
Behind the story of the decades-long journey of xenotransplantation
On this episode of The Story Behind the AP Story, we hear from Lauran Neergaard and Shelby Lum, who have been following the journey of animal to human transplants, or xenotransplants, over the last few years. That includes sharing the procedure's history, more about the people who have received these organs and what risks it may pose.
Shelby Lum, Associated Press: There's more than 100,000 people on the transplant list, and most of these people are waiting for a kidney. Thousands will die on the list before ever being eligible to get one. And many experts say that there will never be enough human donors.
Haya Panjwani, Associated Press: That's why scientists are turning to a procedure known as xenotransplantation.

PANJWANI: I'm Haya Panjwani. On this episode of The Story Behind the AP Story, we hear from Lauren Neergaard and Shelby Lum, who have been following the journey of xenotransplants over the last few years. Lauren says xenotransplantation has actually been tried for many, many years, unsuccessfully.
NEERGAARD: There were a number of chimpanzee transplants back in the 1960s. Probably the most famous of them was Baby Faye in 1984, who lived for 21 days with a baboon's heart. They didn't work because back then, doctors and scientists hadn't really understood exactly how you could overcome the challenge of the immune system. The human immune system immediately recognizes those animal organs and the animal tissue as foreign and attacks.
So if you just took a pig's organ, any old pig organ, and stuck it in a person on the operating table, it would literally turn black before the surgeon's eyes. That's how fast the immune system would attack it. Now, what's happening is we have companies that are gene-editing pigs. They are modifying the genes in pigs so that they are born lacking some of those molecules that say, 'attack me' to the human immune system, and with some additional human genes to make them more compatible with people.
They are already used in, well, we've used them for food, but they're also already used in human medicine in a variety of ways. We get heart valves from pigs. We get skin grafts from pigs, and their organs are very compatible to human organs in the anatomy. They look like human organs, they work like human organs, the blood vessels are in the right place that you can attach them properly.
PANJWANI: Shelby says they've been able to meet some of the pigs used in these procedures.
LUM: And these pigs are bred specifically to be more compatible for human size, but they look like normal pigs. They look exactly like what you would think of a normal pig to look like. The pigs at the research farm are used for some of the early xenotransplant experiments. So these are for some of the decedent studies where these organs are being tested in brain dead donors, as well as some of the early compassionate care cases.
NEERGAARD: We really started seeing the first examples of organs from these gene-edited pigs being tested in brain-dead donors. And so kidneys from pigs were placed in them to see what happened. And this is where Shelby got to do something really cool.
LUM: I got the chance to be in the operating room for this really experimental surgery to test the pig kidney in this brain dead donor whose name is Moe. Moe's family had graciously decided to donate his body to this research to see if this could work. In order to do that, we worked with the university for months, which included sending over my vaccination history. It included getting testing done. I had to get blood work drawn to make sure that I wasn't bringing in anything into this operating room.
So once you're in the operating room, it's pretty similar to a regular kidney transplant surgery. You have the surgeon who's starting the surgery and then surgeons go to the farm and they procure the pig kidney there. And so they fly it back to NYU where the surgery is being done. And in that surgery, they put the pig kidney into the donor. And it turns pink immediately, which is the big sign for a kidney transplant surgery. That's what you wanna see, is this moment where that kidney turns pink. And it was this really incredible moment where the surgeons attached the kidney, you hear all of them chattering, and you hear them say, It's pink.
NEERGAARD: What was really amazing was how quickly this moved from deceased bodies into living people.
PANJWANI: In January 2022, a man named David Bennett became the first person to receive a pig heart transplant.
NEERGAARD: And Dr. Bartley Griffith, who was the surgeon involved here, told us how nervous he was about broaching it and then how this patient responded with humor. He said, you know, what do you think I'm gonna oink? That first operation, that first transplant, lasted only two months.
PANJWANI: The second pig heart attempt with a Navy veteran named Lawrence Faucette also only lasted a little less than two months.
NEERGAARD: Then the kidneys started.
PANJWANI: A man named Rick Slayman and a woman named Lisa Pisano both received gene-edited pig kidneys.
NEERGAARD: Those two patients also died. So this is a point where the doctors are like, 'Whoa, we're not having as much success as we wanted.' And they started looking for patients who were not quite so critically ill as those first four people.
An Alabama woman named Towana Looney has lived the longest with a gene-edited pig kidney. She had it for 130 days before her doctors at NYU had to remove it earlier this month. Looney had been doing very well. She had told us she felt like superwoman. But then, at the beginning of April, her body began rejecting the organ. She's recovered from the surgery and is back home on dialysis again, disappointed, of course. But that record time living with the pig kidney did provide valuable information to scientists.
