Where does India stand in its fight against malaria today?
The concept of the 'last mile' is particularly pertinent to India's malaria elimination efforts. Despite significant progress, most malaria-related illnesses and deaths still occur in remote, tribal, and forested areas. These regions continue to have limited access to timely and quality healthcare, making this final stretch in the fight against malaria both the most difficult and the most important. However, efforts such as strengthening the primary healthcare system through Ayushman Arogya Mandirs (AAM) are improving healthcare delivery. These centers are helping improve healthcare services at the local level by bringing essential diagnosis, treatment, and prevention closer to communities that need them most.
How do you see the integration of malaria services into existing primary healthcare systems evolving, especially under the Ayushman Bharat Health and Wellness Centres framework?
Ayushman Bharat Health and Wellness Centres, now renamed Ayushman Arogya Mandirs (AAMs), have been set up to deliver 12 essential primary healthcare services, including the prevention and control of communicable diseases. In this setup, integrating initiatives that emphasize locally endemic diseases such as malaria is very important, particularly in areas where the disease is widespread. In such regions, early detection, timely treatment, and preventive measures will be key priorities. Strengthening the role of frontline health workers, ensuring a steady supply of diagnostic tools and antimalarial medicines, along with increasing community awareness, are all important steps in making malaria care more accessible through AAMs.
Tata Trusts has been involved in strengthening health systems. How central is primary healthcare in the roadmap to malaria elimination, especially in tribal or hard-to-reach areas?
An effective primary health system is crucial for ensuring community well-being and achieving equitable health outcomes. A strong foundation in primary care can deliver long-term benefits across the population. Tata Trusts' healthcare portfolio has consistently focused on strengthening primary healthcare and health systems, both in urban centers and in hinterlands across the country, while actively supporting government efforts to build resilient and responsive healthcare infrastructure. In Madhya Pradesh, Tata Trusts is supporting the state government in bolstering 500 selected Ayushman Arogya Mandirs (AAMs) to effectively deliver the 12 essential components of primary healthcare, as envisioned under the National Health Mission. The objective is to develop these AAMs into model centers of comprehensive primary care. In Odisha, the state with the highest incidence of malaria cases, the Trusts supported the state government in ensuring early detection and treatment of malaria in selected tribal pockets. This focused intervention has led to a significant reduction in the disease burden across the project areas.
What challenges persist in early diagnosis and treatment of malaria at the primary care level, and how can these be addressed through better workforce or technology support?
Key challenges in early diagnosis and treatment of malaria at the primary care level include delayed detection, limited human resources, and inadequate medical infrastructure, particularly in remote areas. Access to emergency healthcare for severe malaria cases, such as cerebral malaria, renal complications, and anemia in pregnant women, remains a significant barrier. To address these, strengthening Ayushman Arogya Mandirs (AAMs), enhancing diagnostic capacity, and improving emergency transport systems are critical. Targeted efforts involving key stakeholders and public-private partnerships (PPPs) can further aid surveillance, continuous reporting, and deployment of trained personnel in remote areas.
India is closely watching the rollout of the RTS,S/AS01 malaria vaccine in Africa. How do you see the malaria vaccine fitting into India's broader malaria elimination strategy, especially in high-burden states like Odisha, Chhattisgarh, and Jharkhand?
The RTS,S/AS01 malaria vaccine targets
Plasmodium
falciparum (P. falciparum), the parasite that causes the most severe and often fatal cases of malaria. In India, P. falciparum is the most common strain found in tribal and forested regions, especially in high-burden states like Odisha, Chhattisgarh, and Jharkhand, areas that still face major challenges due to limited access to healthcare. Given these conditions, the vaccine could be a valuable addition to India's malaria control efforts. It has the potential to be effective in these high-risk regions if included in the national immunisation programme.
How can digital health tools be leveraged to monitor vaccine coverage, track outcomes, and flag gaps in real-time?
