away the most infectious of the bunch.
Parents could no longer reliably count on herd immunity to keep newborns too young to vaccinate safe from the disease. Pediatricians and emergency room doctors would see patients with measles-related complications they'd likely never encountered in their training or careers.
'I've read a reasonable number of these kinds of papers, and I think that this one is exceptional,' said Dr. Adam Ratner, a New York City pediatric infectious-disease specialist. 'The numbers of cases and adverse outcomes from measles and other vaccine-preventable diseases that they estimate are extremely concerning. These are diseases that U.S. families are not used to thinking about or seeing, and they may become commonplace again in the near future unless we reverse course.'
Even relatively small increases in childhood immunization would prevent that scenario, said Lo, an assistant professor of infectious diseases. If MMR vaccine rates went up by 5%, the country would see only about half as many measles cases in the next 25 years that it saw in the last 25.
'A small fraction of the population here can really make the difference in terms of tipping us into more safe areas,' he said.
Both authors said they considered a further decline in vaccination rates the more likely outcome. Since they first conceived of the study, President Trump took office and appointed as secretary of Health and Human Services Robert F. Kennedy Jr., a longtime critic and skeptic of established vaccine science.
After promising during confirmation hearings that he would not alter the U.S. childhood vaccinations schedule, Kennedy told HHS employees in February that he plans to investigate childhood immunizations.
Further declines in vaccination rates could have significant consequences, the researchers found. Were vaccination rates to fall another 25% from where they are today, the U.S. would see 26.9 million measles cases between now and the end of 2050, along with 80,600 deaths from measles, rubella, polio and diphtheria combined.
'To put this in perspective, most physicians in the U.S. haven't seen a single case of any of these diseases because we have very effective vaccines,' said Dr. Kristina Bryant, a pediatric infectious diseases physician at Norton Children's Hospital in Louisville, Ky.
At a 50% drop, a once-unthinkable scenario, the disease would become endemic again within five years. Over the next 25 years, there would be 51.2 million cases of measles, 9.9 million cases of rubella and 4.3 million cases of poliomyelitis. More than 159,000 people would die from vaccine-preventable diseases. Some 51,000 children would have life-altering neurological complications, and 5,400 would be paralyzed by polio — a disease for which there have been no cases reported since 1993.
A decline of that magnitude in vaccination rates 'would really take something quite unprecedented,' Kiang said. But when it comes to public health, he said, 'what we've learned over the past few months is that our imagination needs to be larger for what is possible.'

Someone with measles can spread the virus four days before and after a rash appears. Measles spreads by breathing in the virus that travels on droplets after people cough or sneeze. It is less common to get measles from objects contaminated with infected droplets, according to Iowa HHS. People can spread measles for several days before getting a rash and several days after the rash develops. It is one of the most contagious infections known to exist, according to Iowa HHS. Should Adults Update Their Measles Vaccinations?

