Diversity Gap Persists in Key Trials for Psoriatic Arthritis

An analysis of 29 pivotal drug trials for psoriatic arthritis revealed significant racial disparities, with White individuals making up the majority of trial participants, whereas Asian, Black, and other racial groups remained underrepresented. Geographic disparities in participant inclusion were also observed, with trial sites primarily located in North America and Europe.
METHODOLOGY:
Researchers conducted a descriptive epidemiologic study to assess the gaps in reporting of racial, ethnic, and geographic data of participants enrolled in pivotal US clinical trials of biologic and targeted synthetic disease-modifying antirheumatic drugs for psoriatic arthritis.
They collected data on 14,845 participants enrolled in 29 trials with start dates from April 2000 to April 2019, using three primary public sources: Journal publications, the Drugs@FDA database, and ClinicalTrials.gov.
Overall, 16 drugs with varying mechanisms were assessed in these trials: Seven trials each of TNF inhibitors and interleukin 17A or 17A/F inhibitors, four trials each of JAK inhibitors and interleukin 23 inhibitors, three trials of phosphodiesterase 4 inhibitors, and two trials each of cytotoxic T-lymphocyte antigen immunoglobulin and interleukin 12/23 inhibitors.
TAKEAWAY:
Data on race were reported in 93% of trials, with the most frequent reporting in journal publications (86%), although 46% of them reported the proportion of only White participants.
Among participants for whom data on race were available, 92% were White, 5% were Asian, 0.6% were Black, 0.3% were American Indian/Alaska Native, and 0.1% were Native Hawaiian/Pacific Islander individuals.
Ethnicity was formally reported in only 41% of trials. The reporting of both race and ethnicity improved over time, reaching full coverage (eight of eight trials) on ClinicalTrials.gov between 2016 and 2020. Among participants for whom data on ethnicity were available, 11% were Hispanic or Latino individuals.
Within the US, the highest participation was reported from Texas (24 trials), Florida and Pennsylvania (22 each), California (20), and Alabama (18), with participation in clinical trials strongly correlated with state population size (correlation coefficient, 0.78; P < .0001). Trial sites were primarily located in North America and Europe, with limited representation in Asia, Africa, and Latin America.

'Standardizing and mandating race and ethnicity reporting across data sources is crucial to ensure transparency and equity. Tailored and multifaceted recruitment strategies are essential to increase diversity and ensure the generalizability of trial results, ultimately aiding clinicians to provide the most effective and informed treatment options for all patients,' the authors wrote.

The Trump administration reportedly plans to cut almost all funding for HIV vaccine research. Experts say the decision comes at a time when research in this field is making substantial progress. Many effective treatments are available for HIV, but these are lifelong commitments that manage a chronic disease rather than cure it. Treatments for HIV infection have come a long way since the 1980s, when too many lives were lost during the epidemic. Today, antiretroviral therapies and other treatments allow people with HIV to live longer lives and, in many cases, prevent the transmission of the virus that causes the disease to other people. Scientists now say the next step in the fight is a vaccine that protects against HIV. However, that next development could be on the chopping block. Trump administration officials reportedly plan to halt funding for a wide array of HIV vaccine research. Researchers told CBS News they have been informed by officials at the National Institutes of Health (NIH) that the Department of Health and Human Services (HHS) has instructed the agency not to issue any more funding during the next fiscal year for HIV vaccine research. NIH officials said HHS officials have instead decided to 'go with currently available approaches to eliminate HIV.' The decision will close down HIV vaccine research projects at the Duke Human Vaccine Institute and the Scripps Research Institute, according to a report in the journal Science. Officials at Moderna also told CBS News that their current clinical trials on HIV vaccines have been put on hold. Experts say the decision to cut funding for an HIV vaccine is short-sighted and reckless. 'I'm stunned by this decision,' said Jake Scott, MD, a clinical associate professor of medicine at Stanford University in California who specializes in infectious diseases. 