Are microplastics hurting our fertility?

In February, Italian researchers announced what they claimed to be a world-first discovery: microplastic particles were found in the ovarian follicular fluid of women undergoing infertility treatment.
The study adds to a growing body of research from around the world showing that tiny plastic particles have made their way into nearly every part of our body, from hair, saliva and blood to breast milk, the liver, kidneys and even the placenta.
The research, published in the Ecotoxicology and Environmental Safety journal , warned of an 'emerging threat to female fertility' and called for 'further investigations to explore the possible damages that these microparticles can cause in terms of human reproduction and female fertility.'
Japan, whose population is rapidly graying and dwindling, is no stranger to infertility. In 2021, 1 in 4.4 couples in the country said they had been tested or treated for conception problems, up from 1 in 8 in 2002.
Could there be a worrisome connection?
Experts say that more rigorous research is needed before saying that microplastics are linked to the infertility crisis. Still, scientists broadly agree that it's time for government regulators and citizens alike to rethink the approach toward plastic in order to protect our health.
Hideshige Takada is one scientist who has been sounding the alarm. A professor emeritus at the Tokyo University of Agriculture and Technology and a pioneer in microplastics pollution research, he's particularly concerned about the effects of chemicals added to plastics.
'Compared with 10 years ago, we know much more about the harmful effects of microplastics,' he says. 'There's been growing evidence that many of the chemicals added to plastics to maintain or improve their functions ... are impacting human health.'
Plastic pollution
Plastic is part of the fabric of everyday life, from bags to bottles to synthetic clothing. And despite efforts by some countries, we keep producing more. In 2023, global production stood at 414 million tons, up from 311 million tons in 2014 and 225 million tons in 2004, according to the manufacturers group Plastics Europe.
Plastic waste now litters the Earth, with much of it ending up in the oceans in the form of tiny fragments, or microplastics. Microplastics absorb toxic chemicals, get ingested by marine animals and climb back through the food chain.

Microplastics are defined as plastic particles less than 5 millimeters in diameter, while nanoplastics are even smaller particles of less than 1 micrometer. They are produced from plastics that we use in daily life, either through litter or through products that fall through the recycling system, such as plastic waste that blows out of trash bins and winds up in rivers or oceans.
When exposed to ultraviolet rays in strong sunlight, plastics decay and break into small pieces. These tiny particles are more easily carried offshore and accumulate in oceans. Synthetic clothes also shed microfibers, which are flushed from washing machines into wastewater, and into streams, rivers and oceans.
Today, microplastics are everywhere in the environment, and as a result, we eat, drink and breathe them regularly.
Endocrine-disrupting chemicals
While there is some research saying microplastics themselves damage our body physically by causing inflammation, Takada says it is really the chemicals that do the most harm.
Plastics contain a host of additives to keep them from degrading, such as antioxidants and UV absorbers. Also, they contain chemicals to add flexibility and prevent them from catching fire.
Many of these chemicals have structures similar to female hormones, and they bind to hormone receptors in the cells when they enter the body, sending the wrong signals. They are known to act as endocrine disruptors, or 'environmental hormones.'
'Female hormones regulate sexual functions, reproduction and brain development, and that's why these chemicals are believed to cause problems in such functions,' Takada says.

'We now know how these additives cause reproductive abnormalities, and we have over 1,000 studies saying additives have been found in the body. All things considered, I feel there's no room to doubt that plastic additives or monomers (the smallest unit of plastics) produced as a result of plastic breakdown affect human health.'
Phthalates and bisphenol A are among the chemicals commonly used in plastics and are known to disrupt hormone functions. PFAS, also known as 'forever chemicals' because of their extreme persistence in the environment, are another group of substances that are commonly found in plastic products and are linked to a wide range of health issues, including cancer.
Currently, the health ministry regulates some of the phthalates in children's toys and food containers and packaging, and sets maximum amounts for bisphenol A that can be transferred into food. Such regulations are effective in keeping the 'direct' exposure to harmful chemicals at bay, but not enough to prevent the 'indirect' exposure, through the fish we consume, the air we breathe or the water we drink, Takada argues.
And regulations on specific chemicals fall short, he says. He cites a 2023 United Nations Environment Program report on chemicals in plastics, which states that over 3,200 chemicals detected in plastics have 'one or more hazardous properties of concern,' including ones that can 'mimic, block or alter the actions of hormones, reduce fertility, damage the nervous system, and/or cause cancer.'
At present, only 130 of the 3,200 chemicals are regulated by international or regional treaties, according to Takada.
