
Donanemab gets Australian nod for treatment of Alzheimer's disease
This week, Australia's Therapeutic Goods Administration (TGA) approved a drug called donanemab for people in the early stages of Alzheimer's disease. Donanemab has previously been approved in a number of other countries, including the United States.
So what is donanemab, and who will be able to access it in Australia? How does donanemab work? There are more than 100 different causes of dementia, but Alzheimer's disease alone accounts for about 70 per cent of these, making it the most common form of dementia. The disease is believed to be caused by the accumulation in the brain of two abnormal proteins, amyloid and tau.
The first is thought to be particularly important, and the 'amyloid hypothesis' – which suggests amyloid is the key cause of Alzheimer's disease – has driven research for many years.
Donanemab is a 'monoclonal antibody' treatment. Antibodies are proteins the immune system produces that bind to harmful foreign 'invaders' in the body, or targets. A monoclonal antibody has one specific target. In the case of donanemab it's the amyloid protein. Donanemab binds to amyloid protein deposits (plaques) in the brain and allows our bodies to remove them. Donanemab is given monthly, via intravenous infusion.
What does the evidence say?
Australia's approval of donanemab comes as a result of a clinical trial involving 1,736 people published in 2023. This trial showed that there was a significant slowing of disease progression in a group of patients who had either early Alzheimer's disease, or mild cognitive impairment with signs of Alzheimer's pathology. Before entering the trial, all patients had the presence of amyloid protein detected via PET scanning.
Participants were randomised, and half received donanemab, while the other half received a placebo, over 18 months. For those who received the active drug, their Alzheimer's disease progressed 35 per cent more slowly over 18 months compared to those who were given the placebo.
Researchers ascertained this using the Integrated Alzheimer's Disease Rating Scale, which measures cognition and function. Those who received donanemab also demonstrated large reductions in the levels of amyloid in the brain (as measured by PET scans). The majority, by the end of the trial, were considered to be below the threshold that would normally indicate the presence of Alzheimer's disease. These results certainly seem to vindicate the amyloid hypothesis, which had been called into question by the results of multiple failed previous studies.
They represent a major advance in our understanding of the disease. That said, patients in the study did not improve in terms of cognition or function. They continued to decline, albeit at a significantly slower rate than those who were not treated.
The actual clinical significance has been a topic of debate. Some experts have questioned whether the meaningfulness of this result to the patient is worth the potential risks.
Is the drug safe?
Some 24 per cent of trial participants receiving the drug experienced brain swelling. The rates rose to 40.6 per cent in those possessing two copies of a gene called ApoE4. Although three-quarters of people who developed brain swelling experienced no symptoms from this, there were three deaths in the treatment group during the study related to donanemab, likely a result of brain swelling. These risks require regular monitoring with MRI scans while the drug is being given.
Around 26.8 per cent of those who received donanemab also experienced small bleeds into the brain (microhaemorrhages) compared to 12.5 per cent of those taking the placebo.
Cost is a barrier:
Reports indicate donanemab could cost anywhere between 40,000 and 80,000 Australian dollars. This puts it beyond the reach of many who might benefit from it.
Eli Lilly, the manufacturer of donanemab, has made an application for the drug to be listed on the Pharmaceutical Benefits Scheme, with a decision pending perhaps within a couple of months. While this would make the drug substantially more affordable for patients, it will represent a large cost to taxpayers. The cost of the drug is in addition to costs associated with the monitoring required to ensure its safety and efficacy (such as doctor visits, MRIs and PET scans).
Who will be able to access it?
This drug is only of benefit for people with early Alzheimer's-type dementia, so not everybody with Alzheimer's disease will get access to it.
Almost 80 per cent of people who were screened to participate in the trial were found unsuitable to proceed. The terms of the TGA approval specify potential patients will first need to be found to have specific levels of amyloid protein in their brains. This would be ascertained either by PET scanning or by lumbar puncture sampling of spinal fluid. Also, patients with two copies of the ApoE4 gene have been ruled unsuitable to receive the drug.
The TGA has judged the risk/benefit profile for this group to be unfavourable. This genetic profile accounts for only 2 per cent of the general population, but 15 per cent of people with Alzheimer's disease.
Improving diagnosis and tempering expectations:
It is estimated more than 400,000 Australians have dementia. But only 13 per cent of people with dementia currently receive a diagnosis within a year of developing symptoms. Given those with very early disease stand to benefit most from this treatment, we need to expand our dementia diagnostic services significantly.
Finally, expectations need to be tempered about what this drug can reasonably achieve. It's important to be mindful this is not a cure.

