Why Kennedy's overhaul of a key CDC committee could lead to ‘vaccine chaos' in the US

The US Centers for Disease Control and Prevention's Advisory Committee on Immunization Practices, or ACIP, will gather this week for its second regular meeting of the year, but this won't be a typical meeting.
Two weeks ago, US Health and Human Services Secretary Robert F. Kennedy Jr., who has a history of vaccine skepticism, upended the independent advisory panel and its planned agenda. All 17 of the previous members were dismissed, and eight new ones were added. Presentation topics central to anti-vaccine activists' arguments were placed on the schedule, despite studies and data about safety.
The swift changes — and concerns that the committee lacks expertise and has potential conflicts of interest — are so concerning that some public health experts and officials said the country risks shifting into 'vaccine chaos,' with different vaccine recommendations at work, inconsistent access and uncertain insurance coverage.
'This week's meeting of the Advisory Committee on Immunization Practices (ACIP) is likely to mark its end – for now – as a vaccine advisory body,' said the staff and advisers of the Vaccine Integrity Project, a group that's seeking to preserve access to vaccines in the event that the government throttles it, in a viewpoint posted Monday.
Dr. Peter Hotez, a pediatrician and co-director of the Texas Children's Hospital Center for Vaccine Development, said that in its new form, 'the ACIP is devoid of any meaningful expertise in vaccines or infectious diseases. It is organized to pursue a pseudoscience agenda. It's a waste of taxpayer dollars that should be dissolved.'
It's a startling shift for a group that has been meeting for more than 60 years, and whose recommendations are so trusted that they're often written into state laws.
ACIP has played a pivotal role in how vaccines are used in the United States. Like an orchestra conductor, it keeps the vaccine delivery system playing well together: directing doctors and other providers on how to use vaccines, telling insurance companies when to cover vaccines, giving states guidance on which vaccines to purchase, helping cities and counties decide which vaccines are needed for school enrollment and cueing the federal government on which vaccines to purchase for low-income children.
'Those recommendations are made in harmony with the input of a lot of consultants from professional groups so that we all agree then on what the recommendations are,' said Dr. William Schaffner, an infectious disease expert at Vanderbilt University in Nashville who has been participating in ACIP meetings for 40 years, both as a voting member and as a liaison member.
'Then, those recommendations basically become the standard of practice,' Schaffner said. 'So that makes the delivery of vaccines very smoothly functioning. Everybody knows who ought to get the vaccines and then when they get them.'
With new uncertainty around ACIP, several groups, including the Vaccine Integrity Project, have taken steps to convene and shape evidence-based recommendations, even without such guidance from the CDC's advisers.
Some states — such as Maine, which this year celebrated record-high immunization rates in school-age kids — have also begun to strip vaccine access laws of references to ACIP recommendations.
But experts are worried that a cacophony of recommendations, however well-meaning, may be confusing to insurers, health care providers and the public.
'We would have certainly, for a period of time, vaccine chaos,' Schaffner said, 'Because it would be uncertain for the providers and uncertain for the public who would be receiving the vaccines, whether your children, adolescents or adults, what was covered and what was recommended.'
Plus, these outside recommendations wouldn't have the force of federal law behind them to compel insurance coverage, said Dr. Fiona Havers, a medical epidemiologist and former senior adviser on vaccine policy at the CDC. Havers recently quit over Kennedy's changes to the committee, saying she no longer had confidence that the data she generated would be viewed objectively or with appropriate scientific rigor.
'If this process continues to completely fall apart and the professional societies like the American Academy of Pediatrics put out their own schedules, that's fine, and that may be where we're going,' Havers said. 'But I personally would not want to trust my child's access to vaccines or my elderly parents' access to vaccines to the goodwill of the insurance companies to follow those recommendations instead of ACIP's.'
Here's what to know about the CDC's vaccine committee ahead of its two-day meeting this week.
ACIP is a group of experts — typically with specialties in pediatrics, infectious diseases, public health or vaccines — who make recommendations to the CDC on the use of vaccines. They tend to come from universities or medical schools. Some have been doctors in private practice. There's also typically a consumer representative on the panel.