Takada acknowledges issues raised regarding the quality of research. For example, the equipment researchers use to collect samples is rife with risks of plastic contamination. Also, there are chemicals in the human body that are very similar to plastics. Researchers may confuse the two, which could lead to the detection of unreasonably high levels of microplastics in the body, he says.
In March, scientists at Irish and British institutions pointed to issues surrounding microplastics research in a commentary published by Nature , calling for 'more rigorous standards, transparency and collaboration' among researchers, policymakers and industrial stakeholders.
They questioned many of the studies published so far, saying they rely on small sample sizes of typically 20 to 50 individuals. (The February ovarian study found microplastics in 14 of the 18 women checked.)
'Modern laboratories are themselves hotspots of nanoplastic and microplastic pollution, and the approaches that are being used to detect plastics make it hard to rule out the possibility of contamination, or prove definitively that plastics are in a sample,' they wrote.
Male infertility
The decline of semen quality, including a decrease in sperm counts and their swimming ability, has long been documented, with some researchers calling for more investigations into its link with Japan's falling birthrate .
Soichiro Ogawa, associate professor of urology at the Fukushima Medical University's School of Medicine, sees many male patients with fertility issues. Based on the available evidence, however, the effects of microplastics or particular chemicals on male infertility remain unknown, he says.
'We can't draw conclusions from just one or two studies,' he says. 'There are so many studies out there, and even peer-reviewed ones are of mixed quality.'
In addition to microplastics and toxic chemicals, many other environmental factors have been studied for their potential links to male infertility, ranging from heat, radiation, electromagnetic waves, pesticides and air pollution. Of all these, smoking poses the biggest risk, Ogawa says, noting that he recommends patients stop smoking to protect their health as well as their family's.

Takada argues there are limits to the evidence-based approach toward individual chemical substances. Modern plastic pollution is very different from 20th-century chemical pollution like the mercury poisoning that caused Minamata disease, where concentrations of specific chemicals were very high and it was relatively easy to identify the cause of the problem, he says.
'In the 21st century, we are dealing with numerous chemical substances or multiple factors that, on their own, may not have a health effect, but when combined, can cause harm,' Takada says. 'We're now in an era where it's increasingly difficult to pinpoint a single substance or phenomenon as the cause of a problem.'
As such, reducing production and consumption of plastics as a whole, instead of regulating individual substances, is most effective to minimize exposure and risks of chemicals to humans, he argues.
Policy changes
Late last year, the 5th session of the U.N. Intergovernmental Negotiating Committee on plastic pollution (INC-5) was held in Busan, South Korea. But at the session, which was meant to be the last one, negotiators failed to reach a legally binding treaty to curb plastic pollution. The countries are meeting again in Geneva, Switzerland, in August.
When it comes to policy changes, Japan may be able to learn from Europe, where the 'nudge' theory is used effectively, says Keita Nakayama, an environmental law expert and lecturer at Waseda University.
The 'nudge,' a concept from behavioral economics, is called the fourth policy approach, after laws, taxes and subsidies. It is a tool to encourage people and businesses to make better decisions without coercing them.

Nakayama cites the case of the European strategy for plastics released by the EU in 2018, which included calling on companies and business groups to make voluntary pledges to use or produce more recycled plastics. He says it worked as a nudge even though the strategy was not legally binding on its own. The same day the strategy was announced, McDonald's said it would switch all of its packaging to renewable, recycled or certified sources in all restaurants worldwide by the end of 2025. Iceland Foods, a Wales-based supermarket chain famous for its frozen meals, also announced on the same day that it will eliminate plastic packaging from all of its brand products by 2023.
Behind such corporate moves were advance coordination with businesses by authorities, which worked as a 'nudge,' Nakayama says.
Japan introduced a plastic strategy of its own in 2019 and enforced a law aimed at promoting the resource circulation for plastics in 2022, but it has been criticized by environmental groups as lacking teeth and being too reliant on recycling instead of reducing the use of plastics.
The introduction of a nationwide plastic bag fee for shoppers in July 2020 did work to reduce plastic waste, halving the volume of plastic bags in circulation to 100,000 tons in two years, according to the Environment Ministry. Now, well over 80% of people turn down plastic bags at retail stores, according to the latest statistics from the Japan Chain Stores Association, up from about 55% in 2019.
But plastic shopping bags make up only a fraction of the country's total plastic waste. Plastic packages and containers — be it cutlery, PET bottles or bento boxes — are still everywhere, accounting for nearly half of the nation's plastic waste.
Japan has been reluctant to regulate plastic production so far, refusing to join a proposal supported by about 100 countries to set a production reduction target during the INC-5 negotiations.
In addition, while 87% of Japan's plastic waste was recycled as of 2022, more than two-thirds of that was done through thermal recycling, which emits carbon dioxide and contributes to global warming.
