Immunisation Advisory Centre staff cut by nearly 40%, leaked document reveals

A public health specialist has warned against cuts to the organisation that provides education, support and clinical advice to the country's immunisation workforce.
A leaked internal document from the Immunisation Advisory Centre (IMAC) described staff numbers being slashed by almost 40 percent and significant reductions to some services after Health New Zealand more than halved its funding.
Neither Health NZ nor UniServices - the University of Auckland company IMAC fell under - would confirm the scale of the cuts or the impact on the centre's workforce.

"It's really important work and it's not getting any easier," he said.
New Zealand was facing historically low vaccination rates, with childhood immunisation coverage dropping back to levels not seen in about 15 years and overall coverage of 80 percent or less, which was a disaster for controlling infectious diseases, Baker said.
He questioned the potential savings from IMAC cuts.
"When economists look at vaccinations, it's about the most cost-effective thing you can do for any country. If you vaccinate enough people you actually stop the infection altogether - that's the idea of herd immunity, that's why New Zealand was so lucky to achieve measles elimination a few years ago," he said.

Health NZ insisted it had increased IMAC funding, while UniServices acknowledged the loss of "some funding" had meant a reduction in services. A current staff member, speaking on the condition of anonymity, confirmed the more sweeping cuts outlined in the restructure document.
Both organisations refused to comment further when asked to explain the discrepancy.
National Public Health Service director Dr Nick Chamberlain said Health NZ had provided a "significant increase in total baseline funding for IMAC this year".
The increase excluded Covid-19 specific contracts, Chamberlain said.
UniServices chief executive Greg Murison said IMAC had cut some services, including reduced hours for its clinical communications centre, moved most courses to online only and reduced staff capacity.
Since its creation almost 30 years ago, IMAC had held the National Immunisation Education and Co-ordination contract, securing an additional contract in 2020 to provide Covid-19 education and clinical support, he said.
Both contracts were rolled into one integrated contract for January 2023 to December 2024, along with some separate Covid-19 funding. IMAC was advised in the middle of last year that Covid-19 funding would likely end from January this year, Murison said.
However a "change process confirmation" document obtained by RNZ detailed deep cuts.
"Heath NZ have offered less than half of the funding we have previously enjoyed for the next IMAC integrated contract," the document said.
"In real terms, the level of funding we have been offered is lower than the funding IMAC had prior to Covid-19."
The new contract meant "a significant reduction in education and clinical support services for the immunisation sector", it said.
Changes included reduced 0800 IMMUNE service hours (an 0800 number that provided support to vaccinators), a reduction in the number of advisors on each shift, reduced medical advisory availability, a freeze on new courses and post-course support, and a reduction in the number of antigen reviews and updates to the immunisation handbook.
The final decision was delayed due to the volume of submissions from staff, who raised concerns about the proposal's risk to clinical safety, saying it "jeopardized the safety and well-being of individuals as well as eroding the public's confidence in the immunisation programme" and hampered the organisation's ability to respond quickly and effectively in a health emergency.
The document confirmed just over 16 roles would go, leaving a new structure of 28.9 full time equivalent positions.

