Good News for Tofacitinib in Recent Study of Acute Severe UC

A head-to-head comparison of the JAK inhibitor drug tofacitinib and chimeric monoclonal antibody infliximab in the treatment of acute severe ulcerative colitis (ASUC) shows that, contrary to concerns, tofacitinib is not associated with worse postoperative complications and in fact may reduce the risk of the need for colectomy.
'Tofacitinib has shown efficacy in managing ASUC, but concerns about postoperative complications have limited its adoption,' reported the authors in research published in Clinical Gastroenterology and Hepatology.
'This study shows that tofacitinib is safe and doesn't impair wound healing or lead to more infections if the patient needs an urgent colectomy, which is unfortunately common in this population,' senior author Jeffrey A. Berinstein, MD, of the Division of Gastroenterology and Hepatology, Michigan Medicine, Ann Arbor, Michigan, told Medscape Medical News.
Recent treatment advances for UC have provided significant benefits in reducing the severity of symptoms; however, about a quarter of patients go on to experience flares, with fecal urgency, rectal bleeding, and severe abdominal pain of ASUC potentially requiring hospitalization.
The standard of care for those patients is rapid induction with intravenous (IV) corticosteroids; however, up to 30% of patients don't respond to those interventions, and even with subsequent treatment of cyclosporine and infliximab helping to reduce the risk for an urgent colectomy, patients often don't respond, and ultimately, up to a third of patients with ASUC end up having to receive a colectomy.
While JAK inhibitor therapies, including tofacitinib and upadacitinib, have recently emerged as potentially important treatment options in such cases, showing reductions in the risk for colectomy, concerns about the drugs' downstream biologic effects have given many clinicians reservations about their use.
'Anecdotally, gastroenterologists and surgeons have expressed concern about JAK inhibitors leading to poor wound healing, as well as increasing both intraoperative and postoperative complications, despite limited data to support these claims,' the authors wrote.
To better understand those possible risks, first author Charlotte Larson, MD, of the Department of Internal Medicine, Michigan Medicine, and colleagues conducted a multicenter, retrospective, case-control study of 109 patients hospitalized with ASUC at two centers in the US and 14 in France.
Of the patients, 41 were treated with tofacitinib and 68 with infliximab prior to colectomy.
Among patients treated with tofacitinib, five (12.2%) received infliximab and four (9.8%) received cyclosporine rescue immediately prior to receiving tofacitinib during the index admission. In the infliximab group, one (1.5%) received rescue cyclosporine.
In a univariate analysis, the tofacitinib-treated patients showed significantly lower overall rates of postoperative complications than infliximab-treated patients (31.7% vs 64.7%; odds ratio [OR], 0.33; P = .006).
The tofacitinib-treated group also had lower rates of serious postoperative complications (12% vs 28.9; OR, 0.20; P = .016).
After adjusting for multivariate factors including age, inflammatory burden, nutrition status, 90-day cumulative corticosteroid exposure and open surgery, there was a trend favoring tofacitinib but no statistically significant difference between the two treatments in terms of serious postoperative complications ( P = .061).
However, a significantly lower rate of overall postoperative complications with tofacitinib was observed after the adjustment (odds ratio, 0.38; P = .023).
Importantly, a subanalysis showed that the 63.4% of tofacitinib-treated patients receiving the standard FDA-approved induction dose of 10 mg twice daily did indeed have significantly lower rates than infliximab-treated patients in terms of serious postoperative complications (OR, .10; P = .031), as well as overall postoperative complications (OR, 0.23; P = .003), whereas neither of the outcomes were significantly improved among the 36.6% of patients who received the higher-intensity thrice-daily tofacitinib dose ( P = .3 and P = .4, respectively).
Further complicating the matter, in a previous case-control study that the research team conducted, it was the off-label, 10 mg thrice-daily dose of tofacitinib that performed favorably and was associated with a significantly lower risk for colectomy than the twice-daily dose (hazard ratio 0.28; P = .018); the twice-daily dose was not protective.
Berinstein added that a hypothesis for the benefits overall, with either dose, is that tofacitinib's anti-inflammatory properties are key.
'We believe that lowering inflammation as much as possible, with the colon less inflamed, could be providing the benefit in lowering complications rate in surgery,' he explained.
Regarding the dosing, 'it's a careful trade-off,' Berinstein added. 'Obviously, we want to avoid the need for a colectomy in the first place, as it is a life-changing surgery, but we don't want to increase the risk of infections.'
