Researchers uncover disturbing health threat impacting older adults across the country: 'Acting now could save lives'
What happens when a nation lives longer but breathes dirtier air? Researchers in Japan set out to answer that question, uncovering the deadly toll of fine particle pollution on the country's oldest citizens.
A study from the University of Tokyo published in the Nature Sustainability journal highlighted the health risks and care disparities that the elderly face from exposure to fine particle pollution, also known as PM2.5.
According to IQAir, PM2.5 levels in Japan are 1.7 times higher than the World Health Organization's recommended limit.
By analyzing 170,000 medical records from across Japan, the researchers found that people over age 65 are disproportionately affected by the health impacts of fine particle pollution and limited access to medical care.
This medical access disparity, the researchers noted, stems from the fact that much of Japan's elderly live in rural areas where health care infrastructure is less developed and more costly to maintain.
"Many rural areas lack the specialized hospitals and trained professionals needed to treat diseases exacerbated by PM2.5, such as strokes and heart attacks," lead author Yin Long said in a press release.
Long added: "As we age, our immune systems weaken and our bodies are less able to defend against pollutants. Even moderate exposure can exacerbate pre-existing conditions, leading to higher hospitalization rates and premature mortality."
Notably, about 30% of Japan's population is age 65 or older, the second-highest proportion among the world's countries and economies after Monaco, according to the World Bank.
As Long explained, aging populations are especially vulnerable to fine particle pollution due to weakened immune systems and declining respiratory and cardiovascular function.
Fine particle pollution has been linked to serious health issues, including heart attacks, strokes, respiratory disease, and early death. Because these particles are so small, they are difficult to avoid, making prevention a major challenge.

These risks are compounded by gaps in access to medical care, especially for aging people living in rural areas. But the consequences don't stop with the elderly. Illness and early retirement among older adults can strain entire families emotionally and financially.
"For some working-age seniors, PM2.5 exposure is linked to increased rates of severe illnesses, forcing many to leave the workforce earlier than planned," Long said in the press release. "This not only affects their financial independence, but also places additional pressure on younger generations to support them."
The researchers highlighted the need for targeted support for aging populations, especially as medical advances allow people to live longer than ever before — in a more polluted environment than ever before.
The researchers hope this new data will motivate lawmakers to take action against fine particle pollution. They recommended stricter pollution policies, including targeted assistance to protect vulnerable populations such as the elderly.
Increasing green infrastructure in urban areas is another key recommendation, as plants can naturally filter pollutants from the air. The researchers also advocated for broader public health support, including subsidies for elderly care and investments in community health programs.
Additionally, they highlighted the need for strengthened telemedicine infrastructure to help elderly residents in urban areas access medical care without distance as a hindrance.
By identifying aging populations as particularly at-risk groups, the researchers said their research can help the government "allocate resources more effectively."
"The health of our elderly is not just a personal matter, it's a public issue with profound social and economic implications," Long said in the press release. "Acting now could save lives and reduce long-term costs for everyone."

It's important to note that while New York City is currently in the top spot for unhealthy air, it will likely drop back down once the aforementioned smoke disappears. A few weeks ago, Denver, Colorado, was ranked among the top 10 worst in the world due to wildfire smoke. As of Wednesday, the city ranked 96th on IQAir's list. IQAir's live Air Quality and Pollution City Ranking ranks cities based on AQI, or Air Quality Index points. Cities/areas are assigned a series of ranges for how healthy or unhealthy the air currently is. To find your city/area, you'll need to allow IQAir to access your location. Once the system locates you, it'll give you your area's AQI, in addition to which main pollutant is contributing to the pollution level. Ranges include: Good, Moderate, Unhealthy for Sensitive Groups, Unhealthy, Very Unhealthy, and Hazardous. NEW YORK (WWTI) – New York DEC Commissioner Amanda Lefton and DOH Commissioner Dr. James McDonald are issuing an Air Quality Health Advisory for fine particulate matter on June 7, 2025, due to wildfire smoke from Canada. Air quality alert issued for much of New York State The pollutant of concern is: Fine Particulate Matter The advisory will be in effect 10 a.m. through 11:59 p.m. The Air Quality Health Advisory regions consist of: Adirondacks, which includes Clinton, Essex, Franklin, Hamilton, northern Herkimer, Lewis, St. Lawrence, and Warren counties; Eastern Lake Ontario, which includes northern Cayuga, Jefferson, Monroe, Oswego, and Wayne counties; Central New York, which includes Allegany, Broome, southern Cayuga, Chemung, Chenango, Cortland, Delaware, southern Herkimer, Livingston, Madison, Onondaga, Oneida, Ontario, Otsego, Tioga, Tompkins, Schuyler, Seneca, Steuben, and Yates, and Western New York, which includes Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, and Wyoming counties. Montréal and Toronto have some of the worst air quality in the world due to wildfire smoke: How to protect yourself, according to an expert
