CDC Data Is Disappearing
Last night, scientists began to hear cryptic and foreboding warnings from colleagues: Go to the CDC website, and download your data now. They were all telling one another the same thing: Data on the website were about to disappear, or be altered, to comply with the Trump administration's ongoing attempt to scrub federal agencies of any mention of gender, DEI, and accessibility. 'I was up until 2 a.m.,' Angela Rasmussen, a virologist at the Vaccine and Infectious Disease Organization at the University of Saskatchewan who relies on the CDC's data to track viral outbreaks, told me. She archived whatever she could.
What they feared quickly came to pass. Already, content from the CDC's Youth Risk Behavior Surveillance System, which includes data from a national survey, has disappeared; so have parts of the Agency for Toxic Substances and Disease Registry's Social Vulnerability Index and the Environmental Justice Index. The CDC's landing page for HIV data has also vanished. And the agency's AtlasPlus tool, which contains nearly 20 years of CDC surveillance data on HIV, hepatitis, sexually transmitted infections, and tuberculosis, is down. Several scientists I talked with told me they had heard directly from contacts at the CDC that the agency has directed employees to scrub any mention of 'gender' from its site and the data that it shares there, replacing it with 'sex.'
The full scope of the purge isn't yet clear. One document obtained by The Atlantic indicated that the government was, as of yesterday evening, intending to target and replace, at a minimum, several 'suggested keywords'—including 'pregnant people, transgender, binary, non-binary, gender, assigned at birth, binary [sic], non-binary [sic], cisgender, queer, gender identity, gender minority, anything with pronouns'—in CDC content. While these terms are often politicized, some represent demographic variables that researchers collect when tracking the ebb and flow of diseases and health conditions across populations. Should they be reworded, or even removed entirely, from data sets to comply with the executive order, researchers and health-care providers might have a much harder time figuring out how diseases affect specific communities—making it more challenging to serve Americans on the whole.
CDC data's 'explicit purpose' is to guide researchers toward the places and people who most need attention, Patrick Sullivan, an epidemiologist at Emory University and a former CDC Epidemic Intelligence Service officer, told me. As the changes unfold before him, he said, 'it's hard to understand how this benefits health.'
When I contacted the CDC, a spokesperson redirected my requests for comment to the Department of Health and Human Services, which did not respond.
The government appears to understand that these changes could have scientific implications: The document directing a review of CDC content suggests that some work could be altered without 'changing the meaning or scientific integrity of the content,' and that any such changes should be considered 'routine.' Changing other content, according to the document, would require review by an expert precisely because any alterations would risk scientific integrity. But the document does not specify how data would be sorted into those categories, or at whose discretion.
'My fear is that in the short term, entire data sets would be taken down,' then reappear with demographic variables removed or altered to conform with DEI restrictions, Katie Biello, an epidemiologist at Brown, told me. Excising mention of gender and sexual orientation, for instance, from public-health data sets could require stripping entire columns of data out. If the government chooses to define sex as binary, transgender people and nonbinary people, among others, could be effectively erased. In response to the ongoing changes, some groups of researchers are now rushing to archive the CDC website in full.
Acknowledging and addressing health differences among demographic groups is a basic epidemiological tenet, Biello told me, 'so we know where to target our health interventions.' She pointed to examples in her own field: Gay men have higher rates of STIs, but lower rates of obesity; transgender women have higher rates of HIV, but lower rates of prostate cancer. More broadly, demographic changes to data sets could limit the country's ability to identify which Americans are most at risk from an expansive list of conditions including adolescent depression, STIs, even sex-specific cancers. Changing data sets in this way would be tantamount to 'erasing our ability to use data and evidence' to care for people, Rachel Hardeman, a health-equity expert at the University of Minnesota, told me.
Jennifer Nuzzo, an epidemiologist at Brown, pointed to mpox as a recent example of how replacing 'gender' with 'sex,' or ignoring sexual orientation, could limit effective public-health responses. At the beginning of the United States' 2022 outbreak, neither researchers nor the public had much clarity on who was most affected, leading to widespread panic. 'Officials were talking about the situation as if it was a risk we equally faced,' Nuzzo said. By collecting detailed demographic information, researchers were able to show that the disease was primarily affecting men who have sex with men, allowing officials to more efficiently allocate resources, including vaccines, and bring the epidemic under control before it affected Americans more widely.