Staff urged UniServices to consider a reduced working week or reduced hours for some jobs to retain more roles.
Some staff whose role had been cut complained of finding out in group meetings or via PowerPoint presentations.
"This is impersonal, callous, distressing. Left me feeling unsupported and undervalued," one said.
The review panel agreed with many of the concerns staff raised and said it had warned Health NZ of clinical safety risks, while risks for the university and its staff had been "communicated at the highest level in the university".
The proposed restructure "was not taken lightly and was made on the need to fit resource within the funding envelope".
Staff warned job cuts would place additional strain on those left behind, heightening the risk of burn-out and resulting in course cancellations.
They described feeling blindsided by the process immediately before Christmas.
The panel said the reduced funding was only confirmed in December, despite the contract coming into force on 1 January, which lead to timing it described as "not ideal".
A current staff member, who RNZ agreed not to name, said the cuts were anything but minor and disputed Health NZ's claim of boosted funding.
"I can definitely confirm funding has been significantly cut and it meant we lost nearly half of our staff at the beginning of the year," she said.
"It's significantly impacted our ability to support the sector and it's put our staff under a lot of pressure."
She said the restructure was taking a toll on staff.
"We're starting to see internally the pressure is building for people who are left, and it means the job is very unenjoyable, which then means senior and experienced staff want to leave," she said.
"When we get that sort of exodus of experience, you lose the crux of our organisation essentially, because it is such a complicated environment you can't just backfill those positions with anybody."
IMAC had pulled back on working with the university's nursing students and
community work with Plunket
, which were not considered a priority, while cuts to the 0800 IMMUNE line had the potential for serious flow-on effects, she said.
"We service about 150 calls a day and on busy days - flu season and that sort of stuff - it goes up to over 200 calls a day," she said.
"A lot of people will turn patients away because they don't feel comfortable vaccinating, or they'll make errors because they couldn't get the information they needed."
The Ministry of Health would not comment on whether a separate contract providing vaccine training, information and communications support in the Pacific Islands had also been reduced, citing commercial sensitivity.

