BANGKOK (Reuters) -Thailand's medical council on Thursday upheld its suspension of two doctors who enabled influential politician Thaksin Shinawatra to spend his prison sentence in hospital, a day ahead of the start of a Supreme Court case that could see him jailed. Thaksin, the driving force behind the current government, returned from 15 years of self-exile in 2023 to serve a prison term for abuse of power and conflicts of interest, but was sent to hospital after only a few hours in jail complaining of chest problems. The polarising billionaire, whose daughter Paetongtarn Shinawatra is prime minister, stayed in a VIP wing of the hospital for six months until his release on parole without a single night in jail, prompting public outrage and deep scepticism about the extent of his ailments. "More than two-thirds of the council voted to uphold the punishments," Medical Council of Thailand vice president, Prasit Watanapa, told reporters. "Members made the decision based on medical principles, evidence and reason." The suspensions could impact a case at the Supreme Court that begins on Friday in which the legality of Thaksin's hospital stay has been challenged, with the possibility the tycoon could be made to serve that time again, in prison. Thaksin, 75, remains a towering figure in Thai politics and though he holds no formal government role, he is highly influential. His lawyer declined to comment on Thursday on the council's decision. The revival of the controversy over Thaksin's hospital stay comes at a challenging time for Paetongtarn's government, which is seeing its popularity dwindle amid a prolonged struggle to spur economic growth and domestic pressure to take a tougher stance on an ongoing border dispute with Cambodia. Thaksin's sentence was originally eight years, but it was commuted to a year by the king and he became eligible for parole after six months. The medical council's vote overrides a veto of its earlier decision by Health Minister Somsak Thapsutin, a Thaksin ally. The council had yet to confirm the duration of the suspension of the two doctors, who it found had issued documents that contained false medical information. They had denied wrongdoing and stood by their medical assessments. Another doctor with the corrections department received a warning for failing to meet medical standards in a referral notice for Thaksin.

Cancer Risk High in Ulcerative Colitis, Varies by Age
Compared with the general population, patients with ulcerative colitis (UC) had an additional risk of developing cancer, with about two to three more cases per 1000 person-years, regardless of immunomodulatory treatment status. Variations in the incidence of cancer were particularly noted with age. METHODOLOGY: Researchers conducted a nationwide cohort study using Swedish health registers with prospectively captured data on patients with incident or prevalent UC from routine medical practice from January 2007 to December 2021. They analysed 63,925 patients with UC who were matched with 593,072 comparators from the general population without inflammatory bowel disease. Exposure to drugs for UC, including thiopurine, TNF inhibitors (infliximab, adalimumab, or golimumab), combined treatment with thiopurine and TNF inhibitors, vedolizumab, ustekinumab, and tofacitinib, was assessed. The absolute and excess occurrence of cancer in patients with UC, regardless of their exposure to specific UC medications, was compared with the incidence of cancer in the general population at 1 year after drug exposure. Incident cancers, including cancers associated with UC, thiopurine, or TNF inhibitors and other common cancers, were identified using codes in treatment-naive patients and those exposed to drugs. TAKEAWAY: Incidence rate differences for any cancer compared with the general population were 2.66 cases per 1000 person-years in the treatment-naive cohort, 3.38 in the thiopurine-treated cohort, 2.69 in the TNF inhibitor-treated cohort, 2.42 in the thiopurine plus TNF inhibitor-treated cohort, and 2.88 in the vedolizumab-treated cohort. The risk for any cancer was higher in the treatment-naive cohort (hazard ratio [HR], 1.12; 95% CI, 1.09-1.16) and in those treated with thiopurine (HR, 1.48; 95% CI, 1.37-1.61), TNF inhibitor (HR, 1.41; 95% CI, 1.24-1.62), thiopurine plus TNF inhibitor (HR, 1.44; 95% CI, 1.19-1.75), and vedolizumab (HR, 1.27; 95% CI, 0.90-1.79) than in the general population. The incidence rate differences for any cancer were more pronounced in middle-aged (40 to < 60 years) and older (≥ 60 years) patients, showing 1.76 to 1.57 cases per 1000 person-years in the naive cohort and 3.45 to 9.69 cases per 1000 person-years in the thiopurine-treated cohort. However, the risks were increased in younger patients (18 to < 40 years) but rarely in older patients. The highest risks were observed for hepatobiliary cancer across treatment groups (naive, thiopurine, TNF inhibitor, and thiopurine plus TNF inhibitor), followed by colorectal cancer and lymphoma that were noted in all these groups except the naive group. IN PRACTICE: "Screening of patients with IBD [inflammatory bowel disease] for other forms of cancer, such as hepatobiliary cancer and basal cell skin carcinoma, has been suggested," the authors wrote. Minor Increase in Stroke Risk After Anti-VEGF Eye Injections
Intravitreal injections of anti-VEGF were associated with a small but significant increase in the risk for stroke among patients with neovascular age-related macular degeneration. The risk was higher with aflibercept and bevacizumab than with ranibizumab and declined 60 days after the last injection. METHODOLOGY: Researchers analyzed data from two Swedish national registries from 2007 to 2019 to investigate whether intravitreal anti-VEGF injections (ranibizumab, aflibercept, and bevacizumab) for age-related macular degeneration were associated with an increased risk for stroke. They identified patients with neovascular age-related macular degeneration who developed stroke or transient ischemic attack within 90 days of receiving an intravitreal anti-VEGF injection and matched them with individuals who experienced stroke but did not receive such treatment. The number of days between the last injection and the occurrence of stroke was categorized into three intervals: 0-30 days, 31-60 days, and 61-90 days. TAKEAWAY: Among 33,585 patients with neovascular age-related macular degeneration receiving anti-VEGF injections, 936 (approximately 2.8%) experienced a stroke within 90 days of the last injection. Patients who received anti-VEGF injections were at a 27% higher risk for stroke within 90 days of the last injection than those who did not receive the treatment (risk ratio [RR], 1.27; 95% CI, 1.22-1.33). The risk for stroke was elevated in the first 0-30 days (RR, 1.38; 95% CI, 1.15-1.66) and 31-60 days (RR, 1.40; 95% CI, 1.09-1.79) after the last injection but declined thereafter. Aflibercept showed the highest risk (RR, 1.36; 95% CI, 1.23-1.52), followed by bevacizumab (RR, 1.26; 95% CI, 1.10-1.46), whereas ranibizumab demonstrated the lowest risk profile. IN PRACTICE: 'For patients with nAMD [neovascular age-related macular degeneration] in need of anti-VEGF treatment who have a history of cerebrovascular events, have atrial fibrillation or hypertension, and who are smokers, it seems prudent to try to reduce stroke risk factors,' the researchers reported.