A scrub such as this could also change how the government allocates funds for long-standing threats to public health, which could widen health-equity gaps, or reverse progress in combatting them. Rates of STIs more generally have recently begun to plateau in the U.S., after decades of steady increase—but altering data that focus interventions on, say, transgender populations, or men who have sex with men, could undo those gains. If no data exist to prove that a health issue concentrates within a particular community, that 'provides a justification to cut funding,' one researcher told me. (Several scientists who spoke with me for this article requested anonymity, for fear of retaliation for speaking out about the loss of federal data.) Sullivan, whose work focuses on HIV surveillance, compared the government's actions to, effectively, destroying the road map to determining who in America most needs screening, pre-exposure prophylaxis, and treatment.
Much of the data on the CDC website have been aggregated from states, so it would be possible for researchers to reassemble those data sets, Nuzzo pointed out. But that's an onerous task, and several scientists told me they never thought they'd be in a position where they'd have to scramble to squirrel away publicly available federal data. Nuzzo also worried that states might be reluctant in the future to share data with the federal government, or might decide not to bother collecting certain data at all. On the most basic scientific level, changing federal-government data means those data become unreliable. Public-health data are collected with the intention of sussing out which populations most need health interventions; altering those data leaves behind a skewed portrait of reality.

Though the infection is lifelong, medicines can keep the virus in check and help people reach lifespans of near-normal length. However, when people don't have access to those medicines, HIV infections progress to an advanced stage called acquired immunodeficiency syndrome (AIDS), which is fatal within about three years if not treated. When a person has AIDS, most of their key, disease-fighting immune cells are lost. This loss of immune protection leaves the person vulnerable to deadly infections and cancers. Although an HIV diagnosis was once a death sentence, scientists have developed treatments that suppress the virus and enable people to live long lives without transmitting the disease to others. Additionally, there are now effective preventive medications that can dramatically reduce the risk of getting HIV in the first place. There is not yet a widespread cure for HIV/AIDS, although a handful of people have been cured of the infection or are in long-term remission thanks to special stem-cell transplants, specially cell transplants from people who have genes that make them resistant to the virus. Scientists are exploring potential avenues for a cure, which could someday mean that people who contract HIV could be rid of the infection rather than having to take medication for life to manage the disease. HIV/AIDS remains a major public health threat worldwide, with an estimated 39.9 million people living with the disease at the end of 2023. Around 630,000 people died from illnesses related to AIDS the same year; by weakening the immune system, AIDS opens the door to these fatal diseases. HIV can spread through contact with an infected person's bodily fluids, although it's important to note that not all bodily fluids can transmit the virus. Bodily fluids that can spread HIV include blood, semen, preseminal fluid, vaginal secretions, breastmilk and rectal discharge (liquid from the anus that's not blood or stool). HIV is not transmitted through saliva, sweat or tears. It's also not spread through the air or through casual contact, such as hugging, shaking hands or sharing food. For transmission to occur, the bodily fluids containing HIV must come into contact with mucous membranes — tissues that line cavities in the body, like the vagina, anus or mouth. The fluids can also transmit HIV when they come into contact with cuts or sores, or when they're introduced to the bloodstream via contaminated needles, for instance. Most people who contract HIV get it through unprotected anal or vaginal sex — meaning sex without a condom or without HIV-preventing medications. People can also contract the virus by sharing the equipment used to inject drugs, such as needles or syringes. Babies can get HIV in the womb, during childbirth or from breastfeeding, if their mother has HIV. People living with HIV who take medicines called ART can suppress the virus to the point that it can't spread via sex. These "virally suppressed" people also have a much lower chance of transmitting HIV to their kids via pregnancy, childbirth or breastfeeding. They are also less likely to spread the virus via shared injection equipment, although experts aren't sure exactly how much the risk is reduced. The symptoms of HIV vary depending on how far the disease has progressed. The virus can spread from one person to another at any stage of the infection, unless the infected