Hidden in plain sight, chronic stress is key to increased dementia and Alzheimer's risk
The probability of any American having dementia in their lifetime may be far greater than previously thought. For instance, a 2025 study that tracked a large sample of American adults across more than three decades found that their average likelihood of developing dementia between ages 55 to 95 was 42%, and that figure was even higher among women, Black adults and those with genetic risk. Now, a great deal of attention is being paid to how to stave off cognitive decline in the aging American population. But what is often missing from this conversation is the role that chronic stress can play in how well people age from a cognitive standpoint, as well as everybody's risk for dementia. We are professors at Penn State in the Center for Healthy Aging, with expertise in health psychology and neuropsychology. We study the pathways by which chronic psychological stress influences the risk of dementia and how it influences the ability to stay healthy as people age. Recent research shows that Americans who are currently middle-aged or older report experiencing more frequent stressful events than previous generations. A key driver behind this increase appears to be rising economic and job insecurity, especially in the wake of the 2007-2009 Great Recession and ongoing shifts in the labor market. Many people stay in the workforce longer due to financial necessity, as Americans are living longer and face greater challenges covering basic expenses in later life. Therefore, it may be more important than ever to understand the pathways by which stress influences cognitive aging. Social isolation and stress Although everyone experiences some stress in daily life, some people experience stress that is more intense, persistent or prolonged. It is this relatively chronic stress that is most consistently linked with poorer health. In a recent review paper, our team summarised how chronic stress is a hidden but powerful factor underlying cognitive aging, or the speed at which your cognitive performance slows down with age. It is hard to overstate the impact of stress on your cognitive health as you age. This is in part because your psychological, behavioral and biological responses to everyday stressful events are closely intertwined, and each can amplify and interact with the other. For instance, living alone can be stressful – particularly for older adults – and being isolated makes it more difficult to live a healthy lifestyle, as well as to detect and get help for signs of cognitive decline. Moreover, stressful experiences – and your reactions to them – can make it harder to sleep well and to engage in other healthy behaviors, like getting enough exercise and maintaining a healthy diet. In turn, insufficient sleep and a lack of physical activity can make it harder to cope with stressful experiences. Prevention efforts A robust body of research highlights the importance of at least 14 different factors that relate to your risk of Alzheimer's disease, a common and devastating form of dementia and other forms of dementia. Although some of these factors may be outside of your control, such as diabetes or depression, many of these factors involve things that people do, such as physical activity, healthy eating and social engagement. What is less well-recognised is that chronic stress is intimately interwoven with all of these factors that relate to dementia risk. Our work and research by others that we reviewed in our recent paper demonstrate that chronic stress can affect brain function and physiology, influence mood and make it harder to maintain healthy habits. Yet, dementia prevention efforts rarely address stress. Avoiding stressful events and difficult life circumstances is typically not an option. Where and how you live and work plays a major role in how much stress you experience. For example, people with lower incomes, less education or those living in disadvantaged neighborhoods often face more frequent stress and have fewer forms of support – such as nearby clinics, access to healthy food, reliable transportation or safe places to exercise or socialise – to help them manage the challenges of aging. As shown in recent work on brain health in rural and underserved communities, these conditions can shape whether people have the chance to stay healthy as they age. Over time, the effects of stress tend to build up, wearing down the body's systems and shaping long-term emotional and social habits. Lifestyle changes The good news is that there are multiple things that can be done to slow or prevent dementia, and our review suggests that these can be enhanced if the role of stress is better understood. Whether you are a young, midlife or an older adult, it is not too early or too late to address the implications of stress on brain health and aging. Here are a few ways you can take direct actions to help manage your level of stress: Follow lifestyle behaviors that can improve healthy aging. These include: following a healthy diet, engaging in physical activity and getting enough sleep. Even small changes in these domains can make a big difference. Prioritise your mental health and well-being to the extent you can. Things as simple as talking about your worries, asking for support from friends and family and going outside regularly can be immensely valuable. If your doctor says that you or someone you care about should follow a new health care regimen, or suggests there are signs of cognitive impairment, ask them what support or advice they have for managing related stress. If you or a loved one feel socially isolated, consider how small shifts could make a difference. For instance, research suggests that adding just one extra interaction a day – even if it's a text message or a brief phone call – can be helpful, and that even interactions with people you don't know well, such as at a coffee shop or doctor's office, can have meaningful benefits. Play Lifelong learning A 2025 study identified stress as one of 17 overlapping factors that affect the odds of developing any brain disease, including stroke, late-life depression and dementia. This work suggests that addressing stress and overlapping issues such as loneliness may have additional health benefits as well. However, not all individuals or families are able to make big changes on their own. Research suggests that community-level and workplace interventions can reduce the risk of dementia. For example, safe and walkable neighborhoods and opportunities for social connection and lifelong learning – such as through community classes and events – have the potential to reduce stress and promote brain health. Importantly, researchers have estimated that even a modest delay in disease onset of Alzheimer's would save hundreds of thousands of dollars for every American affected. Thus, providing incentives to companies who offer stress management resources could ultimately save money as well as help people age more healthfully. In addition, stress related to the stigma around mental health and aging can discourage people from seeking support that would benefit them. Even just thinking about your risk of dementia can be stressful in itself. Things can be done about this, too. For instance, normalising the use of hearing aids and integrating reports of perceived memory and mental health issues into routine primary care and workplace wellness programs could encourage people to engage with preventive services earlier. Although research on potential biomedical treatments is ongoing and important, there is currently no cure for Alzheimer's disease. However, if interventions aimed at reducing stress were prioritised in guidelines for dementia prevention, the benefits could be far-reaching, resulting in both delayed disease onset and improved quality of life for millions of people.

