A medical student was killed when the bar of a gym weight machine struck his head, an inquest heard.
Mohammed Farraj, 21, had been using an exercise step to perform calf raises with a Smith machine at the University of East Anglia in Norwich on Oct 16 2024.
Det Sgt Robert Waring said a Smith machine was a 'horizontal barbell which runs vertically up and down on rails'. The step was not part of the machine.
He said the incident at the Sportspark gym was captured on CCTV.
'It's clear from watching the footage that this was a tragic accident,' he said.
He said the plastic step Farraj was using was 'seen to tip towards him'. He said the bar then 'drops' and 'forces his neck to the ground'.
Set of adjustable stops
Det Sgt Waring said a safety feature on the machine was a set of adjustable stops which 'should be set to the highest position possible' to stop the weight bar.
He said they had been 'left in the very lowest position'.
The plastic step was 'intended for a different purpose' and a weight plate could have been used instead, he added.
Witness Dr Trey Koev, a research scientist at UEA, said: 'I saw him slip onto the platform. The bar carrying the weight came down.'
He said Mr Farraj 'struggled to his feet very rapidly then tumbled, then both my gym partner and I rushed to him as we were the closest to him'.
Dr Koev said there had been red-coloured weights on either end of the bar indicating 25kg each and the bar weighed approximately 15kg, a total weight of 65kg.
Dr Damian Laba, Dr Koev's gym partner, said he was using the bench press when he heard a bang, 'stood up and turned round and saw Mohammed collapsing on the floor'.
Dr Laba, a consultant anaesthetist at the Norfolk and Norwich University Hospital, said: 'I realised this is potentially a life-threatening injury and I shouted for help.'
Warning not on machine
He added that he tried CPR but Mr Farraj died at the scene.
Benjamin Price, head of sports operations at UEA Sportspark, said: 'It appears Mohammed followed another person who had been using that equipment.
'It appears they had put the stops at the lowest level.'
A warning about the stops had not been on the Smith machine at the time but was now in place, Mr Price said.
Hashim Farraj, the victim's father. said his son was a devout Muslim and 'loved life'.
He grew up at the family home in Stoke-on-Trent, was part of a boxing club and 'took his fitness and his health very seriously. He liked to go to the gym and would run 10k in the morning on a regular basis,' he said.
Norfolk assistant coroner Johanna Thompson concluded that Mr Farraj's death was an accident. She extended her condolences to his family and friends.

An English drug dealer who operated as part of a gang to sell narcotics in Aberdeen has been caged for his part in the crime. Byron Smith appeared via video link at Peterhead Sheriff Court on Monday to be sentenced for supplying both cocaine and heroin in Aberdeen two years ago. He had lodged guilty pleas in May. The 22-year-old, whose address was given in court papers as HMP Grampian, will now spend the next 15 months behind bars. As part of the Ace syndicate, Smith operated out of a flat on Park Road in Aberdeen. Fiscal depute Jennifer Pritchard told the court that he came to the attention of officers on November 7 2023, when he was spotted entering the property. 'Conversation could be heard within the address,' she added. 'Entry was subsequently forced.' Within, officers uncovered 123 wraps of drugs and a rectangular container containing cocaine. In total, heroin worth £380 was recovered and cocaine to the tune of £1,700. Smith's defence agent, Leonard Burkinshaw, branded the crime a 'well-known tale in this area'. 'He was just 20 at the time of the offences,' he said. 'He was tasked to perform in this area, my lord. That is essentially the situation he found himself in.' Mr Burkinshaw added that his client was looking to 'start fresh and start a new life' following his conviction. Sentence on Smith had previously been deferred for background reports to give the court information on his suitability for work in the community. Referencing these, Sheriff Craig Findlater noted there was a 'history of failing to comply' with such orders. He added that Smith had already been jailed previously. Ordering him to be locked up for more than a year, Sheriff Findlater said: 'It speaks of a poor attitude on your part. 'You've travelled to Scotland in order to deal drugs. You have already received custodial sentences in Scotland and in England.'

