Investigators: High winds, rotor flap caused fatal Kauai helicopter crash

A helicopter that crashed on Kauaʻi in July, killing the pilot and two passengers, broke up in-flight after it hit turbulence causing the main rotor blades to strike its airframe, National Transportation Safety Board investigators found.
The conclusion was made without data revealing the helicopter's final moments because it wasn't carrying a flight recorder that would have shown its altitude, airspeed and the last actions taken by the pilot.
But the final report contains new details of the high winds around the accident area that day and includes dramatic video footage capturing the weather front that rolled in just before the aircraft went down.
The Robinson R44 Raven II operated by Aloha Helicopter Tours plummeted into the ocean off Kauaʻiʻs Nāpali Coast just after 1 p.m. July 11, 2024, after it broke in half during a one-hour sightseeing tour.
Hikers on the Kalalau trail who had stopped to take photos of the coast told NTSB investigators that before the crash they felt a 'huge, huge gust of wind' strong enough to blow 'a 50-pound backpack off a rock.' They then heard a 'loud bang' and looked toward the noise to see the helicopter falling straight down, the tail section detached.
One witness said the drop took 3-4 seconds and the impact 'made a big, loud splash.'
The wreckage was recovered by the U.S. Coast Guard in 80 feet of water, but only one body was ever found — that of Amy Quintua, a Kentucky woman who was on the tour with her husband James.
An inspection of the wreckage did not show any mechanical irregularities, the NTSB accident investigators said, but they did find evidence that the main rotor blades had hit the airframe — a condition known as 'mast bumping.'
The R44 is one of the world's most popular helicopters because of its affordability, but it also has a high rate of accidents because its rotor blades are susceptible to extreme flapping under certain conditions. Between 2006 and 2016 it had the highest fatality rate of any helicopter flying.
During downdrafts and high winds, the twin blades mounted on a mast above the main cabin can start to oscillate violently up and down. If pilots don't reduce their airspeed or overcorrect the flying controls, the flapping rotors can strike the tail or cabin and result in mid-air disintegration — which was the NTSB's finding in this case.
NTSB investigators found mast bumping on an R44 was also the cause of a 2019 crash over Kailua in which three people died.
The manufacturer Robinson issued its own safety notice about the problem, and the Federal Aviation Administration requires pilots to complete a pilot safety course before flying certain Robinson models including the R44.
The 69-year-old pilot who died in the Kauaʻi accident, Guy Croydon, had completed a Robinson Helicopter Pilot Safety Course in 2011, was an experienced certified flight instructor and had passed his last flight review two weeks prior to the accident, the report said.
His body was not recovered, however, so a toxicology report could not be conducted.
Investigators pointed to the likelihood of weather as a contributing factor in their preliminary report last year, and the detailed study found the weather in the area on July 11 was conducive to the development of strong downdrafts.
Limited radio location data shows the flight path of the helicopter up until 10 minutes before the crash.
The helicopter briefly rose to an altitude of 3,500 feet where wind speeds reached about 44 mph, the tracker shows. Easterly winds over the accident site were estimated at 30 to 37 mph over the accident site, the NTSB's weather study found.
The manufacturer's handbook for R44 pilots prohibits flight when the surface winds, including gusts, exceed 25 knots, or nearly 29 mph.
A week before the accident the National Transportation Safety Board had written to the chairman of the FAA about its continued concerns that some air tour operators and pilots in Hawaiʻi had adopted 'risky weather-related operating practices.'
The NTSB has been pushing the FAA to provide stronger oversight of the island state's busy air tourism industry, especially since 2019, when seven people died in a helicopter crash on Kauaʻi after the pilot continued to fly despite deteriorating weather and poor visibility.
During the investigation into that crash, NTSB investigators heard testimony about a local culture among air tour operators of 'pushing the weather' — even when flying conditions are not optimal. Local Industry representatives have told Civil Beat that pilots always have the final say in whether to go up.
Video footage from one of a new network of weather cameras installed last year by the Federal Aviation Authority to help tour operators assess conditions along the flight path captured the rapid changes in cloud cover and visibility at the time of the accident.