Digital health tools offer significant potential to enhance malaria control efforts. By integrating real-time data systems, we can track vaccine coverage, monitor treatment outcomes, and identify geographic or demographic gaps quickly. Portable microscopes to test for suspected cases of malaria and mobile-based platforms can support frontline workers in capturing and uploading data at the point of care, while dashboards can enable timely decision-making at district and state levels. , a hub-and-spoke telemedicine model was implemented to improve rural healthcare access, reaching villages around Vijayawada. In Uttar Pradesh, the Trusts partnered with NGOs such as

in Vrindavan and Varanasi to support grassroots digital health initiatives. In Telangana, a joint venture with the state government was successfully scaled and evolved into a sustainable program. The experiences from these projects can be leveraged by the government in strengthening the supply chain tracking systems, improving accountability, resource allocation, and program responsiveness—ultimately contributing to more efficient and equitable malaria surveillance and intervention delivery.
Looking ahead, what kind of policy and operational groundwork should India lay now to be malaria vaccine-ready in the coming years?
In the coming years, India's malaria vaccine strategy should focus on areas where Plasmodium falciparum is most common, mainly remote and tribal regions that carry a large share of the malaria burden. To prepare for vaccine rollout, it is important to start building the necessary policies and systems now. This includes identifying high-risk districts through strong disease tracking, planning for the vaccine within the existing immunisation system, and ensuring that cold storage facilities are available even in the most remote locations.
What innovations—whether digital, diagnostic, or programmatic—are you most optimistic about in helping India eliminate malaria by 2030?
Programmatic innovations are crucial to eliminating malaria by 2030. Strengthening disease surveillance with advanced technologies will provide more accurate estimates of the malaria burden, support informed decision-making, and enhance risk prediction. Developing new methods of vector control and personal protection will complement current strategies, improving their effectiveness and sustainability. Through genome mapping of mosquitoes, Tata Institute for Genetics and Society (
TIGS
) has identified over 3,000 previously uncharacterized genes linked to insecticide resistance and parasite transmission. Leveraging this data, TIGS is developing gene-drive technologies aimed at rendering mosquitoes incapable of transmitting the Plasmodium parasite, thereby reducing malaria incidence without harming ecosystems. This innovative approach exemplifies how precision science can be harnessed to address public health challenges in an environmentally conscious manner. Additionally, improving logistics and ensuring the quality of malaria treatments and diagnostics will ensure their consistent availability. Timely detection and diagnosis in both high and low-endemic regions, supported by rapid diagnostic tests and mobile platforms, will be key to preventing further spread.

Delhi govt.'s new primary healthcare centres promise more tests, less wait
Tucked inside the Tis Hazari courts complex, a yellow-painted health outpost, once a Mohalla Clinic, now wears new branding: Urban-Ayushman Arogya Mandir (U-AAM), announced through placards and signboards in Hindi and English. When a group of lawyers walk in, curious about what has changed, doctors tell them that 14 diagnostic tests, previously outsourced, are now conducted in-house. Prenatal and postnatal care, family planning services, and biweekly yoga sessions (yet to begin) are also part of the expanded offering. Upgraded care Inside the main room, handmade posters designate specific areas — a first-aid management area, a wellness space, and a breastfeeding corner, which is placed behind curtains. Smaller rooms branch off into spaces for doctors, pharmacy, registration, diagnostics, and storage. Of the two visitors, one has gastrointestinal issues and the other suffers from weakness. 