Michigan Health and Human Services director details impacts of federal cuts on state-level efforts

Downsizing and mass job cuts put forth by U.S. Secretary of Health and Human Services Robert F. Kennedy Jr., have already hampered public health officials in Michigan, Department of Health and Human Services Director Elizabeth Hertel declared in court filings earlier this month.
Kennedy announced in March that his department would be working to eliminate 20,000 total employees, consolidating DHHS's 28 divisions into 15 and closing half of its regional offices.
However, Michigan Attorney General Dana Nessel and 19 other attorneys general took action, arguing these cuts would functionally dismantle the DHHS, leaving it unable to do its job, with Nessel warning the decision could bring 'immeasurable harm' to the nation and healthcare if the courts do not step in.
A federal judge has since blocked the Trump Administration from issuing further job cuts and reorganizing the Executive Department until a lawsuit brought by a collection of government workers' and healthcare workers unions is resolved.
As part of these cuts, the DHHS is eliminating roughly 3,500 employees at the Food and Drug Administration, 2,400 at the Centers for Disease Control, 1,200 at the National Institutes of Health and 300 at the Centers for Medicare & Medicaid Services.

In a declaration filed as part of the case, Hertel detailed how the reduction of staff and closure of several labs at the CDC alongside cuts at the FDA are hindering the state's work on disease testing, public health and tobacco monitoring.
While the Michigan Department of Health and Human services runs its own Bureau of Laboratories, which is enmeshed with labs operated by the CDC and other states' public health agencies, the CDC's lab closures and staffing cuts have left states to fill the gaps.
The Bureau was directly impacted by the closure of the CDC Viral Hepatitis Laboratory Branch and its discontinuation of hepatitis testing, and according to Hertel has not received consistent communication from the CDC about test discontinuations.
'As a result of sporadic changes to test offerings by CDC laboratories, the MDHHS Bureau of Laboratories has been unable to determine which tests to prioritize with confidence,' Hertel said.
MDHHS also expects to see higher levels of test submissions for Hepatitis C due to the CDC's decision to discontinue testing.
'The MDHHS Bureau of Laboratories serves as the National HIV and Hepatitis C virus NAT Reference Center in cooperation with the CDC and the Association of Public Health Laboratories… if MDHHS shifts its lab capacity to Hepatitis C testing, the MDHHS Bureau of Laboratories staff will have less capacity to respond to other public health emergencies,' Hertel warned.
The department has also felt a lack of central coordination on tests discontinued by the CDC, Hertel said, noting that if the national agency is not at full capacity, there won't be a central coordinating agency for responding to disease outbreaks.
Bureau of Laboratories staff have also reported difficulties contacting CDC staff, Hertel said.
In addition to its impacts on lab testing, DHHS cuts have also significantly impacted the state-level department's work on public health, with Hertel noting that the state relies on a partnership with several CDC programs for infectious disease detection and response activities. As a result, MDHHS staff say communications with CDC subject matter experts is now slow, reduced, or sometimes non-existent.
Because key program staff are no longer with the CDC, there has also been confusion on points of contact and a reduced knowledge base, Hertel said.
'As a result, the CDC has failed to provide timely response to inquiries of urgent nature, reduced or eliminated national calls, and can no longer effectively serve as a national coordinator of infectious diseases efforts,' she said.
As an example, Hertel pointed to reports from her staff that the CDC is no longer sending out-of-state travel notifications about potential disease exposures to Michigan residents, including notices for diseases with critical timelines, like measles. Staff reported that the CDC was not able to coordinate a recent multi-state effort to investigate a complex and urgent case of human rabies, and is no longer hosting 50-state calls on the Highly Pathogenic Avian Influenza response, despite ongoing concerns of a global bird flu outbreak.
' Overall, the mass termination of CDC personnel appears to have created a climate of fear for remaining staff, thereby undermining the ability of remaining CDC staff to effectively carry out their jobs by supporting state health departments like MDHHS,' Hertel said.
These cuts have similarly left MDHHS without access to updated health data and data systems needed to identify emerging health crises and address disparities in healthcare, Hertel said.
Cuts at the CDC have also bled into state-level efforts in preventing HIV, AIDS, viral hepatitis, sexually transmitted infections and tuberculosis.
