Titan submersible tour operator ‘leveraged intimidation tactics' to evade scrutiny

The tour operator responsible for the Titan submersible, which fatally imploded near the wreckage of the Titanic, 'leveraged intimidation tactics … to evade regulatory scrutiny', a US Coast Guard investigation report has concluded.
The incident resulted in the deaths of five people in June 2023 – including British adventurer Hamish Harding and father and son Shahzada and Suleman Dawood.
The chief executive of tour operator OceanGate Expeditions, Stockton Rush, and French national Paul-Henri Nargeolet, were also killed in the incident.

On Tuesday, the US Coast Guard published a 335-page report in which identified eight 'primary causal factors' that led to the fatal implosion.
The report criticised OceanGate's design and testing processes and the continued use of the Titan submersible despite 'a series of incidents that
compromised the integrity of the hull and other critical components'.
Authored by lead investigator Thomas Whalen and marine board chairman Jason Neubauer, it read: 'For several years preceding the incident, OceanGate leveraged intimidation tactics, allowances for scientific operations, and the company's favourable reputation to evade regulatory scrutiny.
'By strategically creating and exploiting regulatory confusion and oversight challenges, OceanGate was ultimately able to operate Titan completely outside of the established deep-sea protocols, which had historically contributed to a strong safety record for commercial submersibles.
'The lack of both third-party oversight and experienced OceanGate employees on staff during their 2023 Titan operations allowed OceanGate's chief executive officer to completely ignore vital inspections, data analyses, and preventative maintenance procedures, culminating in a catastrophic event.'

French Police opened an investigation into the death of the Mr Walsh after his body was discovered lying on the tracks inside the Grands Boulevards metro station in the northeast of the city. He had travelled from Ireland to watch the 16th stage of the Tour de France cycling race. Comeragh Cycling Club, which Mr Walsh had been a member of for the past 10 years, launched a fundraiser for the Kevin Bell Repatriation Trust last month. The club said: "Tomas sadly lost his life in Paris this July. "The Kevin Bell Reparation Trust's help has been invaluable in all aspects of the reparation of Tomas. We've chosen to fundraise for Kevin Bell Repatriation Trust. All funds raised here will go directly to this great cause." The 'In Memory of Tomas Walsh' campaign on iDonate has so far raised €4,648 out of its €5,000 goal. The Walsh family confirmed Tomas' funeral details yesterday. A death notice said: "He will be forever missed but always remembered by his heartbroken parents Frank and Bernie and his sisters Caoimhe and Bróna. "Will be sadly missed by his grand parents Chrissie and Joan, his niece Molly, Caoimhe's partner Paul, his aunts, uncles, cousins and his many, many friends especially those in the Comeragh Cycling Club." Tomas who will be reposing on Friday from 4pm to 7pm, with his family requesting that the house remains "strictly private" outside these hours. A Requiem Mass will take place at 12pm this Saturday in St Nicholas church in Faithlegg, Co Waterford. Burial will take place afterwards in the adjoining cemetery, with family flowers only and donations to the Kevin Bell Repatriation Trust. 'CREDIT TO HIS PARENTS' Online tributes to the cyclist described him as a "pleasant young man with the world in front of him", a "credit to his parents" and a "really great lad". One posted: "A truly lovely lad. You packed so much into your short life", another said: "It's very hard to comprehend the loss of such a talented young man who was so full of the joy of life". Another tribute read: "No words can be said to express how sad we feel at the heartbreaking loss of your son and brother, his young life tragically cut short, may he rest in peace". In a further tribute, a Mother and Nana of young children said: "My deepest sympathy at the loss of your very precious Tomas. "I know the pan of losing a son without warning in the blink of an eye. "The Kevin Bell Repatriation Trust was a lifeline as our son was abroad also. An amazing organisation we were then in return able to give back to the KBRT for other families like ourselves, we held a fundraiser and raised lots of money to give back to the fund. "A very worthy cause. My heart and thoughts are with you all."
