
Column: Health care provider Aunt Martha's commits to continue serving patients who lose Medicaid
But at Aunt Martha's, where 65% to 70% of patients are on Medicaid, the federally qualified health center has no plans to drop patients who lose Medicaid coverage.
'Regardless of whether they have Medicaid coverage or not doesn't change Aunt Martha's commitment to servicing them,' said Garza.
'It's unfortunate, some won't have Medicaid, and I'm not saying by Aunt Martha's serving them that will be enough. They might need care that goes beyond what Aunt Martha's is able to provide. If that's the case, they are going to need to engage other health systems that can provide that additional care.'
For those who lose Medicaid coverage and require that more extensive care, he worries some may not be able to access care. He also fears safety net hospitals, which have already been dealing with Medicaid reimbursements that don't cover the cost of care, won't be able to serve all those in need.
More than 300,000 people in Illinois are expected to lose Medicaid coverage including 40,000 in the south suburbs under the new law, said U.S. Rep. Robin Kelly, a Democrat from Matteson and chair of the Congressional Black Caucus Health Braintrust.
The law includes $1 trillion in cuts to Medicaid, has new 80 hours a month work requirements for Medicaid recipients and requirements that recipients have their eligibility redetermined every six months instead of once a year. Critics say administrative red tape barriers will cause many to lose coverage and note that most Medicaid adults under age 65 are working already or face barriers to work.
The result of the Medicaid cuts will mean those without Medicaid will delay receiving care and end up in emergency rooms sicker and where care is more costly, Kelly said. Health care providers will be forced to serve fewer people, and many rural hospitals will close due to insufficient Medicaid reimbursements, health care advocates and experts have warned.
Roughly 60% of Aunt Martha's Medicaid patients are adults, and 40% are children, said Garza.
'Patients are concerned,' Garza said. 'They are asking us if they are going to lose coverage. The positive thing is many of our patients do work already. But we haven't gotten the exact guidance. We don't have language on the criteria. We don't know how many people would be able to meet the criteria.
'If they have to find a job, are they going to be able to do that? We'll have to see what happens. But we believe several thousand people will be affected, of patients at Aunt Martha's.'
Aunt Martha's message is one of stability, he stressed. In 2023, it received a surprise unsolicited $9 million donation from billionaire MacKenzie Scott's Yield Giving fund after the funder did quiet research into the organization's operations. That funding certainly helps, Garza said.
'We have done the right things over the years to position ourselves to being able to deal with changes,' he said. 'These are certainly dramatic changes. But we're going to be here.
'Does that mean appointments might take a little longer, that it might take longer to get into the clinics? Yeah, but we're not going to deny them. We're still going to see them.'
Aunt Martha's is focused on looking at ways to innovate and create more operational efficiencies. It's experienced at that. Garza noted more than 20 years ago, Aunt Martha's integrated mental health with primary care.
'People thought that was impossible to do back in in early 2000s,' he said. 'We pioneered it in Illinois at Aunt Martha's. From there, we pioneered telehealth as a federally qualified health center. Before the Affordable Care Act, we were already exploring how to provide services to this population in creative, efficient and innovative ways.'
Now among its priorities is looking at how to go above and beyond the telehealth services it provides.
'We have to evaluate the kinds of appointments we provide telehealth for and figure out ways to be more efficient,' Garza said. 'That's part of our approach.'
He said fundraising has also taken on a heightened priority.
'We have to figure out how to do our work, so people aren't left without care.'

