New Hyperkalemia Risk Model for CKD and Diabetes

A new risk model may help identify which patients with chronic kidney disease (CKD) and diabetes are more likely to develop hyperkalemia, granting physicians more confidence in prescribing medications like finerenone. However, researchers caution that further validation is needed.
The model is based on pooled data from two phase 3 trials of finerenone: FIDELIO-DKD (Finerenone in Reducing Kidney Failure and Disease Progression in Diabetic Kidney Disease) and FIGARO-DKD (Finerenone in Reducing Cardiovascular Mortality and Morbidity in Diabetic Kidney Disease). The combined dataset, known as FIDELITY, includes 6355 patients in the placebo groups.
Researchers used this dataset to develop the risk score, identifying seven factors independently associated with the primary outcome of new onset of hyperkalemia (incident serum potassium level > 5.5 mmol/L):
Serum potassium > 4.5 mmol/L
Prior history of hyperkalemia
No use of sodium-glucose co-transporter 2 inhibitor
Urine albumin-to-creatinine ratio > 1000 mg/g
Hemoglobin < 12 g/dL
No use of thiazide-type diuretics
Estimated glomerular filtration rate < 45 mL/min/1.73 m2
The model was subsequently validated using data from the finerenone groups in FIDELITY. In their paper, published online in the European Heart Journal , João Pedro Ferreira, MD, PhD, of the Cardiovascular Research and Development Center at the University of Porto, Porto, Portugal, and coauthors noted that the model could be used to reduce the risk for hyperkalemia among high-risk patients receiving finerenone.
'This could include tailoring of individualized treatment and follow-up strategies (eg, frequency of visits and serum potassium assessments, and use of potassium binders), thus optimizing patient management and potentially improving outcomes,' they noted.
Welcome Results, Validation Required
CKD, which affects more than 800 million people worldwide, is a common complication of diabetes. An estimated 15%-40% of people with CKD in the setting of diabetes may have hyperkalemia, noted Bernard G. Jaar, MD, MPH, clinical director for Nephrology at Johns Hopkins School of Medicine, Baltimore, who was not involved in the study.
In an email exchange with Medscape Medical News , Jaar described the study as 'welcome and much needed.' He noted that hyperkalemia poses a challenge for clinicians, as many medications required for optimal care in CKD — including renin-angiotensin-aldosterone system (RAAS) blockade agents and finerenone — can raise serum potassium.
'This is a true barrier to care,' Jaar wrote. 'Clinicians are worried about the potential complications associated with hyperkalemia, such as arrhythmias and even death.'
Jaar noted that the FIDELITY analysis has several strengths, including its large sample size of patients with varying stages of CKD and degrees of proteinuria, and a risk score model that incorporated readily available clinical variables. He added that it reports a stepwise increase in hyperkalemia risk across risk score tertiles in both placebo and finerenone groups, even though all patients were already on a maximally tolerated RAAS blockade agent.
However, he questioned the study's generalizability, given that it excluded patients if their serum potassium was > 4.8 mmol/L.
'This risk model in predicting hyperkalemia needs to be validated in other populations where diet may be different,' Jaar wrote. 'Also, this needs to be validated in at-risk populations during real-life experience and not only in patients enrolled in clinical trials, who are typically highly motivated and selected.'
Future Applications
Rajiv Agarwal, MD, MS, professor emeritus of medicine at Indiana University and a lead researcher in the finerenone clinical studies, echoed the need for further validation of this tool. Although not involved with the development of the risk model, Agarwal was an author on key publications from the FIGARO-DKD, FIDELIO-DKD, and FIDELITY studies.
Agarwal suggested that this model's approach could be applied to other therapies, including aldosterone synthase inhibitors in a similar population with CKD. He also noted the importance of incorporating validated risk models for hyperkalemia into electronic medical records or apps, particularly in countries with limited access to care, like China and India. This would allow more nuanced approaches for following up patients taking drugs with established hyperkalemia risk.
'If the patient cannot come after a month, can I make a clinical judgment and say, 'Okay, if you came back after 3 months, your risk of hyperkalemia is low enough that I'm willing to take that risk,'' he said. 'Or if the risk of hyperkalemia is high, you can tell the patient who can't come back in 30 days, 'I don't think I want to prescribe this drug for you'.'
The work on the risk model was supported by Bayer, which also funded the FIDELIO-DKD and FIGARO-DKD studies and pooled analysis.

