D.C. Dispatch: Miller-Meeks, Grassley introduce bills on drug costs

Members of Iowa's congressional delegation have introduced legislation they say will help lower drug costs.
U.S. Congress has largely not met this week, with most of the attention in national politics centering on President Donald Trump's moves to close the U.S. Department of Education and militarize a part of the U.S.-Mexico border. Still, Iowa's federal legislators introduced several measures this week, including proposals dealing with the cost of pharmaceuticals.
Rep. Mariannette Miller-Meeks reintroduced the Delinking Revenue from Unfair Gouging (DRUG) Act Tuesday, a bill focused on pharmacy benefit managers (PBMs), the third-party businesses that negotiate prescription drug prices between drug manufacturers, health insurance companies and pharmacies. The Iowa representative also introduced the measure in 2o24.
Miller-Meeks' proposal would require PBMs to charge a flat fee for their service in negotiating prices on a certain drug instead of charging using a percentage of the drug price. The current practice of charging based on a percentage of the drug's costs incentivizes PBMs to promote the use of higher-cost medicines, according to a news release from Miller-Meeks' office, which 'takes money away from patients.'

'Pharmacy benefit managers (PBMs) have excessive influence over the prices patients pay at the pharmacy counter,' Miller-Meeks said in a statement. 'Local Iowa pharmacies are closing due to greedy PBM practices, impacting proximity and access to medications for Iowans. The DRUG Act will put downward pressure on prescription drug prices and insurance premiums by removing the incentive for PBMs to drive up the list price of medications.'
Many pharmacists have called for action to restrict certain business practices by PBMs, which they say are raising drug costs and limiting the ability to fill prescriptions. Iowa state lawmakers are also considering legislation that would place limits on some PBM actions.
Miller-Meeks has advocated for Congress to ban certain PBM practices, like 'patient steering,' encouraging or requiring patients to use certain affiliated pharmacies instead of a pharmacy of their choice and 'spread pricing,' where a PBM charges a higher cost for a drug than what is reimbursed to the pharmacy — practices that are also a focus of the Iowa legislation.
Charles Crain, managing vice president of policy of the advocacy group the National Association of Manufacturers, praised the legislation as a way to help manufacturing employers provide workers affordable health care coverage.
'PBMs increase health care costs by driving up prescription drug list prices, forcing patients to pay more at the pharmacy counter and making it more difficult for manufacturers to offer affordable health care benefits,' Crain said in a statement. 'Manufacturers commend Rep. Miller-Meeks and her colleagues for re-introducing the DRUG Act, which will rein in PBMs operating in the commercial market by removing their perverse incentive to maximize their own profits at the expense of manufacturers and manufacturing workers.'
In the Senate, U.S. Sen. Chuck Grassley co-introduced a bill that would attempt to stop pharmaceutical manufacturers from attempting to shut out competitors by 'product hopping.'
'Product hopping' refers to practices by some pharmaceutical companies to coercively move patients using a branded drug that has an expiring patent to a new patented drug in an effort to stop patients from moving to a generic version of the original product. According to a news release from Grassley, some manufacturers use tactics like destroying the inventory of their old product, raising its price or stating it is unsafe in attempts to discourage people from using the drug.
'Under this practice, it is difficult to switch patients to the cheaper generic when the market protections for the earlier drug expire,' the news release stated. 'This abuse of the patent system forces patients to continue paying high costs for a drug that is substantially similar to their old one for many years to come.'
Grassley, alongside Sens. John Cornyn, R-Texas, Richard Blumenthal, D-Connecticut and Dick Durbin, D-Illinois, introduced the 'Drug Competition Enhancement Act' Monday that would make product hopping' an antitrust violation. Companies engaging in this practice could face enforcement action from the Federal Trade Commission, including being brought to court.
The Iowa Republican said the bipartisan measure would help bring down drug prices by preventing companies from engaging in anticompetitive practices.
'One of my top priorities in the Senate is reducing the cost of prescription drugs,' Grassley said. 'Our bill will bring much needed transparency to drug pricing by cracking down on product hopping and giving Iowans more access to lower-cost generic drugs.'

