
Overdoses increasing at Toronto drop-in centres since supervised consumption sites shut down, network says
Overdoses at drop-in centres in Toronto have sharply increased since the provincial government shut down four supervised consumption sites in April, according to a coalition of community agencies.
Before the supervised consumption sites closed, there were typically less than 10 overdoses a month at drop-in sites — which include churches, food banks and neighbourhood centres — said Melanie Joy, manager of the Toronto Drop-In Network.
Now, these sites are reporting overdoses in the high thirties every month, she said.
Joy said this surge is happening throughout the city, not just in the downtown core.
"It's been a dramatic increase, without an increase in resources or supports," she told CBC Radio's Metro Morning on Friday.

But Joy says TPH does not track overdoses that are happening in drop-in centres, in the community, or at the remaining supervised consumption sites that are still open.
Downtown church has become an 'injection site'
More than 50 community agencies are part of the Toronto Drop-In Network, including Metropolitan United Church, located at Queen Street E. and Church Street.
Between 30 and 50 people use drugs on the church's property over the course of a day, said property manager Frank Cormier.
He said he carries Naloxone on him at all times. Last summer, he said he used it about once a month, but now he's using it two or more times a week.

After the safe injection sites closed, Cormier said people who use drugs are lacking spaces to go for support.
"Right now, Metropolitan United Church's front lawn has become the injection site," he told Metro Morning.
On April 1, the province closed nine supervised consumption sites in Ontario — including four in Toronto — that were located within 200 metres of schools or daycares. The legislation is currently being challenged in court by Kensington Market Overdose Prevention Site, a tenth site that was slated to close but is currently still open due to a legal injunction and fundraising.
The nine sites that did close agreed to become Homelessness and Addiction Recovery Treatment (HART) Hubs, an abstinence-based model that provides access to recovery and treatment systems for people struggling with addictions and mental health issues, but does not provide any drug consumption services.
Province says 9 of 28 HART Hubs opened in April
While the HART Hubs were meant to be fully operational by April 1, Joy said that's "not what we're seeing in reality."
As of last month, several of the HART Hubs don't yet appear to have full slates of services up and running. At Toronto Public Health's HART hub, now running temporarily on the Esplanade, only one program of 10 is operational, with the rest planned to get underway this summer or beyond.
The province said in a statement the nine HART Hubs replacing supervised consumption sites "opened, as planned on April 1, including those in Toronto, ensuring no gaps in the delivery of mental health support services."
The government is spending $550 million to build a total of 28 hubs across the province that will provide people with 24/7 support, the statement reads.
It is continuing to work with the remaining 19 hubs "to finalize operational budgets," the statement said.
Joy said HART Hubs offer services that both people working in the sector and those who use drugs have long called for as additions to the supervised consumption site model, such as more detox beds, enhanced one-on-one case management, and counselling.
But she said the hubs do not include key services typically provided by supervised consumption sites, such as access to clean needles, which help reduce the spread of infectious diseases.
"It's frustrating that there wasn't the foresight to include all these services together in one place," Joy said.
Meanwhile, Cormier said the government failed to adequately plan the transition from supervised consumption sites to HART Hubs.
"The system changed suddenly, drastically, and it didn't seem like anybody thought about what the gap is going to be between one model and the next model," he said.