'I have been coming here for minor health issues like cold and cough for years. Now, more tests are conducted here than before,' says Anil Tomar, who practises in the chambers nearby. The staff at the U-AAM, which offers free primary healthcare services, includes a doctor, a nurse, a multitask worker, and a sweeper-cum-guard. However, two doctors affiliated with the Delhi government are currently overseeing the clinic. Footfall hovers around 70-80 patients daily, according to a doctor at the facility. Quick diagnostics U-AAMs are part of the Delhi government's plan to improve the city's health infrastructure. The government has promised to set up 1,139 such clinics in phases. While some of these are revamped Mohalla Clinics started by the previous Aam Aadmi Party government, many of the others are being newly built. The doctor in charge explains that tests for haemoglobin, blood sugar, malaria, and dengue now happen on-site. 'Reports here now get ready in 30 minutes, which was never seen before,' he says. Another doctor, formerly at Lok Nayak Hospital, adds that most patients come for fever, joint pain, hypertension, and diabetes. 'Earlier, people would queue for hours, losing their daily wages. Now, that will change.' Being inside the court complex, the majority of early users are lawyers, guards, and shopkeepers. However, residents from nearby areas have also begun to visit. Of the 93 diagnostic tests listed under U-AAM, 14, including cervical cancer screening, happen in-house. Seventy-nine other tests are outsourced. The nurse also handles the pharmacy and family planning, while a yoga instructor is expected to start sessions, twice a week, soon. Low awareness In Inderpuri's F Block, another U-AAM quietly functions less than a kilometre from a still-operational Mohalla Clinic. But awareness remains low. 'I haven't heard about the new clinic's benefits. We still go to the old one,' says Vishal, a resident whose house is equidistant from both. Sushma, another local, says she is fine with both the clinics. 'I just want my medicines when I need them,' she says. Meanwhile, former Mohalla Clinic employees, who are currently protesting their removal, say they have had no communication with the new U-AAM teams, who were recently posted at the facilities. -Ammar Aziz Safwi

Bihar: Gaya residents get eye treatment, free of cost under Ayushman Bharat Yojana
Lok Nayak Jaiprakash Eye Hospital, inaugurated five years ago in 2020, has become a landmark location in Bihar's Gaya, owing to its famed eye care centre catering to more than 10 lakh people, not just from the city but also adjoining towns including Nawada, Jehanabad, Aurangabad, and Nalanda. The hospital, situated at Kajha village of Wazirganj block of Gaya, is primarily an eye hospital, providing treatment for all types of eye diseases. The hospital has a modular Operation Theatre, two other OTs, 100 indoor patient beds and is equipped with a pathological lab, a spectacle and medicine shop, world-class diagnostic equipment, and a team of trained eye surgeons and paramedical staff. The hospital is again in the spotlight over providing free eye surgery to thousands of patients under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB PM-JAY). About 40 per cent of the patients in the hospital are Ayushman cardholders and hence have access to medicines as well as surgeries free of cost under the yojana. Many Ayushman Bharat scheme beneficiaries shared their story with IANS and expressed gratitude to Prime Minister Narendra Modi for launching such a scheme. Balinder Singh of Bhedia village in Manpur block said: "I was treated under the Ayushman card. I got timely treatment, food and all facilities. If this card was not there, my treatment would not have been possible." 'In our family, my wife has this card. There is a younger boy, he will also get it made. I thank the central government for this scheme," he added. A woman named Malti Devi, who got her eyes treated at the hospital, said: "My card has been made. The eye operation has been done without any money spent. I want to thank the Modi government. With the help of the Central government, my eyes have been treated." She further stated: "This government is taking special care of the poor. Ayushman cards are being made in rural areas. This initiative is spreading awareness, also. People get to know from one person to another that eye treatment is being done in the hospital. This makes people aware, and they also get themselves treated. If this card had not been there, my eyes would not have been treated. This has been possible because of the Central government, and I want to thank PM Modi for this." Notably, the hospital provided eye care services to 26,240 OPD patients and performed cataract surgery on hundreds of patients through 637 rural eye camps last year. In view of its quality of services and social impact, it was also awarded a certificate of appreciation in 2022 and 2023.