The CDC has also cut the entirety of its Pregnancy Risk Assessment Monitoring System team and shut down the program's data collection system. While Michigan was able to retrieve its 2024 data before the system shutdown, the CDC's team would normally statistically weight that data, meaning the unweighted data obtained by the state is unusable.
'To use the data, MDHHS would be required to contract with a survey statistician to properly weight this data, expending significant resources that would otherwise have been available from CDC staff,' Hertel said.
The CDC also assists the state in monitoring and responding to lead poisoning in children, Hertel said. However, following significant cuts at the Childhood Lead Poisoning Prevention Program, the MDHHS Environmental Health Bureau has received zero communication from the program.
While the CDC previously sent notices about newly identified food and consumer products containing lead, those notices have stopped, Hertel said.
MDHHS has similarly experienced issues in its environmental public health efforts including less frequent communications, the end of public health programming work groups with other states, and limited and inconsistent guidance from the CDC leaving the CDC Tracking Data Explorer without updated state-level data on drinking water, biomonitoring and radon.
While the CDC also helps the state administer several chronic disease and injury control programs, significant staffing shortages and the loss or potential loss of CDC contracts has left MDHHS with little or no guidance, Hertel said, creating uncertainty regarding the future of these programs.
The MDHHS Division of Child and Adolescent Health has also been impacted, Hertel said, with the Administration for Children and Families indefinitely postponing annual training for Personal Responsibility Education Program (PREP) teen pregnancy prevention grantees.
Hertel also pointed to the effectively shuttering of the CDC's Office on Smoking and Health as a major loss, noting the important role it played in preventing and reducing cigarette use by collecting and sharing information on smoking and its health impacts, including state level data on tobacco-use cessation treatments and high-quality reports on tobacco use trends.
She also noted the loss of the FDA's Center for Tobacco Products, which was effectively shuttered when all members of its staff were terminated on April 1.
'Among other duties, [the Center for Tobacco Products] conducted compliance checks on vendors and retailers to ensure that tobacco products are not sold to those under the age of twenty-one, reviewed premarket applications for new tobacco products before they can be marketed in the United States, enforced advertising and promotion restrictions, and educated the public about the risks of tobacco use including the dangers of e-cigarettes and other tobacco products,' Hertel explained, later noting the state relied on both the Center for Tobacco Products and and the Office on Smoking and Health for its Tobacco programs.
If Michigan does not receive support and funds from the Office on Smoking and Health, it stands to lose $2,347,639 in grant funding and other resources, Hertel said. Most of that funding supports Michigan Quitlink, which provides services to all Michigan residents for tobacco dependence treatment at a cost of just over $1.2 million annually. It also supports efforts to prevent youth tobacco use, offer healthcare provider training on tobacco dependence treatment and offer improved resources for schools to address vaping.
A loss of support from the Center for Tobacco Products could also shutter the state's program ensuring retailers comply with laws banning the sale of tobacco products to underaged customers, as it has been unable to approve and process MDHHS's request for additional funds to remain fully operational during a 2-month contract extension, or issue a solicitation for new state funding.
If the state program is shuttered, eight full time staff members would be laid off and $1.485 million dollars would be lost annually, Hertel said.
An MDDHS spokesperson declined a followup interview on Hertel's behalf.