Te Pāti Māori MP Hana-Rawhiti Maipi-Clarke was suspended for seven days - but had also been punished with a 24-hour suspension on the day over a haka all three had performed in Parliament, against the Treaty Principles Bill, in November. It is against the rules of the House for members to leave their seats during a debate - which all three did. Ngarewa-Packer told Saturday Morning that the 21-day suspension, which was seven times harsher than any previous sanction an MP has faced, was not proportionate. "I think the backlash from the public, nationally and internationally, validates that," she said. Previously, the longest suspension for an MP had been three days, given to the former prime minister Robert Muldoon for criticising the speaker in the 1980s. While New Zealand First leader Winston Peters said the duration of the suspension would have been lessened if the Te Pāti Māori MPs had apologised, Ngarewa-Packer said that was never requested by the Privileges Committee. "What we have here is a situation where, and some are calling it Trumpism, we've been a lot more specific - we have an Atlas agenda that has not only crept in, it's stormed in on the shores of Aotearoa and some may not understand what that means, but this is just the extension of the attack on the treaty, on the attack on Indigenous voices. "We made the point the whole way through when we started to see that they weren't going to be able to meet us halfway on anything, even a quarter of the way, on any of the requests for tikanga experts, for legal experts when we knew the bias of the committee." Ngarewa-Packer added that the Privileges Committee process was not equipped to deal with the issue. "We hit a nerve and we can call it a colonial nerve, we can call it institutional nerve... "I think that this will be looked back on at some stage and say how ridiculous we looked back in 2025." Twelve-month prescription renewals: What you need to know
The government's plan will allow some patients to go longer between prescription renewals. Photo: 123RF In the annual flood of Budget coverage a few weeks back, one of the big announcements for the health sector was a move to allow 12-month prescription renewals from 2026. But how is it going to work and how will it affect your health services? RNZ is here to explain it all. The current maximum prescription length for most medicines is three months, but the government's new plan would extend that to 12 months for many medicines. This means that some patients will no longer need to visit their GP every three months to get their standing medication renewed. While some repeat prescriptions can be filled without a doctor's visit now by contacting their office, guidelines by the Medical Council of New Zealand on good prescribing practice say "patients receiving repeat prescriptions should be assessed in person on a regular basis" and repeats are given only with a doctor's discretion. In announcing the plan , Health Minister Simeon Brown said the current system "creates unnecessary barriers for patients on stable, long-term medications like asthma inhalers, insulin for diabetes, and blood pressure tablets. It means added costs for patients and more paperwork for health professionals, taking time away from patients with more urgent or complex needs". Medicines will still have to be dispensed every three months from pharmacies. "It's a win-win for patients and the health system - fewer avoidable hospitalisations, better health outcomes and reduced long term costs," Brown has said. Not until the first quarter of 2026. The increased prescribing limit will also apply to oral contraceptives, which currently have a six-month prescribing limit. Royal New Zealand College of GPs medical director Dr Luke Bradford said that during consultation on the plan, the college had supported a six-month prescription renewal instead of 12. "We submitted on this and said 12 months is probably too long, six months would be reasonable." Dr Bradford said there would not be a centralised list of drugs that are part of the scheme and physicians will need to determine who benefits from 12-month renewals. "It's very much going to be a patient by patient situation and we can't put blanket rules across this. It depends on a whole raft of things but predominantly depends on conditions being managed." If you have multiple prescriptions for multiple conditions, you are far less likely to get 12-month renewals as a physician needs to monitor your ongoing health and possible interactions. "Prescribers will be responsible for determining the clinically appropriate prescription length and can prescribe for up to 12 months, or for shorter periods if they consider that most appropriate," a spokesperson for the Ministry of Health said. "It is expected this change will increase patients' access to medicines and therefore create a higher demand for medicines. Funding is required for the Combined Pharmaceutical Budget to meet the increased demand for medicines," the Ministry of Health spokesperson said. "The Budget includes $10m initial funding in the 2025/26 financial year, with outyear funding held in contingency, pending information gathered from the initial uptake." Some patients won't have to go to the GP quite as often, in theory. GP visits are charged differently from place to place and can run anywhere from $20 to $90 depending on the complicated calculation of fees at individual doctors' offices. The Ministry of Health has said that the average patient who renews prescriptions four times a year could save up to $105 a year on GP fees. It also has said the change could mean less time off work or school for appointments and it could make life easier for people in rural and remote areas who travel to get to their doctor. No. The $5 co-payment for most prescriptions - which was brought back under the current National-led government - will remain unchanged. In a statement to RNZ, the Pharmaceutical Society of New Zealand said it cautiously supported the move, but had concerns. "PSNZ support the increased period in principle, but with some provisos - as always, it's the detail that will matter and we are waiting to receive that. "For example, there is currently no indication that funds to support the extra monitoring and counselling that may be required, when patients are picking up their medication every three months without any interacting with their general practice." If people visit their GPs less often, it will affect the income their practices receive. In its submission on the proposal , the Royal College said prescriptions account for between seven to 10 percent of practice revenue. "The financial impact on practices that rely on revenue from current prescription renewals could be substantial, e.g., one specialist GP calculated a $320,000 loss in a patient population of 14,500," that submission also reported. "There will definitely be an impact on that," Dr Bradford said. At the same time, it won't necessarily reduce the admin work needed. Photo: RNZ / Nick Monro On its website, the Ministry of Health said the Medicines Regulations will need to be amended to increase the maximum length of prescriptions, and IT systems will need to be updated to allow this change. The Medicines Regulations 1984 will need to be amended. Pharmac will make changes to the Pharmaceutical Schedule, while Health NZ will work to support GPs, other prescribers and pharmacies with updating systems and the transition. The Pharmaceutical Society said it also would work to ensure the prescription plan changes go smoothly. "PSNZ will use this time to engage with officials to support a smooth transition and highlight any unintended consequences, particularly any changes in workload and financial pressures that this change could place on pharmacists and their teams," it said in its statement. General Practice Owners Association chair Dr Angus Chambers recently told RNZ that many patients with chronic conditions needed regular reviews by a doctor or nurse. "Just to leave someone for 12 months to have their diabetes deteriorate may well lead to more emergency department visits and hospital admissions." Everyone manages their conditions differently, Dr Bradford said, and no two cases are quite the same. "If you're on five different meds for diabetes you're going to need three-month checkups." In announcing the plan, Simeon Brown said it would give GPs a breather. "Instead of spending time on routine repeat prescriptions, they can focus more on supporting those with complex or deteriorating health needs." Dr Bradford said the current three-month renewals do give a "safety net" for physicians to keep on top of issues. "By taking that away we either say we've still got to do that or the pressure goes on GPs to do it anyway and have to sit their own reminders for those periods of time and follow it up in their hour of clinic time." "The majority of patients will be fine," but a visit to a GP involves a lot more than just checking off a prescription renewal box, Dr Bradford said. "The moment you come in we're watching how you're walking, we're looking at your body language and listening for cues and we're reminding you of the screening you haven't done. "Those things that happen because you come in and say, 'Oh, I'm just here for my asthma medicine, doc,' are at risk of being lost."
