
Engine failure caused paddle steamer crash which left 24 people injured
The world's only seagoing paddle steamer crashed into a pier injuring 24 people following engine failure, investigators have found.
Fewer people might have been injured if passengers had been warned ahead of the impact, a Marine Accident Investigation Branch (MAIB) report said.
The report also found the paddle steamer had 're-entered service with inexperienced crew and worn machinery'.
The Waverley was trying to berth at the pier at Brodick, Arran, on September 3 2020 after a two-hour trip along the island's coast when the crash happened at 4.46pm.
The report said 21 of the 186 passengers on board and three of the 27 crew were injured.
Investigators said many passengers were standing at the time of the crash and either fell to deck or hit fixtures on the boat as they were thrown forward.
Three passengers sustained serious back and pelvis injuries, and two of them were evacuated by helicopter to a mainland hospital.
The other passengers sustained cuts and bruises and four were were taken to a mainland hospital with nine treated at the local hospital on Arran.
The report states: 'No impact warning or instruction to brace was broadcast and 24 of the passengers and crew were injured.
'This number might have been smaller had sufficient warning been given.'
The Waverley's bow was buckled inwards by the impact, resulting in three holes in the bow plating above the waterline.
The report found the engine failed when trying to use to it to slow the ship on approach to the pier and could only be restarted too late.
The Waverley hit the concrete wall at the end of the pier at approximately 2.8 knots.
Investigators said the engine failure was caused by a 'dead centre event' where the high-pressure steam piston came to rest at the top of its travel when the engine was stopped – known as the dead centre position – preventing the engine from running.
It was slow to restart due to an incorrectly secured piston valve.
'Dead centre events were a known phenomenon but they had only occurred infrequently. The chief engineer was unfamiliar with the system indicators that warned of a dead centre event or how to anticipate and prevent its occurrence,' the report states.
'The absence of an effective safety management system and reliance on historic maintenance documentation resulted in the failure to capture maintenance issues accurately.'
The report found the risks of such an event occurring and the associated risks 'had neither been assessed nor effectively mitigated'.
The MAIB said since the crash the ship's operator, Waverley Excursions, has carried out an internal safety review of its management system.
This led to the creation of an electronic planned maintenance system, a requirement for all berthing activities to be fully risk-assessed and a competency-based training and development programme for engine and deck crew.
It has also added awareness of sudden movements while berthing to the passenger safety briefings.
The MAIB has made no recommendations to the company 'in light of the actions taken and the time elapsed since the accident'.
Paul Semple, Waverley Excursions general manager, said: 'I welcome the publication of the MAIB report and support its findings and conclusions.
'The report recognises the extensive work undertaken by Waverley Excursions since the incident back in 2020, including a full review of the safety management system.
'I am pleased to note that given the actions taken by the company and outlined in the report there are no recommendations made by the MAIB.'
He said this year passenger service is due to start in May and this season marks the 50th anniversary since the Waverley's first sailing in preservation.

This season includes two planned trips to the remote Lundy Island – passengers that board the Bristol channel experience will be allowed to step ashore and explore the mark the 200th anniversary of the modern railway the Waverley is giving the "ultimate day out" by connecting three steam railways – the West Somerset, the Brecon Mountain and Lynton & Barnstable railways by taking a paddle steamer and steam train around the Bristol Channel and South West Wales are until 25 June, where the Waverley will be exploring Glasgow and Clyde Coast Summer from 27 June.