In other findings, the tofacitinib group had no increased risk for postoperative venous thrombotic embolisms (VTEs), which is important as tofacitinib exposure has previously been associated with an increased risk for VTEs independent of other prothrombotic factors common to patients with ASUC, including decreased ambulation, active inflammation, corticosteroid use, and major colorectal surgery.
'This observed absence of an increased VTE risk may alleviate some of the hypothetical postoperative safety concern attributed to JAK inhibitor therapy in this high-risk population,' the authors wrote.
Overall, the results underscore that 'providers should feel comfortable using this medication if they need it and if they think it's most likely to help their patients avoid colectomy,' Berinstein said.
'They should not give pause over concerns of postoperative complications because we didn't show that,' he said.
Commenting on the study, Joseph D. Feuerstein, MD, of the Department of Medicine and Division of Gastroenterology, Beth Israel Deaconess Medical Center, Harvard Medical School, Boston, noted that, in general, in patients with ASUC who fail on IV steroids, 'the main treatments are infliximab, cyclosporine, or a JAK inhibitor like tofacitinib or upadacitinib, [and] knowing that if someone needs surgery, the complication rates are similar and that pre-operative use is okay is reassuring.'
Regarding the protective effect observed with some circumstances, 'I don't put too much weight into that,' he noted. '[One] could speculate that it is somehow related to faster half-life of the drug, and it might not sit around as long,' he said.
Feuerstein added that 'the study design being retrospective is a limitation, but this is the best data we have to date.'

The risk that alcohol poses to women's health has mounted over the last two decades, as women have begun to drink more frequently and in larger quantities. Alcohol-related deaths among women more than doubled from 1999 to 2020. And deaths from alcohol-related hepatitis, a disease resulting in severe liver inflammation, nearly tripled among women over the same time period. A new study, published Wednesday in the medical journal Clinical Gastroenterology and Hepatology, similarly points to an elevated risk of alcohol-related liver disease among women. The condition encompasses various types of liver damage due to excessive alcohol consumption. It spans early-stage inflammation to severe scarring, known as cirrhosis, which can lead to liver failure. Women are more susceptible than men to alcohol-related liver disease for several reasons. For one, their bodies tend to have less water weight and a higher percentage of body fat. That combination increases the concentration of alcohol in their blood, which the liver then has to process. Compared with men, women also naturally have lower levels of an enzyme called alcohol dehydrogenase, which helps the body metabolize alcohol. That again leads to higher blood alcohol concentrations. Those physiological differences, combined with the sharp increase in alcohol use and binge drinking, have made women particularly vulnerable to alcohol-related illnesses. 'Historically, there's been differences in prevalence rates [of alcohol use] between men and women. And essentially, that gap has now closed and the ratio between men and women's drinking is almost at 1 to 1,' said Sherry McKee, director of the Yale SCORE Program on Sex Differences in Alcohol Use Disorder. Changes in women's lifestyles have created increasing opportunities to consume alcohol, McKee said. Today's young adults drink less compared with previous generations, but there are now more women in college than men — and college is generally associated with increased alcohol consumption, she said. 'You pair that with the fact that women are delaying childbirth, delaying marriage — it just gives more space for women to continue drinking in the post-college years,' McKee said. Women's heavy alcohol consumption is most common in midlife, said Katherine Keyes, an epidemiology professor at Columbia University's Mailman School of Public Health. Some researchers attribute the trend to stress or office drinking culture, but Keyes said the main reason women are drinking more is for fun. She noted that wine and spirits are often marketed to women as luxury goods or methods of relaxation. Experts said more awareness of the health risks of drinking could help encourage women to scale back. 'It isn't the case that every single person who drinks heavily will get liver disease from it. But we do know that a proportion of those folks do — about 25 to 30%,' said Dr. Jessica Mellinger, a senior staff physician at Henry Ford Health, a Michigan-based health system. The more a person drinks, she added, the greater likelihood they have of getting alcohol-related liver disease of any stage. The study published Wednesday found that heavy drinkers are developing alcohol-related liver disease at more than double the rate compared with 20 years ago. The researchers suspect that's because people vulnerable to liver disease — including women and those with obesity or diabetes — are drinking more compared with decades past. 