
AI Identifies Novel Predictors of TB in People With HIV
An artificial intelligence (AI) model using routinely collected data predicted subsequent development of active tuberculosis (TB), Swiss researchers reported. The AI model outperformed biological tests for latent TB in identifying HIV-positive patients at high risk of developing TB.
As well as immune function and sociodemographic variables, the AI model retained several biomarkers indicative of patients' well-being and metabolism.
In Switzerland and other countries with good access to antiretroviral therapy, TB is a rare but serious co-infection in people living with HIV, frequently linked with late HIV diagnosis. To prevent progression to active TB disease, people known to have latent TB infection can be offered preventive treatment with isoniazid and/or rifampicin.
But detection of latent TB is challenging, especially in people with HIV. In a previous Swiss analysis, a combined approach using interferon gamma release assays (IGRA) and tuberculin skin tests identified only 30% of people who subsequently developed active TB.
'It was worse than tossing a coin,' Joahnnes Nemeth, MD, an attending physician in the department of infectious diseases and hospital epidemiology at the University of Zürich, Zürich, Switzerland, told Medscape Medical News.
The problem is that the tests rely on immune response, which may be impaired. 'You interrogate the very system that is malfunctioning during HIV infection, so it's not a surprise that the tests perform poorly,' he explained.
This led him and his colleagues to look into alternative ways to identify patients at risk. They leveraged data from the Swiss HIV Cohort Study, which includes around 70% of people receiving HIV care in the country.
Over 23 years' worth of data were analyzed using machine learning, a subset of AI that enables computers to learn patterns from data and make predictions without being explicitly programmed for each task. Their machine learning model employed a random forest — an algorithm which combines the outputs from multiple decision trees.
The model looked at data collected at HIV diagnosis in order to predict active TB disease that developed at least 6 months later. Rather than only considering variables which the researchers thought were potential risk factors, the model reviewed all the variables for which they had sufficient data.
'What I really liked about this machine learning approach is that we threw all the data we collect into the machine and just asked it: Can you do something with that?' Nemeth said. 'I think that really paid off.'
The first iteration of the model included 48 variables and had a sensitivity of 70.1% and a specificity of 81.0%. A streamlined second version retained 20 variables — making it computationally less demanding — while delivering a sensitivity of 57.1% and specificity of 77.8%.
Given that biologic tests had a sensitivity of 30% and specificity of 94%, for Nemeth this 'blows everything of the water.' The model doesn't require additional data collection or have the expense of IGRA.
As might be expected, the 20 retained variables included immunological parameters, hematological markers, and sociodemographic factors, but some were more surprising: along with several variables linked with metabolism (cholesterol, high-density lipoprotein, glucose, and creatinine), body mass index, and mean arterial pressure.
The researchers noted that TB is associated with malnutrition and said that some of these markers may reflect metabolic perturbations and compromised muscle mass in people at risk for TB.
The model was first validated on a portion of the Swiss cohort which it was not trained on, and then on a cohort in Austria. Despite the many parallels between the two cohorts, initially the model performed badly in Austria.
The researchers realized the issue stemmed from different migration patterns between the countries: Most people with TB in Switzerland have moved from sub-Saharan Africa, while in Austria, most come from the former Soviet republics. Only after modifying the ethnicity and region of birth variables did the model begin to work effectively.
'This is a cautionary tale,' said Nemeth. 'You go to a very similar setting with a little difference, and all this stops working. With machine learning models, we really have to be careful and test them vigorously before we rely on them.'
Emily Wong, MD, is an associate professor at the University of Alabama at Birmingham who has used AI to aid interpretation of chest radiography in South Africa, but was not involved in the new study.
The Swiss research 'opens one's eyes to the idea that with very large data sets with lots of clinical variables, you can discern meaningful and predictive patterns that predict whether someone will go on to develop TB,' she told Medscape Medical News.
Nemeth is working on an implementation study in which physicians whose patients have never been tested for TB will be randomly allocated to either receive a reminder to test, or a risk score based on the machine learning model. A key question is whether the latter will be enough to convince physicians to take further action, such as offering preventative therapy.
Wong noted that the potential benefits and risks (including liver toxicity) of preventative therapy need to be weighed up for each patient. But a machine learning model could help clinicians to do this.
'The idea that in the future, based on key demographic and clinical information of a person, and maybe including their chest x-ray or IGRA test, or maybe not, we would have a well-functioning clinical decision making tool that would guide a health care worker to make TB prevention decisions for the patient in front of them is definitely a worthy goal,' she said.