New doc explores how world's last seagoing paddle steamer was saved by Sunday Mail readers
Since its launch in 1946 the Waverley has remained a firm favourite with day-trippers for its legendry voyages 'doon the watter' from Glasgow's River Clyde around the west coast islands. It is listed by National Historic Ships UK as 'a vessel of pre-eminent national importance' and was built to replace a previous Waverley sunk in 1940 while helping to evacuate troops from Dunkirk during WW2. But in the mid-1970s she fell into disrepair and faced being sent to the scrapyard before being bought for £1 by the Paddle Steamer Preservation Society (PSPS) which then joined forces with this newspaper to campaign for her survival. Now the tale of how she was rescued is recounted in a new film Saving Waverley featuring previously unseen interviews and footage. Terry Silvester, former Chairman of the Waverley Organisation and a lifelong supporter of the ship, said: 'The appeal was led by the Scottish Sunday Mail, which ran a great campaign with full colour, centre spread, Ten Days To Save The Waverley. 'This brought people's contributions flooding in and we sailed in '76 and, of course, have sailed ever since.' This month the 78-year-old Waverley celebrates her 50th anniversary of cruising with the PSPS after its Spring sailing schedule kicked off on Friday. Much of the footage in the new film, produced by John Dakin and featuring interviews by Nigel Heath, was recorded on a sailing in 2010, but remained undiscovered on a disc for 15 years. He added: 'It really all began when we realised in 1969 that the Waverley had become the last paddle steamer in the world that went to sea. 'The Waverley at that time was owned by the Caledonian Steam Packet. 'We started a campaign in Scotland with my partner in crime Douglas McGovan to try and persuade them to keep the Waverley as a tourist attraction and this was quite successful for a number of years. 'But in the winter of 1973 Douglas got a phone call asking him to go down to Gourock for a meeting. 'He discovered then that the Waverley, laid up for the winter as usual, had the men withdrawn from work on the ship. so it was obvious they were not going to recommission it for 1974. 'So Douglas said will you come to Scotland for the meeting but I said I'm not coming to Scotland in the middle of the winter to be given the ship's bell as a memento. I said 'you go get the bell'. 'He called me and said I've been to the meeting and they offered to give us the bell, but they also offered to give us the rest of the ship as well. 'This was a bit of a shock because the Paddle Steamer Preservation Society was a very small organisation, it had in monetary terms in those days three and six in the bank and this frightened everyone to death. 'The society really was going to try and turn down the offer with honour. 'But three of us said 'look you can't be the Paddle Steamer Preservation Society if you turn down the last chance you're ever going to get of owning a real seagoing paddle steamer'. 'So we said we have to have a go. 'It really was as simple as that.' Named after Sir Walter Scott's debut novel, PS Waverley was built just after World War II as a replacement for a vessel which was sunk during the Dunkirk evacuation. It was in August 1974 that the ship was officially handed over to Waverley Steam Navigation Co. Ltd, a company formed by the PSPS. Douglas McGovan, former chairman of Waverley Excursions said: 'We were just a couple of enthusiasts. 'But much to our astonishment I got a phone call from their general manager inviting me to his office. 'I thought they were going to say the Waverley is finished. 'Thanks for your support but we're going to send her to the yard. 'But instead he said that we are going to offer your society the Waverley for free, as a gift. 'To make the deal legal I had to hand over that famous Scottish £1 note.' A council grant then allowed the society to run the first cruise. And in May 1975 she set sail for the first time under the new deal, departing Glasgow with a send off from the Glasgow Police Pipe Band. But it took the enthusiastic support of Sunday Mail readers to enable them to carry on financially the following year. And to build a programme that was later to extend to visit ports around the UK. Since then Waverley has earned thousands of fans from all around the country, recapturing the Golden Age of Steamers, and was named the National Flagship of the Year 2024 by National Historic Ships UK. In 2003 a heritage restoration programme was carried out at a cost of £7 million and in 2019 there was also a Boiler Refit. Appeal raised £2.3million with donations from over 8500 individuals, the PSPS, the Scottish Government and several trusts and foundations. The new film is available now on DVD for £16.50. All profits from the DVD will be going to volunteer-run charities.

Aberdeen harbourmaster gives insider account after deadly Solong ship crash
Port of Aberdeen's harbourmaster has detailed the complex operation to remove dangerous cargo from the Solong – the fire-ravaged container ship involved in a fatal North Sea collision that remains the focus of a major investigation. Aberdeen's South Harbour is now acting as caretaker for the vessel, which sustained severe structural damage in the crash and the fires that followed. Harbourmaster Benji Morrison told the port's annual general meeting that the Solong is now, 'effectively just a hull'. The Portuguese-flagged vessel arrived under tow on March 28, following a high-impact crash with the stationary Stena Immaculate – a tanker carrying aviation fuel for the United States Air force. The incident claimed the life of one crew member and triggered a large-scale salvage operation. Mr Morrison gave an in-depth account of how his team scrambled to prepare for the ship's arrival with just three days' notice. 'This wasn't something I expected in my first year as harbourmaster,' he said. 'But when the SOSREP calls on a Saturday afternoon, you know it is serious.' The Solong collided with the US-flagged Stena Immaculate on March 10. The crash caused a series of explosions and fires that ravaged the Solong for more than a week. Tragically, 38-year-old Filipino crew member Mark Angelo Pernia lost his life in the incident. The ship's captain, Vladimir Motin of St Petersburg, Russia, has been charged with gross negligence manslaughter. Despite the damage, 36 remaining crew members from both vessels were rescued in a tense operation. Aberdeen was identified as the preferred port to manage the casualty drawing on recent experience handling the MV Lowlands Diamond just months earlier. To prepare for the Solong's arrival, a specialist shore tension system had to be shipped overnight from the Netherlands and rigged quayside in time for the vessel's arrival on Friday morning. 'She was a dead ship with no mooring capability,' Mr Morrison explained. 'We had to decide how we were going to moor this vessel and how we were going to safely keep it there. 'We had to look at everything: environmental risks, emergency procedures, offloading operations, and maintaining safety for the rest of the port. 'We knew there was dangerous cargo on board, that cargo was very fire-damaged and there was a lot of damage from the fire-fighting efforts to extinguish the fire. 'There was also a lot of broken glass on the deck and we had to put a pilot onto the vessel.' The Marine Accident Investigation branch, along with Humberside Police and Police Scotland, boarded the ship as soon as it was secured in South Harbour. In the six weeks since, teams have removed its cargo, offloaded all fuel bunkers, and disposed of contaminated firefighting water in line with waste management regulations. 'The next stage for us now is really caretaking,' said Mr Morrison. 'We are keeping daily watch on this vessel to make sure she maintains her structural integrity and remains safe in port.' The Solong is expected to be towed for recycling, though the final destination has not yet been confirmed. Mr Morrison said the back-to-back handling of two casualty vessels has helped cement the Port of Aberdeen's growing reputation as a key port of refuge. He added: 'We are very well strategically placed as being a port that can offer the UK resilience in times of conflict or national and international emergencies.'