'The modern American drinker looks different than it did 20 years ago,' said Dr. Brian Lee, the study's lead author and a hepatologist at Keck Medicine of the University of Southern California. The researchers used data from a national, government-led survey to measure drinking habits and liver health in the U.S. They characterized heavy drinkers as men who consumed at least 30 grams of alcohol per day — roughly two standard drinks — and women who consumed at least 20 grams per day. Among heavy drinkers, the risk of significant liver damage more than doubled over a 22-year period, from nearly 2% in 1999-2004 to more than 4% in 2013-20. The rate of metabolic syndrome — conditions like obesity or high blood pressure that increase the risk of heart disease, diabetes and stroke — among heavy drinkers also increased during that time, from 26% to nearly 38%. Both obesity and Type 2 diabetes can cause fat to build up in the liver, thereby elevating the risk of liver disease. 'It could be the situation of a perfect storm. We have an increase in alcohol consumption … alongside changing prevalence of these other [health] conditions,' Keyes said. Lee said it's important for people to be honest with their doctors about their alcohol intake, so that doctors can decide whether to screen them for liver disease. 'Your risk of liver disease might be higher than you think,' he said. 'The reality is that liver disease is silent, and most people — even with cirrhosis, which is end-stage liver disease — have no symptoms at all. I always say it's a blessing and a curse that you need very little healthy amounts of liver to feel perfectly fine.' Keyes said women in particular tend to wait longer to seek medical care for heavy drinking due to social stigma. 'It's really becoming this hidden epidemic where women wait too long to see someone about a really serious, alcohol-related condition,' she said. Elieah Boyd faces a long road to recovery, but she is thankful for doctors who reattached her arm and a community that is supporting her healing. Elieah Boyd describes herself as an active and creative person. The Southern California cafe manager loves to surf, to hike, to take her Australian cattle dog running. She paints, travels as often as possible, and makes jewelry and bags. She's also right-handed. Now, she's looking at a long road to recovery after her right arm was severed — and then reattached — in early July. But the 24-year-old sees the future not as a difficult process but as a "fun challenge." "I might be left-handed for a little while," she told USA TODAY from UC Irvine Medical Center, where she's already undergone multiple surgeries, with more to come. "I'll have learn how to live my life left-handed." Boyd was riding a friend's e-bike to meet her boyfriend on July 7 when she came upon railroad tracks that cut through two neighborhoods in Ventura. As she tried to wrangle the heavy bike across the tracks, a passerby offered to help. They were lifting the bike together when Boyd saw an oncoming train. "People around here do it all the time to get from one side of town to the other. Everybody in the city uses this crossway all the time. This just happened to be an unsafe time." She saw the train. 'Everything went silent' "I remember looking down at my arm, and everything went silent for a moment," she remembered. "I think I was in disbelief and I was still standing there. The train was gone so fast... I remember looking at my right arm, grabbing it, and there was nothing there." The man who'd helped her with the bike called for help. He is a retired firefighter, Boyd said, so he knew what to do, making sure she was stabilized and telling first responders to find her arm, which was severed cleanly. "He saw that, and he knew (the arm) was probably still viable," Boyd said. Boyd was taken to the nearest hospital, and then taken by helicopter to UC Irvine Medical Center. After 10 hours in surgery, the arm was reattached. She's had four surgeries since, and more are planned. But her outlook hasn't required any help. "I feel OK, and I'm trying to stay as positive as I can," she said. The pain has been minimal and she's been able to mostly avoid heavy painkillers, while expressing how impressive and compassionate medical professionals at the hospital have been. (A request for comment from UC Irvine was not immediately returned.) According to Massachusetts General Hospital, replantation (or the surgical reattachment of a limb) is a complicated process requiring several steps including removal of damaged tissue, shortening bones, reattaching with pins or plates, and repairing muscles, tendons, blood vessels and nerves. "The patient has the most important role in the recovery process," the hospital notes, and Boyd may have some advantages: Younger patients have better odds of regaining nerve function and feeling, and cleanly-cut limbs like her arm are more likely to recover. Doctors generally consider restoring 60% to 80% of function as an excellent outcome. Community steps up to help a woman with 'an undeniable spark' Her community has stepped up to support her as well, with a GoFundMe that's raised more than $50,000 so far. "She radiates joy, warmth and inclusivity in and out of the water," said photographer Amber Jenks, a friend who met Boyd through the local surf community. "She's not just an incredible longboarder, but someone who genuinely lifts everyone around her." Jenks mentioned Boyd's "undeniable spark," and added, "Her recovery is something we're all rooting for, every single day." Doctors haven't given Boyd a date when she can be released, nor are they certain how much function can be restored to the arm. She's has at least 18 months of physical and occupational therapy ahead, she said. "The amount of support, donations, texts and messages, even from people I don't know ... all their positive words have kept me going," she said. Once she's released from the hospital, she's looking forward to getting back to as much normalcy as possible: all the things she'd done before the accident, like surfing and going to the beach, playing with her dog and hanging out with friends and family.