Toronto and Montreal's air quality is on par with some of the worst in the world due to drifting wildfire smoke. While fires continue to burn in Western Canada, wind has carried smoke from the fires to Ontario, Quebec and areas in the U.S. like Nebraska, Michigan and Wisconsin. The impact of wildfire smoke is so severe, Environment Canada issued air quality statements for the Greater Toronto Area (GTA) and Montréal Island Area. As of 12:25 p.m. EST., IQAir, a Swiss technology company, ranked Montreal as the city with the worst air quality in the world. Toronto came in third place, behind Santiago. Environment Canada is encouraging people, especially the elderly, pregnant people, children and people with chronic health conditions, to avoid spending time outdoors whenever possible. As of June 6, the Canadian Interagency Forest Fire Centre reported 212 active fires in Canada, with 91 considered out of control. Mandatory evacuation orders were issued for parts of Alberta and Saskatchewan due to poor visibility and air quality due to smoke. Manitoba's state of emergency required residents of several communities to evacuate due to air quality and lack of power. Environment Canada has issued a weather alert for parts of Ontario due to poor air quality that may cause people to experience symptoms like cough, headaches as well as nose and throat irritation. The Weather Channel in the U.S. reported wildfire smoke from Canada is visible as far as the Gulf Coast. Winds have caused smoke to impact air quality in states like Minnesota, Iowa, Wisconsin, Michigan as well as North and South Dakota. As the wildfire season kicks off, ways to protect ourselves from smoke are top-of-mind for many. Yahoo Canada previously spoke to a top respirologist in Toronto, who weighed in on the impact of smoke on our lungs, and what to do to stay safe. Read on for everything you need to know. Dr. John Granton, a respirologist at the Toronto General Hospital (University Health Network), told Yahoo Canada wildfires cause air pollution that can have a far-reaching effect on human health. Small air pollutant particles in the smoke, called the PM2.5 particles, are "where a lot of the toxic stuff lives," Granton explained. "That's not filtered by your upper airway, that gets access to your lower airway and into your bloodstream even — and that's where the danger lies. "That can cause asthma attacks, can cause heart attacks, can contribute to hospitalizations and has long-term health outcomes." Even just the smell could cause problems to some, he said. "Being in smog all day long, the smell bothers people; it can cause irritation of the upper airway, some of the larger particles can cause irritation, and cause symptoms. People [who] have chest symptoms or asthma, it can make them feel worse." Granton said smoke isn't easy to escape, but there are some things people can do try and stay safe. According to Granton, "there's not a lot of data to support the health benefits of masking" when it comes to safety from wildfire smoke. But, he said there is research looking into the effectiveness of filters. "Cloth masks or scarves and things are not effective at all," the doctor claimed. But, surgical masks and N95 filter masks "tend to filter those smaller particles," he added, "apparently some of those masks are effective." Granton said "they're not going to filter the gases," though, and there are studies that question whether masks that aren't properly fitted would actually help in the real world. "Whether or not that has a direct health benefit is speculative right now," Granton explained. However, he said it "may be reasonable" for those who have underlying health conditions and those who have to be in the hotspots of the pollution to wear one. "If you have to be outdoors and doing things, then wearing that sort of mask to protect you would be sensible." Though filtration masks could aid in protecting your lungs from smoke, the number one recommendation from experts is to stay indoors. According to Health Canada, the following measures should also be taken: Reduce sources of indoor air pollution (smoking, vacuuming, burning candles, wood stoves) Prevent infiltration of outside air (seal windows, instal a high-quality air filter, set HVAC system to recirculation mode, limit use of exhaust fans when not cooking) Have a functioning CO alarm Use a portable air purifier and air conditioning Granton echoed this advice. "Most people recommend staying indoors as much as possible and not exercising or doing things outside," he reiterated.