Kennedy Jr. had espoused the idea of "medical freedom," the ability of people to make personal health decisions for themselves and their families without corporate or government coercion. It's an idea supported under Kennedy's Make America Healthy Again (MAHA) movement to reduce the prevalence of chronic disease in the U.S. by making healthier lifestyle choices. On topics, such as vaccines, Kennedy has said he wouldn't prevent children from being able to receive vaccines but would leave the choice up to parents. "We should have transparency. We should have informed choice, and if people don't want it, the government shouldn't force them to do it." Some public health experts told ABC News, however, that the HHS has been limiting choices on some products for many Americans despite Kennedy's talk about "freedom of choice." Just last week, Kennedy announced the Centers for Disease Control and Prevention would no longer recommend the COVID-19 vaccine for certain groups. Additionally, Kennedy has called on states to ban recipients of food stamps from being able to use them to purchase soda. He has also praised states for banning fluoride from public drinking water and indicated he will change federal guidance on recommending adding fluoride. The public health experts said Kennedy's actions are setting up a dichotomy on public health. "I think that RFK Jr. has done a really good job of identifying some of the problems [in public health], but it's the solutions that are problematic," Dr. Craig Spencer, an associate professor of the practice of health services, policy and practice at Brown University School of Public Health, told ABC News. "What you're seeing with RFK Jr. and his approach to health is an individualization of public health. It's this idea that you can make decisions for your health, and that's always been true." He went on, "We need to be able to follow their guidance, not just have them tell us, 'Follow your own science.' As the focus shifts from community to individuals, we're losing a lot of that underpinning, which has led to a lot of the gains in public health." Kennedy has repeatedly stated that he is not anti-vaccine and that he supports vaccination. Shortly after Trump's election, Kennedy said in an interview with NBC News that "if vaccines are working for somebody, I'm not going to take them away. People ought to have choice, and that choice ought to be informed by the best information." Separately, in an opinion piece Kennedy wrote for Fox News in March on the nationwide measles outbreak, he said the measles vaccine helps protect individuals and provides "community immunity" but also called the decision to vaccinate a "personal one." However, last week, Kennedy announced the removal of the COVID-19 vaccine from the CDC's immunization schedule for "healthy children and healthy pregnant women." The CDC's immunization schedule is not just a guide for doctors but also determines insurance coverage for most major private plans and Medicaid expansion programs. Following Kennedy's announcement, the schedule was updated noting all children would be eligible for COVID vaccines, but now under a shared-clinical decision-making model -- allowing parents to choose whether their children are vaccinated alongside advice from a doctor. "Regarding the vaccines, HHS is restoring the doctor-patient relationship," HHS spokesperson Andrew Nixon told ABC News in a statement. "We are encouraging those groups to consult with their health care provider to help them make an informed decision. This is freedom of choice." "If you restrict access, you necessarily restrict choice," Dr. Matthew Ferrari, a professor of biology and director of the Center for Infectious Disease Dynamics at Pennsylvania State University, told ABC News. "Those two things are antithetical. You can't do both. You can't say you're allowing choice if you're restricting access." Ferrari said the idea of "medical freedom" is catchy, but public health recommendations are made based on how to protect the most vulnerable individuals. "If you look at the outcomes, if you look at the consequences of that movement, it has been to disproportionately restrict access to -- and restrict support and infrastructure to allow people to access -- preventive medicine," he said. "It's sort of easy to say, 'Well, take the vaccine away. But [vaccines] prevent a future outcome of illness for yourself and for others in the community." Traditionally, the CDC's Advisory Committee for Immunization Practices decides if there is a benefit to a yearly vaccine and who should get it. The independent advisory committee then makes recommendations to the CDC, which has the final say. The committee was set to meet in late June to vote on potential changes to COVID vaccine recommendations. Spencer said Kennedy's bypassing of traditional avenues when it comes to changing vaccine recommendations is also taking away choice from people. "This did not go through the normal process that it should have, and he basically just made a decision for people while at the same time saying that he's going to let people make a decision," Spencer said. Kennedy has also campaigned to prevent Americans from using food stamps -- provided under the Supplemental Nutrition Assistance Program -- to buy candy and soda. "It's nonsensical for U.S. taxpayers to spend tens of billions of dollars subsidizing junk that harms the health of low-income Americans," Kennedy wrote in an opinion piece for The Wall Street Journal last September. Secretary Kennedy is unapologetically taking action to reverse the chronic disease epidemic, not subsidize it with taxpayer dollars. Nutrition experts agree that sugar-sweetened beverages (SSBs) are unhealthy. Frequent consumption of SSBs is linked to health issues such as weight gain, obesity, type 2 diabetes, tooth decay, heart disease and kidney