The decision to fly or not fly underpins a lawsuit filed in March against the tour operator Aloha Helicopter Tours LLC doing business as Ali'i Air Charters by the estates of the two passengers who died.
The filing in the Maui District Court alleges the companies and its employees were negligent in not monitoring the weather prior to takeoff, properly assessing the risk of flying into inclement weather, and failing to abort the flight.
An attorney for the plaintiffs Jacob Lowenthal said Friday that the weather data in the final NTSB report will be a critical component of their case.
Another Aloha Helicopter Tours pilot, Felipe Victoria, who flew over the site about 30 minutes after the accident, told Federal Aviation Administration inspector Seth Cossel that he had called Croydon during his lunch break to ask if he was going up for his afternoon flights.
Croydon said he was, and this helped Felipe inform his own decision to keep flying that day. 'It was Guy's next flight after lunch that was the accident flight.'
Reached by phone Friday the owner of Aloha Helicopter Tours, William Zeffiro, said his attorney had advised him not to comment on the lawsuit or the report.

He was matched with Minnie. "If I'm anxious at home or I have a bad dream, she's right there," Elsner said. Minnie went through two-and-a-half years of training at Helping Paws, working directly with the Hortman family, who volunteered to train her. It started as a high school service project for their daughter, but when she left for college, Mark Hortman finished training Minnie. "Their love for helping others, their love for doing something above themselves, is just living on every day with the gift that I got from their family," Alyssa Golob, executive director with Helping Paws, said. Gilbert never graduated from training school, but went on to be a beloved pet of the Hortman family. Melissa and Mark Hortman and Gilbert were shot and killed on Saturday. "It's evil on its own, and then you add in an innocent dog, and that's a certain kind of evil," Golob said. Minnie is just one of the many examples of Melissa and Mark Hortman's legacy living on, continuing to serve Minnesotans for the better. Since Saturday, Helping Paws donations have been pouring in, totalling more than $10,000. The money will be used to continue connecting veterans with service dogs.

Here's what you'll learn when you read this story: A skull washed on up on the shore of a New Jersey beach back in 1995, with further bone fragments appearing in 1999 and 2013. As a result, the set of remains earned the nickname 'Scattered Man John Doe.' Stumped for nearly three decades, local law enforcement recently turned to students at Ramapo College to employ genetic analysis and historical research to try and identify whose bones had washed ashore. The students were able to determine that the remains belonged to Henry Goodsell, the 29-year-old captain of the merchant ship Oriental, who died during a storm alongside his crew in 1844 This story is a collaboration with Imagine it's 1995. You've just had a good time seeing Batman Forever at the movies. You're cracking open an OK soda, strolling along the beach in Longport, New Jersey, rocking out to Shaggy's 'Boombastic,' when all of a sudden you stumble across a shocking sight: a skull has washed up on the shore. Did somebody drown at the beach? Did you stumble across the remnants of a mob hit? Whose skull is this, and how did it get here? You contact the proper authorities and feel certain they'll find the answer soon enough. Now imagine it's 1999. You've just had an O.K. time seeing Star Wars: Episode I - The Phantom Menace, and have likely forgotten all about that skull on the beach four years ago when, suddenly, news breaks: fragments of bone from the same corpse as the Longport skull have now been found less than two miles away in the nearby town of Margate. Authorities are no closer to identifying the dead man. Now, it's 2013. You just saw Frozen, and as you check your phone on the way out of the theater, you see the news alert: new skeletal remains, likely connected with that 1995 skull, have washed up in Ocean City. Still, law enforcement remains no closer to identifying a body whose skull was first discovered 18 years prior, and they've taken to calling the skeletal remnants 'Scattered Man John Doe.' Well, after 12 more years—a total of 30 since that skull was first discovered Longport, New Jersey—the Scattered Man has finally been identified. Through a combination of genetic analysis and good-old-fashioned historical research—in a collaboration between state law enforcement and the Ramapo College of New Jersey's Investigative Genetic Genealogy Center—the remains have been found to belong to Henry Goodsell, the captain of a merchant ship who died during a storm alongside his crew. Goodsell was only 29 years old when he died, meaning that he spent more years as 'the skull that washed up on the beach' than he ever did as a living, breathing