Health board not clear on design for hospital ventilation system, inquiry finds

One of Scotland's biggest health boards was not clear enough on the design of the ventilation system which delayed the opening of a children's hospital, an inquiry has found.
The Royal Hospital for Children and Young People (RHCYP) and Department of Clinical Neurosciences in Edinburgh were due to open in July 2019 but a delay was announced just hours before.
Eventually opening in March 2021 with the delay believed to have cost almost £17 million, the hospital became a subject of the Scottish Hospitals Inquiry – alongside parts of the Queen Elizabeth University Hospital campus in Glasgow.
Lord Brodie, chairman of the inquiry, published 11 recommendations on Monday as part of his interim report, where he criticised NHS Lothian's handling of the design of the RHCYP and accused it of leaving patients and families 'in the dark' on the delay.
Ahead of the opening of the site, issues with the pressure differentials and air change rates – which can help to protect from airborne diseases – in the ventilation of the paediatric critical care department were found, the inquiry reported.
Lord Brodie said: 'On the basis of the evidence that I have heard, I have concluded that in a project for the construction or refurbishment of a healthcare facility, it is the health board, as the prospective provider of that healthcare, in consultation with relevant stakeholders and clinical and technical advisers, that is best-placed to identify what it requires from the ventilation system.
'This includes the precise output parameters that the ventilation system must meet.
'Accordingly, determining the output parameters of a ventilation system should not be left to the judgment of the project company and its subcontractors during the design phase.
'The design should address how previously determined parameters are to be achieved, not what should be achieved.
'In relation to this project, it is my conclusion that NHS Lothian did not present its requirements for the output specifications of the ventilation system with sufficient clarity and precision.'
Instead, the board relied on the contractor's interpretation of NHS guidance for ventilation systems, the inquiry found, and provided a document described as an 'environmental matrix' which set specifications for the ventilation system.
The matrix contained an 'undetected error' in relation to critical care rooms, the inquiry said.
Jim Crombie, the deputy chief executive of NHS Lothian, said: 'Lord Brodie has identified our failure to provide a clear design brief at the outset as critical to events as well as weaknesses in governance and understanding between contractual partners.
'We are sorry for these failures and also deeply regret that opportunities were missed by all parties over the course of the project to detect and rectify errors in the design of the ventilation regime.
'The report acknowledges the changes that have been put in place in NHS Lothian and the new guidance introduced since 2019 to ensure that lessons are learned and similar events can be prevented in the future.
'Finally, I'd like to thank the relatives of patients affected by the delay who took part in the inquiry for their important contributions.'
As well as the design of the buildings, the inquiry also criticised NHS Lothian's handling of communications with patients and their families around the delay.
'The decision not to open the hospital as planned had a significant impact on patients and their families, who were shocked, scared and deeply disappointed that long-promised new facilities were not to be available for the treatment, in some cases, of children suffering from very serious conditions,' Lord Brodie said.
' Patients and their families were left in the dark as to the reasons why the hospital did not open as planned.
'Communication with patients and families was unsatisfactory in this regard. I have seen this to have been important.
'There is a group of young patients who are very seriously ill and spend a significant portion of their time, sometimes much of their lives, in hospital.
'They are supported by family members or guardians. The hospital becomes, for them, their second home.
'The impact of unclear or poor communication on the wellbeing of patients and their families during what may already be a very difficult, emotional and uncertain period in their lives is significant.'
Mr Crombie added: 'Patient safety was always our driving force in commissioning a first-class hospital building so we know that the discovery of a ventilation problem just days before it was due to open caused shock and significant concern for our patients, their families and our staff.
'We once again reiterate our sincere and unreserved apology to them all for the resulting delay to the transfer of patient services to the new facility.
'I am particularly sorry that patients and their families were let down at a time when they were already worried and vulnerable.'
The final report of the inquiry is expected next year.

Dr Pavan Srireddy, vice-chair of the Royal College of Psychiatrists in Scotland, said: 'While concerning, these figures come as no surprise. The unprecedented demand for services and critical gaps in the mental health workforce is already leading to unacceptably long waits for mental health care and treatment. ‌ 'Working on the frontline we see a postcode lottery of specialist mental health services across the country, but our patients deserve so much better than this. ‌ 'Alarmingly, waiting times may be even higher because the Scottish Government don't consider the rise in people seeking help for neurodevelopmental conditions such as Autism or ADHD." NHS Lothian had the longest list, with 4,287 patients waiting to be seen. This was followed by Greater Glasgow and Clyde on 3,733 and Tayside on 2,821. Grampian had 2,736 patients waiting, Forth Valley had 2,729 and Lanarkshire had 2,097. The only other health board with more than 1,000 people waiting was NHS Fife, on 1,746. Dr Srireddy continued: 'The situation is only set to get worse if there is no action. Targets are missed but what we need is ringfenced funding and resources ploughed in at local level. 'We also urgently need a strategy to not just recruit more psychiatrists, but to retain our experienced doctors who sadly face excessive workloads, stress and burnout due to staff shortages leading them to leave permanent roles. ‌ 'Above all, we need ring-fenced mental health funding to reach the government's own 10% budget commitment and ensure adequate resources are put in place urgently, for recruitment and retention.' Campaigner Peter Todd - who waited five years for mental health treatment - said: "Many of our leaders have no idea much patient's lives are affected by waiting years for psychological treatment because they have no sense of what goes on in the real world. "No doubt we will here covid being used as an excuse. But that doesn't wash because I was forced to wait 5 years back in 2018 due to being a victim of the MP Cyril Smith scandal which I gave evidence at the Historical Abuse Inquiry about. ‌ "I look forward to John Swinney campaigning in Caithness in the run up to the Holyrood election, so he can hear my experience and how my Iife was ripped apart because of his parties inept running of mental health services which forced me to wait five years." Scottish Labour Mental Health spokesperson Paul Sweeney said 'These shocking figures lay bare the true scale of Scotland's mental health crisis and the SNP's woeful failure to deliver the support needed. 'Behind these figures are people who have been failed in their darkest hour by an NHS that is no longer truly free and available at the point of need. ‌ ' NHS staff are working tirelessly to keep overstretched mental health services going, but they are being let down by an SNP government more interested in fiddling the figures than fixing the waiting lists. 'Scots needs NHS mental health services that they can count on and we need a change in government to deliver that.' A Scottish Government spokesperson said: 'The latest Public Health Scotland data shows that more than 74,000 people started treatment in psychological therapies last year. ‌ 'One in two patients are starting treatment within three weeks of referral and there was a significant increase of 7.5% of patients starting treatment in the last quarter. 'This has been made possible by the hard work of our psychology services workforce which has increased by 61.8% in the last decade. 'We want to ensure that all people in Scotland can access the right help, at the right time to support their mental and physical health. We continue to work with NHS Boards that are not on track to meet the standard that 90% of patients begin treatment within 18 weeks of referral.'