Most healthy adults who have followed recommendations for measles vaccination are not in need of additional vaccination, despite recent outbreaks in certain parts of the US, according to experts. 'A complete two-dose measles vaccination provides 97% protection for a lifetime,' said William Schaffner, MD, professor of medicine in the Division of Infectious Diseases at Vanderbilt University, Nashville, Tennessee, in an interview. 'Measles infections that occur despite having received two doses of vaccine are unusual and, when they occur, usually are rather mild,' he added. 'The only measles vaccine available in the US is MMR [measles, mumps, and rubella]; there is no stand-alone measles vaccine,' Schaffner told Medscape Medical News . What the CDC Says The CDC currently recommends two doses of the MMR vaccine for all children, with a first dose at age 12-15 months and a second at age 4-6 years. One high-risk group for whom the CDC recommends MMR vaccination is young adults at college and other post-high school institutions who lack evidence of immunity — they should receive two doses at least 28 days apart. Other adults without evidence of immunity should receive two doses at least 28 days apart if they are in high-risk settings, such as healthcare or international travel. Otherwise, low-risk, healthy adults without evidence of immunity should receive at least one dose of MMR, according to the CDC. The evidence of immunity against MMR is defined by the CDC as at least one of the following: Documentation of adequate vaccination, laboratory confirmation of disease, laboratory evidence of immunity against MMR, or birth before 1957. However, certain groups including pregnant women, individuals with immunocompromising conditions, and those with severe allergies to any of the vaccine components should not receive an MMR vaccine, according to the CDC. Clinical Perspective Prior to 1989, a single dose of measles vaccine was standard practice, said Shirin A. Mazumder, MD, an infectious diseases specialist in Memphis, Tennessee, in an interview. 'One dose of the measles vaccine is approximately 93% effective, and two doses of the vaccine increase the level of protection to 97%,' she said. Individuals born after 1957 and vaccinated between 1963 and 1967 likely received a different version of the current MMR vaccine that was inactivated and less effective, Mazumder noted. 'Individuals vaccinated between 1963 and 1967 can consider receiving an additional vaccine dose to increase their level of protection,' she said. The risk assessment for measles vaccination, as with other vaccinations, should be an open discussion between clinicians and their adult patients, Mazumder told Medscape Medical News . 'In otherwise healthy adults, the measles vaccine is well tolerated and highly effective,' she said. Some adverse effects associated with the vaccine include fever and transient rash, reported in 5% or fewer of adult recipients, as well as transient lymphadenopathy and arthralgias, reported in approximately 20% and 25% of recipients, respectively. ACIP's Impact 'The dismissal of all the sitting members of Advisory Committee on Immunization Practices (ACIP) may have a damaging impact on public health in this country and dismantle a transparent process that has been in place for a long time and saved countless lives,' said Mazumder. 'Traditionally, ACIP was made up of an independent and diverse group of advisors, and the previous meetings were open to the public with a very transparent process,' she told Medscape Medical News . 'Physicians rely on the decisions made by ACIP to help guide the recommendations passed on to their patients, and there is a concern that the ongoing changes will make it harder for clinicians to care for their patients,' she said. Decisions made by ACIP shape immunization schedules and also impact insurance coverage, which affects public access to vaccines, Mazumder added. Overall, the potential impact of the newly reconstituted ACIP on adult measles vaccination, and on vaccination in general, remains to be seen, Schaffner told Medscape Medical News . 'We need to give this some time and be prepared to continue to give our good advice to our patients,' he said.

NEW YORK — All 17 experts recently dismissed from a government vaccine advisory panel published an essay Monday decrying 'destabilizing decisions' made by U.S. Health Secretary Robert F. Kennedy Jr. that could lead to more preventable disease spread. Kennedy last week announced he would 'retire' the entire panel that guides U.S. vaccine policy. He also quietly removed Dr. Melinda Wharton — the veteran Centers for Disease Control and Prevention official who coordinated the committee's meetings. Two days later, he named eight new people to the influential panel. The list included a scientist who criticized COVID-19 vaccines, a leading critic of pandemic-era lockdowns and someone who worked with a group widely considered to be a leading source of vaccine misinformation. 'We are deeply concerned that these destabilizing decisions, made without clear rationale, may roll back the achievements of U.S. immunization policy, impact people's access to lifesaving vaccines, and ultimately put U.S. families at risk of dangerous and preventable illnesses,' the 17 panelists wrote in the Journal of the American Medical Assn. The new committee is scheduled to meet next week. The agenda for that meeting has not yet been posted, but a recent federal notice said votes are expected on vaccinations against flu, COVID-19, HPV, RSV and meningococcal bacteria. In addition to Wharton's removal, CDC immunization staff have been cut and agency experts who gather or present data to committee members have resigned. One, Dr. Lakshmi Panagiotakopoulos, resigned after 12 years at CDC, disclosing her decision early this month in a note to members of a COVID-19 vaccines work group. Her decision came after Kennedy decided — without consulting the vaccine advisers — to pull back COVID-19 vaccination recommendations for healthy children and pregnant women. 'My career in public health and vaccinology started with a deep-seated desire to help the most vulnerable members of our population, and that is not something I am able to continue doing in this role,' she wrote in a message viewed by the Associated Press. Those CDC personnel losses will make it hard for a group of new outside advisers to quickly come up to speed and make fact-based decisions about which vaccines to recommend to the public, the former committee members said. 'The termination of all members and its leadership in a single action undermines the committee's capacity to operate effectively and efficiently, aside from raising questions about competence,' they wrote. A spokesperson for the U.S. Department of Health and Human Services did not respond to the JAMA commentary, but instead pointed to Kennedy's previous comments on the committee. Kennedy, a leading voice in the anti-vaccine movement before becoming the U.S. government's top health official, has accused the committee of being too closely aligned with vaccine manufacturers and of rubber-stamping vaccines. The Advisory Committee on Immunization Practices, created in 1964, makes recommendations to the CDC director on how vaccines that have been approved by the Food and Drug Administration should be used. CDC directors almost always approve those recommendations, which are widely heeded by doctors and guide vaccination programs. ACIP policies require members to state past collaborations with vaccine companies and to recuse themselves from votes in which they had a conflict of interest, but Kennedy has dismissed those safeguards as weak.
