
New Qantas rule after turbulence shock
Qantas has promised to step up its staff safety procedures following a shock turbulence event that injured three cabin crew.
The incident happened on descent into Brisbane Airport during a Sydney to Brisbane flight in early May.
About 36km out from the airport, the aircraft experienced unexpected and severe turbulence, with three cabin crew unrestrained as the plane started shaking.
Two crew members received minor injuries, including a facial injury and concussion, while a third was seriously injured with a fractured ankle, an ATSB report into the event released this week has revealed.
'The ATSB found that the captain did not inform the cabin crew about the expected turbulence during descent, likely due to not being aware of its severity,' the safety agency said.
The turbulence event was then followed by a communication breakdown between different parts of the cabin, with the pilot landing the plane despite three crew members and a passenger still unrestrained.
'Following the turbulence, the captain instructed all passengers and crew to return to their seats and fasten seatbelts. 'However, two cabin crew and two passengers remained unrestrained in the rear galley to assist the seriously injured crew member during landing,' the report states. 'When the flight crew were informed of this, the captain repeated the instruction that everyone besides the injured crew member was to return to their seats for landing.
'Assuming the cabin would be secured after the repeated instruction, the flight crew proceeded with the landing, unaware that four crew and passengers remained unrestrained.'
The aircraft, a narrow body twin-engine Boeing 737-800, was fitted with a weather radar system that detects and locates precipitation bearing clouds on the flight path, giving pilot a visual indication in colour of a cloud's intensity.
'The captain did not recall identifying areas of turbulence on the weather radar on descent or report that they were experiencing any difficulties operating the weather radar,' the report states.
On descent, the captain observed 'an approaching cumulus cloud' along the flight path but determined it did not pose an immediate hazard based on a visual assessment and the lack of radar indications, the report found.
'As a result, the captain did not perform the 'turbulence' PA, which would have prompted the cabin crew to immediately secure themselves in the nearest seat or wedge themselves in the aisle to prepare for the turbulence encounter.' After the event, a crew member who suffered an undiagnosed concussion then worked on subsequent flights.
The ATSB said Qantas 'lacked a procedure to ensure cabin crew fitness was assessed after a significant injury'.
'This increased the risk that a crew member could continue to operate while being unfit for duty,' the report states.
In response to the investigation, Qantas said it would overhaul its medical assessments processes, 'regardless of visible impact'.
'Following an internal investigation, we promptly expanded our Group-wide medical escalation and post-incident medical assessment processes, ensuring all crew on-board a flight are assessed following an incident, regardless of visible impact,' a Qantas spokeswoman told NewsWire.
The investigation drew on information from the captain of the flight, the customer service manager, the R2 primary cabin crew member, Qantas, Qantas' safety manager, the Civil Aviation Safety Authority and the Bureau of Meteorology for its report.

Fatal aircraft crash due to radio, visibility failures
Radio call failures and blocked visibility on two runways likely contributed to a mid-air light plane collision that killed two people, an aviation report has found. Former commercial pilot David Maddern and his wife Jan, both in their 60s, were killed when their Jabiru J430 collided midair with a Piper Pawnee glider tug aircraft on Queensland's Sunshine Coast in July 2023. The final Australian Transport Safety Bureau report found Mr Maddern's aircraft was unable to receive or transmit radio calls and trees blocked the view between two runways, saying it likely contributed to the crash. On the fateful morning, the Pawnee aircraft was returning to land at the non-controlled Caboolture aerodrome while Mr Maddern was preparing to depart from an intersecting runway. Pilots at Caboolture rely on making radio calls and visual cues - commonly called "alerted see-and-avoid" - to prevent crashes, safety bureau Chief Commissioner Angus Mitchell said. The Pawnee was due to touch down but then a Cessna 172, piloted by a solo student, crossed the runway and was unaware of the other aircraft. This led the Pawnee to abort the landing by applying power to begin climbing back into the air while radioing other pilots about the decision. But at the same time, Mr Maddern's aircraft lifted off from the