So what should individuals do to reduce health risks?
Takada says separating plastic from our food is key. Plastics degrade with heat, so avoiding reheating ready-made food in plastic containers and switching to glass instead of plastic for food storage is a good starting point, he says. PET bottles could also contain harmful chemicals in their caps, he adds.
'It's impossible to eliminate all sources of chemicals associated with plastics, but it would be helpful to reduce their use wherever possible,' he says. 'They don't stay long in the body, so reducing your daily exposure lowers your long-term risks.'

With touchdown planned for 4:17 a.m. on Friday, Japan time, the lander, Resilience, began descending from an altitude of around 100 kilometers but was unable to decelerate to the required speed, ispace said. "Based on the circumstances, it is currently assumed that the lander likely performed a hard landing on the lunar surface," the company said, adding communication had been lost. The Tokyo-based company's CEO Takeshi Hakamada apologized to supporters at a press conference, saying the outcome was "disappointing." The company will strive to identify the cause of the failure and make another attempt in 2027 with a new lander. "We would like to catch up as quickly as possible" with U.S. companies that have already achieved the feat, he said. The company's first attempt to reach the Moon's northern hemisphere in April 2023 with a different spacecraft was unsuccessful, with the lander likely having crashed on the lunar surface. Irregularities in the altitude measurement system at that time led the lander to eventually run out of fuel during descent, resulting in a free fall to the Moon's surface. U.S. company Intuitive Machines Inc. subsequently became the first private firm in the world to successfully land a spacecraft on the lunar surface in February 2024. Resilience, transporting a rover and equipment to carry out experiments, lifted off from Cape Canaveral, Florida, on Jan. 15, sharing a SpaceX rocket launch with U.S. firm Firefly Aerospace Inc.'s lunar lander. Firefly's Blue Ghost, which took a different route from the ispace lander to reach the surface, touched down on the Moon on March 2. After entering lunar orbit in May, the Japanese lander -- 2.3 meters high and 2.6 meters wide -- started descending shortly after 3 a.m., with a planned deceleration from 580 km to 2 km per hour by the firing of its engine toward the lunar surface. However, data transmission stopped at an altitude of 192 meters while the lander was moving at a faster speed than expected, possibly due to the malfunction of an altitude measurement sensor. "As of 8 a.m. on June 6, 2025, mission controllers had determined that it is unlikely that communication with the lander will be restored," ispace said in a press release. The International Atomic Energy Agency has released a new report on analysis of samples of treated and diluted water discharged into the ocean from the Fukushima Daiichi Nuclear Power Plant. It says the results are consistent with a 2023 report which "concluded that the discharge would have a negligible radiological impact on people and the environment." The IAEA published the latest report as part of additional monitoring measures that began last year. The number of locations for collecting samples was increased as part of the measures. The report covers a sample of seawater collected near the plant in October last year. It says experts from research institutions in China, South Korea and Switzerland took part in the analysis of the seawater collected near the plant. Japan's Foreign Ministry says missions for conducting the additional measures have been carried out three times so far, and that further survey results are expected to be published in due course. The ministry says Japan's government "will continue to maintain close coordination with the IAEA in sharing information with the international community in a highly transparent manner and fostering a better understanding of the safety of the discharge of ALPS treated water within both the domestic and international communities." ALPS is the Advanced Liquid Processing System, used to treat water that accumulates at the plant. Water used to cool molten fuel at the Fukushima plant has been mixing with rain and groundwater. The accumulated water is being treated to remove most radioactive substances, but still contains tritium. Before releasing the treated water into the ocean, the plant's operator dilutes it to reduce the tritium level to about one-seventh of the World Health Organization's guidance level for drinking water.

