New drug could reduce risk of early-onset Alzheimer's disease

An experimental drug appears to reduce the risk of Alzheimer's in people 'destined' to develop the disease in middle age.
The study, published in the journal The Lancet Neurology, involved 73 people with rare, inherited genetic mutations that cause the overproduction of the protein amyloid in the brain. This all but guarantees they will develop Alzheimer's disease in their 30s, 40s or 50s.
For 22 patients who had no cognitive problems at the start of the study, and who received the anti-amyloid drug the longest – an average of eight years – the treatment lowered the risk of developing symptoms by half.
The results boost hopes similar treatments being developed to target more common forms of Alzheimer's - that appear when people reach their sixties, seventies and eighties - might also be effective.
These findings provide new evidence to support the theory that amyloid leads to Alzheimer's disease.
It's thought amyloid forms toxic plaques around brain cells and as the brain becomes affected, there is also a decrease in chemical messengers, called neurotransmitters, involved in sending messages between brain cells.
The study suggests removing such plaques or stopping them form forming can prevent symptoms of the memory robbing disease.
'Everyone in this study was destined to develop Alzheimer's disease and some of them haven't yet,' said senior study author Randall Bateman of Washington University in St Louis, Missouri.
'We don't yet know how long they will remain symptom-free – maybe a few years or maybe decades.
'In order to give them the best opportunity to stay cognitively normal, we have continued treatment with another anti-amyloid antibody in hopes they will never develop symptoms at all.
'What we do know is that it's possible at least to delay the onset of the symptoms of Alzheimer's disease and give people more years of healthy life.'
The study participants consisted of people who had originally enrolled in the first Alzheimer's prevention trial in the world, launched in 2012, and then continued into an extension of the trial in which they received an anti-amyloid drug.
All participants in the trial had no-to-very-mild cognitive decline and were within 15 years before to 10 years after their expected age of Alzheimer's onset, based on family history.
When the initial trial concluded in 2020 researchers reported that one of the drugs, gantenerumab, lowered amyloid levels in the brain.
But the researchers did not see evidence of cognitive benefit then because the group without symptoms – regardless of whether they were on drug or placebo – hadn't cognitively declined.
As a result, the researchers continued to study the drug to determine whether higher doses or longer treatment could prevent or delay cognitive decline, and found it could reduce the risk of developing symptoms from 100 per cent to 50 per cent.
Gantenerumab is no longer made by Roch, the company that developed it, because it did not slow symptoms of a more common non-genetic form of Alzheimer's in a trial of 1,900 participants.
But there are other anti-amyloid drugs being developed.
Although the trial was limited to people with genetic forms of Alzheimer's that lead to early onset, researchers expect that the study's results will inform prevention and treatment efforts for other forms of Alzheimer's disease.
Both early-onset and late-onset Alzheimer's disease can start with amyloid slowly collecting in the brain two decades before memory and thinking problems arise.
Dr Richard Oakley, associate director of research and innovation at the Alzheimer's Society, and who was not involved in the study, said it showed there is 'hope on the horizon' to beat Alzheimer's.
'These exciting early-stage results hint that long-term anti-amyloid treatments, started before Alzheimer's disease symptoms appear, could potentially delay symptom onset,' he said.
'However, these results need to be treated with caution; this trial focuses on a very small group of individuals with genetic forms of Alzheimer's disease. Longer-term follow up of this group and larger studies will tell us more about the effect of these drugs in these types of Alzheimer's.'

It is also a staple herb in Mediterranean meals. ‌ In one study, people who breathed in rosemary's scent performed better on memory tasks compared to those in an unscented environment. ‌ Dipa Kamdar, Senior Lecturer in Pharmacy Practice at Kingston University, said: " There are several mechanisms at play. For starters, rosemary stimulates blood circulation, including to the brain, helping deliver more oxygen and nutrients, which may improve mental clarity. It also has calming properties; some studies suggest its aroma can reduce anxiety and improve sleep. Lower stress can mean better focus and memory retention. "Rosemary contains compounds that interact with the brain's neurotransmitters. One such compound, 1,8-cineole, helps prevent the breakdown of acetylcholine, a brain chemical essential for learning and memory. By preserving acetylcholine, rosemary may help support cognitive performance, especially as we age." She added: " Rosemary is packed with antioxidants, which help protect brain cells from damage caused by oxidative stress – a major factor in cognitive decline. Rosemary is rich in phytochemicals, plant compounds with health-enhancing effects. One of the most powerful is carnosic acid, an antioxidant and anti-inflammatory agent that helps shield brain cells from harm, particularly from the kinds of damage linked to Alzheimer's disease." In a study this year. scientists found a version of carnosic acid appeared to improve memory, boosted the number of synapses (the connections between brain cells), and reduced harmful Alzheimer's related proteins like amyloid-beta and tau. The same compound could be used to treat type 2 diabetes, cardiovascular disease and Parkinson's disease. Dipa told The Conversation: " Rosemary's benefits could extend well beyond the brain. It's been used traditionally to ease digestion, relieve bloating and reduce inflammation. Compounds like rosmarinic acid and ursolic acid are known for their anti-inflammatory effects throughout the body. Rosemary may even benefit the skin – a review suggests it can help soothe acne and eczema, while carnosic acid may offer anti-ageing benefits by protecting skin from sun damage. ‌ " Rosemary oil also has antimicrobial properties, showing promise in food preservation and potential pharmaceutical applications by inhibiting the growth of bacteria and fungi. For most people, rosemary is safe when used in food, teas or aromatherapy. But concentrated doses or extracts can pose risks. Consuming large amounts may cause vomiting or, in rare cases, seizures – particularly in people with epilepsy. "There's also a theoretical risk of rosemary stimulating uterine contractions, so pregnant people should avoid high doses. Because rosemary can interact with some medications – such as blood thinners – it's best to check with a healthcare provider before taking large amounts in supplement form. "Rosemary is more than just a kitchen staple. It's a natural remedy with ancient roots and modern scientific backing. Brent Franson wasn't afraid of dying. He was afraid of dying from Alzheimer's disease. He remembers seeing sufferers on TV growing up, with their listless expressions and inability to walk. It left him certain he did not want to develop the devastating condition. His worst fear was revealed when he used the Death Clock - an artificial intelligence that predicts - down to the minute - when and how a person will die. Franson invented the app, which analyzes a person's body metrics and lifestyle, and input his own information. The 43-year-old was shocked when the Death Clock projected his demise at 76 years old. Among possible causes of his death: Alzheimer's. It made sense, Franson thought. In his 20s, he had become addicted to anti depressants and sedatives, which left him unable to sleep properly for a decade - a risk factor for neurological decline - and he had a gene that predisposed him to dementia. Franson was also working late, eating large late-night dinners and balancing his role as husband and father-of-three. Determined to change his fate, however, Franson made major lifestyle changes - becoming what he called a 'sleep athlete.' He fine-tuned his schedule to make time for rest and exercise and to create a better work-life balance. One year later, he ran his information through the Death Clock again and it revised its estimate — adding an extra nine years to his life expectancy. The most likely cause of death? Alzheimer's was still a possibility, but so were cardiovascular diseases and cancer. 'Something does have to kill you eventually,' Franson told He added: 'My biggest fear in life, it's not death. It's Alzheimer's. Just the thought of having to live with Alzheimer's — so this was really a wake-up call for me.' As the aging population of the US continues to grow, so will the rates of dementia. Currently, an estimated 6.8million Americans have Alzheimer's disease - the most common form of dementia - the vast majority of whom are over 65 years old. By 2050, this number is projected to rise to nearly 13 million. A lack of sleep - especially a lack of deep sleep - has been linked to an increased risk of Alzheimer's in numerous studies. Experts say it stops the body from clearing out waste products from the brain — raising the risk of dangerous amyloid beta protein clumps forming that raise someone's risk of the disease. More than one in three adults — 50 to 70million people — regularly do not meet the CDC's recommendation of at least seven hours of sleep every night. One study has suggested sleeping six or fewer hours per night before age 70 is linked to a 30 percent increased risk of dementia compared to people getting seven hours or more. It can also drive other unhealthy behaviors, like exercising less or eating more sugary treats, which can also raise risk. To boost his sleep, and slash his risk of Alzheimer's, Franson began by banning all mobile phones from the bedroom. He set a consistent bedtime of 9.30pm, and made sure to have his last meal at 5.30pm every night. Scientists say a consistent bedtime helps the body's circadian rhythm while eating too close to bedtime means the body is still digesting, disrupting sleep. Some suggest it can take three to four hours for a meal to transit into the intestines. He also lowered the bedroom temperature to 60 degrees Fahrenheit (15 degrees Celsius). Experts recommend sleeping at temperatures between 60 and 67 degrees to reduce the body's internal temperature and ensure a restful night's sleep. He had two sets of blackout blinds installed in his bedroom, as well. One that descends horizontally from the ceiling, and another that moves vertically across the window, to block out all light while he sleeps. Another lifestyle change was cutting out alcohol. Alcohol has also been linked to sleep disruption, reducing the amount of time spent in REM sleep — a stage for memory consolidation — disrupting the sleep cycle. The Alzheimer's Society says drinking alcohol also reduces the volume of the brain's white matter, which helps the brain transmit signals between different regions. This can lead to issues with the way the brain functions. Alcohol can also shrink the parts of the brain involved in memory. Franson also makes sure to exercise during the day, saying he alternates between weight lifting, cardio exercises and yoga. A July 2022 study published in the journal Neurology analyzed the health information of 501,400 people from a British health database over 11 years. Researchers found people who engaged in consistent vigorous activity — sports or working out — had a 35 percent reduced risk of developing dementia. Even people who reported completing household chores saw a benefit and had a 21 percent lower risk of dementia. A separate October 2022 meta-analysis of 38 international studies found people who engaged in activities like regularly walking, running, dancing, playing sports or swimming had a 17 percent reduced risk of developing Alzheimer's disease compared to those who did not. And while there is plenty of fancy and expensive health tech, Franson said sleep, diet and exercise are more than 90 percent of the puzzle of longevity. He added that nine percent was cancer screenings and about one percent was 'all this crazy stuff that people like to talk about.' Franson added: 'The discussion in the longevity space is often focused on the wrong thing. 'People devote a lot of time talking about things like Ashwaganda or full-body scans when, compared to sleep, diet and exercise, these are going to make only tiny incremental differences.'