Despite Kennedy's claims when he removed all 17 previous members of the panel this month, members of the committee are closely scrutinized for conflicts of interest, and any conflicts are publicly disclosed. Members also recuse themselves from a vote if they have a financial interest in its outcome.
As of Tuesday evening, HHS had not posted updated ethics statements or conflicts of interest reports for the new incoming members.
Although the US Food and Drug Administration decides whether immunizations are allowed to be used in the United States, it's the CDC's job to recommend how those products should be used. It does so based on recommendations from ACIP.
After a vote, ACIP recommendations are forwarded to the CDC director for approval. The CDC isn't bound by the committee's recommendation but ordinarily follows it. The HHS secretary has the final signoff.
The committee typically meets three times a year to vote on the use of vaccines in the US, and more frequently during emergencies like Covid-19, when they met as often as once a week to pore over data from around the world. These meetings, which are public and typically broadcast, can span two or even three days.
Kennedy has accused ACIP of being a 'rubber stamp for industry profit-taking agendas,' but the committee hasn't always recommended the use of vaccines, even when they've already gotten approval from the FDA.
After data showed that the FluMist nasal spray vaccine had little to no effectiveness during the 2015-16 flu season, the committee said it shouldn't be used during the next two flu seasons. FluMist was again recommended during the 2018-19 season after data demonstrated better effectiveness.
In 2023, the first fall and winter season that the RSV vaccine was available for older adults, ACIP recommended it for adults 60 and older with shared decision-making, meaning it was available only after consultation with a health care provider on its risks and benefits. In 2024, ACIP removed the shared clinical decision-making requirement, making it easier to access. The panel recommended that adults 75 and older get one dose of any available RSV vaccine, while adults 60 to 74 needed one only if they were at higher risk of severe RSV infections.
After Kennedy dismissed all 17 ACIP members who were appointed during the Biden administration, he replaced them with eight of his own picks.
There can be up to 19 voting members, according to ACIP's charter.
Most of the new members have little expertise in infectious diseases or vaccines, and some have served as expert witnesses in vaccine injury cases. One has been associated with a group that exaggerates the rare risks and harms of vaccines while casting doubt on their benefits. Another has done studies purporting to show the harms of the Covid-19 vaccines, reporting conclusions that run counter to most other evidence on the safety and effectiveness of these shots. Independent fact checks of these studies suggest that their conclusions may be based on flawed methods.
Aside from voting members, there are two more tiers of membership.
There are six 'ex-officio' members from government health agencies such as the Centers for Medicare and Medicaid Services and the FDA. The meeting typically hears from these members about policy questions they can help answer. Ex-officio members don't typically vote, but they can do so if there aren't enough voting members to have a quorum – which is defined by the Federal Advisory Committee Act as a majority, or one more than half.
The last tier of membership includes the liaison members: non-voting members from the 30 nonprofits and professional groups that are spelled out in the committee's charter. Liaison members may be added by the HHS secretary. These members often speak up during discussions and after voting has taken place.
As of Monday night, many of the liaison members had not been sent a video link to participate in the meeting, Schaffner said, when normally they would have gotten that already.
Voting members can serve up to four-year terms. They are considered special government employees and subject to federal conflict of interest safeguards. They aren't paid for their service but are compensated for their travel to and from meetings and for their meals.
In addition to regular meetings, members of ACIP convene outside of public meetings in 'work groups' that focus on a specific vaccine or disease of interest. These groups must include at least two voting members but may also consist of liaison and ex-officio members as well as outside experts. They often spend months making deep dives into medical research and present their findings to the voting members of the committee at each meeting.
The working groups often look at vaccines that are nearing FDA approval, to be sure they are ready to issue advice shortly after that agency makes the determination that a vaccine is safe and effective.
ACIP's recommendations are used in several ways:
To guide patient care: ACIP sets what's known as the vaccine schedule, a table that charts out which inoculations should be given by what ages. Many states use these vaccines schedules to set their own policies for school enrollment, for example.
To determine insurance coverage: Under the Affordable Care Act, when ACIP recommends a vaccine, it has to be covered by most insurance plans with no co-pays to patients.
To determine which vaccines are purchased for the Vaccines for Children (VFC) program: The CDC's Vaccines for Children program bulk purchases vaccines that are then distributed to states, which, in turn, distribute them to doctor's offices. It provides vaccines at no cost to children whose families wouldn't otherwise be able to afford the shots. Roughly half of children in the US get their vaccines through the VFC.