Bihar pushes up its Ayushman Bharat health insurance coverage
Bihar is now among the Indian States leading in the enrolment of cardholders under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), with 3.94 crore Ayushman cards created till date, according to data released by the State government. It has also helped beneficiaries save over ₹1,000 crore in out-of-pocket medical expenses within a single year. Bihar is the third most populous State in the country, with about 80% rural population, and a literacy rate of less than 70%. It faces health challenges, including high infant and maternal mortality rates, malnutrition, and limited access to quality healthcare. As part of its drive to ensure maximum coverage, a recent four-day drive, from May 26-30, brought 17.38 lakh more cardholders into the Central government-run cashless health insurance scheme, offering treatment for secondary and tertiary care hospitalisation for the economically weaker sections of society. Uttar Pradesh and Madhya Pradesh are the other two States that have recently indicated a high enrolment rate. The flagship health insurance scheme, launched in 2018, offers cashless healthcare treatment of up to ₹5 lakh per family per year. Initially, the State Government had adopted the Socio Economic Caste Census (SECC 2011) data for implementing the AB-PMJAY scheme, Shailesh Chandra Diwakar, Administrative Officer, Bihar Swasthya Suraksha Samiti (Ayushman Bharat), said on the recent focus to expand the coverage of the Ayushman card. In February 2024, the Bihar Government adopted the National Food Security Act (NFSA) database and provided coverage to all NFSA-registered family members. 'This saw many more ration cardholders coming into the AB-PMJAY fold. More than 2.8 crore Ayushman cards were created in 2024,' Mr. Diwakar said. Besides this, the State has launched various measures, including an enhanced cash incentive of ₹15 (from ₹5) to ASHA workers involved in verified enrolments; camps at railway stations to capture those in need when people travel back into the State for festivals; and door-to-door campaigns. 'These extensive mop-up rounds are for individuals who have been left behind,' Shashank Shekhar Sinha, CEO, Ayushman Bharat (PMJAY-Ayushman Bharat Digital Mission), said. The State has also recently registered enrolment of 2.82 lakh persons under the Ayushman Vaya Vandana card (for senior citizens 70 years and above). According to the State government, the total claims paid rose from 3.03 lakh cases in FY 2023-24 to 7.52 lakh cases in FY 2024-25, and more than 2.83 crore Ayushman cards were created in 2024. A first time Ayushman card user, speaking of his enrolment, and experience with availing the treatment, said that he had had his card made only after he reached the hospital to be treated for cataract. 'Though we had heard about the Ayushman card, since we didn't need hospitalisation, we never got the card made,' Lalan Choudhary, a resident of Bihar's Nalanda district, who is now undergoing free treatment at a private healthcare facility in Gaya under the AB-PMJAY, said. 'This is a problem we are facing in the State — though people know about the card, they will enrol under the scheme often only when they seek treatment,' Mr. Diwakar added. The State is now trying to rope in eligible individuals who have not opted for the scheme. Khushi Praveen, 19, who was operated for spinal deformity 10 days ago, speaking about her use of the Ayushman card, said that she had been suffering for a nearly a year without money to secure treatment and surgery. 'Though I needed surgery, without money, there was no access to healthcare. My doctor in the private sector suggested that I get my Ayushman card and get operated. Using the card, I got surgery and am healing well now, and hope to be able to walk soon,' Ms. Praveen said. Parmilla Devi, 50, had her cataract operation performed under the scheme. 'My husband and I work as farmers and couldn't have afforded the surgery otherwise. My husband is also now planning to apply for his card also,' Ms. Devi said. Rajaram Prasad, Chief Surgeon, Gaya district, said that in Gaya district (one of the 38 districts in Bihar), which is seeing robust enrolment and use of the card, in 2024-25, a total of 18,734 people were treated in government hospitals at an expense of about ₹15 crore under the Ayushman scheme. 'Affordable, quality medical care is now no longer a distant dream for the rural population of Bihar,' Dr. Prasad said. Besides the enrolment of more individuals under the scheme, Bihar is also looking at enhancing the number of hospital service providers. Currently, over 1,100 hospitals are empanelled under the scheme in Bihar, 50% of which are private hospitals, a senior Bihar health official.