Medicaid changes will hurt family caregivers, experts warn
Medicaid cuts under President Trump's sweeping tax and spending package will harm family caregivers, experts warn, by reducing access to health care for themselves and the people they care for, which could then lead to more caregiving responsibilities. The Congressional Budget Office estimates the package will reduce Medicaid spending by roughly $911 billion over the next 10 years and increase the number of uninsured Americans by up to 10 million. Some of those who could lose coverage are among the 8 million — or 13 percent — of family caregivers in the United States who receive their health insurance coverage through Medicaid, according to the National Alliance of Caregiving. 'We are very concerned of the impact of the just finalized Medicaid cuts on the community of family caregivers,' Jason Resendez, president and CEO of the alliance, said during a call with reporters earlier this week. Medicaid recipients will be subject to more frequent eligibility checks, in part, due to revised work requirements for the joint state and federal program. Now, adults between the ages of 19 and 64 will need to work or participate in community service activities for at least 80 hours a month to be eligible for health care coverage under Medicaid. There are some exceptions for parents with dependents as well as for those deemed 'medically frail' or who are pregnant or postpartum, according to the health care policy nonprofit KFF. Many caregivers cannot work outside the home because of the intense care their family members need, or can only work limited hours, which can make fulfilling Medicaid work requirements difficult to impossible. This was the case for Lisa Tschudi, host of caregiving podcast 'Love Doesn't Pay the Bills,' who stayed home full time to take care of her daughter who has ataxic cerebral palsy and epilepsy. 'We really did not have other options,' she said. 'I, many times, tried to line up some non-me care for her during the workday and a job for myself, and I never really got my start in a paid job in that way.' Her daughter's epilepsy was poorly controlled for years as a child and teen, which required her to travel for frequent doctors' appointments on top of taking care of her younger daughter. 'It was a lot to manage,' she said. Work requirements might force some family caregivers to look for outside help, if that is an option, which represents a new expense and, potentially, a new challenge to navigate. 'Even if you can find outside providers to come in … families often find that they are not reliable,' said Elizabeth Edwards, senior attorney at the National Health Law Program. 'Some of that inconsistency of how people show up as providers can mean it's very hard to hold a job.' Family caregivers also already spend huge amounts of time navigating numerous bureaucratic hurdles, and new work requirements will add to the paperwork they have to fill out to prove they are eligible for health care under Medicaid. This extra administrative work also increases the likelihood of errors occurring in the eligibility system, which could delay coverage or prevent some from being enrolled altogether, according to Edwards. That is what happened to many Medicaid recipients in Arkansas and Georgia when the two states implemented work requirements in 2018 and 2023, respectively. More than 18,000 people in Arkansas lost Medicaid coverage over the 10-month period the state rolled out work requirements without increasing employment, according to a KFF analysis. Georgia still has one of the highest uninsured rates in the country at 12 percent, according to the Commonwealth Fund. 'We anticipate [them] not just being faced with these eligibility issues, but family caregivers losing Medicaid coverage because of these additional hurdles that they'll be forced to traverse,' Resendez said. About 11 million family caregivers in the U.S. receive payment for the care they provide, according to Resendez. Those payments primarily come through home and community-based services and consumer-directed programs at the state level. But those programs will likely start to lose funding as states are forced to decide what to fund with fewer Medicaid dollars, experts told The Hill. 'When states have less money and are forced to make decisions, home and community-based services are the first optional benefits to get cut,' Resendez said. Tschudi, as well as her husband and second daughter, are all paid family caregivers under a home and community-based service their home state of Oregon's Medicaid program provides. Without that program, her family would likely have to go back to unpaid caregiving, which would not be financially possible at this time. 'I don't wish it on anyone,' Tschudi said about the struggles that come with unpaid caregiving. 'I really think you leave families in an impossible situation when you don't pay for caregiving.'