Testing a single piece of AI software currently takes more than nine months NHS Lothian is one of two health boards in Scotland partnering with an evaluation company to speed up the safe adoption of artificial intelligence (AI) in the healthcare system. The £1 million Innovate UK-funded project brings together NHS Lothian and NHS Greater Glasgow and Clyde hospitals, Edinburgh and Glasgow universities, and technology companies. ‌ Testing a single piece of AI software currently takes more than nine months, and with more than 200 AI products available for a single hospital department, checking all of them individually using traditional clinical trial or service evaluation models isn't realistic. The result is that very few trials turn into real-world solutions, meaning that patients, hospitals and AI developers all lose out. ‌ The collaboration will use Aival's independent evaluation platform to assess AI systems in a secure environment, with all patient data anonymised and remaining within an NHS secure environment. The project will assess the use of the Aival platform to compare six commercial AI products for the triage of stroke and lung cancer patients from AI developers InferVision, Annalise-AI and – who are all collaborating with the programme. AI products are often calibrated to the locations where they are being used to allow for differences in the local population, meaning they may not generalise to different groups without extra training. The performance of AI products changes over time – even when the product itself isn't altered. This is known as 'drift', and can happen as a patient population changes, as the commonness of a disease shifts, or a scanner's software is updated. Dr Rishi Ramaesh, Consultant Radiologist and Innovation Fellow at NHS Lothian, said: 'Artificial intelligence has tremendous potential to improve patient care, but healthcare leaders need confidence that these systems are safe and effective. This project will help healthcare leaders to evaluate AI, and make sure that new technologies deliver real benefits for patients.' Luciana D'Adderio, Edinburgh University academic and AI evaluation and assurance expert, said: 'AI technology is achieving widespread deployment across healthcare settings, yet its assurance has not received the rigorous attention it demands. ‌ 'The current frameworks for evaluation and monitoring fall short in adequately assuring AI systems, primarily because AI represents a fundamental departure from the traditional technologies these frameworks were designed to assess. 'There is an urgent need for innovative tools and technologies for AI assurance, which themselves must undergo thorough evaluation and validation. This critical challenge forms the cornerstone of our groundbreaking project.' ‌ Kanwal Bhatia, CEO and founder of Aival, said: 'It's vital that we monitor and check AI that's being used in decisions on patients' health, to ensure the best outcomes for patients. 'Any healthcare workers using AI need to be sure that the product is doing what it says it does – not just now, but five years in the future. While many NHS teams understand this, they currently lack the tools to properly evaluate the AI products. ‌ 'Putting in place effective validation systems will encourage trust and adoption of AI, and will deliver cost savings and growth in the NHS and in private healthcare. 'We work hand in hand with NHS leaders, clinical and technical teams to provide the expertise and software to ensure that their AI systems are doing what they're supposed to do.'

'Extremely alarming': 164 children waiting over a year for mental health care

There are 164 children who have been waiting more than a year for mental health services in Scotland – marking a 20% increase from this time last year. An additional 253 young people have been waiting between nine months and a year for treatment with NHS Scotland's Child and Adolescent Mental Health Services (CAMHS). The Scottish Children's Services Coalition (SCSC), an alliance of leading providers, called the figures 'extremely alarming'. 'Each one of these statistics is an individual, and we would urge the Scottish Government to ensure the adequate resourcing of mental health services for our children and young people so that they can get the care and support they need, without lengthy waits,' a spokesperson for the SCSC said. 'We are facing a mental health emergency, and many of our children and young people are at breaking point, with stress and anxiety reaching alarming levels as they battle with the long shadow of lockdown and the rising cost of living. 'This is also having a negative impact on classroom behaviour, affecting the young people concerned, their fellow pupils and staff.' Although Public Health Scotland has insisted that the number of children starting treatment within 18 weeks of referral is improving, the number of people waiting for over 18 weeks has increased across the board since December 2024. From December 2024 to March 2025, the number of children waiting over a year has increased from 145 to 164. Since March 2024, that figure has risen from 137. The number of children waiting between five and nine months has also increased since December from 424 to 499. The only category that's fallen is the number of children waiting between nine months and a year – from 283 to 253. Most of the children who have been waiting for more than 12 months are in the NHS Lothian district. NHS Lothian has the highest number of children on the CAMHS waitlist of any other health board, and it accounts for 88% of the children waiting more than a year for treatment.