To determine coverage by the Vaccine Injury Compensation Program: When ACIP recommends a vaccine for pregnant women and children, it is automatically added to the list of vaccines covered by the Vaccine Injury Compensation Program. As with many medical treatments, the benefits of vaccines come with rare risks. These risks, or adverse events, are maintained on a list by the program, which compensates people who've experienced them and can prove they are linked to a covered vaccine. Because the program shields vaccine manufacturers from lawsuits, it has come under fire from Kennedy, who has said that people should be able to file suit in court for their injuries.

Managing Mental Health in UC: Tips for Clinicians
Mental health concerns are common among individuals living with ulcerative colitis (UC), often affecting treatment adherence, symptom management, and overall well-being. To explore practical strategies for supporting mental health in this population, Alissa Hershberger, MSN, RN, CCRN, CNE, for Medscape, spoke with Kathryn N. Tomasino, PhD, a clinical health psychologist and co-director of the Behavioral Medicine for Digestive Health Program at Northwestern Memorial Hospital. Dr Tomasino specializes in evidence-based psychological interventions for patients with chronic gastrointestinal conditions. In this interview, she shares actionable guidance for clinicians working with patients who have UC. Read on for her insights. You've worked extensively with patients who have chronic digestive conditions. What unique mental health challenges do individuals with UC typically face? Alissa K. Hershberger, MSN, RN, CCRN, CNE Right off the bat, among individuals with UC, there are higher rates of depression and anxiety as well as posttraumatic stress disorder symptoms related to the medical condition. Part of that is because of the emotional impact of hearing the initial diagnosis and the condition flares that require a shift in coping. However, there is also a biological pathway which is particularly linked with depression. Depression can drive inflammation, and inflammation can also drive depression, so there's a reciprocal relationship. And on top of that, getting diagnosed with a lifelong medical condition requires some psychological adjustment. Other unique mental health challenges might include limitations on travel or specific foods for certain periods of time, and if nutrition support is required, there might be an impact on body image or sexual functioning. Additionally, pain can be difficult to cope with from a psychological perspective, leading to increased anxiety and depression. Fatigue can also be an issue, both during flares and when UC is in remission. For the most part, though, once UC is under control, patients can feel relatively normal compared to what they expect when they first get the diagnosis. So, while there are certainly unique mental health challenges, there are also plenty of people who adjust to the diagnosis and live a full life. How do you approach conversations about mental health with patients who may not initially see the connection between their physical condition and emotional well-being? The first thing that I do is establish some credibility and buy-in for working with a psychologist. I provide some background about how long I've been working with people with UC, how large and well known our psychology program is, and that UC can affect mental health, and mental health can also impact the disease. We get very scientific and describe the research and statistics behind why it is important to manage mental health in chronic GI disorders. This helps normalize the process and prevents people from feeling like they are being 'sent to a psychologist' because there is something wrong with them, or that these symptoms are all in their head. It can be tricky, though, because even after being told they are in remission, they might still be experiencing symptoms. In that case, it can be easy for people to feel like they are being told that things are all in their head, but we explain that this is where the brain-gut connection comes in. Many people with inflammatory bowel disease (IBD) have overlapping mind-gut disorders in addition to the IBD. That means they could be in remission from a pathology and colonoscopy perspective, but they are still experiencing symptoms like urgency, diarrhea, or abdominal pain. The patient's physical symptoms are potentially related to changes in the enteric nervous system, leading to these symptoms even in the absence of inflammation. This is where interventions from a psychological and behavioral standpoint can help. Can you speak about the importance of routine mental health screening in gastroenterology practices, particularly for patients with UC? This is one of my favorite questions because routine screening is so important. We have actually been working on a quality improvement project to implement regular screening through the electronic medical record for our patients with UC. The goal is to make it a seamless process where the patient can fill out an evidence-based questionnaire before their appointment, and the provider is notified so they can review it. If they have a high score, something that requires a little bit more care, then our GI psychology team also receives a notification. This helps normalize the idea that mental health