I'm a wellness writer, and I get overwhelmed by health advice, too. Here is how I prioritize
After a long day of work, I climb onto the couch to make plans for the rest of the week, and my mind buzzes with everything I could put on my priority list. I should spend less time on social media, but it is also good to stay connected to long-distance friends. I need to drink water regularly, but does my tap water have endocrine-disrupting chemicals? Eating leftovers rather than restaurant meals is healthier, but I shouldn't store food in plastic because I need to reduce my exposure to microplastics. I hate running, but I should give it a try. Will that one carrot salad that influencers like really make a big difference in my nutrition? What is a wellness writer to do? If I am overwhelmed by all the ways to improve wellness — and it's my job — I am sure you could be, too. Here is what I have learned about finding the balance. One of the most valuable pieces of advice I have received when it comes to wellness is 'first things first.' It is tempting to look for ways to cut sugar and salt out of my diet when I read a study on their health impacts or to spend the whole day researching the chemicals used in my homewares to see whether I should replace them. But then I am reminded to focus on the most immediate things. Am I eating enough? Am I sleeping well? Did I get some good movement in my day? Have I seen my friends and family? If the answer to any of those things is no, or even a not really, then nothing else is important to me. It is better that I make sure I am eating a meal that nourishes me than that I nitpick the calories or contents. It is better to sleep soundly and peacefully than to stress all night about microplastics. It is better to spend time laughing with people I care about than to sit in a room filled with red light panels. These aren't just my personal priorities. Regular exercise, good sleep and a balanced, plant-based diet have consistently shown to be key in a healthy, long life. And more evidence is pointing to the quality of social connections being an important aspect of longevity as well. (My colleagues wrote those stories.) Once those foundational bases are covered, then you and I can move on to fine-tuning the nutrition, exercise, products and wellness practices that make up our lives. Let's say you have those essential aspects of your life mostly managed, and you are ready to get more detailed in your wellness. Making changes often takes time, effort and money. There is a lot of research that many of the products commonly found in homes contain potentially dangerous chemicals such as PFAS or endocrine-disrupting chemicals. The class of chemicals known as PFAS, or perfluoroalkyl and polyfluoroalkyl substances, are sometimes called 'forever chemicals' because they don't fully break down in the environment and are commonly found in household products. They are endocrine-disrupting chemicals, according to the Endocrine Society. Chemicals in PFAS categories have been linked with cancers, endocrine-related conditions and developmental delays in children. It may not be realistic to get rid of everything in your home and start from scratch. Instead, find ways to make a reasonable change that can have a big impact. For example, finding a good water filter is one step that can address endocrine-disrupting chemicals, PFAS and other potential contaminants. The Environmental Working Group also has a tap water database so that people can search their drinking-water quality by ZIP code and use a water filtration guide to pick a system that is right for them. NSF, formerly the National Sanitation Foundation, also has a list of recommended filters. If you are hoping to change some personal care or other home products, start with the things to which you aren't particularly attached, said Dr. Alexa Friedman, a senior scientist on the healthy living team at the Environmental Working Group, in an earlier article. Friedman has curly hair, so she prefers a certain shampoo and conditioner, but she said there are other personal care products in which she might not notice as much of a loss if she were to switch to those with lower levels of endocrine-disrupting chemicals. Plenty of reputable online databases are available to the public to search for safer personal care products, such as developed by allergists and dermatologists with the Mayo Clinic, and Skin Deep from the Environmental Working Group, said Dr. Michael Bloom, a professor of global and community health at George Mason University's College of Public Health in Fairfax, Virginia. Nutrition and exercise are other wellness categories in which an abundance of good guidance also creates plenty of ways to make you feel overwhelmed. How you eat. The Mediterranean diet, which focuses on plants, healthy fat sources, whole grains and lean proteins such as fish, nuts and legumes, has consistently ranked as one of the healthiest dietary patterns and has been linked to many health benefits. Studies have also shown the importance of reducing sodium and sugar for longer lives with less risk of chronic disease. How you move. Likewise, exercise has been linked to better mental health, lower risk of cognitive decline and reduction in chronic diseases. Adults should get at least 150 minutes of moderate physical activity and two days of strength training a week, according to the US Centers for Disease Control and Prevention. But if all you can do now is get a couple of thousand more steps in your day, 10 minutes of exercise or more beans in your diet, those are good places to start, experts say. How to create a habit. The most sustainable way to alter diet and exercise habits is to do so in slow, manageable steps, according to a 2017 study. Adding something into your day — exercise, nutrients or even more sleep — takes willpower, which you can wrestle into place in the short term but isn't a good strategy for long-term change, said Katy Milkman, the James G. Dinan Professor at the Wharton School of the University of Pennsylvania. But good habits and a well-established routine can help you make the choices you are trying to stick to, Milkman pointed out earlier. Try making small additions of health-promoting behaviors — even just one change a week to make it stick — to what you are already doing rather than scrapping all your habits and starting from scratch.