VIENNA — The use of expanded-criteria donors for kidney transplantation in older and high-risk recipients did not provide the same survival benefit as the use of standard donors, according to an analysis of data from more than 64,000 individuals in the European Renal Association (ERA) Registry, using target trial emulation. 'When determining who is a suitable transplant candidate, we shouldn't only focus on the patient alone, because the outcomes will also depend on donor quality,' said study presenter Rachel Hellemans, MD, PhD, a nephrologist at Antwerp University Hospital, Belgium. 'Of course, ideally, we would like to give a transplant to everyone, but shortage is a reality,' she continued. The possibilities for older patients to receive a standard-criteria donor kidney depend on local allocation systems and waiting times. Waiting an extended period for a transplant carries the risk of clinical deterioration, Hellemans pointed out, and patients may want to 'settle for an expanded-criteria donor kidney — although, in general, these kidneys perform less well.' 'Even in the absence of a true survival difference, patients may still value this kind of transplantation to improve the quality of their remaining life years,' she said. But clinicians need to be careful 'to interpret and communicate the results of our study' she cautioned, 'because, despite a very thorough pretransplant workup,' and 'careful decision-making as a transplant team, there remains an element of unpredictability and some very real risks, especially in the early post-transplant period.' Exploring the Margins of Survival Benefits The research presented here at the 62nd ERA Congress 2025 was based on an analysis of data on more than 64,000 individuals on the European Renal Association (ERA) Registry. 'Transplantation is, no doubt, the optimal treatment strategy for many of our patients with kidney failure,' said Hellemans, 'and it often leads to substantially better survival compared to continued dialysis, but its successes have also made us push the boundaries.' She pointed out that, due to a lack of suitable donor organs, clinicians have increasingly turned to less-than-ideal donors, such as those who are older, have more comorbidities, or who have died due to circulatory disorders. On the other hand, patients on dialysis now have improved survival, said Hellemans. Therefore, the question becomes: 'Where do the margins lie for the survival benefit with transplantation?' 'Although this may sound like a simple question,' she continued, 'it's actually a very difficult one to answer.' Ideally, a research question like this would be answered with a randomized control trial (RCT), 'which is impossible for ethical and practical reasons.' The next best step is to turn to registry data, but this is fraught with methodological pitfalls. Hellemans said that approximately half of such studies in the field suffer from avoidable biases, likely leading to an overestimation of the true benefit of transplantation. Moreover, the most recent comprehensive assessment of the impact of recipient age and comorbidities, and donor quality on mortality risk in older patients, was published in 2013, and Hellemans pointed out that this is a fast-evolving field, and so regular updates are required. The researchers therefore turned to the ERA Registry, which contains data on 64,013 adults from France, Catalonia (in Spain), Denmark, Norway, and the United Kingdom who were on dialysis and wait-listed for a first deceased donor kidney transplant between 2000 and 2019. The study investigators looked at 5-year survival with transplantation vs continued dialysis, stratified by donor type: standard-criteria donor kidneys, or those from donors younger than 60 years of age without significant risk factors for poor kidney function; and expanded-criteria donor kidneys, which includes all donors aged ≥ 60 years, and aged 50-59 years who had at least two of the following: a history of arterial hypertension, death from cerebrovascular accident, and/or last serum creatinine > 1.5 mg/dL. The recipients were also stratified by age and presence of comorbidities, namely diabetes, and a history of cardiovascular disease. To overcome the limitations of using registry data, the researchers turned to a methodological framework known as target trial emulation. Here, they treated their observational data as if it was from an RCT, in which transplantation was considered the intervention, and each transplant launched one of a series of sequential trials comparing the outcome with that seen for patients who remained on dialysis. They then controlled for country, time on dialysis prior to wait-listing, calendar year of transplantation, patient sex, cause of kidney failure, and diabetes. This way, Hellemans explained, they could avoid the biases that normally come with registry-based studies. Provided there is no important and measured confounding, target trial emulation can 'achieve a level of evidence that closely approximates that of a true RCT.' Standard Criteria Donor vs Extended Criteria Donor The results showed that, no matter the recipient's age, the 5-year adjusted survival rate was substantially better for transplant patients who received a standard-criteria donor organ than those who remained on dialysis. However, when the researchers turned to expanded-criteria donors, they found that the survival benefit from transplant decreased with increasing recipient age — to the extent that, among those who received a donor organ after circulatory death, the advantage all but disappeared. Among patients aged 75 years and older, 5-year survival rates for recipients who received kidneys from extended criteria donors were estimated at 57%–58%, only slightly higher than the 54% in those who remained on dialysis. A similar pattern was seen when looking at recipients with diabetes and those with cardiovascular disease: standard-criteria donors were associated with a survival benefit with transplant over remaining on dialysis — no matter the recipient's age — while the benefit dropped off sharply with increasing age in patients receiving expanded-criteria donor organs. A key factor was the increased early post-transplant mortality observed in older patients receiving expanded criteria kidney donations, Hellemans reported. There was a sharp rise in mortality risk compared with staying on dialysis in the first 10 months after undergoing transplant, followed by a drop-off in risk until, at 5 years, the hazard ratio for death was 1.01 (95% CI, 0.74-1.36) for the two approaches, she explained. Limitations of the study include the heterogeneous nature of the donor population, and potential residual confounding factors due to the lack of information on the functional status of the patients, said Hellemans. In addition, the presence of diabetes and cardiovascular disease were recorded only at the time of wait-listing, thus they did not include incident cases during follow-up. Moreover, 5-year survival as an outcome has its limitations, she pointed out, as few patients remained on dialysis beyond this timeframe. Informed Discussions With Patients 'The breadth of data we could access via the ERA Registry showed that the survival advantage of a transplant plateaus for the very oldest or highest-risk patients who are likely to receive an expanded-criteria or circulatory-death donor kidney,' said Vianda Stel, PhD, associate professor in the Department of Medical Informatics at Amsterdam UMC, the Netherlands, and director of the ERA Registry, in a press release. 