Protect AR Rights ballot title to overturn recent ballot title laws rejected
LITTLE ROCK, Ark. – Protect AR Rights' first ballot title submission to the Arkansas attorney general was rejected on Monday. The group has stated that the submission is intended to place before voters the chance to overturn ballot initiative laws passed during the most recent general assembly. Citing concerns about security, the legislature passed multiple laws tightening the requirements for ballot referendums to be accepted, including increased restrictions on signature gathering and referendum language. Arkansas bill aims to let attorney general determine constitutionality of ballot initiatives One of those laws requiring ballot titles to be at no more than an eighth-grade reading level was the primary reason Attorney General Tim Griffin rejected the Protect AR Rights submission. 'I must reject your popular name and ballot title because the ballot title does not comply with Act 602 of 2025,' Griffin wrote in his opinion. 'As explained above, Act 602 prohibits me from certifying ballot titles that are above an eighth-grade reading level under the Flesch-Kincaid Grade Level formula. The ballot title you have submitted ranks at grade 11.5. Thus, your ballot title requires significant revisions before it complies with the Act.' The attorney general also cited ambiguity in two parts of the ballot title's language. Biden State Department Spokesman Now Says Israel Is Committing War Crimes
Former State Department spokesperson Matthew Miller says that he personally believes Israel has been committing war crimes against the Palestinian people — including during his tenure in the Biden administration, where, as a major voice of U.S. foreign policy, he daily defended Israel's flattening of Gaza to the American public. Speaking to Sky News' Mark Stone on the 'Trump 100' podcast, Miller said on Monday that it is 'without a doubt true that Israel has committed war crimes' in the territory. When Stone asked if he held this belief while serving as spokesperson, he said yes. 'Look, one of the things about being a spokesperson is you're not a spokesperson for yourself. You're a spokesperson for the president, the administration — and you espouse the positions of the administration,' Miller said. 'And when you're not in the administration, you can just give your own opinions.' The ex-State Department official said he believes Israeli soldiers committed war crimes in individual instances rather than as part of a wider policy of systematically erasing the Palestinian people, as muchof the international community has now accused the Israeli government. 'We were expecting it coming,' Hamid Bendaas, spokesperson for the pro-Palestinian nonprofit IMEU Policy Project, told HuffPost of Miller's admission. 'But I think this is the first foray of that — of Biden admin officials being like, 'Oh yeah, we were hard on Israel, we weren't super pro-Israel,' after saying for over a year that they were the most pro-Israel administration of all time, and a lot of the reporting since then obviously backing it up.' 'But [Miller] is saying that he's speaking about covering war crimes, right? It's not even covering up Biden's age or stuff like that,' Bendaas continued. 'But covering up a war crime that you know happened, and saying that doesn't happen because you're following orders? That's basically what he's saying he did. It's just a different thing.' United Nations officials,human rights groups and some world leaders have described Israel's actions as genocidal, a term Miller rejected. Still, his comments on Monday represent a huge pivot from the statements he made every day on behalf of former Secretary of State Antony Blinken while heclashed with reporters who tried to push himon the U.S. role in unconditionally supporting and arming an Israeli offensive that has now reached its nineteenth month. 'It has to be said that Miller is also guilty of, at minimum, facilitating these war crimes,' progressive strategist Rania Batrice posted on X. 'Matthew Miller, almost daily, not only lied to the public and the press, he did it in a shitty condescending way, in an attempt to mislead the public.' Though Miller insisted that he believed Israeli war crimes were individual acts, the State Department was aware that the Israeli government was committing possible war crimes. Experts within the U.S. government concluded last year that Israel was intentionally blocking humanitarian aid from entering Gaza — a major development that would legally require the Biden administration to stop sending weapons to Israel. Blinken rejected the findings and delivered a report to Congress without that information, leading report author Stacy Gilbert to resign in protest. Gilbert was one of many Biden appointees who, unlike Miller, resigned over the administration's unconditional support for Israel in its bombing campaign. Biden resignee and A New Policy co-founder Josh Paul told HuffPost, 'The Biden Administration's wanton lawbreaking when it came to what was happening in Gaza was no less a scandal than Watergate, and in the context of its human impact, probably a greater one.' 'I'm happy to be punitive to Matt Miller, but we also want to see more people be doing what Matt Miller is doing in a way, because it's a sign that they are recognizing that public opinion has shifted pretty rapidly on this,' Bendaas said. 'And especially on the Democratic side, it's just extremely unpopular to be pro-Israel in the way that Democrats have been for the last few decades.' Unpacking rumor that Stephen Miller's wife, Katie, left him for Elon Musk