symptoms can be a part of the condition. Untreated anxiety, depression, and trauma can impact patients' quality of life and their ability to cope with and adhere to recommended medical treatments, so screening and intervention is incredibly important. Around 95% of patients want to be asked about mental health by their gastroenterologist, so there has been a call for this to be part of routine care. I recently contributed to a consensus statement that emphasizes the importance of screening for and treating depression and anxiety in gastroenterology practices. What role can cognitive-behavioral therapy or acceptance-based approaches play in helping patients manage both UC symptoms and emotional distress? There are a lot of different evidence-based treatments available to help address the mental health aspects of IBD. As a cognitive-behavioral therapist, I would say that they mostly fall under the umbrella of cognitive-behavioral therapy. We also do a lot of acceptance and commitment therapy and mindfulness-based interventions. Another huge component of treatment can be relaxation training and gut-directed hypnotherapy. As far as the specific treatment plan, it really depends on the patients' presenting issue. We do an initial intake and evaluation where we have a conversation about the patient's goals and possible interventions that could help them manage symptoms and live the fullest life possible. We know that a variety of lifestyle factors can impact the symptom experience of UC and the likelihood of relapsing, so behaviorally, we really work on stress management, improving sleep, setting boundaries, coping with pain, and diet. We've seen that addressing mental health alongside medical treatment can improve outcomes in terms of patient adherence, including regularly taking prescribed medications, stress management, and lifestyle changes. Can you share any practical strategies or tools that patients with UC can use at home to reduce stress or anxiety tied to symptom flares? One of the first strategies we recommend is diaphragmatic breathing, which is a specific breathing technique that promotes digestion and reduces pain and stress through stimulation of the vagus nerve and activation of the parasympathetic nervous system response. Another strategy we suggest is optimizing sleep because UC flares can have a big impact on sleep, often causing people to wake up in the middle of the night because of pain. There are also benefits related to learning about UC and connecting with others who have the condition. The Crohn's & Colitis Foundation has an incredible website where they help connect people with support groups. There are also several mobile apps that have come out recently that target coping skills, help promote self-management, and reduce anxiety and stress related to the condition. Additionally, Melissa Hunt, a psychologist at the University of Pennsylvania, wrote, Coping with Crohn's and Colitis: A Patient and Clinician's Guide to CBT for IBD, a book that acts as a self-help guide for people with IBD; it is an excellent resource for patients to use at home. Is there anything else you'd like to discuss related to the maintenance of mental health in patients with UC? I think it is important to remember that there is such a wide range of experiences for people with UC; it can be mild or, at the more severe end, require surgery. There is a lot of variation in between, so I think it's important to consider that when thinking about how mental health and body image can be affected. Often, the big fear for patients with UC is surgery, but many people who have had the surgery are relieved that they are now living their full life again and not suffering with so much pain and discomfort. This is another reason I would suggest having patients talk to their provider, a psychologist, or other people with the condition — to work through some of those fears.

Just remember to avoid spot training since research shows that focusing on a single area with targeted exercises won't result in fat loss in just that spot. Instead, when you exercise, your body breaks down fat stores from various areas, not just where you're focusing your efforts. To achieve your desired fat loss, you'll want to focus your efforts on moving your body regularly and maintaining a balanced diet. We've detailed seven easy habits you can adopt to progress toward your goal. 1. Get your steps in Walking is an ideal workout outside the gym. It can be done around the neighborhood or at a park. On good weather days, you can also get some much-needed fresh air. Plus, it's free, and you can take your dog with you (if you have one). Walking is also a body fat buster. One study found that healthy postmenopausal women lost 3.9% of body fat after 30 weeks of walking and 1.8% after 15 weeks of walking. While we can't target specific areas, walking can help lose belly fat. According to a December 2024 meta-analysis, 30 minutes of aerobic exercise (like walking) per week may help adults with obesity or those who are overweight slightly reduce waist circumference, body weight and measures of fat. However, the greatest improvement was seen with 150 minutes per week. 2. Try intermittent fasting One diet trend that has risen in popularity over the years is intermittent fasting. As the name suggests, this is where people fast for a certain time, and then