B.C. doctor fired for refusing COVID-19 vaccine loses appeal
The B.C. Supreme Court has dismissed an appeal from a doctor who was fired for refusing the COVID-19 vaccine in 2021. Dr. Theresa Szezepaniak was appealing a 2023 decision from the B.C. Hospital Appeal Board (HAB), which largely upheld the Interior Health authority's decision to suspend her hospitalist privileges at Royal Inland Hospital in Kamloops, B.C. The appeal board had ruled that Szezepaniak's refusal of the shot in 2021 amounted to neglect of her obligations as a hospitalist. Szezepaniak, who had to sell her home and move to a different town to find work after the decision, said that her Charter rights were breached by the HAB decision and asked a Supreme Court justice to set it aside. However, Justice Steven Wilson said the Charter did not apply to Interior Health's decision to suspend Szezepaniak's privileges, as it was an operational decision and not one that was directly controlled by government. "I do not accept that a hospital board's ability to exclude a practitioner from the hospital for failing to comply with the [bylaws] is a decision that is governmental in nature," his decision, published Thursday, read. Szezepaniak had argued that the HAB was upholding discipline based on government legislation, in which case her Charter-protected rights to life, liberty and security of the person — and specifically her right to earn an income to support her family — would have been breached. But the court disagreed, and said that even if the Charter were to apply to the HAB's decision, Szezepaniak's rights were not breached in this instance. That was because, the court noted, the Charter does not protect the right to work in a particular job or position, and Szezepaniak's firing was a result of her decision to not get vaccinated. Contract terminated Szezepaniak's contract with Interior Health was terminated on Nov. 16, 2021, after she declined the vaccine, which was required to continue working in B.C. hospitals under an order from Provincial Health Officer Dr. Bonnie Henry. Her privileges, which granted her the right to provide care at Royal Inland Hospital in Kamloops, were officially cancelled by the health authority in August 2022, and Szezepaniak cited the Charter in an appeal to the HAB shortly thereafter. In both the current Supreme Court case and that HAB decision, the issue was not whether the doctor would be forced to get the vaccine — but rather, the consequences that arose from her decision to decline it. In a Nov. 20, 2023, decision, a HAB panel concluded that Interior Health didn't challenge Szezekpaniak's right to refuse the vaccine, but it did hold her accountable for the fact that that choice left her unable to work under provincial law. "Having the right to make a decision, and your right to do so acknowledged, or respected, is not the same as being held responsible for the consequences," the panel's decision reads. Although the appeal board did not reinstate Szezepaniak's hospitalist privileges, it found the health authority should have suspended rather than cancelled them in August 2022, saying Interior Health should have waited to cancel them if she wasn't vaccinated in time for her next annual review. 'Black mark' Szezepaniak, who is now based in 100 Mile House as a family physician, worked in B.C. hospitals for 21 years before she was fired. She said there was a "black mark" against her name due to the discipline that she received, and that she suffered significant emotional and financial consequences after the firing. Ultimately, however, the court found that the loss of income and her subsequent relocation to find work were not related to the discipline she received — but rather a consequence of her decision to not get the vaccine following the provincial order. Notice of liability A few days after Szezepaniak was barred from working, Royal Inland Hospital's chief of staff emailed to say there were three options for unvaccinated staff: obtain an exemption, resign, or face cancellation of their privileges. Szezepaniak replied with an email saying she would not be "blackmailed or coerced into receiving an experimental injection," the HAB panel decision says. On Nov. 12, a few days before she was fired, she sent an 18-page letter to a health authority manager titled, "NOTICE OF LIABILITY regarding the B.C. Government's Mandatory Testing/Vaccination Policy."

Ontario's health critic calls for details after Grey Bruce health board reps fired
Ontario's NDP health critic is asking the province for more details about why all seven municipal representatives on Grey Bruce Public Health's 11-member board of directors were fired last week, while the four provincial appointees remain in place. France Gélinas said a vaguely-worded health ministry statement in response to questions about the firings doesn't go far enough to explain such a significant change in governance. The health unit serves a wide area around the Bruce Peninsula, including the communities of Owen Sound, Walkerton and Tobermory. "We need transparency here," said Gélinas, who represents the Nickel Belt riding. "The government cannot go on making serious decisions about the governance of public health based on ... what?" Gélinas was reacting to news the board's municipal representatives each received letters last week from board chair Nick Saunders advising of them of the "termination of all municipal appointees to the board" effective Aug. 6. The health unit is governed by an 11-member board which includes seven elected municipal politicians: three from Bruce County and four from Grey County. In response to questions from CBC News about the firings, the health ministry said Ontario's chief medical officer of health launched an assessment of the health unit's operations in 2023 following "multiple complaints" about the board's operations, finances and governance. The health ministry's statement said a report that outlines the medical officer of health's findings has been sent to Saunders "and we are currently awaiting their action plan." The statement doesn't offer any specifics about why the municipal board members were removed. The report has also not been made public. The ministry's statement said the health unit continues to operate normally and the province expects all board members — including those now removed — "to work collaboratively to inform their response to the chief medical officer's report." CBC News reached out to the health ministry for more clarification, but did not receive a response on Tuesday. Gélinas said the province should at least make public the chief medical officer's report. "The government appointees got to stay but the municipal leaders .... all got fired, more or less," said Gélinas. "If this has to do with the report, prove it. Otherwise, it has to do with political manoeuvring and getting people they want in charge of public health." CBC News reached out to all the fired board members for comment. Kevin Eccles, a former board member who is also mayor of the municipality of West Grey, said in an email response he would like to speak about what's happened but said after receiving legal advice "I am not making a statement at this time." Grey Country Warden Andrea Matrosovs and Bruce County Warden Luke Charbonneau issued a joint statement Monday saying they want to work with the health ministry "on the best solutions for residents." Bruce County warden 'concerned' In an interview with CBC News on Tuesday, Charbonneau said he's not been given a clear justification about why the municipal representatives were removed. He's also not seen the report mentioned in the health ministry's statement. "The county of Bruce is concerned about the whole situation," he said. "I have not seen a reasoning that provides a justifiable reason for the letters that were sent terminating our board members." The changes at Grey Bruce Public Health follow moves by the Doug Ford government to step in and take over in cases where local politicians appear to be struggling with fiscal management, including London's largest public school board and hospital. The municipal representatives removed by last week's decision include: Grey County: Andrea Matrosovs (The Blue Mountains). Sue Carleton (Township of Georgian Bluffs).