person is taking ART and has reached "viral suppression" (see glossary). The initial stage is called "acute HIV infection." Within two to four weeks of contracting the virus, many people develop a flu-like illness involving symptoms like fever, headache, rash and sore throat. These symptoms can last from a few days to a few weeks. Some people have no symptoms at this stage, however. The viral load, or amount of HIV in the blood, at this stage is very high. The second stage of the disease is "chronic HIV infection," during which the virus continues to multiply but at a slower speed than during acute infection. This stage is also called "clinical latency" or "asymptomatic HIV infection," as many people don't feel sick during it. People can remain in this stage of the disease for 10 to 15 years, though some pass through it more quickly. As the virus multiplies, levels of an important type of immune cell — CD4 T lymphocytes — decline. Without treatment, the disease will eventually enter its most advanced stage: AIDS. This can come with a wide range of symptoms, including rapid weight loss; recurring fever; night sweats; extreme tiredness; prolonged swelling of the lymph nodes; diarrhea; sores of the mouth, anus or genitals; and blotches on or under the skin or inside the mouth, nose or eyelids. It can also trigger neurological problems, like memory loss. AIDS raises the risk of severe bacterial infections and cancers, including lymphomas and Kaposi's sarcoma. It can also worsen viral infections, such as hepatitis B and mpox. Without any treatment, people with AIDS typically survive about three years. HIV and AIDS are related, in that AIDS is the most advanced stage of an HIV infection, and therefore, the HIV virus causes both conditions. AIDS can also be called a "stage 3 HIV infection." AIDS is defined in part by a very low CD4 count of fewer than 200 CD4 cells per cubic millimeter (mm3) of blood. Generally speaking, the CD4 counts of healthy teens and adults are around 500 to 1,200 cells/mm3. Anything below 500 cells/mm3 is considered low, and 200 cells/mm3 marks the threshold for an AIDS diagnosis. Doctors also diagnose AIDS by considering a patient's history of "AIDS-defining illnesses." These are medical conditions often seen in people with AIDS because their immune systems can't fight the illnesses off. They include "opportunistic" infections — those caused by germs that wouldn't necessarily harm a person with a well-functioning immune system. Such infections include a fungal infection called extrapulmonary cryptococcosis, recurrent blood infections with Salmonella bacteria, the parasitic infection toxoplasmosis, and lower respiratory infections caused by the herpes simplex virus. The bacterial disease tuberculosis poses a major risk to people with AIDS, and it is currently the leading cause of death for people living with HIV/AIDS worldwide. AIDS-defining illnesses also include cancers such as Kaposi's sarcoma, Burkitt's lymphoma and invasive cervical cancer. Others include HIV encephalopathy, which affects brain function, and HIV wasting syndrome, which causes extreme weight loss and weakness. Complications of AIDS-defining illnesses raise the risk of death, but the degree of risk varies among diseases. At all three stages of the infection, HIV is treated with antiretroviral therapy (ART) — combinations of medications that drive down the amount of HIV in the blood. Different ART drugs work in different ways to keep the amount of virus, or viral load, in check. They are available as daily pills or as shots given periodically throughout the year, depending on the person's treatment plan. It's key for patients to take their medication as prescribed, because missing pills or shots can open the door for the virus to multiply, as well as develop drug resistance, which causes the medication to work less well. ART medications can also interact with other drugs and carry some risk of serious side effects, so patients work with their medical providers to figure out which drug combination is best for them. The goal of ART is "viral suppression," which describes when a person's viral load falls low enough that there are 200 or fewer copies of the virus's genetic material per milliliter (mL) of blood. Historically, tests weren't sensitive enough to detect levels of HIV below that threshold, so doctors called this level "undetectable." Studies also found that people who reach viral suppression can't transmit the virus via sex; have a lower chance of spreading the virus through pregnancy, childbirth or breastfeeding; and likely have a lower chance of spreading it through needle sharing. This is why the slogan "undetectable equals untransmittable," or "U = U," was coined. Nowadays, some tests for HIV are extremely sensitive, so they can detect viral loads significantly below 200 copies/mL. However, experts emphasize that 200 copies/mL is still the critical threshold at which transmission risk becomes extremely low. If a person with HIV/AIDS develops another medical condition, such as an AIDS-defining illness, the individual would receive treatment for that condition in