eat at other scheduled times. One study review found that subjects who fasted intermittently had weight loss ranging from 0.8% to 13%. The idea is to force the body to use up its immediately accessible sugar stores and start burning fat. A perk of intermittent fasting is that you can customize it to your preference and ability to abstain from food. According to Johns Hopkins Medicine, fasting can last for a certain number of hours every day or even just eating one meal per day for two days out of the week. For instance, you might only eat during an 8-hour period each day and fast the rest of the day. It's important to note that intermittent fasting is not for everyone, especially those at risk for disordered eating or during pregnancy. Before trying intermittent fasting, consult with your doctor to ensure you follow the best plan for you and your goals. 3. Lift heavier weights This point might seem counterintuitive since we just covered how you can't burn fat in one specific region, no matter how many stomach crunches you do. You can balance weight training to target multiple muscle groups or work on certain body parts as part of a whole-body workout. That may give you more balance and a lean appearance and help with body composition. If you don't have dumbbells at home, check out these household items that double as weights. Weight training can also help you lose fat while building muscle. Studies show that 3 pounds of lean muscle weight gain corresponded to 4 pounds of fat weight loss. Resistance training has also been shown to reduce body fat percentage, body fat mass and visceral fat (the fat that surrounds your organs). The National Institute on Aging recommends resistance training, either with weights or bodyweight exercises like pushups, for at least two days a week, so it can be a fairly easy exercise to fit into your schedule. 4. Like walking, you can do it around the neighborhood or in a park, so it's free. If you're concerned about the weather, you can also find an indoor track at a gym or community center. You can also consider getting a treadmill or elliptical to run or jog at home. Sprint training, where you switch how fast you run every several seconds, is especially good at busting the fat. The Centers for Disease Control also recommends 150 minutes of moderate-intensity aerobic activity, 75 minutes of vigorous-intensity activity or a mix of the two every week. The CDC lists walking at a rate of a 15-minute mile as moderate activity and jogging or running as vigorous. 5. Focus on high-intensity interval training This type of exercise, often shortened to HIIT, involves exercising as hard as possible for short bursts and then spending some time doing lower-intensity training. A perk of this exercise is that it can be any activity that gets your heart pounding, from jumping jacks to stair climbing, so it's customizable based on how much room you have and what equipment you have around. It's a fat buster too. It may result in a modest reduction of overall and abdominal fat. You can do 30 seconds to several minutes of hard-as-you-can working out and then 1 to 5 minutes of recovering with a lower-intensity exercise level. These workouts typically last about 30 minutes, including 5-minute warm-ups and cool-downs, but they can be tailored to suit comfort and fitness levels. Typically, the aim is to do these sessions five times a week. 6. Eat the right foods You can also focus on your diet. While there are no foods that'll magically burn your fat, there are foods that can increase your metabolism. Most of these foods are high in protein and good-for-you fats, making you feel fuller longer. Some foods to incorporate into your diet if you're trying to reduce fat, as listed by the CDC, Healthline and the World Health Organization, include: Fat-free, sugar-free yogurt, such as Greek yogurt Fatty fish like tuna, herring or salmon Eggs Veggies Fruit Green tea Whey protein Olive oil Beans Grilled chicken 7. Prioritize quality sleep We tend to associate burning fat with endless exercise and painfully restrictive diets. Getting a good amount of rest can also help get rid of the fat. Staying awake for too long can make us eat sugary foods to stay awake, give us tired and ineffective workouts, and may even contribute to stress and inflammation, leading to poor workout recoveries. One study found that not getting enough sleep decreased the proportion of fat weight loss by 55%. Another found that better sleep quality was linked to more weight and fat loss. Another study found a positive relationship between sleep duration and body fat loss. The Mayo Clinic recommends adults get seven or more hours of sleep at night. Sleep needs can vary by individual, so adjust higher if seven doesn't quite feel like enough. The bottom line When it comes to losing fat at home, you have many different options. You could try walking, running, high-intensity interval training or bodyweight training, all of which have studies backing up their fat loss capabilities. You might also try adjusting your diet. Eat foods that are low in saturated fat and sugar and ones that can keep you full longer. Opt for high-protein or low-calorie foods like grilled chicken, beans, eggs or green tea. You can also try intermittent fasting. Finally, make sure you're getting enough sleep. A good amount of sleep is also associated with fat loss.