'There is no scientific or medical evidence to justify these cuts at the exact moment this field is showing real promise,' he told Healthline. Carl Baloney Jr., the chief executive officer-elect of AIDS United, agreed. 'Eliminating funding for HIV vaccine research undermines decades of scientific progress and turns our back on a future where HIV could be preventable for all, regardless of where someone lives, their income, or access to healthcare,' he told Healthline. Why an HIV vaccine is important Experts say that treatments for HIV are incredibly effective. However, they note that most involve daily adherence and aren't necessarily readily available or affordable for many people. 'There are a lot of good options, but they can be really expensive,' Scott said. 'These medications are also not a cure. They are a lifetime burden.' The experts add that people with low levels of HIV in their system can still have weakened immune systems. That can raise the risk of serious infections as well as inflammation that can lead to conditions such as heart disease. 'A vaccine can help prevent all this,' said Scott. Experts note that a vaccine research program may be difficult to put back together even if a new administration restored funding in the near future. They say it took decades to build these programs and restarting them would take time. In addition, researchers will leave the field of HIV prevention to set up shop in another industry that is receiving funding. 'We could lose an entire generation of scientists,' said Scott. 'This is setting the field back a decade or more at a critical time.' 'This isn't just about canceling [a] clinical trial. It's about sidelining the scientists, institutions, and community partners driving innovation forward,' added Baloney. 'These setbacks could delay the development of a successful HIV vaccine by years or even decades.' How scientists fought against HIV The first treatment for HIV was approved by the Food and Drug Administration (FDA) in 1987. Azidothymidine (AZT) was first developed in 1964 as a treatment for cancer. It was ineffective in that usage, but in the 1980s, scientists discovered AZT could suppress HIV replication without damaging healthy cells. It helped treat people with AIDS as well as people who were HIV positive but had no symptoms. In the 1990s, other nucleoside reverse transcriptase inhibitors (NRTIs) were developed and approved. Laboratory tests to measure viral load and cell counts accelerated this research. From there, scientists experimented with combining drugs to help counter the HIV virus's ability to mutate and replicate. In 1996, a triple-drug therapy proved effective in thwarting HIV replication and creating a barrier against drug resistance. Since then, these antiretroviral drugs have become more effective and more available. The effectiveness of these medications is nothing short of miraculous. In the 1980s, the average life expectancy after an AIDS diagnosis was one year. Today, people who adhere to combination antiretroviral drug therapies can expect to live a near-normal life span. In some cases, the medications can reduce the HIV viral load in a person to the point where the virus is undetectable and can't be transmitted to another person. How is HIV treated today? More than 50 types of HIV medications are now approved for use. Some of the more commonly used antiretroviral medications are: Combination NRTI drugs that include Truvada and Descovy. These medications work by preventing HIV from converting its RNA into DNA. This prevents the virus from making copies of itself.. Integrase strand transfer inhibitors (INSTIs) that include Vocabria and Biktarvy. These drugs work by blocking an enzyme that HIV uses to put HIV DNA into human DNA inside cells. Protease inhibitors (PIs) such as Lexiva and Crixivan. These medications work by blocking an enzyme that HIV needs as part of its life cycle. Non-nucleoside reverse transcriptase inhibitors (NNRTIs) that include Intelence and Viramune. These drugs work by preventing the HIV virus from making copies of itself. Entry inhibitors such as Fuzeon and Selzentry. These medications work by blocking HIV from entering CD4 T cells. In addition, there are drugs known as Cytochrome P4503A (CYP3A) inhibitors, such as Tybost and Norvir, that help boost the levels of HIV medications in the bloodstream. There are also medications known as post-attachment inhibitors that when used with antiretroviral drugs can help prevent HIV from entering immune cells. Trogarzo was the first of these drugs to become available, having been approved in 2018. In addition, there are attachment inhibitor medications, a newer form of HIV drug that works by attaching to a viral protein, which prevents that protein from entering healthy T cells. Only one type of this medication, Rubokia, is currently available, having been approved in 2020. Most people with HIV are given medications, but there also are long-acting injections that are given once a month or once every other month. Scott said these treatments are cures that have turned HIV into a 'managed chronic disease' to the point where he and other colleagues now refer to AIDS as 'advanced HIV.' Baloney said, however, there are limits to how much treatments can do. 