intersecting runway. Mr Maddern started a left turn trying to avoid the Pawnee but the two aircraft collided at 130 feet, the ATSB found. The Jabiru plunged to the ground, killing Mr Maddern and his wife Jan. The Pawnee remained flyable despite damage to its left wing and was landed safely by the uninjured pilot. The ATSB investigation found the Pawnee pilot had made multiple radio calls about their intention to land and the decision to abort but did not hear any from Mr Maddern. "Based on the Jabiru pilot's apparent unawareness of the Pawnee until just before the collision, and most witnesses not recalling hearing any calls from the Jabiru throughout the event, it is likely that the Jabiru pilot could not transmit or hear radio calls," Mr Mitchell said. "Because of this, and a stand of trees between the intersecting runways that blocked visibility between them, neither pilot was aware of the other aircraft." The ATSB ruled the aerodrome operator did not effectively manage or inform pilots about the risk of the trees or buildings on the runway that could impact visibility. Since the crash, the aerodrome has prohibited simultaneous runway operations and mandated take-off radio calls. The investigation also found that the regulatory guidance that pilots relied on when using non-controlled aerodromes did not clearly define what an "active runway" was. The chief regulator, the Civil Aviation Safety Authority, has decided to remove all references to the term "active runway" to prevent confusion. Mr Mitchell said the tragic accident highlights the risk of "see-and-avoid" strategies to prevent crashes. He urged pilots to mitigate the risks of the strategy by using two-way radio calls but also being mindful that communication can be missed or misinterpreted.

Inside, the pavilion-like roof utilises sustainable heating systems and gives a spacious, airy feeling alongside walls of glass to let natural light in and break up hours of claustrophobic, temperature-controlled travel. Small portions are currently boarded up – but don't feel obtrusive – as the airport undergoes a $US1.3 billion refurbishment known as the 'Great Hall Program' to be completed in 2027. Check in It's the age of unmanned check-in kiosks and a few self-service screens are stalled by a 'loading' death spiral. I soon ask for human assistance anyway, because the 23-kilogram baggage limit on United Airlines won't compute with the 30-kilogram limit I've been allocated by booking Denver-Sydney through Qantas as a Silver Frequent Flyer. Fortunately, the most important upgrades of the Great Hall Program are 17 new security screening lanes – all operating at rapid pace when I go through West Security Checkpoint. There's a five-minute wait before I remove my coat, shoes, belt and a newly acquired cowgirl hat to go through. The most time-consuming process is getting to the actual gates - Denver airport has the largest land footprint in the US and it's a 2.3-kilometre walk to the furthest gate. But the journey is straightforward via an automated underground train transporting passengers to A, B and C concourses. Food + drink I pop out at Concourse B where there's a plethora of options with quick eats led by American juggernauts Chick-fil-A, Jamba Juice, Shake Shack, SmashBurger and Starbucks. Modern Market appears to be the healthiest option with salads, soups and sandwiches. Local Colorado brewery New Belgium Brewing keeps the boys on a buck's weekend watered, while Denver's legendary brunch diner Snooze A.M. Eatery serves all-day breakfast to the time-zone-addled. Retail therapy Duty free includes the usual perfumes, makeup and alcohol stores alongside knickknacks and convenience items. The independent Denver bookstore chain of Tattered Cover Book Stores vies for my remaining American dollars. Alas, those end up going to a $US20 neck pillow after kicking myself when I realise I left mine in the hotel. Passing time Having arrived at the rental car drop-off three hours before my flight, the meandering journey to my gate leaves little time before boarding. I'd otherwise check out the golf simulator on Concourse A, the express day spa on Concourse C, or make friends with the Canine Airport Therapy Squad (CATS) team of happy pooches and one feline trotting around. But for 20 minutes I'm fully content to log into the airport's free Wi-Fi, sink into rows of padded seating in front of floor-to-ceiling glass and watch the sun set over the Rocky Mountains. The verdict Loading My worst airport experience ever was in this very terminal 10 years ago on a public holiday, when I waited in a snaking security queue that filled the entrance hall and extended out the automatic doors into the snow. Today, recent upgrades make it one of the best. Security screening is no longer the Wild West and accessing Colorado's four national parks and 28 ski resorts just got easier.