diseases, according to the CDC. Kristina Petersen, an associate professor in the department of nutritional sciences at Pennsylvania State University, told ABC News there is a crisis of diet-related diseases in the U.S., which increase the risk of disability and reduces lifespan. However, she said there needs to be strong evidence of the benefits of restrictive policies if they are to be put in place. "In terms of limiting people's choices, it is important to consider all the different roles that food plays in someone's life, and so obviously we want people eating nutritious foods, but also we need to acknowledge that food is a source of enjoyment," Petersen said. "A lot of social situations revolve around food. So, when we're thinking about reducing people's access to given foods, we need to think about the consequences of that." One unintended consequence could be an eligible family not signing up for SNAP benefits because of the restrictions, she said. Even if a ban on buying candy and soda with SNAP benefits does occur, Petersen said she is not aware of any evidence that shows banning certain foods leads to healthier diets. She added that the nation's dietary guidelines are written to emphasize healthy foods like fruits and vegetable rather than telling people to avoid or restrict less healthy foods. "All foods can be consumed as part of a healthy dietary pattern. It's really just the amount and the frequency that determines whether that pattern is helpful overall or less helpful," Petersen said. "People can have small indulgences, but really, we're interested in what is their pattern over a period of time." Providing incentives for purchasing healthier foods may be more effective and still allow people to have choice, Petersen said. A 2018 study used a model simulation to study the effects of food incentives, disincentives or restrictions in SNAP. One of the simulations involving incentives for foods such as fruits, vegetables, nuts, whole grains, fish and plant-based oils found to have the most substantial health benefits and be the most cost-effective. "Things like fruits and vegetables, they do tend to be more expensive, so if you incentivize them by providing more benefits … that's making the dollar go further, and it's kind of making the economic piece of this a bit stronger," Petersen said. "A lot of this is framed around personal choice. Rather than restricting access to, how can we give people more access to healthy foods? I think that's going to have the greatest benefit here." Woman dies of brain-eating amoeba after using tap water in nasal rinse device
A woman in Texas has died after contracting a fatal brain amoeba from contaminated water. The patient, 71, had used a nasal irrigation device filled with unboiled tap water from an RV's water faucet, according to an alert from the U.S. Centers for Disease Control and Prevention (CDC). The "previously healthy" woman was staying at a local campground at the time, the agency stated. Dangerous Fungus Could Spread To Parts Of Us, Researchers Claim Within four days of using the nasal rinse, the woman developed "severe neurologic symptoms," including fever, headache and altered mental status. The patient was treated for a potential infection of primary amebic meningoencephalitis (PAM), a rare, deadly brain infection caused by Naegleria fowleri, also known as the brain-eating amoeba. Testing of the patient's cerebrospinal fluid confirmed she had contracted N. fowleri, according to the CDC. "This case reinforces the potential for serious health risks associated with improper use of nasal irrigation devices, as well as the importance of maintaining RV water quality and ensuring that municipal water systems adhere to regulatory standards," the health agency stated. Naegleria fowleri can also be a risk when swimming in fresh water. It is more prevalent during hotter months, as the amoeba grows best in high air and water temperatures. Fox News Digital previously spoke with Tammy Lundstrom, chief medical officer and infectious disease specialist for Trinity Health in Michigan, about the risks of infection. "The risk of brain-eating amoeba is very low," she said. "Fewer than 10 people in the U.S. every year get infected — but unfortunately, most cases are fatal. There are only a handful of survivors of known cases." The death rate for Naegleria fowleri exceeds 97%, per the CDC. The initial symptoms of PAM usually begin about five days after exposure, but they can be noticed sooner. Early signs usually include headache, nausea, fever and/or vomiting, the CDC's website states. As the infection progresses, people may experience confusion, stiff neck, disorientation, hallucinations, seizures and coma. Dangerous Fungus Spreading In Us Hospitals Has 'Rapidly Increased' "People usually start to feel ill one to 12 days after water exposure," Lundstrom said. "Early symptoms should prompt a medical evaluation, as they are also signs of bacterial meningitis." Death can occur anywhere between one and 18 days of infection, at an average of five days. To prevent contracting the fatal infection, the CDC recommends using "distilled, sterile or boiled and cooled tap water for nasal irrigation," as "improperly maintained" municipal water and RV water systems carry the risk of disease. It is also best to avoid immersing your head in the water when swimming in summer, Lundstrom told Fox News Digital. "Infection occurs when water harboring the amoeba goes up a person's nose, usually during swimming," she said. "It is not known why some people get infected and others, even swimming companions, do not." Drinking contaminated water does not present a risk, and the infection does not spread from one person to another, Lundstrom added. Because the amoeba is found in soil, the CDC also recommends avoiding stirring up the sediment at the bottom of lakes, ponds and rivers.