In late May and early June 2025, rumors swirled that the wife of White House adviser Stephen Miller, Katie Miller, had left him for tech magnate Elon Musk, who until then had served as the new U.S. Department of Government Efficiency's public face. Some internet users even suggested that the affair had started with Stephen Miller's consent in an arrangement some described as a "throuple." For example, user @ on Bluesky relayed these claims: (Bluesky user @ The post read: Oh I am not sh**posting, folks. Stephen Miller, his wife, and Elon are widely rumored to be a throuple. Now Miller's wife is leaving the Trump team to be with Musk & LITERALLY PER THE NEW YORK TIMES Stephen is angrily subtweeting Musk. They actually put it in the paper of record that he was mad The post on Bluesky had received 7,600 likes and 3,200 reshares as of this writing. The same rumor appeared several times on Bluesky, but also on X, where the official account of the Democratic National Committee tagged Miller in its post of a single empty chair in a supposed hotel room in reaction to rumors that Miller's wife had left government to work for Musk in the private sector (archived): The empty chair represented a meme known as the "cuck chair," a chair on which someone supposedly sits while they watch their spouse have sex with someone else. This post had amassed 2.1 million views and 32,000 likes as of this writing, further spreading the unsubstantiated claim that Miller had cheated on her husband with Musk. Snopes has reached out to the Miller couple and Musk, seeking details. However, because we were not able to confirm or disprove the story, we have left it unrated. The rumor started as Musk, CEO of Tesla and owner of SpaceX, announced his departure from DOGE. Bluesky user Snyder (to whom we have reached out for comment and await a reply) referenced a New York Times story published on May 28, 2025, which indeed referenced Miller criticizing Musk on X without naming him. Miller posted on X (archived) in response to Musk lambasting the "One Big Beautiful" budget bill the U.S. House of Representatives had voted on, which experts, including analysts at the Tax Foundation, said would cause the budget deficit to balloon. The article read: However, Stephen Miller, the White House deputy chief of staff for policy, shot back at Mr. Musk on social media without naming him. Mr. Miller asserted that the bill would reduce the deficit — despite multiple independent analyses saying otherwise — and noted that the cuts made by Mr. Musk's team were unrelated to the spending bill. Lower down in the thread on Bluesky, Snyder linked to a story published on Feb. 27, 2025, in Wired, which Snyder said "strongly implied throupledom." The story explained that while Stephen Miller had assumed the position of White House deputy chief of staff for policy and homeland security adviser, his wife had been appointed as "top communications official at DOGE," directly with Musk. Citing an anonymous Republican source, the story went on to suggest that this had been by design: The Millers are seen inside Trumpworld as glorified babysitters for Musk, tasked with ensuring he stays within bounds, insofar as that's possible. "He gets a lot of grace," the first Republican said of Musk. "Many people aren't nervous, because Stephen Miller is deeply involved. And Katie." This Republican compared Musk to a preteen child. However, the Wired story implied more salacious suspicions about the relationship between the Millers and Musk without making them explicit: Musk's relationship with the Millers has become a subject of great intrigue in Washington as DOGE continues to wreak havoc on the federal government. Little is known about how often they interact outside of work and how the relationship grew over the late stages of the campaign into the transition. "If you can find out anything about Stephen Miller's social life, I don't wanna know the answer," says a longtime Republican operative who knows the couple personally. Given the anonymous sourcing, Snopes was unable to independently verify Wired's reporting. We have reached out to Snyder on Bluesky asking if the user heard the rumor independently. However, on May 28, 2025, a CNN report that Katie Miller had left DOGE to work for Musk full time gave new life to claims that the relationship between Musk and the pair was more complicated than it seemed. Citing three unnamed sources, CNN said Miller was now helping arrange interviews for Musk. Snopes was unable to independently verify CNN's reporting because it was based on anonymous sources. Though several internet users took it to mean that she had left her husband, with whom she shares three children, the reality is that Katie Miller had the status of "special government employee," which precluded her working more than 130 days per year for the U.S. government. As such, she would have had to leave government work regardless of which job she took on after her stint at DOGE. While she did not confirm the story, she had reshared on X one post by Musk regarding his departure from the government, as well as a screen capture of one of his quotes about DOGE, suggesting she may indeed be working as his spokeswoman. Two days later, on May 30, 2025, Musk stood in the White House Oval Office, speaking to the press alongside U.S. President Donald Trump about his return to the private sector. Musk appeared with a bruise around his right eye. While Musk said he owed his black eye to his son, whom he said he had instructed to punch him, the internet once again lit up with speculation (archived): The post had gained 2.7 million views and 41,000 likes as of this writing. Snopes reached out to the Millers as well as Musk asking them to provide further details. We will update this report should they respond.