So what's next? A New Hampshire man received a pig kidney at Mass General Hospital in January. Like Looney, he was not as sick as some of the prior recipients when he was transplanted, and so far, so good. Chinese researchers also recently announced a pig kidney transplant there, so this is gaining momentum.
LUM: Looney issued a statement saying the outcome wasn't what anyone had hoped for, but she's hoping that it can inspire others in this journey as xenotransplantation is studied more. And there are people who are very interested in getting a xenotransplant.
PANJWANI: With any new scientific procedures, there come risks.
NEERGAARD: So one of the risks, of course, is that this will fail. The doctors will all say those first four were actually not failures because of how much they learned from each one of those patients. They will tell you those are names that will go down in medical history because they learned from each and every one of them. The other side of the risk, though, is the fact that we are using animal organs in people. And that does come with concern about whether there would be any possible transmission of animal viruses to people. One of the ethical challenges is whether these people are fully consenting to an experiment. Because so far, they have been people who really have been out of other options. There also are some ethical issues about how the animals are treated, how we're using animals for the good of humans. And so that's something that the proponents of this believe is helped by the fact that we're using pigs, which are also used for food already.
LUM: Lauren and I have talked with a few people who have expressed interest in getting one. Because for so many people living on dialysis, being in the chair several times a week, it is just not the best quality of life. They don't feel good, they aren't able to do the things they want to, and so there are people who are really interested in being part of these early studies.
NEERGAARD: I think the main theme that we've heard from all of the patients that we have managed to speak with so far, or their families, or even in the case of Moe, the man whose body was used for that early experiment that lasted two months, there's this common theme that whatever happens to them, they hope that this will teach the surgeon something and the scientist something so that eventually this will work and help other people. That's every person that Shelby and I have talked to has said that. The Food and Drug Administration has given United Therapeutics permission to do a clinical trial using pig kidneys created by their subsidiary Revivacor Incorporated on a small number of participants. They'll do surgeries on the first few patients, see how it goes, they're there for a few months and then they'll ramp up to larger numbers.

Texas hospital that discharged woman with doomed pregnancy broke the law, inquiry finds
WASHINGTON — A Texas hospital that repeatedly sent a woman who was bleeding and in pain home without ending her nonviable, life-threatening pregnancy violated the law, according to a newly released federal investigation. The government's findings, which have not been previously reported, were a small victory for 36-year-old Kyleigh Thurman, who ultimately lost part of her reproductive system after being discharged after receiving no help from her hometown emergency room for her dangerous ectopic pregnancy. But a new policy the Trump administration announced Tuesday has thrown into doubt the federal government's stance on hospitals' denying women emergency abortions, even when they are at risk for serious infection, organ loss or severe hemorrhaging. Thurman had hoped the federal government's investigation, which was concluded last year and issued a report in April, would send a clear message that ectopic pregnancies must be treated by hospitals in Texas, which has one of the nation's strictest abortion bans. 'I didn't want anyone else to have to go through this,' Thurman said in an interview with the Associated Press from her Texas home this week. 'I put a lot of the responsibility on the state Uncertainty regarding emergency abortion access Women around the country have been denied emergency abortions for their life-threatening pregnancies after states swiftly enacted abortion restrictions in response to a 2022 ruling from the U.S. Supreme Court, which includes three appointees of President Donald Trump. The guidance issued by the Biden administration in 2022 was an effort to preserve access to emergency abortions for extreme cases in which women were in medical emergencies. It directed hospitals — even ones in states with severe restrictions — to provide abortions in those emergency cases. If hospitals did not comply, they would be in violation of a federal law and risk losing some federal funds. The Centers for Medicare and Medicaid Services, the federal agency responsible for enforcing the law and inspecting hospitals, announced on Tuesday it would revoke the Biden-era guidance around emergency abortions. CMS administrator Dr. Mehmet Oz said in a social media post on Wednesday that the revocation of the policy would not prevent pregnant women from getting treatment in medical emergencies. 