Police are treating the incident as a hate crime. Spray-painted Christian crosses and the words "No Masjid" were discovered on the former Abergavenny library just weeks after the council's Labour-led cabinet agreed to grant a 30-year lease to Monmouthshire Muslim Community Association (MMCA). ‌ The group, a not-for-profit organisation, intends to convert the building into the county's first mosque and community centre, serving the needs of Monmouthshire's growing and diverse Muslim population. ‌ The incident has been widely condemned by local residents and faith leaders as a shocking display of religious intolerance in a town known for its history of peaceful dissent and diversity. The building, located in the town centre, has been out of use as a library since 2015 after services were moved to Abergavenny Town Hall. Since then it has housed several council-run services including adult education and most recently the pupil referral unit (PRU) for children unable to attend mainstream education. In May the council awarded the lease to MMCA citing the association's clear demonstration of local need. Andrew Lewis, a local resident, welcomed their work and said: "The Muslim community locally have been doing some great community events. ‌ "I can only see that increasing with having their own dedicated community space. We have had them talking with Scouts as part of the World Faiths badge and I went to a Ramadan celebration and education evening at Crickhowell High School." Although the cabinet decision was approved, three councillors called in the lease agreement for review following concerns raised members of the community. The review meeting was scheduled to take place on Wednesday, June 11, at Monmouthshire County Hall in Usk. ‌ The vandalism, which occurred just a day before that review, has triggered anger and soul-searching in the community. "I am disgusted at the vandals who have used a precious Christian symbol to object to a former library being used for Muslims to worship in our town," said Reverend Gareth Wilde, an accredited Baptist minister. "Abergavenny has a rich, peaceful history of non-conformist dissent from the 17th century where its adherents fought for religious freedom for those who didn't follow the established Church of England. ‌ "These would include Baptists, Congregationalists, Presbyterian and various other chapel-attending congregations. "Muslims are only asking to be treated with the same equality and respect and if anything should be prioritised above Christian churches wishing to use a community facility. "As an accredited Baptist minister I wholeheartedly agree with the MCC decision to let the Carnegie Library to Monmouthshire Muslim Community Association. Please be kind to Muslims in Wales." ‌ Another saddened resident said: "Never thought I'd see this in Abergavenny. A very sad day. We need to respect and understand everyone's beliefs and peacefully coexist in a society that has so much freedom." One local added: "I always thought that Abergavenny was a tolerant caring community and hopefully this abhorrent graffiti was perpetrated by some hateful person outside of our community. It remains a dark stain on us all. "This is no different from the religious persecution shown by the Nazis to the Jewish communities in the 1930s. Previous generations made a supreme sacrifice to overcome this kind of inhumanity. ‌ "It saddens me that the gross transgressions of the past are rearing their ugly head again." In response to the incident a sign has been placed over the graffiti reading: "What unites us is big + beautiful, what divides us is small + MEAN!" A Gwent Police spokesman said: 'We received a call at about 8.45am on Tuesday, June 10, reporting criminal damage after graffiti had been sprayed on the old library in Abergavenny overnight. ‌ 'This has now been cleaned off and we are treating it as a hate crime.' Inspector Emma Sowrey said: 'We have been carrying out onquiries to identify those responsible including house-to-house and reviewing CCTV. So far we've been able to establish that the damage happened at about 3am on June 10. 'I understand the concern that may be felt amongst our communities following this appalling incident which we are treating as a hate crime. ‌ 'We will not tolerate any such behaviour and are determined to identify the offenders and bring them to justice.' His victims were left shaken by the incidents, and were taken to hospital in order to be tested for various conditions. Adam Smith, 33, was a serving prisoner at HMP Parc in Bridgend when he carried out the disgusting assaults on staff members at the jail. He had been sentenced to six years imprisonment for inflicting grievous bodily harm. ‌ A sentencing hearing at Cardiff Crown Court on Wednesday heard the first assault took place on November 17, 2023, when a prison officer was mingling with inmates. For the latest court reports sign up to our crime newsletter. ‌ The officer felt a wet substance hit the back of his head, and fall down his back and legs. He described it as "cold, runny wax", and he could smell faeces and urine. The officer tipped the back of his head to stop liquid getting into his eyes, and was described as "distressed and shaken". He was taken to hospital for a routine check. Article continues below Smith was later identified on CCTV and he was also overheard saying "I potted an officer." When confronted, he claimed he had been "paid to do it". On February 2 last year, another officer was attacked when Smith sprayed the contents of a plastic bottle and was laughing as he did so. The contents went on the officer's face and upper body and he could smell urine. ‌ The officer slipped on urine on the floor and felt a pain in his left as he grappled with the defendant, who continued to laugh. His clothes were wet and his eyes were stinging, and he was required to attend hospital for his eyes to be washed with a syringe. The officer was left "angry and upset" by the incident. A third officer was assaulted by Smith on March 20 last year when the defendant walked towards him and punched him to the mouth. ‌ Prosecutor Jenny Yeo said the officer felt blood in his mouth and his teeth felt loose. As a result, his teeth had to be realigned and fixed back into place. Smith, now of HMP Swaleside, pleaded guilty to two counts of administering a noxious or poisonous substance and assault occasioning actual bodily harm. The court heard he had 28 previous convictions, including three offences of assault occasioning actual bodily harm, 12 offences of battery and inflicting grievous bodily harm. Article continues below In mitigation, David Pinnell said his client had since been diagnosed with ADHD, for which he was now receiving medication, and wanted to reengage with his partner and three children. Judge Rhys Rowlands sentenced him to a total of 42 months imprisonment, which he will serve consecutively to his current sentence.