addition to their ART regimen. There is no widespread cure for HIV/AIDS. However, a handful of people have been cured of their HIV infections through stem cell transplants, and a few more are considered "potentially" cured via the same process. Stem cells can develop into different types of cells in the body. In certain cancers that affect blood cells, stem cell transplants can be used to replace the cells lost in the course of cancer treatments such as chemotherapy. Each individual who has been cured of HIV also had one of these cancers, so their doctors searched for stem cell donors who carry a rare gene that makes them resistant to HIV infection. By swapping in cells from an HIV-resistant individual, the procedure essentially locks the virus out of the patient's CD4 cells. There is one exception to this rule: A person known as the "Geneva patient" was potentially cured of HIV after a stem cell transplant, but the donor didn't have this special genetic resistance. It's unclear exactly why the man entered long-term remission from the infection after this procedure, but scientists are investigating. There have also been a couple of cases in which people's own immune systems somehow rallied against the virus and controlled it without treatment; these people are known as "elite controllers." Scientists hope to learn from both the stem cell recipients and from elite controllers to discover cures that could reach far more people with HIV/AIDS. Meanwhile, some researchers are exploring the use of gene-editing tools like CRISPR to cure the infection, while others are investigating the use of drugs and modified immune cells. Antiretroviral therapy (ART) – Combinations of medications that lower the amount of HIV in a person's blood. These drugs, given as pills or shots, prevent the viral infection from progressing to AIDS and dramatically lower a person's risk of complications and of transmitting the virus to others. Pre-exposure prophylaxis (PrEP) – Medicines that people at risk of being exposed to HIV take to prevent the infection. Viral load – The amount of HIV in a person's blood. This is measured in terms of the number of HIV RNA molecules — the virus's genetic material — found in a milliliter of blood. It's an important way to measure how well ART is working. Viral suppression – When a person's viral load falls to 200 copies/mL or lower. Viral suppression is the goal of ART, as it both lowers a person's likelihood of spreading the virus and extends their lifespan by preventing the infection from progressing to AIDS. CD4 T lymphocyte – A type of white blood cell that helps coordinate the actions of other immune cells to fight infections. HIV infects CD4 cells and uses them to multiply while the virus depletes the number of CD4 cells in the body. The delayed distribution may lead to increased health risks and strain on the healthcare systems in Africa. A former US official and aid worker revealed that the contraceptives, valued at around $11 million, include condoms, birth control pills, implants, and intrauterine devices (IUDs), which were originally intended to help women in low-income countries, particularly in sub-Saharan Africa, avoid unplanned pregnancies and protect themselves from sexually transmitted infections like HIV. However, the supplies are now stuck in storage facilities in Belgium and Dubai. According to sources at Reuters, the U.S. The Agency for International Development (USAID) has decided not to pay for their delivery and is no longer donating them as part of foreign aid programs. This delay stems from policy changes implemented by the former U.S. President Donald Trump, who reduced foreign assistance under his 'America First' approach: USAID gives update A recent internal memo from USAID, dated April, highlighted the urgent need to transfer the stock to prevent waste and minimize further costs. Despite this warning, a concrete plan has yet to be announced. With each passing month, the contraceptives edge closer to expiration. A senior U.S. State Department officials further confirmed that the contraceptives remain in storage, but no final decision has been made regarding their fate. The situation, according to the US official, is both frustrating and urgent for many health workers and aid groups. Another former USAID official said the stalled shipment represents nearly 20% of all contraceptives the U.S. usually donates each year, adding that if no solution is found soon, the entire stock could be destroyed, at a cost of hundreds of thousands of dollars. 'These supplies were meant to help women who have little or no access to family planning—girls fleeing conflict, mothers in refugee camps, and young women at risk of early pregnancy, ' one source said. ' The condoms in the shipment were also meant to help prevent the spread of HIV, which continues to impact millions across the region.' It added. Karen Hong, head of supply chain at the United Nations Population Fund (UNFPA), said her team is already working on an alternative plan. ' We cannot dwell on an issue for too long; when urgency and clarity don't align, we have to move on,' she said.