Jan Beutel was half-watching a live stream of Kleines Nesthorn, a mountain peak in the Swiss Alps, when he realized its cacophony of creaks and rumbles was getting louder. He dropped his work, turned up the sound and found himself unable to look away. 'The whole screen exploded,' he said. Beutel, a computer engineer specializing in mountain monitoring, had just witnessed a glacier collapse. On May 28, an avalanche of millions of tons of ice and rock barreled down the slope, burying Blatten, a centuries-old village nestled in the valley below. Local authorities had already evacuated the village after parts of the mountain had crumbled onto the glacier; a 64-year old man believed to have stayed remains missing. But no one expected an event of this magnitude. Successive rock avalanches onto the glacier increased the pressure on the ice, causing it to melt faster and the glacier to accelerate, eventually destabilizing it and pushing it from its bed. The collapse was sudden, violent and catastrophic. 'This one just left no moment to catch a breath,' Beutel said. The underlying causes will take time to unravel. A collapse of this magnitude would have been set in motion by geological factors going back decades at least, said Matthias Huss, a glaciologist at the Swiss university ETH Zürich. But it's 'likely climate change is involved,' he said, as warming temperatures melt the ice that holds mountains together. It's a problem affecting mountains across the planet. People have long been fascinated with mountains for their dramatic beauty. Some make their homes beneath them — around 1 billion live in mountain communities — others are drawn by adventure, the challenge of conquering peaks. These majestic landscapes have always been dangerous, but as the world warms, they are becoming much more unpredictable and much deadlier. 'We do not fully understand the hazard at the moment, nor how the dangers are changing with climate change,' said David Petley, an Earth scientist at the University of Hull in England. Snowy and icy mountains are inherently sensitive to climate change. Very high mountains are etched with fractures filled with ice — called permafrost — which glues them together. As the permafrost thaws, mountains can become destabilized. 'We are seeing more large rock slope collapses in many mountains as a result,' Petley told CNN. Glaciers are also melting at a terrifyingly rapid rate, especially in regions such as the Alps and the Andes, which face the possibility of a glacier-free future. As these rivers of ancient ice disappear, they expose mountain faces, causing more rocks to fall. There have been several big collapses in the Alps in recent years as ice melts and permafrost thaws. In July 2022, about 64,000 tons of water, rock and ice broke off from the Marmolada Glacier in northern Italy after unusually hot weather caused massive melting. The subsequent ice avalanche killed 11 people hiking a popular trail. In 2023, the peak of Fluchthorn, a mountain on the border between Switzerland and Austria, collapsed as permafrost thawed, sending more than 100,000 cubic meters of rock into the valley below. 'This really seems to be something new. There seems to be a trend in such big events in high mountain areas,' Huss said. Melting glaciers can also form lakes, which can become so full that they burst their banks, sending water and debris cascading down mountainsides. In 2023, a permafrost landslide caused a large glacial lake in Sikkim, India, to break its banks, causing a catastrophic deluge that killed at least 55 people. Last year, a glacial lake outburst caused destructive flooding in Juneau, Alaska — a now regular occurrence for the city. After two years in a row of destructive glacial flooding, Juneau is scrambling to erect temporary flood barriers ahead of the next melting season, the Anchorage Daily News reported this week. As well as melting ice, there's another hazard destabilizing mountains: rain. Extreme precipitation is increasingly falling on mountains as rain instead of snow, said Mohammed Ombadi, an assistant professor at the University of Michigan College of Engineering. His research shows every 1 degree Celsius of global warming increases extreme rainfall events by 15%. This pushes up the risks of flooding, landslides and soil erosion. Northern Hemisphere mountains will become 'hotspots' for extreme rain, Ombadi said. Heavy rainfall this month in Sikkim, a Himalayan state in northern India, triggered a series of landslides, killing at least three people. Images show deep muddy scars carved into the mountain, with buildings and trees obliterated. Scientists do have tools to monitor mountains and warn communities. 'There are fantastic instruments that can predict quite accurately when a rock mass (or) ice mass is going to come down,' Huss said. The difficulty is knowing where to look when a landscape is constantly changing in unpredictable ways. 'This is what climate change actually does… there are more new and previously unrecognized situations,' Huss said. These are particularly hard to deal with in developing countries, which don't have the resources for extensive monitoring. Scientists say the only way to reduce the impact of the climate crisis on mountains is to bring down global temperatures, but some changes are already locked in. 'Even if we manage to stabilize the climate right now, (glaciers) will continue to retreat significantly,' Huss said. Almost 40% of the world's glaciers are already doomed, according to a new study. 'We could have maybe avoided most of (the negative impacts) if we had acted 50 years ago and brought down CO2 emissions. But we failed,' Huss added. The consequences are hitting as the numbers of people living in and visiting mountains increases. 'We're just more exposed than we used to be,' he said. Ludovic Ravanel, an Alpine climber and geomorphologist who focuses on mountains' response to global warming, has a front line view of the increasing dangers of these landscapes. Mountains are the 'most convincing' hotspots of climate change, he told CNN. When he's focused on the science, he keeps his emotions at arm's length. But as a father, and a mountaineer, it hits him. 'I see just how critical the situation is. And even then, we're only at the very beginning.'