Light pollution is making you sick, fat. Yet regulatory practices remain scant

The deaths of actor Gene Hackman and his wife Betsy Arakawa made headlines some months ago after their bodies were found at their home in Santa Fe, United States. While Ms. Arakawa was found to have died of hantavirus pulmonary syndrome, a rare rodent-borne disease, Mr. Hackman's autopsy findings indicated severe heart disease as well as advanced stages of Alzheimer's disease. The actor was 95 at the time of his death. What is Alzheimer's disease and what do you need to know about it? Read on, to find out. What is Alzheimer's disease? Alzheimer's disease is the most common form of dementia, which is an umbrella term for several diseases that affect the brain, impairing memory, thinking, reasoning, affecting a person's ability to function well. Dementia is not a normal part of ageing. Alzheimer's disease, the World Health Organization (WHO) states, may contribute to 60–70% of dementia cases. Dementia is currently the seventh-leading cause of death and one of the major causes of disability and dependency among older people globally. India is expected to have about 7.6 million afflicted by 2030, as per the Dementia in India Report, 2020, majority of whom are likely to be diagnosed with Alzheimer's. Alzheimer's is a progressive, neurodegenerative disease, which means that over time, brain cells are damaged and die, leading the brain to shrink. What causes Alzheimer's disease? The exact causes that lead to Alzheimer's disease are still being studied and are not fully understood. Scientists believe the disease may be caused by the abnormal build-up of proteins in the brain. The brain is made up of billions of nerve cells or neurons that are connected in networks. When a protein called beta amyloid builds up and clumps together, it forms plaques that affect the communication between neurons. Another protein, the tau protein, becomes twisted into strands called tangles, which also affect the functioning of the cells, and damages them. When the cells become damaged and lose their connections with each other, they eventually die. The destruction and death of the brain nerve cells cause the symptoms of Alzheimer's. Genetics, environmental and environmental factors are believed to contribute to the cause. Who is at risk for Alzheimer's disease? Age, being 65 years or older, is one of the primary risk factors. Other risk factors include a family history of Alzheimer's, smoking, a traumatic head injury, untreated depression as well as lifestyle conditions associated with heart disease such as obesity, high blood pressure and high blood sugar. The WHO also lists drinking too much alcohol, being physically inactive and being socially isolated as risk factors for dementia. What are the signs and symptoms? One of the first and most common symptoms may be memory loss -- difficulty in remembering recent events or conversations and newly-learned information. Other symptoms involve thinking and reasoning abilities as well as changes in speech, moods, personality and behaviour. These can include: confusion, difficulties in paying bills and managing finances, difficulties in day-to-day activities such as cooking or driving and poor judgement. Personality changes can include suspicion of others, mood swings, hallucinations, anxiety, depression, social withdrawal and anger or aggression. What is the prognosis? The rate of progression for Alzheimer's disease varies. In the later stages of the disease, a person may lose physical abilities, may need help with all activities, and may lost the ability to communicate. Some people may live several years after a diagnosis, while others may live for fewer years. The stage of the disease at the time of diagnosis and other factors of health may affect life expectancy. How is it treated? It is important to see a doctor if you are concerned about memory or other thinking skills or if you have these concerns about a family member. At present, there is no cure for Alzheimer's., however treatment can help manage symptoms and treat the progression of the disease. Early treatment is optimal to help preserve brain health. Medicines may be prescribed where necessary. This apart, medication may also be given for symptoms such as depression, hallucinations, agitation and mood changes. Opting for a healthy lifestyle including adequate exercise, well-balanced, nutritious meals, keeping up with hobbies and social activities, may all help. Creating a supportive and caring environment at home, one that is calm, structured, is also important to help improve well-being and the ability to function.