Four ways NIOSH's Spokane Research Lab, now facing closure, has improved workers' safety
Jun. 7—Spokane is home to the National Institute of Occupational Safety and Health's biggest facility in the western United States, where researchers have worked to prevent harm to workers in mining, commercial fishing, wildland firefighting and other dangerous jobs. Now, NIOSH's Spokane Research Laboratory is at risk of closure after the Trump administration notified nearly all its employees in March that their jobs would be eliminated by the beginning of July. Those terminations are on hold after courts in California ruled that President Donald Trump's mass firing of federal workers likely violated the Constitution, but the more than 80 employees in Spokane remain on paid administrative leave and their fate is unclear. Facing pressure from both Democrats and Republicans in Congress, Health Secretary Robert F. Kennedy Jr., who leads NIOSH's parent agency, has brought back about 300 of the 900 workers who were terminated — none of them in Spokane. The Department of Health and Human Services did not respond to questions from The Spokesman-Review about the rationale for the mass firing and any plans to reinstate more workers. Here are four examples of the research NIOSH employees in Spokane have conducted to improve safety for workers. 1. Protecting oil and gas workers from deadly fumes For years, federal regulations required workers in the oil and gas industry to open hatches on top of huge storage tanks to take samples of the oil inside. In 2013, a doctor at the University of California, San Francisco contacted NIOSH and the Occupational Safety and Health Administration, a regulatory agency, about the deaths of two of those workers. Working with the physician and OSHA, NIOSH researchers in Spokane used their database of fatalities in the oil and gas industry and found a total of nine similar deaths that occurred from 2010 to 2014. The deceased workers ranged in age from 20 to 63, and coroners attributed most of the deaths to heart disease, with no autopsy conducted in one case, but the NIOSH data showed a clear pattern. The truck drivers and other workers, who often worked alone, were found "collapsed over open hatch" or "slumped over on catwalk next to tank" — all while doing the same job task, "collecting sample" or "gauging." After NIOSH researchers identified the pattern of deaths, all likely due to workers opening a hatch and being engulfed in a plume of hydrocarbon gases and air that lacked oxygen, they worked with other federal agencies and the oil and gas industry to implement new regulations and safer methods of gauging the tanks' contents. 2. Detecting airborne silica to prevent lung disease in miners At hard-rock mines like Lucky Friday in the Silver Valley east of Coeur d'Alene miners can't get to valuable minerals without blasting, crushing and grinding a lot of crystalline silica, the most common mineral on earth. But when a miner breathes in silica dust, it damages the lungs and causes silicosis, an incurable disease that causes severe breathing problems and can lead to death. Currently, the standard method of measuring airborne silica involves sending a sample to a lab and waiting days or even weeks for a result, said Art Miller, a particle scientist who researched silica detection at the Spokane Research Lab until he retired in 2020. Miller and his colleagues at NIOSH facilities in Spokane and Pittsburgh, Pennsylvania, have worked for years to develop faster methods of detecting dangerous levels of silica dust. The researchers have developed a tool that produces same-day test results, but a project in Spokane to create a wearable, real-time silica monitor that would immediately alert workers to dangerous levels of dust is now in jeopardy. 3. Making commercial fishing vessels safer The NIOSH facility in Spokane houses the agency's research on maritime safety, covering waters from Alaska's Bering Sea to the Gulf of Mexico, which the Trump administration recently renamed as the Gulf of America. After a 15-year-old boy working on a shrimp boat in the Gulf died in 2012 when his clothing got caught in a winch, the Coast Guard asked NIOSH to look into the prevalence of such entanglements and find a solution. Researchers found 35 severe injuries, eight of them fatal, had occurred on shrimping vessels between 2000 and 2011 when workers got entangled in winches used to haul nets out of the water. NIOSH designed guards to fit several common winch models and made them available to shrimpers. The crab fishing season in the Bering Sea, made famous by the reality TV show "Deadliest Catch," has long been one of the world's most dangerous workplaces. From 1990 to 1999, an average of eight crab fishermen died each season. When NIOSH researchers analyzed the fatalities, they found that most were caused by boats capsizing because they were overladen with crab pots as they raced to maximize their catch in the brief season. Based on those findings, the Coast Guard began enforcing limits on crab pots, and fatalities in the still-dangerous industry fell to an average of one per year in the following 15 years. 4. Keeping miners safe deep underground The Lucky Friday Mine in Mullan, owned by Coeur d'Alene-based Hecla Mining, has the deepest mine shaft in the world at more than 9,500 feet underground. The mine shut down for more than a year and required more than $30 million in upgrades after a rock burst and other accidents killed two miners and injured seven others in 2011. NIOSH's Spokane Mining Research Division has worked with companies to improve safety at Lucky Friday and other hard-rock mines in the western United States for decades, developing techniques and technologies that have been applied by miners around the world. Brad Seymour, a NIOSH mining engineer and union steward in Spokane, started researching ground support methods to prevent deadly collapses in 1986, when the office was part of the now-defunct U.S. Bureau of Mines. Early in his career, he helped to improve cemented backfill techniques — filling underground voids with mill tailings and other material to prevent collapse — at the Cannon Mine in Wenatchee. Those improvements were adopted by other mines, he said, helping fuel a gold mining boom in Nevada in the 1990s and now improving safety and efficiency at North Idaho mines like Lucky Friday and the Galena Complex.