'Current treatments and prevention tools have transformed HIV into a manageable chronic condition, but they are not a cure and they're not accessible to everyone,' he said. 'An HIV vaccine would be a game-changer, especially for communities facing systemic barriers to care.' Preventive measures for HIV Even with the available treatments, experts agree that it's better for a person if they don't contract HIV in the first place. They say condom use, along with dental dams and gloves, can be effective barriers to contracting HIV. Limiting sexual partners is also recommended, as are sterile needles for intravenous drug users. Getting tested for HIV is also an important component. It's estimated that more than 1 million people in the United States have HIV, and 13% of them don't know they have contracted the virus. There are medications available that can be taken as a precaution or after potential exposure to HIV. These drugs include: Preexposure prophylaxis (PrEP): These medications can be taken as a daily pill or a bimonthly injection. The first injectable PrEP drug was Apretude, which was approved by the FDA in 2021. Truvada can also be used as PrEP therapy. These medications help prevent HIV from getting a foothold in the body. Postexposure prophylaxis (PEP) drugs: These are designed to be taken within 72 hours of potential exposure to HIV. It is a pill ingested once a day for 28 days. Lenacapavir: This injectable drug has been tested in clinical trials as a potential PrEP therapy. The FDA is scheduled to vote on its approval on June 19. Experts say all these preventive measures are good practices, but they note that vaccines are still the most effective.

I found out that changes leading to neurodegenerative disease begin in your 20s and 30s. Most people don't realize that your brain doesn't suddenly age in your 60s or 70s. Now is the time to start promoting and preserving your brain health. The sooner you understand cognitive decline and what you can do to curb it now, the better your brain health will be in later years. If you can improve your brain health today, you'll reduce the risk of brain disease in the future, according to experts. I spoke by email with Dr. Rudolph Tanzi, a leading expert on Alzheimer's and neurodegenerative diseases. He is the Director of the Genetics and Aging Research Unit and the McCance Center for Brain health At Massachusetts General Hospital. Tanzi recommends that you talk with your primary care provider first and walk through the McCance Brain Care Score™-- the multi-dimensional brain care tracker to see where you stand today. Your score shows where you can improve your brain health. The brain health expert developed a plan to help you remember the everyday steps to take care of your brain now to preserve and promote brain health and prevent other age-related diseases, including heart disease, diabetes and cancer. Here is Tanzi's six-step SHIELD method with additional research that supports his plan. 1. S stands for sleep. Sleep deprivation hampers your ability to deal with stress. Tanzi emphasizes the importance of getting seven to eight hours of sleep each night. He says sleep serves to clear away the plaques that lead to cognitive decline, adding that plaques begin decades before signs of decline. So, getting good sleep at any age (even power naps) can improve your brain health. If you're looking for ways to improve the quality of your sleep, he suggests visiting The American Heart Association's guide to healthy sleep. 2. H is for handling stress. In six easy steps, meditation reduces cortisol levels by 25% and alters brain activity so you're less error prone and make fewer mistakes. Tanzi advises that you establish a meditation practice and manage expectations that can reduce stress. He recommends visiting the Benson-Henry Institute for Mind Body Medicine website for ongoing tips and guided meditations and the Mental Health & Well-being guide from The American Heart Association. 