Smoke and mirrors: How big tobacco manipulates science in Japan

In April 2019, I joined Philip Morris Japan as director of medical and scientific affairs. I'm a doctor — and a smoker — and after 25 years working in global health for the Foreign Ministry, I was attracted by the smoke-free vision of Philip Morris International, the world's largest tobacco company. Philip Morris, headquartered in Stamford, Connecticut, states that it is building its future on 'smoke-free products that — while not risk-free — are a far better choice than cigarette smoking.' Products such as its Iqos 'heat-not-burn' device, which heats tobacco without setting it alight, offer a better alternative for adults 'who would otherwise continue smoking cigarettes,' the company says, claiming that this amounts to 'tobacco harm reduction.' Yet while working at Philip Morris Japan, I came to view this as an illusion, a vision used to mislead customers around the world, including in Japan. It was here that Iqos was first launched in 2014 and the country remains the most successful market for the device — with Iqos' global revenue now surpassing that of Marlboro, the world's bestselling cigarette brand, also owned by Philip Morris. I decided to come clean after witnessing how the company used what I believe amounts to bribery to manipulate scientific research at some of Japan's leading universities to show that heated tobacco products are less harmful than cigarettes — despite this not being backed by independent science. After raising my concerns within Philip Morris Japan to little effect and my firing in October 2019, I decided to share my story with government authorities and the media. I am now advocating for an end to improper and potentially illegal practices in this industry and for Japan's tobacco policy to change. I became one of only a handful of whistleblowers in the history of the global tobacco industry to help break the spell that so many are under. Dubious harm reduction claims Philip Morris International says that it has 'dedicated more than 1,460 scientists, engineers and technicians, and invested over USD 14 billion in the research and development of innovative smoke-free products.' It also claims that, according to 'laboratory tests,' the aerosol (smoke) from Iqos and similar products 'has an average of 95% lower levels of harmful and potentially harmful constituents (HPHCs) and is less toxic than cigarette smoke.' Without directly stating that Iqos are less harmful than cigarettes, Philip Morris' message is a smart combination of four talking points: Heated tobacco's aerosol has less HPHC, tobacco harm is reduced, Iqos is a better alternative to cigarettes and therefore better for public health, and the company is committed to a smoke-free future. In April 2019, the United States Food and Drug Administration (FDA) authorized the marketing of Iqos, a decision 'largely predicated on scientific research provided by PMI (Philip Morris International) in its application, including its clinical and post-market studies from Japan,' as stated in a paper by the Tobacco Control Research Group of the University of Bath in the United Kingdom. In July 2020, the FDA went on to approve Iqos as a modified risk tobacco product, namely one that reduces the risk or harm of tobacco-related disease — a profoundly disappointing decision and one based on the lower levels of HPHC detected in heated tobacco smoke. However, in an earlier deliberation, the FDA's own Tobacco Products Scientific Advisory Committee denied that such reduced exposure was likely to lead to a 'measurable and substantial reduction in morbidity and mortality.' In addition, in a study published last year, a group of researchers from Yokohama City University found that the cigarette smoke extract of heated tobacco products and combustible cigarettes both induce cytotoxicity, i.e., cell damage. Yet many view heated tobacco products as reducing the risk of tobacco-related diseases, especially in Japan, the country with the highest use of these products globally. Such beliefs are 'in defiance of independent research evidence,' according to The BMJ medical journal. Regardless of how many harmful constituents are present in Iqos and similar devices' smoke, what really counts is how much actual harm they cause the human body. Where there's smoke, there's fire When I joined Philip Morris Japan, I inherited two cooperation projects with the University of Tokyo and Kyoto University. In the former case, the company was paying ¥36 million ($250,000) annually to a private consultancy firm owned by a University of Tokyo professor. As I told my superiors at the time, the firm didn't provide Philip Morris Japan with deliverables that justified such a high fee: These amounted to some low-quality academic articles and a medical industry forum at the university on innovation in and public acceptance of heat-not-burn tobacco products (and vaccines). The real purpose of the ¥36 million, I learned through company documents and colleagues, was to facilitate the hiring of an economist employed by Philip Morris International as a visiting scholar within the professor's university department and then to publish research under the university's name. I raised my concerns internally: Given the substantial size of the payments and the lack of due diligence when it came to the relationship with the consultancy firm, I pointed out that this arrangement posed reputational risks for Philip Morris. I feared that the payments could amount to bribery. Ultimately, the economist wasn't hired as a visiting scholar and — while the professor didn't disclose the payments his company was receiving from Philip Morris Japan to the university — a university investigation was conducted in 2020, five years after the consultancy had started receiving ¥36 million a year from my former employer. The university found no conflicts of interest or breaches of its code of conduct. This is despite its ethics guidelines stating that its employees cannot use their position 'to advance private interests for their own benefit or for the benefit of organizations to which they belong.' Visitors of a tobacco products store in Tokyo are only allowed to use heated tobacco products like Philip Morris' Iqos. If Japan is to make its indoor smoking ban effective, it must extend it to smoking rooms for heated tobacco products. | REUTERS In the Kyoto University case, Philip Morris Japan