man. But all the more remarkable is how long Goodsell's body had been lost, awaiting discovery: the storm that claimed Goodsell's life occurred in the winter of 1844. Ramapo's search for answers began in the fall of 2023, when they sent a DNA sample from the remains to Intermountain Forensics 'who uploaded the sample to GEDmatch and FamilyTreeDNA in February of 2024,' per a Ramapo College press release. This allowed the Ramapo undergraduates to trace the genealogy of the deceased. The students found evidence of ancestry 'dating back to the 1600s, with genetic relatives hailing from Litchfield and Fairfield counties in Connecticut.' For a full year, students at Ramapo hit the books, volunteering their time to try and triangulate deceased or missing figures with ties to Connecticut who may have been involved in shipwrecks off of the coast of New Jersey. That's when they discovered two news articles from December of 1844, which spoke of the sad fate of the vessel Oriental: 'The students learned that five crew members were aboard the Oriental, which departed from Connecticut en route to Philadelphia, PA, to deliver 60 tons of marble for use by Girard College, a college preparatory boarding school that opened in 1848. The ship was wrecked off the coast of Brigantine Shoal in 1844. It was reported the ship likely sprung a leak and went down less than one mile from the shoreline, and all crew members died. The captain of the ship was 29-year-old Henry Goodsell.' From there, the students proposed Goodsell as a possible identification to New Jersey State Police, and they in turn (in March of 2025) collected a DNA sample from one of Goodsell's descendants. A match was confirmed by NJSP on April 8, 2025, finally resolving the mystery of the Scattered Man John Doe. 'Identifying human remains is one of the most solemn and challenging responsibilities law enforcement is charged with,' Chief of County Detectives Patrick Snyder at the Atlantic County Prosecutor's Office stated in the press release. 'Law enforcement works hard knowing that behind every case is a promise: that no one will be forgotten, and that we will pursue the truth until families have the answers they deserve.' The identification of Goodsell also demonstrates the potential of modern genetic analysis to solve cases that have gone even colder than we ever thought solvable. 'Using modern genealogy testing to identify bone fragments from the 19th century is a powerful reminder of our unwavering commitment to resolving cases no matter how old,' noted Colonel Patrick J. Callahan, NJSP superintendent. 'The ability to bring answers to families—even generations later—shows how far science and dedication can take us.' So now, it's 2025. For those who took advantage of these sales to upgrade their kitchen by adding a new stove, you will want to make sure to burn in the oven before you cook. When you first turn it on, you may notice an odd smell filling your home. As alarming as it may be, it's completely normal, so long as the odor is oily or of something burning (and not gas -- that's rather more serious). Learn what causes the smell and how to get rid of it before using your new oven for cooking. What's that smell? According to Samsung, the burning smell caused by a new oven is due to the "insulation surrounding the oven cavity emitting odors the first few times it is exposed to the extreme heat inside of the oven." This is the smell of a bonding agent curing. Also, if you fail to remove some of the zip ties used to secure the inserts during transit before turning it on for the first time, you may also smell the plastic melting and burning. Appliance maker Belling, on the other hand, explains that the smell and light smoke you may see on the first use is from a protective coating of oil that is used during the manufacturing process. Whatever the case, new ovens smell. The best way to get rid of the smell is to run a proper burn-in cycle. It's recommended that you not cook any food prior to a burn-in, as the smell (and potentially the taste) can stick with your food. How to break in your new oven No matter what type of oven you have, the first thing you should do is remove any zip or twist ties. Then use warm, soapy water and a washcloth to wipe down any racks and the insides of the oven. Once you have wiped down all the surfaces inside the oven, it's time to start the burn-in. While instructions may vary from one manufacturer to the next, the premise of every "burn-in" process is the same: Turn on the fan in the ventilation hood and, if possible, open a few nearby windows. Set the oven to high heat, between 400 to 550 degrees Fahrenheit (204 to 288 degrees Celsius). Let the oven run at this temperature for between 30 minutes and an hour. If the smell persists after the cycle, repeat the process once or twice more. Once you no longer smell oil or a burning odor, allow the oven to fully cool and use a warm, soapy wash cloth to wipe down the inside of the oven a final time.
