DOSH issues stop-work order after fatal Shah Alam crane accident
SHAH ALAM: The Selangor Department of Occupational Safety and Health (DOSH) issued a stop-work order at a construction site in Shah Alam after a worker was fatally crushed by a tower crane counterweight two days ago.
Selangor DOSH said it dispatched a team of investigating officers to conduct an investigation on the same day.
They added that the prohibition notice will remain in effect until the investigation at the scene is completed.
The employer has been instructed to conduct an internal investigation to identify the cause and implement corrective measures.
"This accident involved a foreign worker who is believed to have been crushed by a counterweight block from a tower crane hook assembly.
"Preliminary investigations revealed that the victim was a contractor's worker tasked with dismantling the tower crane, who was in the process of replacing the counterweight block of the hook assembly.
"While giving instructions to the tower crane operator to lift the hook block, the counterweight block fell and struck the victim.
"The victim sustained severe head injuries and was pronounced dead at the scene," the department said.
Further investigations are underway to determine the causes of the accident and the parties responsible.
The employer could face legal action if found in violation of the provisions under the Occupational Safety and Health Act 1994, which carries a maximum fine of RM 500,000 if found guilty.