paid ¥18 million a year to research contractor CMIC, which in turn paid ¥16.5 million to a professor from the university. Kyoto University's ethics committee approved a CMIC-sponsored research project to be conducted by the professor, but no mention of Philip Morris was made in the contract between the firm and the institution or in the university's official records. The research — which focused on Japanese smokers undergoing smoking cessation treatment and was also provided to the FDA — was supervised by CMIC. In my view, the professor did not provide a labor input equivalent to the ¥16.5 million he was receiving, therefore also raising my suspicions that this amounted to bribery. While it is standard practice for companies, including in the tobacco industry, to sponsor scientific research, what also worried me was that in this case, this was not out in the open. Philip Morris Japan's compliance office investigated my allegations of wrongful conduct, but these were found to be without merit. I was given a negative performance evaluation and told I had failed my probation period at the company. Eventually, I was dismissed. I believe this was a form of retaliation for having raised my concerns, though my former employer denies my termination was unlawful. However rigorously Japanese scientific research standards are applied, they cannot fully prevent wrongdoing by people committed to working for both research and industry in unethical and potentially corrupt ways. After all, a vision founded on deliberate scientific fraud is an illusion. Fighting back Once I was fired, I could start to truly fight back. Where raising the alarm internally hadn't worked, the only option was to look outside. 'Sunlight is said to be the best of disinfectants,' late U.S. Supreme Court Justice Louis Brendeis once wrote. I approached the Japanese health and finance ministries, with the latter exercising significant authority over tobacco policy, from price and tax approval to regulating marketing and health warnings. No serious action came of this. In May 2020, I also received a subpoena from the U.S. Securities and Exchange Commission (SEC), who I had contacted on suspicion that Philip Morris' actions may have breached the U.S. Foreign Corrupt Practices Act. Under it, a publicly traded American company is prohibited from bribing foreign officials — including employees at state-funded universities. While I am not aware of an ongoing SEC case, it is not standard practice for the commission to divulge such information — not even to whistleblowers — so as to preserve the integrity of investigations. What I do know is that not long after my interview, Philip Morris International hired a new general counsel, Suzanne Rich Folsom, a corporate governance expert with rich experience in investigations and compliance. As well as talking to government officials, I also turned to the media. I was in touch with a reporter from a major Japanese newspaper and, despite their initial interest in my case, nothing came of it. Later, I found that Philip Morris International was sponsoring a conference organized by the same media outlet and that the tobacco conglomerate's CEO, Jacek Olczak, was due to speak at the event. But the sponsorship was cancelled just a few days before the forum, after several speakers had withdrawn their attendance in protest. Yet as recently as this March, two other Japanese outlets published interviews with Olczak, including statements that heated tobacco products substantially decrease exposure to cigarettes' harmful substances and that they present fewer health harms compared to their combustible counterparts. Perhaps I can't blame these newspapers for publicizing misleading tobacco industry talking points. After all, I myself had once believed in the positive health effects of heated tobacco products and Philip Morris' smoke-free vision. After my revelations failed to spark much attention in Japan, I turned to academics and journalists in the U.K. The University of Bath's Tobacco Control Research Group published articles detailing how Philip Morris Japan tried to 'exploit science' at Japanese universities. In Nicotine & Tobacco Research, it wrote that 'some of PMI's most significant clinical and post-market studies were conducted in Japan ... However, our findings raise concerns about the integrity of this evidence base and, by extension, the true harms of PMI's products.' Still in the U.K., The Bureau of Investigative Journalism published an expose and several stories as a result of my whistleblowing. In them, Japan is described as a 'tobacco state' because of the close relationship between tobacco companies and government agencies. For example, Japan Tobacco International, one of the world's largest tobacco manufacturers, is one-third owned by the finance ministry. Yet these revelations were largely ignored by the Japanese media. Moving forward My message is simple. Japan should address and properly regulate the entire spectrum of nicotine delivery products, from the most to the least harmful. If heated tobacco products are allowed, Japan should be consistent in its policy and also permit e-cigarettes: These contain nicotine but not tobacco and are classified as medicinal rather than tobacco products under Japanese law, which bans their sale. E-cigarettes are not risk-free and many health authorities express concern about their use, especially among young people. Yet these devices have been found to be less harmful than smoking conventional cigarettes, as detailed by sources such as National Health Service in the U.K. and John Hopkins University in the U.S. 'For people who smoke, legal e-cigarettes are an option to help them stop,' according to Cancer Research U.K., the world's largest independent cancer nonprofit. Also, Japanese regulators should ban heated tobacco smoking rooms, which it approved when the country adopted its indoor smoking ban in 2020: An outcome that Philip Morris Japan lobbied for, targeting select politicians with — guess what — favorable scientific research. When I was dismissed from the company, I faced turmoil and financial concerns. I was initially afraid to have my name out in public, but I was inspired to speak out by those who revealed the sexual abuse they had faced in the Japanese entertainment sector. Every time I pass an Iqos store, I cannot help but feel that I did the right thing in making my voice heard to protect public health.
