Sponge test ‘could benefit thousands of people' with Barrett's oesophagus

Thousands of people being monitored for food pipe cancer could be spared invasive tests thanks to a 'sponge on a string' check, research suggests.
A new study published in the Lancet medical journal found invasive endoscopies could be replaced for over half of patients with Barrett's oesophagus.
This condition occurs when stomach acid damages the lining of the oesophagus (the food pipe). Some cells may grow abnormally and develop into oesophageal cancer.
At the moment, the NHS offers regular monitoring of the condition every two to five years with an endoscopy, which involves passing a tube with a camera into the oesophagus.
However, a new, less invasive test called a capsule sponge test could be used instead for some patients, researchers believe.
The test is already being used in some NHS hospitals to diagnose the condition.
The new Lancet study, funded by Cancer Research UK, NHS England Cancer Alliance and Innovate UK, was carried out at 13 hospitals in the UK.
Some 910 people took part and were given the capsule test.
This involves a patient swallowing a capsule attached to a string. The capsule dissolves in their stomach and leaves a small sponge.
The sponge is then pulled out via the string by NHS staff after a few minutes, during which time it collects cells from the oesophageal lining.
In the study, around 15% of patients were found to be at high risk of developing cancer, while more than half (54%) were classed as low risk.
The authors concluded that the capsule sponge could then be used to monitor patients with low-risk Barrett's oesophagus instead of regular endoscopies, and the test could be administered by nurses in GP practices.
The study was from researchers at the University of Cambridge, Addenbrooke's Hospital and Queen Mary University of London.
Professor Peter Sasieni from the Cancer Research UK's cancer prevention trials unit at Queen Mary University of London said: 'Our findings suggest that the capsule sponge could help stratify patients with Barrett's oesophagus by risk and that half of them will fall into the low-risk group.
'Given that the risk of these individuals progressing to dysplasia (abnormal cells) and then to oesophageal cancer is so low, it should be safe to replace their usual endoscopy with the capsule sponge.'
Professor Rebecca Fitzgerald, from Cambridge, added: 'We are very excited by these results, which could lead to a test that is much more accessible and less operator dependent to improve standards for monitoring for patients with Barrett's across the NHS and beyond.'
Chief executive of Cancer Research UK, Michelle Mitchell, said: 'Survival rates for oesophageal cancer have remained unacceptably low for decades, with fewer than 20% of patients surviving for five or more years after diagnosis.
'Early detection is vital if we are to change this grim statistic.
'The Cancer Research UK–backed capsule sponge represents one of the most promising breakthroughs in early detection we have seen to date.
'These new findings bring us closer to transforming the way we diagnose and ultimately treat this disease.
'If adopted widely, this innovative approach could spare significant numbers of people from discomfort and invasive endoscopies. By bringing this more accessible alternative into community care, we have the potential to save more lives.'

Common garden snails and slugs could now be considered venomous, according to scientists. In a study that shakes up the definition of venom, researchers explain it is not just the bites of snakes and spiders that are classed as venomous, but also the saliva of aphids and the chemicals released by slugs. This change in definition will mean that tens of thousands of additional species could now be considered venomous. According to the research, published in the journal Trends in Ecology & Evolution, substances like saliva all function with the same evolutionary function in mind: to manipulate another organism's body against its interests. The paper, led by the Natural History Museum's venom expert Dr Ronald Jenner, argues that venom should be redefined as any internally delivered secretion that one organism uses to make a physiological change in another living organism. This means that insects such as cicadas, aphids and shield bugs - which suck the sap of plants and inject toxic secretions to disable plant defences - and garden snails and slugs, which use toxins to manipulate their sexual mates, join the ranks of snakes and scorpions that use venom on prey. Dr Jenner said: 'This redefinition helps us understand venom not as a narrow weapon, but as a widespread evolutionary strategy. 'If you look at what the proboscis of a mosquito does when it's in your skin, it injects toxins that suppress the immune system so that the animal can safely take a blood meal without being swatted away. On a molecular level it shows a lot of similarities to what happens when a viper bites, say, a bunny. 'Conceptually they work on exactly the same system: a conflict arena between two organisms that is mediated by injected toxins. And that's venom.' Researchers also found the venom in wasps, bees and ants, as well as bugs and aphids, were originally used on plants rather than animals. Slugs and snails also inject potential partners with toxins during sexual courtship. Examples range from snails that shoot love-darts coated with bioactive molecules to manipulate their partners against their interest, to male blowflies whose barbed phallus injects a secretion that prevents females from mating again. These sexual secretions, the researchers argue, also fit their definition of venom because substances are internally delivered to manipulate the recipient in a conflict of evolutionary interests. The authors hope this redefinition could mean that scientists from traditionally separate fields can combine forces to accelerate understanding of venom biology.

DIY tests are to be given to women who have consistently missed offers for cervical screening, in a bid to prevent cases of cervical cancer. The tests, which can be performed at home, look for the human papillomavirus (HPV) – the virus which is linked to the majority of cases of cervical cancer. After being approved by the UK National Screening Committee in March, the NHS in England is going to offer the tests to certain women. The rollout is expected to begin in January 2026. But many people don't take up the offer with a number citing a lack of time, discomfort or embarrassment as a reason for them not going. Data published by the NHS last November showed five million women are not up to date with routine check-ups. Now officials have said that at-home kits will be offered to women who have rarely or have never attended their cervical screening. The initiative will see women sent a self-sampling kit in the post which can be returned via pre-paid mail. People who were found to have the HPV virus will be encouraged to attend an in-person appointment with a clinician. Research has suggested that offering DIY testing kits could boost uptake. The YouScreen trial, which was led by King's College London in partnership with NHS England, found that offering self-sampling kits to 'under-screened' women when they attend their GP practice and by posting kits to women's homes could boost uptake in England by about 400,000 each year. The Department of Health and Social Care said the new approach could potentially save about 5,000 lives a year across England. It said that the new programme 'specifically targets those groups consistently missing vital appointments' including: younger women, those from minority ethnic backgrounds, people with disabilities and LGBT plus people. Health and Social Care Secretary Wes Streeting said: 'These self-sampling kits represent healthcare that works around people's lives, not the other way around. 'They put women firmly in control of their own health, ensuring we catch more cancers at their earliest, most treatable stages. 'Our 10-year health plan will fundamentally reform the NHS, shifting focus from treating illness to preventing it before it starts. 'We know the earlier cancer is diagnosed the better the chances are of survival. 'By making screening more convenient, we're tackling the barriers that keep millions of women from potentially life-saving tests.' Dr Anita Lim, chief investigator of the YouScreen trial from King's College London, added: 'This is a significant step forward for cervical cancer prevention and brings us closer to the NHS goal of eliminating the disease by 2040. 'It's hugely positive to see this now reflected in national policy, helping more people get protected from this highly preventable cancer.' Michelle Kane, NHS England's director of screening, said: 'There are a number of reasons that stop some women taking up the offer of screening and we hope the introduction of self-testing will encourage more women to take up this life-saving test in a way that works for them. 'I'd encourage anyone who gets an invite for a cervical screening, either from their local GP practice or the NHS App, to attend and if you have any worrying symptoms, please contact your GP. It could save your life.'