Oakland County issues alert for measles exposure site in Rochester Hills
FOX 2 - The Oakland County Health Division is notifying the public about a Rochester Hills measles exposure site involving a person from another county. The exposure site is a medical office building at 3950 S. Rochester Road at the corner of South Boulevard on Tuesday, June 3, according to a release from the Oakland County Health Division. Anyone there between 8 a.m. and 7:30 p.m. may have been exposed to measles. If you think you were exposed to measles at this location on June 3, Immune Globulin (IG) treatment is effective within six days of exposure for high-risk individuals. Those considered high-risk include women who are pregnant, unvaccinated children under age 5 and those who have a weakened immune system due to illness and disease including diabetes or HIV, malnutrition and/or medications. If you are not eligible for IG, monitor symptoms through June 24. If symptoms develop, call ahead to your health care provider. Individuals born in or before 1957 are considered immune. Measles is a highly contagious, vaccine-preventable disease that is spread by direct person-to-person contact and through the air. The Health Division recommends unvaccinated individuals ages 1 year and older receive a measles vaccination to protect themselves and those around them. Anyone who doesn't have a record of two MMR vaccines, unsure if they've have been vaccinated, or unsure if you've had measles in the past, needs to contact their health care provider. Measles can live for up to two hours in the air where an infected person coughed or sneezed. Symptoms of measles usually begin 7-14 days after exposure, but can appear up to 21 days after exposure, and may include: High fever (may spike to over 104°F Cough Runny nose Red, watery eyes (conjunctivitis) Tiny white spots on the inner cheeks, gums and roof of mouth (Koplik Spots) 2-3 days after symptoms begin. A rash that is red, raised, blotchy; usually starts on face, spreads to trunk, arms and legs 3-5 days after symptoms begin. UCSD students protest Trump cuts to science research funding
SAN DIEGO (FOX 5/KUSI) — For the second time this year, students and faculty at the University of California, San Diego (UCSD) staged a protest outside Geisel Library, demanding the federal government reverse deep education funding cuts imposed by the Trump administration — cuts they say are threatening research, jobs and the future of scientific innovation. Dozens of protesters gathered on campus, holding signs and sharing personal stories about how the loss of federal research grants is impacting their work and well-being. Dozens possibly detained by federal officials amid immigration raids in Los Angeles 'This year has been filled with many moments of insecurity,' said Eleanor Ketterer-Sykes, a first-year Ph.D. student in the neuroscience program. 'UCSD is one of the best science schools in the country because of its renowned research labs, you have access to labs at the Salk Institute, Sanford and Scripps as well as the VA Hospital — but that future is in jeopardy.' The Trump administration has canceled hundreds of research grants in recent months, citing concerns over 'ideologically driven science.' The cuts have targeted studies ranging from HIV prevention to violence prevention in children. UCSD Chancellor Pradeep Khosla warned in an April letter that the university stands to lose between $75 million and $500 million annually as a result. 'As of May 30, there have been more than 150 federal grants terminated, resulting in a loss of $30 million,' said Lisa Eyler, a UCSD psychiatry professor. 'These cuts are already leading to layoffs among instructors, staff scientists, and support personnel like librarians.' 'No Kings Day' protests planned across California on June 14 The ripple effects could be felt beyond UCSD. Stanley Maloy, emeritus professor of microbiology at San Diego State University, said the cuts threaten the pipeline of future scientists and engineers. 'Reduced positions mean talented students are left behind,' Maloy said. 'As this innovation workforce dries up, our economy is going to suffer.' Eyler echoed those concerns, warning of a generational loss in scientific progress. 'There will be a gaping hole in the pipeline of future scientists, engineers and healthcare providers, which could result in the potential loss of an entire generation of great scientific thinkers,' she said. Thousands in San Diego protest cuts to federal education funds For Ketterer-Sykes, the issue is personal. She entered the neuroscience program hoping to make life-saving breakthroughs. 'So many people rely on scientific innovation — whether it be to find cures for diseases like tuberculosis or Alzheimer's disease,' she said. 'It really is vital to humanity.'
