Wealth, wellness and wellbeing: why healthier ageing isn't just about personal choices

Older adults in the UK's wealthiest bracket are nearly twice as likely to eat well, stay active and avoid depression.
We've all heard it before: eat your five-a-day, and try to get some exercise. It's advice that's simple in theory, yet in practice, not everyone is able to follow it. So what's standing in the way?
Our research examined this question in depth. Using data from UK adults over the age of 50, we explored how socioeconomic status affects the likelihood of meeting the World Health Organisation's recommendations for physical activity and diet. These guidelines include at least 150 minutes of moderate-intensity (or 75 minutes of vigorous-intensity) physical activity per week and a daily intake of at least five portions of fruit and vegetables.
What we found points to a clear and concerning disparity. Wealthier older adults are nearly twice as likely to meet both exercise and dietary recommendations compared to their less affluent peers. And perhaps even more striking, those who don't meet these health guidelines are significantly more likely to suffer from depression.
We analysed survey responses from more than 3,000 adults aged 50 to 90, using data from the English Longitudinal Study of Ageing. While nearly 70% of participants reported doing some form of physical activity, the data revealed a sharp wealth divide.
Adults in the highest wealth quintile (the top 20%) were almost twice as likely to be physically active as those in the lowest quintile. A similar pattern emerged for diet. Over 70% of those in the wealthiest group reported meeting the five-a-day guideline, compared to just over 40% in the lowest income bracket.
This matters because not meeting government guidelines for physical activity and diet can have serious long-term health consequences. Regular exercise is known to increase HDL (or 'good') cholesterol, improve cardiovascular health, and reduce the risk of chronic conditions like type 2 diabetes, heart disease, and some cancers.
It also benefits brain health by lowering inflammation and even promoting the growth of new brain cells. Similarly, diets rich in fibre, vitamins, and antioxidants – found in fruits and vegetables – are associated with lower the risks of disease and cognitive decline, including conditions like Alzheimer's.
Depression disparity
But the impact isn't just physical. Our research also explored links between lifestyle and mental health. Around 19% of participants met the criteria for clinical depression, with the highest risk found among women, people living alone, smokers and those with lower incomes.
Alarmingly, rates of depression were nearly three times higher among those in the lowest wealth quintile (32.6% were depressed) compared to those in the highest (11.1%).
Lifestyle clearly played a role in depression levels. Among inactive participants, 30% reported symptoms of depression – more than double the rate seen in those who were physically active (13.7%). Likewise, those who didn't meet the five-a-day guideline had a depression rate of 23.4%, compared to 15.7% among those who did.
These results suggest that staying physically active and eating well not only improves physical health but may also play a protective role in mental wellbeing. Yet not everyone has equal access to the resources, time, or environments that support healthy living. There is also the role of social isolation as a compounding factor.
Social disconnection is strongly linked to both poor physical and mental health, including depression and even increased mortality risk. Physical activity programmes that also offer social interaction, such as walking groups or community exercise classes, may provide even greater benefits.
Healthy ageing for everyone
The evidence shows that health disparities in later life are deeply tied to wealth and socioeconomic status. This means that addressing them requires more than encouraging personal responsibility – it calls for policy action.
Financial barriers to healthy food and physical activity need to be tackled through targeted programmes, subsidies and infrastructure investments. Making healthy options accessible and affordable – especially for those in lower-income groups – will benefit people and reduce strain on healthcare systems.