'This arms clinicians with guidance to have informed discussions with their patients. The message is not 'don't transplant older people,' ' she said, but rather 'be open about uncertainty when the numbers say benefit may be marginal.' 'We were always convinced that we can give a benefit by giving a transplant to every patient on the waiting list,' Christoph Wanner, MD, PhD, professor of medicine and head of the Division of Nephrology, University of Würzburg, Germany, told Medscape Medical News . 'And now we see that, in those aged 75 years, they will not have a benefit with the expanded donor criteria.' He said that the 'big question' is whether the current findings will affect clinical decision-making. 'I think dialysis doctors will respond to this and maybe keep patients on dialysis for various reasons and not push them into transplant. This new data give us the justification' to make that argument. Given that the pool of available organs cannot be expanded, Wanner believes that current waiting lists could be rationalized so that 'the organs that are available could be directed to a smaller proportion of patients, and therefore people would benefit' from lower wait times. Daniel W. Coyne, MD, professor of medicine, Nephrology/Internal Medicine at Washington University in St. Louis, Missouri, commented on X (formerly Twitter) that the benefit with expanded-criteria donation decreases with older age 'is not what my transplant group is telling our patients in the US.' Amid record-high spending for onboard and shoreside experiences since the pandemic, demand for premier and private shore excursions has reached new heights. Guests are increasingly seeking to spend time on land in a comfortable setting with only their immediate travel companions. That often means a private car rather than a tour bus and an itinerary tailored to their particular needs and cravings. Shore Excursions Group CEO Paul Kiritsy said that trend is growing in Europe in particular. It doesn't matter the ship size or whether it's contemporary or luxury, said Rinat Glinert of Venture Ashore, another third-party shore excursion provider. But there are two groups she's noticed who are particularly drawn to private experiences: travelers with accessibility challenges and families. Those groups find the convenience of a private driver and a tailored itinerary to be worth the extra expense, Glinert said. Tim Harwood, owner of MyExcursions, which specializes in higher-end, personalized experiences, said last year was the company's best ever. "A lot of people are still just looking for the personal experience to cover some of the main sites, but they don't want to step on a coach or they may be a little less mobile, and they want to have that comfort of knowing they can proceed at their own pace," he said. Cruise lines are seeing similar shore excursion trends. Christine Manjencic, Norwegian Cruise Line Holdings' vice president of destination services operations, said the company has noticed a shift over the past few years in its guests' shore excursion patterns. "We are seeing them interested in small-group tours and more immersive excursions such as Go Local experiences where they can truly dive into the culture and history of the destination they are visiting," she said. "Our classic shore excursions where guests can see highlights of the destination with a large group are still popular among first-time visitors, but we are also seeing an increase in guests who prefer a more intimate and exclusive experience that enables them to get that perfect picture for their social media." Whether it's cooking classes or a stop at a restaurant with a particularly scenic vista, guests are looking to taste the flavor of a destination on their brief stop off of the ship, said Glinert of Venture Ashore and Harwood of MyExcursions. "I think that's one of the things that really connects you to a destination," Glinert said. "You have those core memories of something that you did that was speaking to more than just your sense of sight, so I think there's a lot there that's building in the culinary space. We've got requests for people who want to go truffle hunting. All kinds of stuff." Guests spending more on excursions The interest in more premium excursions, and excursions overall, coincides with continued increased spending. Sometimes guests spend more on excursions than the cruise itself, Glinert said. "They're really looking for something that is going to elevate their cruise experience, and they're willing to pay for that," she said. Shore Excursions Group, which recently sold its 5 millionth tour, has seen the average order value increase by nearly 40% since the pandemic, Kiritsy said. The company pays commissions and has tracked an increase in travel advisor sales after integrating its own marketing into popular booking platforms advisors use like Odysseus, Revelex and VacationPort. The group's sales are up 50% from such platforms, thanks to a 200% increase in advisors activating Shore Excursions Group offers, Kiritsy said. All of this means excursions are selling out more quickly and bookings are happening earlier, Kiritsy said. For Caribbean excursions, guests are booking an average of 10 weeks prior to sailing, and in destinations like Alaska and Europe, it's 14 weeks prior, he said. In Europe, new restrictions on vehicle sizes for accessing certain sites have also caused excursions to book up because they simply can't accommodate as many people as they previously could, Glinert said. Shore Excursions Group is suggesting that advisors book excursions early to ensure clients have the experiences they want, Kiritsy said. "Once the cruise is booked and you know where you're going, we definitely recommend securing the excursions," he said. "They definitely will sell out, and you don't want to miss out on the best excursions in each of those ports."