'The Biden Administration created confusion, but EMTALA is clear and the law has not changed: women will receive care for miscarriage, ectopic pregnancy, and medical emergencies in all fifty states—this has not and will never change in the Trump Administration,' Oz wrote, using the acronyms for the Emergency Medical Treatment and Labor Act. The law, which remains intact and requires doctors to provide stabilizing treatment, was one of the few ways that Thurman was able to hold the emergency room accountable after she didn't receive any help from staff at Ascension Seton Williamson in Round Rock, Texas, in February 2023, a few months after Texas enacted its strict abortion ban. An ectopic pregnancy left untreated Emergency room staff observed that Thurman's hormone levels had dropped, a pregnancy was not visible in her uterus and a structure was blocking her fallopian tube — all telltale signs of an ectopic pregnancy, when a fetus implants outside of the uterus and has no room to grow. If left untreated, ectopic pregnancies can rupture, causing organ damage, hemorrhage or even death. Thurman, however, was sent home and given a pamphlet on miscarriage for her first pregnancy. She returned three days later, still bleeding, and was given an injected drug intended to end the pregnancy, but it was too late. Days later, she showed up again at the emergency room, hemorrhaging because the fertilized egg growing on Thurman's fallopian tube ruptured it. She underwent an emergency surgery that removed part of her reproductive system. CMS launched its investigation of how Ascension Seton Williamson handled Thurman's case late last year, shortly after she filed a complaint. Investigators concluded the hospital failed to give her a proper medical screening exam, including an evaluation with an OB-GYN. The hospital violated the federal Emergency Medical Treatment and Labor Act, which requires emergency rooms to provide stabilizing treatment to all patients. Thurman was 'at risk for deterioration of her health and wellbeing as a result of an untreated medical condition,' the investigation said in its report, which was publicly released last month. Ascension, a vast hospital system that has facilities across multiple states, did not respond to questions about Thurman's case, saying only that it is 'is committed to providing high-quality care to all who seek our services.' Penalties for doctors, hospital staff Doctors and legal experts have warned that abortion restrictions like the one Texas enacted have discouraged emergency room staff from aborting dangerous and nonviable pregnancies, even when a woman's life is imperiled. The stakes are especially high in Texas, where doctors face up to 99 years in prison if convicted of performing an illegal abortion. Lawmakers in the state are weighing a law that would remove criminal penalties for doctors who provide abortions in certain medical emergencies. 'We see patients with miscarriages being denied care, bleeding out in parking lots. We see patients with nonviable pregnancies being told to continue those to term,' said Molly Duane, an attorney at the Center for Reproductive Rights that represented Thurman. 'This is not, maybe, what some people thought abortion bans would look like, but this is the reality.' The Biden administration routinely warned hospitals that they needed to provide abortions when a woman's health was in jeopardy, even suing Idaho over its state law that initially prohibited nearly all abortions, unless a woman's life was on the line. Questions remain about hospital investigations But CMS' announcement on Tuesday raises questions about whether such investigations will continue if hospitals do not provide abortions for women in medical emergencies. The agency said it will still enforce the law, 'including for identified emergency medical conditions that place the health of a pregnant woman or her unborn child in serious jeopardy.' While states like Texas have clarified that ectopic pregnancies can legally be treated with abortions, the laws do not provide for every complication that might arise during a pregnancy. Several women in Texas have sued the state for its law, which has prevented women from terminating pregnancies in cases where their fetuses had deadly fetal anomalies or they went into labor too early for the fetus to survive. Thurman worries pregnant patients with serious complications still won't be able to get the help they may need in Texas emergency rooms. 'You cannot predict the ways a pregnancy can go,' Thurman said. 'It can happen to anyone, still. There's still so many ways in which pregnancies that aren't ectopic can be deadly.'

Join scientists as they drive into hailstorms to study the costly weather extreme
As severe storms once again soak, twist and pelt the nation's midsection, a team of dozens of scientists is driving into them to study one of the nation's costliest but least-appreciated weather dangers: Hail. Hail rarely kills, but it hammers roofs, cars and crops to the tune of $10 billion a year in damage in the U.S. So in one of the few federally funded science studies remaining after Trump administration cuts, teams from several universities are observing storms from the inside and seeing how the hail forms. Project ICECHIP has already collected and dissected hail the size of small cantaloupes, along with ice balls of all sizes and shapes. Scientists in two hail-dimpled vehicles with special mesh protecting the windshields are driving straight into the heart of the storms, an area known as the 'shaft' where the hail pelting is the most intense. It's a first-of-its-kind icy twist on tornado chasing. 'It's an interesting experience. It sounds like somebody on the outside of your vehicle is hitting you with a hammer,' said Northern Illinois University meteorology professor Victor Gensini, one of the lead researchers. A team of journalists from The Associated Press joined them this week in a several-day trek across the Great Plains, starting Tuesday morning in northern Texas with a weather briefing before joining a caravan of scientists and students looking for ice. Driving toward the most extreme forecasts The caravan features more than a dozen radar trucks and weather balloon launching vehicles. At each site, the scientists load and unload drones, lasers and cameras and other specialized equipment. There are foam pads to measure hail impact and experimental roofing material. There are even special person-sized funnels to collect pristine hail before it hits the ground and becomes tainted with dirt. Already in treks across Kansas, Oklahoma and Texas, the team has found hail measuring more than 5 inches (13 centimeters) in diameter — bigger than a softball, but not quite a soccer ball. The team's equipment and vehicles already sport dings, dimples and dents that scientists show off like battle scars. 