As populations continue to age, promoting health in later life is a public health priority. But that effort will only succeed if it recognises – and works to reduce – the inequalities that hold people back from living healthy, fulfilling lives. DM

Dr Marion Morkel, chief medical officer at Sanlam, says the increase in claims for diseases which have a good prognosis when detected early puts the spotlight on the importance of regular screenings and checks. 'The World Health Organisation recommends that annual health screenings and checks start in our thirties, and we urge South Africans to prioritise these as part of their yearly routine.' ALSO READ: Liberty pays out R600 million for two-pot retirement system in 2024 What the claims statistics show for deaths and diseases The claims statistics show that: R6.62 billion was paid across all claims, including R5.47 billion in death and funeral claims; R501.6 million in disability, loss of income and impairment claims and R650.3 million in severe illness and injury claims. Over the past six years, the group has paid out more than R36 billion in claims, with 2024 being one of the highest annual claim totals aside from the Covid-19 peak in 2021. Cancer accounted for 54% of all severe illness claims, up 30% in the number of cancer claims from 2023. Prostate cancer claims in men doubled, while breast cancer claims in women increased by 33%. Cardiovascular conditions made up 20% of death claims and 17% of disability claims. Sanlam paid more than 99% of all death claims. The highest claim was R36.2 million. Morkel says lifestyle diseases came through very strongly in the claims data, with cardiovascular disease and cancer the top two causes of death and disability. 'Cancer claims under the severe illness category increased by 30%, while prostate cancer claims doubled in men and breast cancer claims increased by 33% in women.' 'These increases, while concerning, are partly the result of the knock-on effect of the Covid-19 pandemic when proactive screenings were delayed for several years.' ALSO READ: How to ensure that your future life insurance claim is paid out Claims statistics also show increase in strokes among women She points out that this year's data also shows an increase in stroke and musculoskeletal disorders among women, including women in the younger age groups under the age of 50. 'While much of the data mirrors that of other countries, South Africa's high blood pressure rate is among the highest in the world, an early indicator of heart disease and stroke.' 'Regular screenings and self-checks, especially for breast cancer, are essential. For heart health, know your numbers: blood pressure, cholesterol, glucose and BMI. And once you reach your mid-40s to early 50s, make regular check-ups a priority.' The claims statistics also show: Disability claims skewed male, with 60% filed for men, compared to 40% for women. Sickness income claims had a higher representation of female clients (61% female vs 39% male), with 20% of total claims for women for being pregnancy and childbirth. Severe illness claims were relatively gender-balanced (52% female, 48% male), reinforcing the universal health risks across both genders. Emotional struggles: Low iron levels can contribute to anxiety, depression, and mood swings. Physical symptoms: From headaches and pale skin to brittle nails and hair loss, anaemia's effects are visible and often distressing. Left untreated, severe cases of iron deficiency anaemia can lead to life-threatening complications, including heart failure and damage to vital organs. A local solution: the 'Iron It Out' campaign In response to this growing crisis, two South African healthcare professionals, Sister Karin Davidson of the Cape Town Infusion Centre and Sister Pippa Hime of The Infusion Room, have launched a national awareness campaign called Iron It Out. This grassroots initiative is designed to educate teens, parents, and caregivers about the symptoms, causes, and treatment of anaemia. 'Iron deficiency anaemia is a silent epidemic robbing teens of their energy, focus, and potential,' says Davidson. 'What makes it worse is that it's entirely preventable and treatable with proper awareness and intervention.' Recognising the symptoms One of the key messages of the Iron It Out campaign is that fatigue and exhaustion are not normal. Teens, parents, and teachers need to be on the lookout for these common but often overlooked symptoms of anaemia: Persistent tiredness or mental fog. Mouth ulcers, hair loss, or brittle nails. Headaches, pale skin, and shortness of breath. Restless legs, cravings for non-food items (pica), and cold intolerance. 'Low iron stores can affect every part of a teen's life, from their physical health to their mental well-being,' explains Sister Hime. 'We want young people to understand that these symptoms are not just 'part of being a teenager' they're warning signs that your body needs help.' What 'Iron It Out' offers The campaign is bringing much-needed awareness and resources to South African teens. Key elements of the initiative include: 1. School workshops: Interactive sessions led by healthcare professionals to educate teens about anaemia and how to prevent it. 2. Health screenings: In partnership with clinics and mobile health units, the campaign offers easy access to iron testing. 3. Symptom tracker tools: A practical tracker helps teens identify potential warning signs early. 4. Community outreach: By working with parents, teachers, and caregivers, the campaign ensures the message reaches every corner of South Africa. Anaemia isn't just a health issue, it's a quality-of-life issue. For South African teens, correcting iron deficiency could mean better grades, improved athletic performance, and an overall happier, healthier life.
