Report highlights ‘significant concerns' over hospital maternity services

The trust has apologised to families and said that a 'comprehensive improvement plan' is in place.
A report by NHS England's Maternity Safety Support Programme highlights how maternity services in Leeds 'had a challenge in responding to families who have experienced harm and poor outcomes' which led to several families seeking an independent inquiry.
It highlights a 'challenging' culture in the organisation.
The report says learning from incidents was 'not robust' and describes how staff felt safety concerns were de-escalated without being resolved.
Around 9,000 babies are born each year across St James University Hospital and Leeds General Infirmary, which are run by the trust.
The trust is one of several which are being examined as part of a national review into NHS maternity care.
The first part of the review will see experts investigate up to 10 of the most concerning maternity and neonatal units, including Leeds, with a view to give affected families answers as quickly as possible.

The second will be a 'system-wide' look at maternity and neonatal care, uniting lessons from past maternity inquiries to create one clear set of actions designed to improve national NHS care.
The investigation will report back before the end of the year.
Law firm Irwin Mitchell said that it is representing a number of affected families, including parents whose babies have either died or been left with life-long disabilities or mothers who have been injured while giving birth.
Medical negligence lawyer Rachelle Mahapatra, from the firm's Leeds office, said: 'This latest report yet again lays bare the scale of the problems within Leeds's maternity services.
'That more than 100 recommendations have been made will yet again cause concerns and upset among families, including those we continue to represent.
'One of the most damning areas of concern is issues that have blighted maternity services have been allowed to continue because of a lack of learning.
'It's imperative that when something goes wrong in the NHS, families are listened to, problems are analysed and reviewed – and any recommendations implemented to prevent the same mistakes happening time and time again.'
Rabina Tindale, chief nurse at Leeds Teaching Hospitals NHS Trust, said: 'This report has highlighted significant areas where we need to improve our maternity services, and my priority is to make sure we urgently take action to deliver the recommendations.
'I would like to apologise to all the families who have received maternity care with us which has fallen short of the high standard we aim to provide.
'It is vital that as a trust we listen more to our families and understand their experiences and concerns so we can address these and ensure everyone's experience is of the highest standard.
'We have already started making improvements to our services, and have a comprehensive improvement plan in place.
'Our commitment is to ensure every family feels safe, supported, and respected throughout their maternity journey with us.'

Cases in England topped 85,000 the highest since records began in 1918 A vaccine for gonorrhoea is now available at sexual health clinics in England as part of a world-first scheme. ‌ The vaccination programme is expected to save the NHS £7.9 million over the next decade and combat increasing levels of antibiotic-resistant strains of the disease. ‌ The move aims to tackle rising levels of the sexually transmitted infection (STI) after cases in England topped 85,000 in 2023, the highest since records began in 1918. ‌ The free jab will be on offer from Monday to patients at the highest risk of the sexually transmitted infection, including gay and bisexual men with a recent history of multiple sexual partners or a bacterial STI. The vaccine is an existing jab, known as 4CMenB, that is currently used to protect people against the meningococcal B disease, a serious bacterial infection that can cause meningitis and sepsis. It is used in the routine childhood programme and given to babies at eight weeks, 16 weeks and one year. ‌ The programme is targeted to those most at risk and could prevent up to 100,000 cases. Gonorrhoea disproportionately impacts specific communities, such as those in deprived areas, people of black Caribbean ethnicity, and gay, bisexual and other men who have sex with men, according to the Joint Committee on Vaccination and Immunisation (JCVI). Patients getting the gonorrhoea vaccine will also be offered jabs for mpox, human papillomavirus (HPV), and hepatitis A and B at their appointment. ‌ Ashley Dalton, the minister for public health and prevention, said: 'Rolling out this world-leading gonorrhoea vaccination programme in sexual health clinics in England represents a major breakthrough in preventing an infection that has reached record levels. 'This government's world-first vaccination programme will help turn the tide on infections, as well as tackling head-on the growing threat of antibiotic resistance. 'I strongly encourage anyone who is eligible to come forward for vaccination, to protect not only yourselves but also your sexual partners.' ‌ Consultant Epidemiologist at the UK Health Security Agency, Dr Sema Mandal said: "This roll out is hugely welcome as we're currently seeing very concerning numbers of gonorrhoea, including even more worryingly antibiotic resistant gonorrhoea. The vaccine will give much needed protection to those that need it most - making the UK a world leader in the fight against gonorrhoea. "And it's excellent to see that at the same time other important vaccines will continue to be offered in sexual health services to those eligible, protecting against mpox, hepatitis A and B, and HPV. Sexually transmitted infections aren't just an inconvenience – they can have a major impact on your health and your sexual partners, so if offered I strongly urge you to get these jabs, you'll be protecting yourself as well as others. Don't put it off and regret it later." What are the symptoms of gonorrhoea? According to the NHS, typical symptoms of gonorrhoea include a thick green or yellow discharge from the vagina or penis, and pain when urinating. In women, it can present as bleeding between periods. However, around one in 10 infected men and almost half of infected women do not experience any symptoms - meaning it's important to get tested if you've had unprotected sex. You can visit your local sexual health clinic to have a test, where a professional will perform a quick, painless test with a swab.