'We got a few good whacks,' said forensic engineer Tim Marshall, who was carrying roofing samples to see if there were ways shingles could better handle hail. 'I look at broken, busted stuff all the time.' At Tuesday's weather briefing, retired National Weather Service forecaster David Imy pointed to potential hot spots this week in Texas, Oklahoma and New Mexico. Computer models show the potential for a 'monster storm down here near the Red River" later in the week, he said. Acting on the latest forecasts, Gensini and other leaders told the team to head to Altus, Oklahoma, but be ready to cross the Red River back into Texas at a moment's notice. A few hours after his briefing, Imy had the opportunity to chase one of the bigger storms, packing what radar showed was large hail at 8,000 feet (2,438 meters) in the air. Because of the warm air closer to the surface, the hail was only pea sized by the time it hit the ground. But the outing still provided good data and beautiful views for Imy, who was with a group that stationed themselves about a half-mile from the center of the storm. 'Beautiful colors: turquoise, bluish green, teal,' Imy said, pointing to the mushroom shaped cloud dominating the sky. 'This is beauty to me and also seeing the power of nature.' A costly but overlooked severe weather problem This is not just a bunch of scientists looking for an adrenaline rush or another sequel to the movie 'Twister.' It's serious science research into weather that damages a lot of crops in the Midwest, Gensini said. Hail damage is so costly that the insurance industry is helping to pay for the mission, which is primarily funded by the National Science Foundation. 'These are the stones that do the most damage to lives and property,' Gensini said. 'We want the biggest hail possible.' A 2024 study by Gensini found that as the world warms from human-caused climate change, small hailstones will become less likely while the larger ones become more common. The bigger, more damaging ones that the ICECHIP team is studying are projected to increase 15% to 75% this century depending on how much the world warms. That's because the stronger updrafts in storms would keep stones aloft longer to get bigger, but the heat would melt the tinier ones. The experiment is unique because of the combination of driving into the hail and deploying numerous radars and weather balloons to get an overall picture of how the storms work, Gensini said, adding that hail is often overlooked because researchers have considered it a lower priority than other extreme weather events. Outside scientists said the research mission looks promising because there are a lot of unanswered questions about hail. Hail is the No. 1 reason for soaring costs in billion-dollar weather disasters in the United States, said meteorologist Jeff Masters, who cofounded Weather Underground and is now at Yale Climate Connections. 'Now a large part of that reason is because we simply have more people with more stuff in harm's way," said Masters, who wasn't part of the research. 'Insurance has become unaffordable in a lot of places and hail has become a big reason." Trump administration revokes guidance requiring hospitals to provide emergency abortions
The Trump administration announced on Tuesday that it would revoke guidance to the nation's hospitals that directed them to provide emergency abortions to women when they are necessary to stabilize their medical condition. That guidance was issued to hospitals in 2022, weeks after the U.S. Supreme Court upended national abortion rights in the U.S. It was an effort by the Biden administration to preserve abortion access for extreme cases in which women were experiencing medical emergencies and needed an abortion to prevent organ loss or severe hemorrhaging, among other serious complications. An Associated Press investigation last year found that, even with that guidance, dozens of pregnant women were being turned away from emergency rooms, including some who needed emergency abortions. Biden argued that hospitals — including states with near-total bans — needed to provide emergency abortions under the Emergency Medical Treatment and Active Labor Act. That law requires emergency rooms that receive Medicare dollars to provide an exam and stabilizing treatment for all patients. Nearly all emergency rooms in the U.S. rely on Medicare funds. The Centers for Medicare and Medicaid Services, which investigates hospitals that are not in compliance, said in a statement on Tuesday that it was rescinding the Biden-era guidance. The agency, however, will continue to enforce the law, 'including for identified emergency medical conditions that place the health of a pregnant woman or her unborn child in serious jeopardy.' But CMS added that it would also 'rectify any perceived legal confusion and instability created by the former administration's actions.' The Biden administration sued Idaho over its abortion law that initially only allowed abortions to save the life of the mother. The federal government had argued before the U.S. Supreme Court last year that Idaho's law was in conflict with the federal law, which requires stabilizing treatment that prevents a patient's condition from worsening. The U.S. Supreme Court issued a procedural ruling in the case last year that left key questions unanswered about whether doctors in abortion ban states can terminate pregnancies when a woman is at risk of serious infection, organ loss or hemorrhage.
