UPDATE : 12 :30 p.m. Jade Palace Two in Kailua-Kona was allowed to reopen after passing a follow-up inspection by the state Health Department, which issued a green 'pass ' placard on May 22. The restaurant was shut down earlier this week for multiple health code violations, including pest activity and contaminated food. To reopen, the owner was required to discard all unsafe food, hire pest control, disinfect the kitchen and retrain staff. State inspectors confirmed that all issues had been resolved and no imminent health hazard remained. PREVIOUS COVERAGE The state Health Department has shut down a Kailua-Kona restaurant after a routine inspection uncovered an active cockroach infestation, contaminated food and multiple food safety violations. Jade Palace Two, located at 75-5595 Palani Road, was issued a red 'closed ' placard on Wednesday and must remain closed until all violations are corrected and a follow-up inspection is completed. The restaurant is operated by Minh Tuan Tran. According to the Department of Health, an inspector saw live roaches in food, on food-contact surfaces and underneath equipment. A container of flour was found infested with roaches, and cooked wontons were seen cooling on the floor. Contaminated food was ordered to be discarded immediately. Other violations included :—An employee returning from a smoke break without washing hands ;—Improper food storage, including raw chicken and beef stored above cooked broccoli ;—Ready-to-eat items in the refrigerator without use-by dates ;—A dishwasher with no sanitizer, and handwashing sinks used for cleaning pans ;—Grease, dust and grime buildup throughout the kitchen. The department is requiring a professional pest control service, deep cleaning and disinfection of the facility and retraining for all food handlers. Jade Palace Two will be allowed to reopen only after it meets all food safety requirements and passes reinspection. The Food Safety Branch conducts routine inspections of food establishments statewide to ensure sanitary conditions and protect public health. Santa Monica's Bay Cities Italian Deli & Bakery closed on Wednesday after inspectors found several health code violations, mainly a vermin infestation. The Los Angeles County Department of Public Health described the vermin infestation violation as "major" in its reasoning for Bay Cities' closure. Inspectors also listed nine other violations, totaling 24 points. According to the department's health grading system for restaurants, markets and bakeries, Bay Cities would have received a C rating. "All food facilities that have a critical violation may be subject to closure regardless of the score received on the inspection report," public health officials wrote on the department's website. "Food facilities that score below seventy percent (70%) twice within a twelve (12) month period are subject to closure and further legal action." It's unclear when Bay Cities will reopen. According to public health records, the market has failed one other inspection in the last five years. In the September 2024 public health report, inspectors found two critical violations for hand washing and improper sanitization of utensils and equipment. Public health officials said they carried out the inspection after someone got sick after eating the market's signature "Godmother sandwich." The family-owned market opened in 1925 and gained a following for its sandwiches. "When you dine at our Italian deli and bakery in Santa Monica, California, you can expect the highest quality food made with the freshest ingredients, along with friendly, personal customer service," the market wrote on its website.

Banh Mi Spot in Philadelphia's Fishtown neighborhood shut down for health violations

A Philadelphia restaurant is temporarily shut down after inspectors found several health code violations this week. A report from the city's health department shows Banh Mi Spot in Fishtown had violations related to rodents, handwashing, food storage and general cleanliness when inspectors visited on Tuesday. These are some of the violations listed on the report: An employee eating in the food prep area Employees not properly washing their hands because soap was not available or the soap dispenser was broken Old food residue in the handwashing sink in the prep area Mouse droppings in the food prep area and utensils Mouse droppings found along floor perimeters throughout the facility Dead mouse on the floor in the basement storage area Food that was not properly labeled An employee with long hair not wearing a hat or hairnet in the kitchen Single-use gloves worn for multiple tasks A dusty wall vent above to-go container storage General cleanliness issues including food residue on the kitchen floor and rags lying on the counter In an Instagram post, Banh Mi Spot said it's closed until further notice but plans to reopen as soon as possible. "We are truly sorry for letting everyone down!" the message from the team says in part. "Despite it being a big blow to our business, we have worked tirelessly over the last 24 hours to ensure the safety of our customers," the post says. According to the report, the restaurant, which is located at 1425 Frankford Avenue, was ordered to stop operating for a minimum of 48 hours. The restaurant will have to pay a fee and pass another inspection to reopen. According to a menu online, the restaurant serves banh mi, spring rolls, vermicelli and other Vietnamese dishes and drinks. According to Instagram, Banh Mi Spot opened in August 2024.