Charlotte Hollins Anderton, 23, plummeted 30ft while working as a camp leader in the US and endured an eight-hour wait for rescue with a fractured spine, two shattered feet and internal bleeding A British woman has suffered a "life changing accident" while working as a camp leader in the US and has been stuck in hospital ever since, her family have said. ‌ Charlotte Hollins Anderton, 23, from Abergele, Wales, plummeted 30ft and lay with a fractured spine, two shattered feet and internal bleeding for eight hours before rescue arrived. Her heartbroken family say she required numerous surgeries and intensive care treatment - and have since set up a fundraising appeal to assist in bringing the young Brit home, North Wales Live reports. A statement on the GoFundMe page reads: "Our beautiful Charlotte was tragically involved in a life changing accident whilst working as a camp leader this summer. ‌ ‌ "She has suffered a life threatening fall on the 4th July and has been hospitalised since. Her fall resulted in spinal and foot fractures requiring multiple surgeries, internal bleeding, and admission to the intensive care unit." It comes after a woman, 20, was found dead on bus with 26 iPhones glued to her body. Charlotte's family said: "She's also dealing with intense emotional trauma from the fall and it breaks our hearts that we can't be there to help. But on top of all of the unimaginable pain and suffering, while we have been deciding what to do as a family, we were just hit with another worry that her insurance company will not be covering the cost of repatriation from America to back home." ‌ The family added that Charlotte urgently needs to return home to the UK so that she can access specialist care via the NHS and continue her recovery with them by her side. "It is because of this that we are reaching out to her friends, and our community around us, to ask if anyone can spare anything to help us with the costs related to her care and journey home," the statement adds. The fundraiser has almost reached £10,000, but Charlotte's family say her repatriation currently costs £45,000. "We know this is a difficult time financially for so many, and if you are not able to donate, then PLEASE can we ask you to share this far and wide, on your social media and around your work," they said. The family further explained they never thought they would find themselves in such a situation, asking for support when all they desired "is to have her home." The statement continued: "We will be keeping everyone updated every step of the way, and thank you all so much already for your kind words, promises of donations and support. "It means more to us than you could imagine. Thank you so much."

Doctors in Scotland are being asked for their views on the future of the profession. A 'once-in-a-generation' project has been launched to look at the current shape of the health service and gauge the views of the profession on how it should look in the next 15 to 20 years. The health service has gone through a period of prolonged struggle in the past decade, with high waiting times and long lists of Scots yet to be seen or treated. Multiple political parties have said there is a need for reform in the NHS to ensure it is sustainable in the long term, but few concrete ideas have been put forward by Government. As part of the first phase of the Future Medical Workforce initiative, doctors are being asked to come forward for a series of focus groups looking at the future of training, recruitment and workforce planning – but the project will not cover the pay and conditions of medics. Health secretary Neil Gray said: 'This is a once-in-a-generation opportunity to shape the future of the medical workforce and create a sustainable, fulfilling career path for doctors in Scotland. 'We know the NHS faces major challenges, and it is vital that those living the reality of what it means to be a doctor today are at the heart of shaping what comes next. 'I strongly encourage doctors across Scotland to get involved in this work. Your voice matters and your insight will help ensure that the future of our health service is informed, inclusive and resilient.'