3. I is for interaction with friends. Tanzi points out that loneliness doubles the risk for Alzheimer's disease. In 2023, Dr. Vivek Murthy, the U.S. Surgeon General cited loneliness as a serious mental health hazard. Research shows that social connections improve your brain health. People with greater levels of social engagement have more robust gray matter in regions of the brain relevant in dementia. 4. E is for exercise. Regular exercise boosts heart rate and blood flow, which may help reduce the risk of cognitive decline. According to Tanzi, exercise induces new nerve cell growth to strengthen brain regions affected in Alzheimer's disease and triggers the breakdown of brain amyloid. He insists that finding an exercise that works for you is key. He recommends starting small and working up to the recommended 150 minutes per week, to have an impact on your brain health. 5. L is for learning new things. Your brain learns from novelty, which promotes adaptive learning by resetting key brain circuits and enhances your ability to update new ideas into existing neurological frameworks. 'Learning new things strengthens and increases the number of synapses in your brain, the connections between nerve cells storing your memories,' Tanzi states. 'Synapse loss correlates most with the degree of dementia. The more synapses you make, the more you have to spare. Puzzles aren't necessarily the answer, either.' He cites a recent Harvard Health article that concludes, 'Practicing a new and challenging activity is a good bet for building and maintaining cognitive skills.' He suggests that activities such as learning a new language or taking a class have major benefits for brain health. 6. D is for diet. Vitamin B is essential for brain wellness and can be found in eggs, whole grains, fish, avocados and citrus fruits. In Tanzi's opinion, the best diet for the brain is the Mediterranean diet, which minimizes red meat and is rich in fiber from fruit and vegetables. 'A plant based diet balances the bacteria in your gut, called the 'gut microbiome,' he points out. "A healthy gut microbiome has also been shown to reduce amyloid plaque and brain neuroinflammation, a major killer of nerve cells in the brain.' NAD+ (stands for nicotinamide adenine dinucleotide), a coenzyme critical to key cellular processes such as ATP production (converting food to energy in the cell), mitochondrial function and DNA repair. According to Dr. Andrew Shao, senior vice president of Global Scientific & Regulatory Affairs at Niagen Bioscience, it's essential to brain health because it supports neural health, powers cellular energy, repairs damaged DNA and activates a family of proteins that help regulate inflammation and protect neurons. Shao told me that NAD+ levels decline as we age (starting as early as our 30s) from stressors we face every day such as overeating, alcohol consumption, sun exposure, sedentary lifestyle and environmental pollutants. Shao adds that the decline in NAD+ impacts cellular repair and energy production, rendering the brain more vulnerable to neurodegeneration. He explains that the most effective way to support your NAD+ levels is with a form of vitamin B3 (nicotinamide riboside or NR). He notes that 1000mg of the oral supplement Niagen is a safe and effective supplement of vitamin B3 that restores NAD+ levels by up to 150%, supporting the cellular processes that keep your brain healthy and resilient as the levels decline with age. Grocery chain Meijer has announced the recall of certain packages of dark chocolate almonds due to contamination with an undeclared allergen. The company announced the recall June 13, stating that packages of Frederik's Dark Chocolate Almonds may also contain dark chocolate cashews, according to a press release. Those with a cashew allergy run the risk of experiencing a life-threatening reaction if they consume the product, as the nuts were not declared on the packaging. Two different packages of Frederik's Dark Chocolate Almonds were affected by the recall: The black stand-up pouches sold at Meijer stores in Michigan, Indiana, Illinois, Ohio, Kentucky, and Wisconsin with a sell-by date of 05/07/2026 or 05/28/2026, and the 8-count, 1.5-ounce multi-pack boxes with a sell-by date of 05/05/2026. No illnesses have been reported. Customers who have purchased the product are urged to return it to Meijer to claim a full refund. The Chocolate Truffle flavor has an allergen label that reads, 'may contain tree nuts,' while the Rocky Road flavor declares almonds as one of the ingredients. In a statement made to People, a representative for Breyers said, 'People with an almond allergy should not consume the product due to risk of serious or life-threatening allergic reaction.
